Naval Medicine...Admiral Nelson said “Everyone is a bachelor when beyond Gibraltar” so it is not surprising that a great deal of naval medicine in the 18th and 19th centuries was related to the consequences of carousing on shore in ports all around the world. I am pleasurably working my way through the 20 Patrick O’Brian novels about Captain Jack Aubery and his spy/physician Stephen Maturin. In the West Indies there is a club called “The Patrick O’Brian Widows’ Club” which is made up of the wives of men who read and re-read the novels over and over again. It is about 10 years since I read them first time (when my wife asked me not to read them again because she wanted to talk to me about things other than sailing) and I have managed, until recently, to hold off quite valiantly. But the medicine of that age is so well portrayed and the life of an 18th century doctor so strange that I couldn’t prevent myself.

200 years ago most doctors were working on theories of health that had been developed over 2500 years ago. No-one had considered the idea of testing conflicting ideas, and memory, being what it is, doctors had tended to remember the good results and forgotten the bad. Why else would draining a pint or two of blood from an already weak man be thought of as helpful? The word ‘scientist’ was not recognised – they called themselves natural philosophers. They concentrated on describing everything they saw and many had prodigious intellect and seem nowadays to have been true polymaths with interests covering everything from earthworms to the workings of the brain.

Britain and France produced the best doctors in the world, but only doctors qualifying from

Oxford and Cambridge were allowed to work in London. Most people learned by watching their seniors and qualified when they felt they had seen enough or had paid enough for their degree. Ships doctors were usually surgeons who would not have had a degree of any sort would have had a basic salary of £5 a month and another £5 for every 100 cases of venereal disease they treated. Physicians (that is doctors who had a degree, bought or otherwise) were considered to be gentleman and would have looked down on surgeons

Apart from venereal disease, the daily ration of half a pint of rum twice a day, even though it was mixed with water, ensured a regular supply of alcohol related injuries and illnesses. If there were casualties of battles, the surgeons would be the ones amputating and coping as best they could.

82% of deaths in the navy were due to disease or accidents and 12% from sinking; only 6% were from battles. Scurvy is from a lack of vitamin C and was the biggest killer of sailors. It was thought to be infectious and due to damp clothes. It causes teeth to fall out, gums to rot, liver disease, depression and death and was discovered to be preventable in 1743 but the royal navy didn’t start handing out limes as a routine to sailors until a good 50 years later. The limes immediately reduced the death rate on long trips from 1 in 3, to about 1 in 11.

However, in general, the common causes of disease were thought to be due either to the weather, the season or poor constitution. Wound infections were thought to be due to invisible particles coming from rotting food and swamps. Changes in the air, certain foods and not enough exercise or poor sleep would disturb the balance

of the body’s functions and treatment was aimed at restoring the balance. The theory of the four humours (blood, phlegm, black bile and yellow bile) explained both the symptoms and the remedies. So, fever was treated by inducing vomiting and sleep, with an absence of red meat. Red wine was to be encouraged. Bleeding was reserved the most seriously ill, or as a precaution against illness if approaching a swampy area or the equator. Mental illness was common, and was best treated with rum. Powdered Spanish flies in alcohol cause skin blisters when applied and were thought to reduce inflammation.

At this time, medicines were marketed by all sorts of quacks and peddlers, with many mixtrures having exotic names such as Lucatellus’ balsam, or Algaroth. Their claims for cure were unproven and probably one of the only effective treatments the doctor had in his bag was Peruvian bark – which contains quinine. It was used to treat fevers and because many of the exotic fevers would have been malaria, for which it is effective, it was used to treat all other fevers, albeit ineffectively. Mercury and rhubarb were used to induce diarrhoea and morphine (as laudanum) was used to cause constipation and to sedate or reduce pain. Syphilis was treated with mercury (“the blue pill”).

Doctors in the 18th century probably achieved very little in terms of curing people. Those who were going to get better, did so irrespective of what the doctor did. Indeed, apart from Peruvian bark, many of the treatments were harmful and probably increased the chances of dying. Once limes were introduced the biggest killer of all became rare and perhaps this is a reminder that in fact public and occupational health save far more lives than individual doctors, even today.

I get a lot of people coming to me asking for help with stress and anxiety, and I’ve written previously about how acupuncture regularly helps such people.

Treatment usually involves some ‘homework’. It is amazing how many people do not give themselves any real down time, or ‘Me Time’ as I call it. Their busy lives revolve around work and kids and housework and helping others, with no thought for themselves.

Just take half an or so out to yourself, doing nothing except relaxing, take a walk or sit in the garden, listen to the birds, really contemplating sights, sounds and smells. Doing this can help to switch your mind of for a short while, and allow you to just be ‘in the now’ i.e. be a human ‘being’ for once, not a human ‘doing’. But do it where you won’t be interrupted, even if you shut yourself in the bedroom with strict instructions not to be disturbed! By the way, ‘vegging out’ in front of the telly doesn’t count!

Ok, so not everyone can just switch off their thoughts, so this is where relaxation techniques such as meditation, yoga, Tai Ji and so on come in. It really does make a profound difference, so next time you’re feeling overwhelmed by it all, reward yourself with some ‘me time’. If you want more advice how on to do this, give me a call.

Are you due to retire over the next few years?

If you are planning to retire over the next few years there are a number of things you should be considering. For example if you are invested in a private pension are you invested in lower risk funds that will better protect your fund values if the stock market goes down shortly before you retire? If you are not sure what type of funds you are invested in ask an Independent Financial Adviser. You can ring the Department for Work and Pensions in Newcastle and obtain the amount of pension you are likely to get or you can request this on line. Once you have a better idea of your total retirement income is it likely to be enough and if not what are your options. Can I increase my state pension if I’m not entitled to the full

amount, can I increase my company or private pension contributions or should I save into another type of account such as an ISA? Do you know how much tax free lump you may be entitled to from your company or private pension and what you might do with this lump sum – save, invest or spend? Are you aware that you can shop around for the best annuity rate to provide your pension income rather than staying with your existing private pension provider? There can be a considerable difference between the best annuity rate available in the market and the rate offered by your existing provider and this open market option as it is called is available on all private pension plans at no cost. It can certainly pay to shop around for the best rate and an Independent Financial Adviser is best placed to help you do this. Are you also

aware that some insurance companies offer better rates if you have some health issues or smoke or have previously smoked? There are also now a number of flexible retirement options that you can consider with your pension fund other than buying a guaranteed but inflexible income for the rest of your life? These flexible options include the ability to select a specific amount of income for a specific length of time subject to minimum and maximum limits imposed by pension rules or to select a death benefit to be paid to your estate in the event of death before for example the age of 75. The decisions made at retirement can last a long time and in many instances can not be changed afterwards so fully discussing your options and objectives with an Independent Financial Adviser is strongly recommended.
