II. INCIDENTS MONITORED/REPORTED

A. Floodings

Region VI

• Sigma, Capiz affecting 13 Barangays - Agbo, Amaga, Cogon, Dayhagon, Poblacion Norte, Poblacion Sur, Guintas, Pagbunitan, Bangon-bangon, Mangoso, Capuyhan, Tawog, and Matangcong and in Panitan, Capiz affecting Brgys Cabanghan and Cabugao

Region VII

• Flooding occurred in Sibulan, Valencia, and Dumague te City all in Negros Oriental

Region IX

• Zamboanga del Norte (Polanco)- Affected families were temporarily evacuated to higher grounds for safety - flood waters had already subsided

• Barangays Turno (Sitio Wakwak) and Dicayas in Dipol og City on 15 December 2011 at around 10:00 AM.

• Sixty (60) families/300 persons from Sitio Wakwak, Brgy Turno evacuated to Turno Elementary School. Six (6) houses and one (1) Day Care Center were totally destroyed by raging waters. Five (5) other sitios were also affected but no report of displacement. Residents in Brgy. Dicayas, opted to stay with their respective houses in spite of the reported rise of flood level.

Region X

• Massive flooding occurred in the following areas due to the overflowing of Mandulog and Agus Rivers: Lanao del Norte - Iligan City (24 brgys); Bukidnon (Valencia City); Misamis Oriental - El Salvador City (Brgy. Mougan) and Cagayan de Oro City (29 Brgys; Misamis Occidental (Ozamis City (6 Brgys) and Clarin (1 Brgy)

Region XI

• Compostela Valley Province (Municipalities of Compostela (2 Brgys), Monkayo (3 Brgys), Nabunturan (2 brgys , New Bataan(1 Brgy), and Montevista (1 Brgy)

• A total of 106 families from Compostela evacuated to the Municipal Gymnasium and Central Elementary Schools and 18 families from Montevista at the Municipal Gymnasium

• Davao del Norte (17 December 2011) with 514 families affected in Brgy. Napungas, Asuncion

B. Landslides Region XI

• Compostela Valley – Moncayo (Purok 4, Purok 22 and Mt. Diwata, Diwalwa)

• Lanao del Sur in Kapai, (Brgys: Togonuan; Parao; Malimono; and Dimagalig) and Bubong (36 Barangays) and due to the overflowing of the Tagoloan River and Siguan River

C. Sea Mishaps Region VII

• One Cargo Ship “Ever Transport Cargo” anchored at Brgy. Calindagan, Dumaguete City sank due to strong winds and big waves. Said cargo ship is owned by Kherl Lines loaded with Asia Brewery products

• LCT Tampi Twin halfway submerged while moored at the Maayo Private Wharf in Sibulan, Negros Oriental due to big waves. The sea water entered through the open portion of its stern and thereby flooded the engine room

• A pump boat “Bantay Dagat” owned by LGU Lazi, Siquijor sank while docked at the Lazi Port when battered by strong winds and big waves

2. POWER/ELECTRICITY/COMMUNICATIONS

� Region X - 60% power has been restored in Cagayan de Oro as of 5:17 PM 18 December 2011

� Region VII - Some power lines of National Grid Corporation were

destroyed. About 70% of power was interrupted in the province of Negros Oriental in due course affecting water, communication, and transportation facilities. Power outages in Tanjay and Siaton, Negros Oriental

� Region X - Power interruption in Valencia City, Iligan City, and Ozamiz

City

� Region XI - Communication issues has been resolved as of 18 December 2011

� CARAGA- power interruption in Agusan del Sur ( Municipalities of

Lianga, San Agustin, Marihatag, Bayabas, San Miguel, Cagwait, Tago, Cortes, Carmen, Madrid, Lanusa, Lingig, and Tandag City

IV. EMERGENCY PREPAREDNESS AND RESPONSE

National and Regional Preparedness

NATIONAL RESPONSE

• NDRRMC OpCen disseminated NDRRMC Advisories of TD Sendong prior to its entry in the Philippine Area of Responsibility (PA)), Severe Weather Bulletins, Flood Advisories, and Gale Warnings to all regions affected through SMS and facsimile and uploaded the same at the NDRRMC Website, Facebook, and Twitter accounts for further dissemination to their respective Local Disaster Risk Reduction and Management Councils (LDRRMCs) from the provincial down to the municipal levels

