The pills people take to relieve headaches and migraines may be making things much worse, according to the National Institute for Health and Clinical Excellence (Nice) in guidance to the health service for England and Wales.

As many as one in 50 people suffer continual headaches because of “medication overuse”, Nice reports. The problem begins with taking the odd painkiller for tension headaches or migraines, which usually works. But some people take the pills more and more often, until they are on tablets for more than half the days in a month. Nice says that if this goes on for more than three months the medica-tion ends up causing the problem it is intended to cure.

So what alternatives are there to continually popping pills? The British Osteopathic Association comments, “tension-type and neck related headaches are the most common form of headache. If you suffer from tension-type headache, then regular gentle exercise will almost certainly help - such as yoga, Pilates, swimming, walking. Some people find it hard to relax and probably don’t realise how tense they are. If someone suffers from headaches it may also be neck related due to stiffness or restrictions of the neck. Osteopaths look to find the cause of tensions or stiffness and seek to relieve it by helping the body achieve more mobility and flexibility.

Other lifestyle factors such as poor posture at a desk or tiredness can lead to tension-type or neck related headaches. Osteopaths look to relieve this tension and give advice on posture and exercises to do to help prevent the tension or stiffness from building up.

HEALTH ADVICE:Chronic Kidney Disease

This dreadful-sounding disease is really, for most people, not a disease at all, but a description of what happens to us all as we get older. You may have been to the doctor recently or seen a copy of a letter that your doctor has sent and seen these words. You have been struck to the core with a fear that you are dying; that the doctor has been keeping a dreadful secret from you. You don’t need to book in with the funeral directors and get measured up just yet because I will explain all.The word ‘chronic’ (from the greek Chronos meaning time) is used by doctors to mean something that goes on for a long time. It doesn’t mean anything bad, or severe, or painful, or indeed anything except prolonged. A chronic nail infection means you have had it for a long time: not that it is severe. (The opposite to ‘chronic’ is ‘acute’ which is also often mis-understood by non medical people).

Over the last few years there has been increasing interest in the normal function of kidneys and measuring it using a test called the glomerular filtration rate. This measures how effective your kidneys are at filtering out the toxins that your body makes. The rate at which you kidneys gradually deteriorate is determined by a whole host of things including your lifestyle, your age, your race, and whether you have conditions such as diabetes or high blood pressure and whether you smoke.

Chronic kidney disease (or CKD) is divided into 5 levels, with 1 and 2 being considered normal kidneys, and 3,4 and 5 indicating that the kidneys are not working as well as they perhaps could. In the whole of the population of Britain, about 1 in 10 people have CKD 3 (the mildest form). About half of people over 75 will have CKD 3 or 4 or 5.

It is useful to know whether you have CKD 3 because there is evidence to show that improving health generally in these early stages maintains good kidney function and reduces the likelihood of any worsening towards CKD 4 and 5. Most people with CKD 3 don’t need any treatment. However rates of deterioration vary considerably between people and so regular checks such as blood pressure, excluding diabetes, checking for protein in the urine and keeping blood cholesterol levels low are important. (Note for the smokers here: smoking damages most body systems and stopping smoking is probably the biggest step you can make to improve your health generally).

People with chronic kidney disease usually have other health problems which means they are being regularly tested and reviewed for these anyway. Or, they tend to be older and ought to be monitored every 6 months or so by blood pressure checks and blood tests so that the kidney function can be watched. A yearly check on the urine is used to see

whether there is much protein in it and if there is then that is a trigger for the doctor to think about whether treatment ought to be started.

The main treatment for CKD (after, or as well as, improving lifestyle) is a group of drugs imaginatively called the ACEs (angiotensin converting enzymes). These were introduced in the 1990s as a treatment for blood pressure but they were noticed to improve kidney function as well. Generally they have practically no side-effects but once in a while, when starting them, some people find that their kidney function can worsen. For this reason people who are starting an ACE, or increasing the dose of their ACE, should have a blood test 4 weeks later to check they are working properly.

Since CKD3 or more is so common, it is a shame that the only medical term we have for this condition makes it sound so awful. The fact that chronic kidney disease level 1 and 2 indicates that you have normal kidneys shows how poor the term is.

