    DIABETIC FOOT DISORDERS:A CLINICAL PRACTICE GUIDELINE (2006 revision)

    ABSTRACT: The prevalence of diabetes mellitus is growing at epidemic proportions in the United States and

    worldwide. Most alarming is the steady increase in type 2 diabetes, especially among young and obese people. An

    estimated 7 of the US population has diabetes, and because of the increased longevity of this population, dia !

    betes!associated complications are e"pected to rise in prevalence.

    #oot ulcerations, infections, $harcot neuroarthropathy, and peripheral arterial disease fre%uently result in gan !

    grene and lower limb amputation. $onse%uently, foot disorders are leading causes of hospitali&ation for persons

    with diabetes and account for billion!dollar e"penditures annually in the US. Although not all foot complications

    can be prevented, dramatic reductions in fre%uency have been achieved by ta'ing a multidisciplinary approach topatient management. Using this concept, the authors present a clinical practice guideline for diabetic foot disor !

    ders based on currently available evidence, committee consensus, and current clinical practice. The pathophysiol !

    ogy and treatment of diabetic foot ulcers, infections, and the diabetic $harcot foot are reviewed. (hile these guide

    ! lines cannot and should not dictate the care of all affected patients, they provide evidence!based guidance for gen

    ! eral patterns of practice. )f these concepts are embraced and incorporated into patient management protocols, a

    ma*or reduction in diabetic limb amputations is certainly an attainable goal.

    The 0reva4en"e o6 diabetes me44it&s is gro-ing at

    e0idemi" 0ro0ortions in the Bnited #tates and -or4d-ide

    ?1@. Most a4arming is the steady in"rease in ty0e 2 diabetes,

    es0e"ia44y among yo&ng and obese 0ersons. An estimated

    C o6 Ameri"ans are a664i"ted -ith diabetes, and -ith the

    B4"eration, in6e"tion, gangrene, and am0&tation are signi6i"ant "om04i"ations o6 the disease, estimated to "ost bi44ions

    and morbidity thro&gh a00ro0riate eviden"ebased 0reven

    tion and management 0roto"o4s.

    Taking a m&4tidis"i04inary a00roa"h to diabeti" 6oot dis

    orders, many "enters 6rom aro&nd the -or4d have noted

    "onsistent im0rovement in 4imb sa4vage rates. 3ith this

    0remise as o&r "entra4 theme, the a&thors 0resent this "4ini
