Mission StatementThe mission of the Kern Center is to provide high qualityphysical recreation opportunities through the medium of sport;to provide programs and services that promote the three areasof wellness—mind, body and spirit; and to create anatmosphere that encourages individuals to develop lifelongpatterns of positive recreational activity regardless of skill level.The Kern Center’s programs and facilities are available to allstudents, faculty and staff of MSOE without regard to age, sex,race or disability.

Having been constructed to meetthe recreational, athletic andwellness needs of the MSOEcommunity, the Kern Centerstrives to promote a healthy mind,body and spirit. We at the KernCenter invite all to pursue theirown level of fitness in this state-of-the-art facility. Recognized as oneof the top facilities in the nation, the Kern Center hasbeen able to positively enhance the campus atmosphereat MSOE. Through usage by students, alumni andcommunity and national organizations, the KernCenter looks to become a focal point for not onlyMSOE, but the city of Milwaukee. Whether you skate,jog, shoot baskets or lift weights, the Kern Center has aplace for you.

Opportunities for ParticipationIntercollegiate Athletics offers an environment tocontinue striving for competitive excellence in sport.Teams compete in the Northern Athletic Conference,Midwest Intercollegiate Volleyball Association or theMidwest Collegiate Hockey Association with a goal ofattaining an NCAA Division III NationalChampionship. For more information go to:www.msoe.edu/athletics/ Club Sports provide an in-depth sport experience thatmay include instruction, practice and competition withother colleges and universities. Intramural Sports and special events provide a varietyof competitive sport opportunities within the MSOEcommunity.Open Recreation programs allow individuals toparticipate in unstructured physical activity such asstrength training, cardiovascular training, basketball orjogging to enhance their quality of life and fitness level.

Memberships All MSOE enrolled students and faculty/staff areconsidered Kern Center members. Memberships areavailable to both spouses and dependents of students orfaculty/staff. Kern Center memberships also areavailable to MSOE alumni, individuals in thecommunity and corporations (see page nine).Individual memberships are only available to those 18years and older. Those ages 6 years or older are requiredto have a membership ID to enter the Kern Center.

Facility Usage Guidelines

Assumption of RiskPlease be aware that engaging in physical activitiesinvolves a higher degree of risk than normal activities.Individuals participating in recreational activities andprograms do so at their own risk and are encouraged to be examined by a physician prior to participating,regardless of age. MSOE and the Kern Center can not be responsible for injuries sustained while utilizing the facility.

General Policies and ProceduresUsers are expected to respect the Kern Center andrights of other users. Any user who engages indisorderly conduct, including verbal or physical abuseof an employee or other user is subject to immediateremoval from the Kern Center and possible loss ofmembership privileges.• Use of the Kern Center is a privilege not a right.• It is the responsibility of the Kern Center user to beaware of building policies.• The Kern Center is a tobacco-free facility.Smoking and/or chewing is not permitted. Thisincludes the lobby, parking garage and entry areas.• Alcohol, drugs and banned substances are notallowed in the facility.• Kern Center is not responsible for lost or stolen items. • Patrons are encouraged to leave all valuables at home.• Dunking/hanging on the basketball rims is not allowed.• Proper athletic footwear is required. Only non-marking athletic shoes are permitted on activity surfaces.• No spitting anywhere in the building except in trash receptacles.• The use of video or photo equipment in the facility isstrictly prohibited unless Kern Center administrationhas granted prior approval. Permission must beobtained from individuals being video taped in theseapproved situations.• The use of “camera/video” phones or any videorecording devices in locker rooms or activity areas isstrictly prohibited.• Posted rules – participants must observe all postedpolicies/regulations and verbal instruction from KernCenter staff.