• Directed RDRRMCs concerned through the OCD Regional Centers to undertake precautionary measures in their respective areas of responsibility (AOR) and subsequently advised local DRRMCs to initiate pre-emptive evacuation of population in low-lying and mountainous areas if situation warrants

• In view of the entry of TD Sendong,NDRRMC OpCen was activated effective 11:00 AM, 15 December 2011 under “Blue Alert” status to monitor the situation and provide necessary actions and on Dec 16, 2011 it was elevated to Red Alert Status wherein

respective member agencies rendered duty at NDRRMC OpCen and prepositioned their respective resources in terms of supplies, equipment and manpower

• The NDRRMC convened at around 11:00 AM of 17 December 2011, presided over by the SND and Chairperson, NDRRMC, attended by all member agencies. His Excellency President Benigno Simeon C. Aquino III provided guidance to the NDRRMC Member Agencies. – TAB E

PAGASA • Continuously providing NDRRMC OpCen with updates on General Flood

Bulletins, Gale Warnings, and Severe Weather Bulletins on Tropical Storm “SENDONG”

• PAGASA detailed personnel at the NDRRMC OpCen for prompt coordination with their bureau especially with the Weather Forecasting Section, Weather Division

AFP • 4ID dispatched Rapid Assessment Teams (RRT) and the Technical

Operations Group 10 (TOG10) to the affected areas of Region X • CENTCOM-AFP directed two (2) UH-1H helicopters with personnel to

complement the 5052nd SRS in Mactan Airbase to augment SAR operations in Sibulan, Valencia, Tanjay, and Dumaguete City

PCG • As early as 15 December 2011 all Coast Guard Districts were directed to

ensure high state of preparedness in respective Stations, Detachments and Operational Units/Disaster Response Assets

• District Commanders were directed to take more proactive measures in notifying or recalling fishermen before the onset or during severe weather conditions

• Coast Guard Districts were reminded to always make sure that their respective Stations and Detachments promptly initiate and encourage public information dissemination through the use of tri-media and LGU’s to warn the coastal residents and local fishermen

• All Districts were advised to coordinate with PCG Auxillary, LGU’s, OCD, PDCC and media in disseminating information to warn/recall fishermen and to take precautionary measures to prevent untoward incidents; reminded all shipping agencies to strictly follow HPCG Memo 01-09 (Guidelines on Movements of Motorboats/Vessels in the Absence of Weather Bulletin) and HPCG SOP No. 002-09 (Interim Guidelines on PCG Alert Condition/levels)

DILG • DILG Disaster Monitoring System is reactivated effective 15 December 2011

in keeping with established disaster protocols • DILG RD IX directed all PDs, CDs and CLGOOs to coordinate with their LCEs

for the activation of their Disaster Monitoring Systems effective immediately, and also to coordinate with the OCD

• DILG Zamboanga City requested Mayor Lobregat to immediately activate the City Disaster Monitoring System

PNP • PNP Police Community Relations issued memorandum to RDs, PROs 5, 6, 7,

8, 9, 10, 11, and 12 advising PNP units/offices/stations under their AOR to alert their respective SAR teams and water assets for possible deployment;

assisted in the dissemination of weather bulletin in the community to ensure maximum police visibility in affected areas; assisted LDRRMC and LGUs in providing security at the evacuation centers; continued to monitor weather updates from PAGASA and NDRRMC websites; and advised all residents in low-lying and mountainous areas under PSWS to take precautionary measures against possible flashfloods and landslide, and those living in coastal areas to take preventive measure against big waves or storm surges generated by the tropical storm

• The PNP National Disaster Operations Center had been activated effective 1200H, 17 December 2011. PCRG Personnel were likewise sent to the NDRRMC Operations Center for monitoring and liasoning duties

DOH • HEMS OpCen was on Code Blue Alert since 18 December 2011; sent Staff to

NDRRMC; coordinated with PAGASA regarding the details about TS “Sendong”; coordinated with RHEMS Coordinators in all the affected regions; sent Memo to all HEMS regional Offices; and continuously monitor the situation

• CHD X sent Rapid Health Assessment Team for the flooded areas; pre-positioned drugs and medicines worth PhP50,000.00 in six (6) provinces; deployed responders; and coordinated with PDRRMC, PRC, Rescue 118, and other responders

• CHD CARAGA pre-positioned medicines and supplies; issued alert messages to local HEMS in respective provinces and cities for appropriate response and coordination; and attended meeting with PDRRMC Surigao del Norte on 16 December 2011

