    Dear Dr. Winter, I need implants in my upper and lowerarches. I have had several consultations and opinions all overthe board! Some of the consults said I only needed 4 implantsfor the upper and 4 for the lower and others have said I need

     just two for the lower. I did tell them I wanted to be able tohave teeth that didn’t come out and I don’t have a lot of bonein either arch. What are your thoughts? Thanks, Jack J.

    Dear Jack, there are several good questions here. I will tryand break them down and I encourage you and the readers toread my articles on Upgradeable Dentistry and AdvancedTreatment Planning under “publications” at:www.hamptondental associates .com where I have writtenarticles about these very questions. I also outline in variouspublications the differences between removable overdenturesand fixed implant bridges and show examples of each type of implant treatment.

    First of all, without a consultation and x-rays I cannotcomment on your specific situation. The number of implantsyou require is dependent upon the quality and quantity of your remaining bone, your budget and the type of prosthesisyou require. Whether the implant restoration should be“fixed” or non-removable by the patient or whether it isacceptable to remove the implant supported denture and takeit out at night is a personal decision and both have advantagesand disadvantages. Most people that lose their teeth want“fixed” teeth. In addition, the patient’s health, medication(s)and anatomy all play a role in deciding upon the ideal implantrestoration. It is important to note that sometimes we can startwith a removable implant denture due to budgetaryconstraints and add implants over a year or two and Upgradeto a fixed restoration or bridge when finances allow!

    Next, we can discuss the number of required implants. Inthe upper arch most people have already been wearing adenture or partial and they have already undergone bone loss.As bone is lost in the upper jaw the sinuses expand and inorder to place implants in this area there is a need to “growthe bone” by doing a sinus grafting procedure and possibly ablock grafting procedure. This sinus augmentation is aprocedure that will allow implants of normal length to beplaced in the posterior upper jaw. A way to bypass this is azygomatic or long implant that is used in an “all-on-four”procedure. This procedure will place 4 implants at angles thatare used to support a full arch of denture teeth or a “hybrid”prosthesis. This may be a predictable procedure but replacing14 teeth (24 roots) with 4 implants doesn’t provide as large amargin of safety as using 7-9 implants per arch. This is amatter of philosophy and the treating implant dentist andsurgeon will be able to explain their philosophy based upon

    their education, skill and judgment.As far as the two-implant option given to you for your

    lower arch, this is strictly to support a lower overdenture,which is removable. In order to have a fixed restoration(non-removable) you would require a minimum of 6maxillary and 5 mandibular implants in my practice. Thatbeing said if you are able to place more implants and over-engineer the situation you are always better off, as theseextra implants will redistribute chewing forces over 6-9

    B Y REP. DAN KNODL(R-GERMANTOWN)Now that the state budget is over, it is

    time to take a look at funding for the newarena in Milwaukee. I have decided tosupport the Legislature’s plan to keep the

    Bucks, as it is going to be prudent for thestate to do so, instead of letting themleave and the state being on the hook forthe outdated Bradley Center.

    The Bradley Center, while createdwith good intent, has become a financialburden. Millions of dollars in debt serv-ice and maintenance payments are set tobe due if the Bucks leave. As we are cur-rently bringing in more tax revenue thanwe are paying for debt service and main-tenance on the existing building, keepingthe Bucks here will allow for us to contin-ue to bring in a positive cash flow.

    We also expect to see revenueincreases in the near future. One exam-

    ple of cash flow that is projected to seefurther growth is the levied tax on travel-ing sports business professionals, alsoknown as the “jock tax.” Due to a newnine-year television deal that takes effectin 2016-17, there will be an increase inthe salaries of players. The increase insalaries will lead to higher tax revenuesfrom the jock tax. Take LeBron James,who is due to make $22.9 million for the

    2015-2016 season, for example. Jameswill make $280,128.05 per game nextseason. For every game he plays inMilwaukee, he will contribute$21,429.80 to the state’s general fund.Add up all of the players and team

    employees who contribute to jock taxrevenue and the state will be taking inover $6.5 million next season alone.

    Another idea that was initially pro-posed by Assembly leadership monthsago is to put a surcharge on every ticketthat is sold at the new arena. Protectingtaxpayer dollars is central to my coreprinciples and I believe that imposing auser fee on those who actually use thenew arena is good public policy. If therest of the state is expected to pay up to$80 million over 20 years, those individ-uals who use the facility should also becontributing to help cover the cost.