• Failure to comply with instructions or with policiesmay result in suspension of Kern Center privileges.• Food/drink are permitted in designated areas only.• Glass containers of any kind are prohibited at all times within the facility. • Food and chewing gum are not allowed in the FitnessCenter or on the track.• Objects or equipment with sharp edges of any kindare not allowed in the wrestling room.• No martial arts weapons will be permitted without authorization from the Kern Centeradministrative staff.• Coaching, personal instruction or organized activitiesare prohibited other than those approved by the KernCenter. Scheduling and reservations for Kern Centerfacilities must be made through the director at (414) 277-2672.• Rollerblades, roller skates, skateboards, roller shoesand bicycles are not permitted in the facility.• Posters/advertising must be approved by the KernCenter or the Student Life Office prior to itsplacement on facility bulletin boards or counters. • Injuries which occur during recreational activitiesmust be reported immediately to the buildingsupervisor at the control desk, and an accident injuryreport must be completed.• Members and guests assume all responsibility ofpersonal injury. Should Kern Center staff deem itnecessary for an injured person to receive medicalassistance, appropriate medical personnel will becontacted at the injured party’s expense.• Helium balloons are not allowed in the facility.• Kern Center reserves the right to alter any building oractivity policy.

Entry/Exit of BuildingThe Kern Center exists to serve its members. To helpensure that we can offer the atmosphere and privilegesexpected, members will be asked to show their KernCenter membership ID (MSOE student, MSOEemployee or membership picture ID card) each timethey enter the center. If there is a question as to thevalidity of an ID, you may be asked to show a secondpiece of photo ID. Once your membership status isconfirmed, you will be admitted. ID cards are nottransferable and are for the exclusive use of theindividual named on the card. There is a $10replacement ID card fee. ID cards are renewable; donot discard them. Entry/exit of the facility must alwaysbe through the designated main entrance and exit.

Violators are subject to disciplinary action. All patronsmust exit the building upon closing time.

Guest PolicyRegistering a guest can be done either in person orover the phone at (414) 277-2850. The guest fee is$5 per guest.The ability to bring guests is a privilege, not a right.• Guest registration must occur on the desired day.• When registering a guest in advance, the sponsor willneed to provide the following: date of visit, sponsor’sname, sponsor’s student or membership ID numberand guest’s name. • The sponsor must have a current, valid ID card. Theguest will also be required to show some form ofcurrent photo ID. The guest must leave their IDat the Control desk; when they leave the facilitythe ID will be returned. Guests 16 or older musthave photo ID to gain access into the KernCenter.• The sponsor must accompany the guest to the facility,check the guest in and stay with the guest duringhis/her visit. Guests that require disciplinary actionmay result in the sponsor’s membership beingsuspended or revoked (no refunds) or the loss ofguest privileges.• Guests may not “page” or search for a sponsoralready in the building.• Solicitation of guest passes will not be permitted.Known solicitors will be denied entry.• Guest passes must be used at the time of purchase. No refunds.• A guest may not check out equipment. The sponsormay check out equipment to be used by the guest, butthe sponsor is responsible for the equipment.• Each member is allowed to bring up to three guestsper day for visits. Requests for more guests must beapproved by the Kern Center director or assistantdirector. • Children under age 16 must be supervised by an adultat all times.• Guests are not allowed to bring in children orspectators unless they are registered as guests.• Registered MSOE alumni (without membership)may sponsor themselves for a $5 per day visit.• During heavy facility usage, the guest pass processmay be suspended.

Activity Information, Policies and Procedures

Locker RoomsLockers are available on a day-use basis only and are included as part of your membership. Members andguests may supply their own lock, towel and toiletries. Daily lock rental is available for $1 and daily towelrental is available for $1. Lock and towel rental is freefor MSOE students with proper ID.Note that extremely soiled towels will be refused forexchange. Personal locks may not be left on lockersovernight. Personal locks will be cut off at the end ofeach day and contents will be bagged. There is a $5“lock cut-off ” fee to have personal items returned inthese situations. Personal items will only be held for 30days. After 30 days, these items will be consideredabandoned and shall be donated to a local charity. Children over the age of four are not permitted in theopposite-sex locker room.

Please keep the Kern Center clean. Pick up anddispose of trash and litter. Glass containers arestrictly prohibited in all areas of the Kern Center. Warning: Wet surfaces in the locker rooms may beslippery. Please watch your step.