DSWD • Pre-positioned PhP10.17 Million Standby Funds and PhP57.69 Million worth

of Relief Supplies in all DSWD-FOs along the typhoon path, amounting to PhP67.86 Million:

• DSWD NROC airlifted the following relief goods to DSWD FO X the followin: 300 sacks of assorted clothing, 5,000 packs of clothing, 1, 000 boxes corned beef , , 10,000 blankets, 10,000 plastic mats , 500 boxes noodles and 1,500 bottles of bottled water (500 ml) worth PhP4,352,6 10.00

• UN-WFP donated 312 boxes High Energy biscuits; Mayn ilad (1 unit water purifier, 247 pcs water jug and 62 cases bot tled water); and PNRC (2 units water bladder) through the DSWD NROC worth PhP12,724,165.44. Said donation was transported t o DSWD FO X

• DSWD CO prepositioned an additional PhP500,000.00 standby funds to DSWD FO-VII as augmentation assistance to the flo od victims of Ts Sendong and another P28,070,400.00 was also prepao sitoned to DSWD FO X intended for Cash for work Program for the af fected municipalities of Misamis Oriental and lanao del norte.

• A total of PhP1,863,252.00 worth of relief assistance were provided by the combined resources of DSWD and Local Government Units (LGUs): (TAB F)

REGIONAL RESPONSE

RDRRMC/OCD Regional Operations Center were activated to ensure timely dissemination of Severe Weather Bulletins, Gale Warning and Flood Advisories to RDRRMC Member-Agencies, P/C/MDRRMCs, tri-media and other stakeholders; alerted and coordinated with local DRRMCs regarding situation in their area of responsibilities and preparedness activities to be undertaken including pre-emptive evacuation whenever necessary; alerted Rescue Groups; AFP, PNP, BFP, PCG, Local Responders, PRC and Private Responders to be on standby for possible assistance/evacuation activities; and alerted the residents in low-lying and

mountainous areas against possible flashfloods and landslides and those living in the coastal areas to be alert against big waves or storm surges to be generated by the tropical storm

Region I

• DPWH Region I expedited clearing operations in the areas affected by landslide utilizing heavy equipment and private contractors

Region VI

• Professional Regulation Commission Region 6 is closely monitoring the situation in anticipation to the scheduled PRC Board for Nursing in Iloilo City on 18 December 2011

Region VII • PCG-DCEV issued hold orders to vessel plying areas under public storm warning

signals specifically Bohol, Negros, and Siquijor routes • PDRRMCs Cebu, Bohol, Negros Oriental and Siquior were convened on 14

December 2011 to review pro-active and operational measures of council members; assessment and coordination with the OCD Regional OpCen is being done on a regular basis; and monitoring of hazard-prone areas

• DRRMC Cebu City sent teams to affected areas for road clearing operations • MDRRMC Amlan and Barangay-based Rescue Teams conducted road-clearing

operations and pre-emptive evacuations in identified hazard areas • BFP Dumaguete, 79 th IB and 302 nd Bde, PA based in Negros Oriental

mobilized to assist the affected populace in areas affected by flooding • CDRRMC Dumaguete City and PDRRMC Negros Oriental simultaneously held

Emergency Meetings on 17 December 2011 to assess extent of damage and needs requirement of affected families

• 302nd Bde, PA provided search and rescue support to LGUs affected • Provincial Public Safety Company (PPSC) Negros Oriental deployed one (1)

team to the affected areas of Sibulan for SAR operations • DSWD VII in close coordination with the OCD and the AFP are now facilitating the

shipment of relief goods and the possible withdrawal of 100 bags of rice from NFA Dumaguete City; and has a standby fund worth PHP445,000.00 and PHP 4,000,000.00 worth of relief goods

• DOH VII dispatched a medical team from the HEMS to augment the emergency medical services in the affected areas

• RDRRMC VII convened an emergency meeting yesterday, 18 December 2011 at 5:15PM, with DILG RD, DPWH VII, CENTCOM-AFP, PRO VII,PCGDCEV, DOH VII, and OCD VII to assess the extent of damages in Negros Oriental

• Joint SAR Team of CGS Dumaguete City together with Negros Oriental SAR team, BFP Dumaguete, PNP Dumaguete, PNP Marig, 211th & 204 PCGA Squadron conducted SAR operations for M/V Ever Transport