    Under the current plan: the state will

    be getting out of the arena business,maintenance and operations will be paidfor by the team, any cost overruns will bepaid by the Bucks, and there are clawback provisions to ensure that they wouldhave to pay back the public investment if they were to leave Milwaukee.

    Milwaukee is our only world-classcity and we need to continue to improveit as best we can. Keeping the Bucks here

    is integral to that improvement. My voteis yes on the arena and I fully expect

    Milwaukee area Democrats to step up,without larding up the proposal, and dotheir part by voting yes as well.Prosperity should be a nonpartisan issue.Let’s get this done.

    Village of Menomonee Falls staff havebegun the process of updating the village'sComprehensive Plan.

    What the plan is and how it will be updat-ed was explained during a presentation theVillage Board July 6.

    Director of Community Development MattCarran said the plan is "the long term vision,the guide for our community regarding the

    physical, social and economic developmentfor the community."Carran said the current plan was adopt-

    ed in 2008, two years before communitieswere required by state law to have such aplan in place. State law also requires the planbe updated every 10 years. Carran said thevillage is seeking input from residents regard-ing the update of the plan. He said the newplan will included updated census informa-tion.

    "This is an update, we're not re-writing theplan. We're just basically bringing in newinformation," Carran said.

    He said the village is seeking input fromresidents regarding what they want for thecommunity. That information can then be put

    into the goals and objectives of the plan.Tyler Zwagerman, community develop-

    ment planning technician, said the first phaseof the update began in January 2015 whenstaff looked at existing data for the communi-ty. Zwagerman said staff also examinedtrends that are affecting the village in 2015,such as population and household growth,and an aging population. He said that phase

    was presented to the Plan Commission inMay.The second phase, which is starting now,

    involves identifying issues and the vision forwhere the community will go based on feed-back from the public, Zwagerman said. Aspart of this step, a community survey was ini-tiated in June, with community workshops andoutreach events planned. Staff will continueto update the village on the plan and howresidents can be involved in its development.

    According to Zwagerman, the current sur-vey has been done primarily online, withadditional hard copies available. He saidthere have been 150 responses received sofar. He said that 95 percent of respondentsare from village residents. He said the survey

    will be open until August.In addition, he said there have been

    "mobile outreach" at events such as theFarmer's Market, with plans to have outreachevents such as concerts in Village Park andFalls Fest. Community workshops are alsoplanned for from 4-7 p.m. July 30 and Aug.11 at the Village Hall board room. Updatesfor the plan will be available at

    www.menomonee-falls.org/2015plan.Later during the meeting, VillageManager Mark Fitzgerald suggested havingoutreach at a the Neighborhood Watchevent that was later in t he week. Trustee SteveRaymonds suggested having outreach duringthe corn roast which precedes the first homegame for Menomonee Falls varsity football.Trustee Katie Kress suggested outreach atYoung Professional events organized by theMenomonee Falls Chamber of Commerce.

    According to Zwygerman, he said a"health advisory committee" has been creat-ed as part of the plan update, which willaddress long term goals and will incude rep-resentatives from local health officials andhealth care providers.

    Also, a Steering Committee wilrole for the goal setting part of the will include representatives from villathe village president, the school disother local officials.

    Once the plan has been drafted,to the Plan Commission and then bereview by the public for 30 dchanges will be made by staff based

    back. It will go back to the Plan Cobefore going to the Village Board consideration. The current timeline final consideration by the Village early 2016.

    During questioning by the boardsaid the plan update is being done bstaff. By comparison, the original pcreated after the village hired outssultants to work on the plan.

    "It should have very little, if any,the budget because we're not hiringants," Carran said. He added that"very proud" of the work that has beso far by village staff.

    Registration is now open for CommunityMemorial Foundation’s 18th annualWheeling for Healing family bike ride. Theannual ride will be held on Saturday, Aug. 8,and will raise funds to support the CancerCare Center at Froedtert & the MedicalCollege of Wisconsin Community MemorialHospital.

    Wheeling for Healing invites riders of allages and abilities to participate. Routesbegin and end at Community MemorialHospital in Menomonee Falls. Riders maychoose from four routes through MenomoneeFalls and surrounding communities. Theseinclude a family-friendly 4-mile route; aleisurely 14-mile route; a more challenging33-mile route; or a 62-mile route to WestBend for avid cyclists. Participants may signup to ride individually, as a family or as ateam.

    A post-ride celebration at CommunityMemorial Hospital begins at 9:45 a.m.,offering delicious food, entertainment, raffles,children’s activities and more. ChrisGloninger, WISN12 meteorologist andreporter, will serve as the Wheeling forHealing ride marshal.