Recreational Equipment Issue A limited supply of recreational equipment is availablefor members to check out. All equipment is on a first-come first-serve basis. Members can check outbasketballs, volleyballs and other equipment at theEquipment Issue Desk on level two at no charge.Members are responsible for all equipment checked outto them and will be charged replacement costs for alllost, stolen or damaged equipment. Members will needto exchange their membership ID for check-outequipment. Check-out equipment is to stay in the KernCenter at all times.

Fitness Center Cardiovascular Ellipseand Strength Training AreaThe Kern Center is proud to offer more than 100pieces of strength training and cardiovascularequipment from Life Fitness and Hammer Strength.Located just off the entry lobby of level one, the

Fitness Center offers a Strength Training Area that will keep both the first-time exerciser or the work-out fanatic content and motivated. The upperlevel of the Fitness Center houses the CardiovascularEllipse featuring 38 cardio machines with heart rateread outs and automated programming. Treadmills to bikes to cross-trainers, all the newest equipment to keep your heart pumping, are ready for action.Instruction is available in the proper use and functionof both strength training and cardiovascularequipment. Contact the Fitness Center staff or call(414) 277- 2672 for more information.

Fitness Center Policies• Do not drop weights or weight stacks or prop weightplates up against facility walls, pillars or equipment.• Closed-toe and closed-heel shoes are required in thecardio/weight areas (NO sandals, flip-flops or aqua socks).• Shirts must be worn while working out in thecardio/weight areas. Shirts must cover front (top ofchest to waist) and full back.

Important: Since the Kern Center cannot beresponsible for lost or stolen items, always make sureyour locker is well-secured. It is recommended thatyou leave all valuables at home.

• Return all weights and equipment to appropriatestorage after use. Break down all weight bars after use.• Those under the age of 14 are not allowed in thecardio/weight areas, unless supervised by an adult. • Collars are recommended on weight bars for your safety.• Limit use of cardio equipment to 30 minutes (sign-up sheet during peak hours) per workout.• Patrons are expected to dry off the weight benchesand cardio equipment after their use (spray bottle andtowel are provided).• Spotters are recommended for any overhead lift orsquat movement using free weights.• Do not re-arrange cardio or weight room layout.Recommendations for Fitness Center Use• Warm-up and stretch prior to using strength trainingor cardiovascular equipment.• Use proper breathing technique during strength training.• Use a spotter. Lift only the amount of weight you cansafely manage.• Do not continue to work out if you experiencedizziness, nausea, chest pains or other similarsymptoms.

Track• Inside lane of track is for walking. • Users must abide by daily track direction

Monday/Tuesday/Friday/Saturday counter-clockwiseWednesday/Thursday/Sunday clockwise

• 10 laps on the inside lane is equal to one mile. • Track is 160 meters in length.

Ice ArenaThe Kern Center houses an NHL-sized hockey rinkutilized both for athletic contests and recreationalactivities. Programming includes: intramurals, openskate, special event skating, drop-in hockey and rink icefor figure skaters and their coaches. Services includeboth skate rental and skate sharpening.Public Open Skating (skate rental fee extra):• MSOE students with verifying ID may skate for free during Open Skate (skate rental fees are still applicable).• $5 adults• $3 children 12 or younger and seniors (age 62 and older)MSOE Open Skating:• MSOE students with verifying ID may skate for free during Open Skating. Most sessions are heldon Sunday nights and after MSOE hockey games.Skate Sharpening: $5 per pair (hockey skates orfigure/goalie skates).Ice Arena Policies and ProceduresAll Kern Center Facility Usage Guidelines apply plus:• All rental groups wishing to use a locker room mustsign out a key at the Skate Rental Desk. MembershipID and car keys are required for a security deposit.• Stretching and light calisthenics will be restricted tothe east side of lower level 2. Absolutely no running isallowed through hallways or bleachers during rentalor ice arena activities.• All patrons are expected to help provide a safe andclean environment by disposing of all trash in properreceptacles. Key areas include, but are not limited to: locker rooms, players’ benches, scorer’s box andarena seating.• Ice use is by permission/schedule and/or contractedtime only. Each new rental or event as determined byIce Arena staff, is required to have the ice resurfaced.