• Rescued crew of M/V Ever Transport was temporarily turned over to the LGU of Brgy Calindagan, Dumaguete City, where they were fed and given medical assistance and were billeted at the barangay hall

Region IX

• PDRRMC ZDN and CDRRMC Dipolog City conducted emergency meeting to assess the condition, damages and needs of reported families/persons

• CDRRMC Dipolog City is in constant coordination with MDCC Polanco Action Officer for any development of Polanco River; provided rubber boat to augment rescue equipment; and conducted assessment and profiling of affected families in

collaboration with CSWD; and provided 1 sack of rice, noodles, and sardines to support relief operations in BDCC Turno

• BDRRMC Turno headed by PB Meynardo Baes with the s upport of rescue teams evacuated the affected families and brought them to Turno Elementary School for temporary shelter; established community kitchen; and provided initial food assistance to the evacuees

• 101st Bde Rescue Team headed by Capt. Art Villanueva tog ether with Special Forces assigned in the area responded in Municipality of Polanco and Dipolog City and assisted in transporting passengers trapped by floods along the National Highway at Brgy Guinles, Polanco

• Polanco MPS led by PI Carmelo Cruzada Velasco, one (1) team from PPSC, Zamboanga del Norte PPO together with troops of 101st IB PA, LGU Polanco, and PRC ZDN chapter conducted SAR operations at severely affected areas

• Dapitan City Police Station led by PSupt Glenn Macaio Dulawan together with one (1) team from PPSC, AFP Condor 3, and CDRRMC Dapitan conducted SAR operations in affected areas; provided assistance to the vehicles passing the National Highway near Brgys Sulangon, Oyan, Burgos, and Ilaya; assisted in the evacuation of all the families near the Dapitan river to a safer place

• ORACIS Radio Communications Group headed by Dr. Asis provided radio communication link, rescue ropes, transport vehicles and additional rescuers in the Municipality of Polanco and Dipolog City

• Dakak Beach Rescue Group headed by Roel Hamoy assisted in the conduct of rescue operations using two (2) jet skis

• Coast Guard Station in Dipolog City augmented the rescue operations utilizing 1 speedboat and 2 rubber boats

• Local Government of Polongco distributed 30 bags of noodles, sardines, mineral water, rice and canned goods to the affected families

• LGU Polanco distributed 30 bags of noodles, sardines, mineral water, rice, and canned goods amounting to PHP 7,500.00

• CSWD Dipolog provided Food and other services for the affected families in the Dipolog City flashfloods

Region X

• At the height of flooding, 10 evacuation centers were being utilized in Cagayan de Oro City, Misamis Oriental

• PCG Iligan deployed one (1) Search and Rescue (SAR) to Brgy Hinaplanon, Iligan City and rescued three (3) unidentified persons with bruises

• CHD X raised to Code White Alert and conducted rapid health assessment in the affected areas; prepositioned medicine worth PHP 50,000.00 per province (all 6 provinces in region X) and coordinated with the PDRRMC, Philippine National Red Cross, Rescue 118, and other responders

• CDRRMC Iligan established four (4) evacuation centers at the height of flooding, namely: City Central School; Sta. Filomena Gym; Santiago Covered Court; and the Upper Hinaplanon Barangay Hall & covered court for the affected families in Iligan City

• TOG-PAF mobilized three (3) choppers: 2 from Davao and 1 from Butuan • RDRRMC/OCD X Operations Center conducted ocular inspection in affected

areas; assessment of immediate needs; leading the mobilization of resources, assets, and responders; and continuing survey and monitoring of affected areas

• Members of the 4ID-PA, TOG-PAF, PNP, and OROALERT were deployed to different areas to rescue stranded commuters and persons in areas reachable by the rescue teams

• CDO CDRRMC established evacuation centers in the City Central School, West Central School, and Macasandig Covered Court. Survivors were given basic needs

• The 10 th Tactical Operations Group conducted search and rescue operations and aerial survey of the affected areas

• PNP Maritime Office X dispatched three teams with three rubber boats to various areas in Cagayan de Oro City

• Regional Director PCSupt Juel Corpus Adriatico of PRO X together with the Regional Chief Directorial Staff, members of Quad Staff, and PRO 10 Ladies headed by Mrs Joji Adriatico went to Iligan City to oversee the continuing SAR operations being conducted by the combined elements of ICPO, RPSB10, Lanao del Norte PPO, and RTS10 led by City Director PSSupt Celso Regencia; distributed food, water, and other relief goods worth PHP120,000 to the 300 affected families staying at the MSU-Iligan Institute of Technology; and distributed relief goods worth PHP 100,000.00 to 350 affected families in Brgy Balulang, Cagayan de Oro City

• PRO X deployed 10 SAR teams to conduct SAR operations • 1002nd Maritime Police Station (MARPSTA) Iligan City led by PINSP Renato G.

