Dear Dr. Winter, I need implants in my upper and lowerarches. I have had several consultations and opinions all overthe board! Some of the consults said I only needed 4 implantsfor the upper and 4 for the lower and others have said I need
just two for the lower. I did tell them I wanted to be able tohave teeth that didnt come out and I dont have a lot of bonein either arch. What are your thoughts? Thanks, Jack J.
Dear Jack, there are several good questions here. I will tryand break them down and I encourage you and the readers toread my articles on Upgradeable Dentistry and AdvancedTreatment Planning under publications at:www.hamptondental associates .com where I have writtenarticles about these very questions. I also outline in variouspublications the differences between removable overdenturesand fixed implant bridges and show examples of each type ofimplant treatment.
First of all, without a consultation and x-rays I cannotcomment on your specific situation. The number of implantsyou require is dependent upon the quality and quantity ofyour remaining bone, your budget and the type of prosthesisyou require. Whether the implant restoration should befixed or non-removable by the patient or whether it isacceptable to remove the implant supported denture and takeit out at night is a personal decision and both have advantagesand disadvantages. Most people that lose their teeth wantfixed teeth. In addition, the patients health, medication(s)and anatomy all play a role in deciding upon the ideal implantrestoration. It is important to note that sometimes we can start
with a removable implant denture due to budgetaryconstraints and add implants over a year or two and Upgradeto a fixed restoration or bridge when finances allow!
Next, we can discuss the number of required implants. Inthe upper arch most people have already been wearing adenture or partial and they have already undergone bone loss.As bone is lost in the upper jaw the sinuses expand and inorder to place implants in this area there is a need to growthe bone by doing a sinus grafting procedure and possibly ablock grafting procedure. This sinus augmentation is aprocedure that will allow implants of normal length to beplaced in the posterior upper jaw. A way to bypass this is azygomatic or long implant that is used in an all-on-fourprocedure. This procedure will place 4 implants at angles thatare used to support a full arch of denture teeth or a hybridprosthesis. This may be a predictable procedure but replacing14 teeth (24 roots) with 4 implants doesnt provide as large amargin of safety as using 7-9 implants per arch. This is amatter of philosophy and the treating implant dentist andsurgeon will be able to explain their philosophy based upon
their education, skill and judgment.As far as the two-implant option given to you for your
lower arch, this is strictly to support a lower overdenture,which is removable. In order to have a fixed restoration(non-removable) you would require a minimum of 6maxillary and 5 mandibular implants in my practice. Thatbeing said if you are able to place more implants and over-engineer the situation you are always better off, as theseextra implants will redistribute chewing forces over 6-9implants, instead of just four.

Volunteers In Partnership withCommunity Memorial Hospital recentlyreceived the 2014 Wisconsin Award forVolunteer Excellence (WAVE) from thePartners of Wisconsin Hospital Association(WHA). The volunteers earned the stateaward for the community garden project inthe category of community outreachand/or collaboration.
The volunteers at Froedtert & theMedical College of Wisconsin CommunityMemorial Hospital plant, maintain and har-
vest the garden every year on the hospitalgrounds. Produce harvested from the gar-den is distributed to patients at theCommunity Outreach Health Clinic as wellas local food pantries in Menomonee Falls,Germantown and Sussex. The gardenaddresses the need for nutritious foodoptions, especially for local families whomay unable to afford them.
The award was presented at thePartners of WHA State Convention at the KICenter in Green Bay in mid-October. MikeHau, lead gardener and volunteer, accept-ed the WAVE on behalf of the volunteersand the hospital. This award is a wonder-ful recognition for our volunteer organiza-tion as well as our hospital, said Sue
Schuelke, director of volunteer services atCommunity Memorial Hospital. We arevery proud of our community garden aswell as all projects and efforts that our vol-unteers support at Community MemorialHospital.
There are four categories in which anorganization can apply for the statesWAVE recognition, they include:

The ubiquitous buzzword in todays cul-ture in stressand, unfortunately, it is notgoing anywhere. The question is how to dealwith it and keep it in check.
Stress is a fact of life, but being stressedout is not. We dont always have control overwhat happens to us, says Allen Elkin, Ph.D.,director of the Stress ManagementCounseling Center in New York City, in aReaders Digest article. Yet that doesntmean we have to react to a difficult, chal-lenging situation by becoming frazzled orfeeling overwhelmed or distraught. Beingoverly anxious is not just a mental hazard; itsa physical one too. The more stressed out weare the more vulnerable we are to colds, flu,and a host of chronic or life-threatening ill-nesses. And the less open we are to the beau-ty and pleasure of life.
Donna Bennett is a recreational therapistin the inpatient unit in Mental Health Servicesat Froedtert & the Medical College of
Wisconsin Community Memorial Hospital.She has spent 22 years in the field of mentalhealth and she has a lot to say on the subjectof stress.
Stress is growing (among people intodays society), she said in a phone inter-view with the Express News. We want to domore, there are more options on the things wecan do and we never relax anymore. We arealways multi-tasking.
Multi-tasking, however, is by its very defi-nition a flawed approach, Bennett pointedout.
Research shows that multi-tasking is notreally effective. You are not in the momentattending to the task. You are more forgetfuland it takes longer to accomplish the task,she said.
Know your triggers: As they say,knowledge is power. Be aware of whatmight set off a stress attack, the mental healthprofessional said. The big stressors are exact-ly what one would thinkfamily, work, com-munication issues in both of these areas,finances, she listed. There is also the worry
of how to get things done with both the hus-band and wife working and the kids in vari-ous activities.
Make a plan: Once you know whatwill trigger your stress, be proactive in deal-ing with it, said Bennett. Make a plan forhow to cope. Look at what your options areand have a strategy in place, she said.
Engage in physical activity: One wayto handle stress better is to literally work itout. Do something physical, Bennett recom-mended. You dont have to go running, butdo somethinggo for a walk, stretch, let someenergy out.
Quiet your mind: Two easy ways toaccomplish this are reading and meditat-ing. Meditationcalming the mind, focusingon deep, slow breaths and counting to fivefor each inhale and exhaleis very helpfulwhen done right, said Bennett. "You can doit anytime, anywhere you need to. It is notsomething we are used to doing, but it reallyhelps. When a person is stressed they tend totake shallow, what I call anxious breaths inthe upper part of the chest.
Make a spiritual connection: Peoplewith no spiritual base tend to more stressedout, according to the experts. It doesnt haveto be a specific religion or church, or even achurch at all, explained Bennett. Just hav-
ing a connection to nature can help. Cognitively engage: The mental health
professional urges individuals to use theirbrain. Take part in an activity in which youhave to focus to be successfula puzzle, aword search Sudoku, she said. Even someelectronic games work for this.
However, sometimes its necessary to... Pull the plug: Take a break from elec-
tronics, especially at night, said Bennett.They stimulate your brain and it is hard to
fall asleep which contributes to stres Have a support system: Y
your people, Bennett stated simpcant make it on this earth without yple. You need your family and frienyou dont have friends, make somevolunteering and taking recreationand things like that.
Most of the time stress can be hhouse, but sometimes the situationdire and in those cases...
Make a call: There are hotlwarmlines available in each county,said. Hotlines are for when you arand a warmline is for when you justalk about your stressors.
Too many people sweep theunder the carpet and hope it will vwill not, stated Bennett.
If you dont do anything and ywilling to make changes in your lifebehaviors, things are not going to geshe said. There are so many estress...concentration problems, issues, back and neck pain...theres relation to cancer. At a certain pbody fights back and says, NoNotice me!
It is all within our power to mchange, Bennett said.

Nowyou've picked it up to finally put it awayand you realize - you have nowhere toput it. You have too much clutter in yourhome. So what do you do? How do youdecide where to start so you can reducethe clutter in your home and make yourwalkways safe once more?
Getting started is easier than youthink. If you want to take control of clut-ter, get a few boxes - start with one for
each room in your home and beginorganizing one room at a time.
Begin this initiative by removingeverything on top of your cabinets,tables and in bookcases, and then placeit all in a box, says Keith McCleary, aca-demic director of Interior Design at TheArt Institute of York - Pennsylvania. Ifthere are other random accessories inthe room, remove those, too. Keep justthe basic furnishings. Now sit with theroom in its simplest form for a short time.
"In room design, make good deci-sions about what you choose to put inthe room and, often more importantly, inwhat you choose to leave out,"McCleary says.