• Noon-time Hockey League/Drop-In Hockey/Stick and Puck participants will be required to wear protective equipment, specifically a HECCapproved helmet.• All Ice Arena group users must read, sign and arebound to contractual agreement.• There are no designated areas for ball playing, streethockey or any stick and puck activities on lower level1 or 2.• Parents are responsible for their children’s behavior onand off the ice. Children not involved in ice activitiesare to remain in the supervision of a parent.• USA Hockey’s Zero Tolerance Policies will be strictly enforced.• The American Hockey Coaches Association “Code of Conduct” ideals will be required by hockey participants.

ConcessionsStandard concession fare is available during homeathletic events in the Arena or Ice Arena concession stands.Facility RentalThe Kern Center facilities will be available for limitedrental opportunities. Whether it be a team practice,company “sports day,” or a school field trip, the Kern Center can handle your event. Please call (414) 277-2672 for additional rental information.

Student EmploymentEmployment opportunities are available at the KernCenter, but are limited to MSOE students. Positionsare available in the following areas: control desk, fitnesscenter, building supervision, ice arena, equipment issue,skate rental and special events. CPR/AED certificationis required for some positions (training provided byMSOE). Open to both Federal Work Study andtraditional student workers. Please call (414) 277-2301for more information.

CounselingStudents face numerous common concerns includingstudy habits, class attendance, social conflicts, self-certainty and career decision-making. Personalproblems, whether they have their origin on or offcampus, often deter students from their formaleducational objectives.Counseling Services provides a setting in whichstudents may discuss privately with a counselor anyproblems that affect these educational goals. Thecounselor can help students focus on realistic solutionsto these and other potential problem areas so studentswill experience personal and academic satisfaction andsuccess. Counseling Services also invites you toparticipate in the many health, development andwellness programs offered for the personal andprofessional growth of all members of the MSOEcampus community.For more information or to make an appointment, stopby the Counseling Services Office in the Kern Center,or call (414) 277-7590.

Health ServicesHealth Services provides basic services such as first aid,routine screenings, treatment of minor illnesses orinjuries, and physician referral. Limited supplies of over-the-counter medications will be provided to the patientas appropriate. Students who require medical, dental oroptical treatment can contact the Health Services Officefor information regarding professional practitioners andhealth care facilities located near the university.Students who wish to continue the ongoing medicaltreatment they are receiving at home or requirespecialized care should contact the Health Services stafffor more information and referral to the appropriate

local specialist, physician or hospital. Charges for such care rendered on and off campus will be assumedby the patient.In the event of sudden illness or the need for emergencycare, the patient may contact Health Services at (414) 277-7590 or Public Safety at (414) 277-7159 for evaluation and assistance. Public Safety officers areavailable to assist in the transport of any patient in theevent of an emergency.

Office of Servant-LeadershipWith a major gift from the Susan & Richard PieperFamily Foundation, MSOE has established The PieperFamily Endowed Chair for Servant-Leadership at the university.The concept and principles of servant-leadership wereset forth in a 1970 publication, The Servant as Leader,by Robert K. Greenleaf, a former AT&T executive whofounded in 1964 what is now the Robert K. GreenleafCenter in Indianapolis. Club Sports ProgramThe club sports program at MSOE providesopportunities for students, faculty and staff who desire amore in-depth experience of sports participation than isprovided in intramural or open recreation programs.Through club sports participation, student leadershipskills are developed in the management of both teamsand club organization. ObjectivesThe objective of the intramural sports program is tooffer a variety of physical activities, promote a high level of physical fitness, enrich social competence,transform spectators into active participants and develophealthy relationships.

How to Get Involved in the Intramural Sports ProgramStudents living in the residence halls and members offraternities or sororities may want to enter an intramuralprogram with others in their organization. Contact the residence hall director, the intramural coordinator or enter a team yourself. Students residing off-campus or any other member of the MSOE community mayenter intramural activities by making direct contact with the coordinator.