You probably know that GPs are monitored for all sorts of things nowadays and that they are compared with their peers locally and nationally. These comparisons range from the ease of making an appointment to whether they have checked the circulation in the feet of all their diabetics. There is a raft of measurable indicators. (look at www.qof.ic.nhs.uk/search/ in your own practice). As one of the doctors who is responsible for identifying and looking at practices where performance could be better (and I am pleased to say that locally the quality is extremely high), I can see the point in collecting information about who has CKD3 or more. After all, if as a GP, you don’t know whether a patient has CKD, you can’t think about preventing it from getting worse. But the term chronic kidney disease is alarming. GPs have no other term they can use, and if they don’t use it, the doctor can look as though they are offering poor care.

So, next time you see your notes, if you notice the term chronic kidney disease, say thank you to your doctor for thinking ahead; remember your greek, and think to yourself: I haven’t got a disease, it is just that my kidneys are older than I know I really am.

Just when you thought that all was done in the garden for this year - out comes the sun, and there’s loads to do again. Planning for the following year is something that gardeners of the most established gardens will no doubtedly and meticulously undertake to ensure the best from next years show of flowers.

Scarify and aerate existing lawns. Brush sharp sand into the spiked holes.

Last chance for a lawn autumn feed. Keep mowing - but with blades set

higher. Still time to plant some bulbs - but

soon. Out with the summer bedding plants

when finished flowering. Clear up all fallen leaves and put

them on the compost heap. Keep dead-heading roses - they

could flower until Christmas, especially with this weather!

It’s planting time for shrubs, conifers, perennials, roses and trees.

Plant out your spring flowering plants. Pansies, wallflowers etcBring in your Dahlia tubers.

Carry on with shrub cuttings. A good time for dividing your hardy

perennials. It is usually a good time to paint

fencing and trellis. The plants can be moved back from the fence more easily now.

Carry on picking fruit as it ripens Plant out your spring cabbage - but

net them against hungry pigeons. Plant out autumn onion sets and

garlic. Dig up your remaining potatoes

- not much use after they are hit by frost!

Grease bands on the fruit trees. As soon as leaves have fallen, treat

peaches and nectarines with copper fungicide to help control leaf curl.

Spray cherries with copper as well - it helps prevent bacterial canker.

Start digging in some organic manure into the veg. plot.

All of the Asparagus ‘ferns’ can now be cut down.

Prune your blackberries - Simply cut out all of the growths that have fruited this year.

Any tender plants need to be bought into the greenhouse now!

Remove all traces of shading from greenhouse glass - wash down if dirty.

Line your greenhouse with bubble polythene - if you need to conserve heat for the winter.

Make sure that heaters are working - before you need them!

Reduce watering now, and only water in the mornings.Still keep an eye out for hot days, when your ventilators may need opening.

Carolyn Bailey is an artist who specialises in painting portraits of horses, dogs and cats, and who also paints people’s homes and historic buildings. She has a lifetime’s experience of painting and works in any medium but feels that watercolours and pastels can be the most sensitive for capturing the subject’s character.Carolyn has painted many much-lovepets and prize-winning horses which are

commissioned as either special gifts or

as awards. One such was a young show horse who has won at all his shows. His owner was so delighted at receiving the portrait, saying that ’it was the best prize I could have won’.

Another painting Carolyn did was of a show pony for his proud owner’s birthday. Her mother wrote ‘I wanted to let you know that my daughter loves her painting of her pony. Thank you for your work on this. I have recommended you to a friend of mine, so she may be in touch regarding her dogs.’

Carolyn has painted many dogs and a recent one was a complete surprise as a present from a husband to his wife.

Carolyn later received an email from his wife saying ‘ I want to say thank you for the beautiful painting you did of Harvey and Oli, our dogs’. She wrote, ‘I opened it and thought ‘Oh, that’s really nice it’s two Schnauzers” to “Oh my goodness, it’s Harvey and Oli, beautiful!!”This is Carolyn’s particular gift – to capture the subject’s real spirit and likeness.

Carolyn also paints gardens, landscapes and flowers in an impressionist style, creating vibrant unique works of art.Carolyn is very happy to take commissions and is taking orders now for Christmas.