Rama together with elements from the PCG-Iligan conducted joint SAR operations at the Orchids Village, Santiago, Iligan City

• DSWD X provided initial augmentation to the LGU of CDO: 40 bags of rice, 40 cases of sardines, 20 big kettles, 10 big kawa, 10 pieces of sandok, an 241 family packs, all worth PHP 170,400.00

Region XI

• PDRRMC Comval and MDRRMCs Compostela, Monkayo, and Montevista extended relief assistance to the affected families and assisted in the evacuation operations

• PDRRMC Davao del Norte conducted damage and needs assessment in the area and closely monitored the level of flood waters in Asuncion

• Monkayo MDRRMC conducted search and rescue operations

CARAGA • PDRRMC Surigao del Norte convened at 8:30 AM while CDRRMC of Surigao

City convened at 9:00 AM on 16 December 2011 • PDRRMCs and MDRRMCs conducted damage assessment and needs analysis

in the affected areas • PCG issued a notice to mariners to denied all vessel departure including motor

bancas, fishing boats, small boats; information was received from PCG Nasipit, Agusan del Norte Station that there are 200 passengers stranded at Nasipit Port bound to Jagna, Bohol; Port Authority coordinated with MDRRMC of Nasipit to support the stranded passengers

• 36th IB of PA stationed at Tago, Surigao del Sur, alerted their troops and sent out teams to monitor Tago River and other flood prone areas in their AOR

• Received information from CAAP Butuan, Bancasi Airport, Cebu Pacific flight from Manila to Butuan was cancelled, but PAL flight from Manila to Butuan had landed and departed Butuan back to Manila as scheduled

• DPWH Regional Office activated their Regional and District Disaster and Risk Management Teams and deployed along flood and landslide prone areas

• PNP Hinatuan provided assistance in the evacuation of the victims and are providing security in the evacuation centers

• Surigao de Norte Governor Matugas cancelled the scheduled Barangay Development Forum in Surigao City

• Acting ARMM Governor Adiong gave relief goods to the affected families in Lanao del Sur

• Elements of the 64th Infantry Batallion are helping the affected LGUs of Lanao del Sur in their SAR operations

1. The number of casualties is alarming. If USEC Ramos had a 3-day lead time to disseminate warning advisories to Hinatuan, Surigao del Sur re-landfall of TS “Sendong”, there is a need to advance more than 3-day warning to mitigate the impact of incoming typhoons or natural calamities. DILG in tandem with DOST should look into the mitigation aspect of areas with high exposure to disaster risks, and employ prioritization as to their potential impact for disasters.

2. Consideration should include long-term mitigatio n measures to address siltation of rivers, mining and deforestation.

3. These high risk areas should be identified for h uman settlements and development be relocated into safe habitation .

4. Assets to include military response capabilities can be transferred before the 3-day warning whenever a typhoon will affect co mmunities at risks. A review of the protocols to include maintenance and transporting costs of these air, land, and maritime assets should be unde rtaken.

5. Schedule a Presidential inspection and visit to affected communities in Cagayan de Oro City and Iligan City on Tuesday, 20 December 2011

6. Mandatory meeting or formal conference will be conducted by the Council and the Office of the President with LGUs for threatened areas due to incoming typhoons and avoid water-related disasters. This is to prevent the people react and will include calibration of warning. For DILG to look into the identification or designation of focal point/person among LGUs likely to be affected by natural calamities as OPR decision-maker.

7. There is a need to come up with a Crisis Manual for Natural Disasters. The thirteen (13) barangays reported in Cagayan de Oro City to be situated along the two (2) swollen river systems which river beds already silted are like “disaster waiting to happen”, if the 93 bodies recovered by the AFP are officially confirmed already as casualties.

8. DPWH to render an Assessment Report on Monday, 26 December 2011 on the on-going reconstruction of the main roads in Ilocos Norte particularly Pagudpud which was affected by landslides due to monsoon rains. This will also include the diversion roads, if any.