Intramural Advisory CouncilThe Intramural Advisory Council is a student board.The members of this board are students who have beeninvolved in the intramural sports program via

employment or participation. Preferably, candidateswould be of sophomore standing or higher. Themembers do not receive compensation. The board meetsthroughout the year when necessary. The duties of theboard include serving with the coordinator to hearstudent appeals, determining eligibility, offeringrecommendations, and helping with program feedbackand input for intramural sports development.

EntriesInformation concerning all intramural sports activities isavailable in the Athletic Department between 10 a.m.and 4 p.m., Monday through Friday. Entry proceduresare as follows:A. TEAM SPORTS - All official entries will beaccepted only at the Athletic Department asdesignated by sport. Official rosters will be handedout at the captains’ meeting. A $25 forfeit fee willbe charged to each team, and will be returned tothem after the end of the league, provided the teamis in good standing.

B. ROSTER ADDITIONS - In order to compete, allpersons must have their names and ID numbers onthe official team roster in the Athletic Department24 hours prior to the contest. A team may addplayers to its roster for two weeks after the first dayof league play. Student ID numbers MUST beincluded on the roster for each participant to belegal. NO EXCEPTIONS! Playing someone that isnot listed on the team roster will result in a forfeit.

EligibilityResponsibility for EligibilityThe intramural sports program assumes the responsibilityfor the eligibility of students participating ONLY whencalled to its attention or when obvious infractions arenoticed by the intramural staff or officials. Organization captains will be responsible for checkingthe eligibility of their own players and they should alsocheck the eligibility of their opponents. Each participantis responsible for his/her eligibility. If a team captain hasa question concerning the eligibility status of a player onhis/her team, the Athletic Department is available toassist in determining this status.Since participation in the intramural sports program istotally voluntary, neither the university nor intramuralsports will accept responsibility for injuries sustainedwhile participating in scheduled intramural contests orin the general recreation program. The field or courtsupervisor should be notified in the case of an injury andan accident report should be filled out.

Who is EligibleEligibility rules exist for the protection of each team

and player in order to fulfill the general and specificobjectives of the various intramural sports activities.A. Except as provided by these rules, all undergraduateand graduate students currently enrolled in theuniversity, and faculty/staff, shall be eligible toparticipate in intramural sports.

B. A player shall not be permitted to play with morethan one intramural team in the same sport, or totransfer from one intramural team to another afterhis/her name appears on the scorecard of ascheduled game. The exception to this rule involvesa team forfeiting its first game or a team droppedfrom the league without having played a game. Inthese instances, players may transfer to anotherteam as long as it is done prior to the rosteraddition deadline. Players who play under anassumed name, or whose names are not on theofficial team roster, are ineligible to participate anda forfeit will be assessed.

C. Regarding a player participating on more than oneteam, the following guidelines will designate theplayer’s ineligibility:1. At the time a player is suspected of playing onmore than one team, the on-duty supervisor willask for identification from the player.

2. The supervisor will cross-check the involvedrosters and determine if the player’s nameappears on both team rosters.

3. If the player’s name appears on both rosters andhe/she has participated in previous games forboth teams, the player will be informed at thetime about a possible eligibility problem.

4. If a player has participated for two teams, theteam the player first participated on according tothe scorecard and scheduled game will retain theplaying rights of that particular player. Thesecond team the player participated on will bepenalized for using an ineligible player. Theplayer will be suspended from further play onboth teams for the remainder of the season.

5. After the supervisor has checked the rosters of both teams, an incident report will be filed. If a game has been delayed due to the suspectedineligibility of one player, both captains will be informed of the supervisor’s findings. Theteam involved will be warned that the gamecould result in a forfeiture due to the use of an illegal player. The final decision will be made the following working day by theintramural coordinator.

D. Varsity or professional players are not eligible toplay that particular sport. Players who haveparticipated in semi-professional leagues also areconsidered professional. This rule is designed withthe intention of preventing intramural sportsdomination by players who have receivedexperienced coaching and intensive practice. It ismade in the best interest of all intramuralparticipants.

E. A former varsity player may participate inIntramurals one year after having appeared on avarsity roster. No more than two former varsityplayers may be on the same roster.

Consequences of IneligibilityA. A team using an ineligible player will forfeit allgames in which the ineligible player participated,and the ineligible player will be banned fromfurther competition in that sport for the remainderof the sport season. During tournament play, ateam using an ineligible player must forfeit thatgame and will be excluded from furthertournament play.

B. Any player using an assumed name shall be banned from that sport and related sports for aperiod of one year or more from the date theoffense was committed.

MisconductA. Any player verbally abusing or attempting to strikean opponent, teammate, official, supervisor orspectator shall automatically be suspended for aminimum of two games and possibly longer (at thediscretion of the intramural coordinator andathletic director).

B. Any player striking an opponent, teammate orspectator shall automatically be suspended for aminimum of three games and possibly longer (atthe discretion of the intramural coordinator andathletic director).

C. Any player striking or attempting to strike anofficial or supervisor shall automatically besuspended from intramurals for one year.

D. If ejected from a game for misconduct, the playermust sit out the remainder of that contest and theteam’s next scheduled contest at the discretion ofthe intramural coordinator. It is the responsibilityof the ejected player to discuss his/her status withthe intramural coordinator on the next workingday following the contest.

E. All individuals are responsible for their own actions.A team’s action is the responsibility of the teamcaptain. Failure to comply with the rules of the sportas well as eligibility and protest rules may result infuture loss of intramural privileges.F. Repeated unsportsmanlike conduct, disregard for intramural rules or excessively rough play will result in a forfeiture of the contest. Theintramural coordinator is responsible for making this decision and may do so without the official’s recommendation.

Equipment AbuseAny person who willfully causes destruction of, ordamage to, equipment shall assume responsibility for the damage and will be assessed the full cost of repairs or replacement. He or she may be suspended from allintramural participation for an indefinite period of time.

ProtestsAll protests must be made in writing to the intramuralcoordinator no later than noon the next working dayfollowing the contest in question. A $25 must beincluded. When a protest situation arises during acontest, it is the responsibility of the team captain tonotify the officials/supervisor IMMEDIATELY that thecontest is under protest. The supervisor will make a fullreport of the contest under protest. No further contestsinvolving the teams under protest may be played until adecision is made. The coordinator will review thesituation. A review of the protest situation will include:A. Reading the written incident/protest report fromthe supervisor.

B. Reading the written protest from the team captain.C. Calling the supervisor and/or officials intoconference to discuss the incident.

D. Making a decision based on the above information.If the decision is to uphold the protest, the captainsfrom both teams will be notified and given the basis ofthe decision (and the $25 will be returned). If theprotest is denied, the team captain requesting the protestwill be notified and told the reason for the denial (andthe $25 will not be returned). The intramuralcoordinator will then give the option of appealing the decision to the athletic director. The athletic director will:A. Read the protest.B. Review the coordinator’s decision.C. Discuss the incident.D. Inform the team captain of the decision.

Alcohol or Drug Abuse Alcoholic beverages and drugs are not permitted at any intramural event, including activities held at off-campus playing areas. Contests will not be played andwill be forfeited if alcohol or drugs are present. Thispolicy includes spectators who are present at the contest.Participants who appear intoxicated due to alcohol or drug use will not be permitted to participate inintramural sports contests. The supervisor and/orofficials assigned to the contest have the authority and responsibility to make decisions regarding studentswho are not permitted to participate, i.e., student’sbreath smells of alcohol or a student’s actions andlanguage are offensive.Teams/individuals guilty of alcohol or drug use will beasked to leave the playing area and may be barred fromfurther competition.

Forfeit PolicyThe Intramural Sports Office, sports supervisor or sportsofficial shall declare a forfeit if:A. A team uses a player who does not have an IDnumber listed on the score sheet or listed on theirofficial team roster.

B. A team uses a player who also is playing for anotherteam in the same sport.

C. An individual in a singles event or a team is notready to play at the scheduled game time.

Forfeiture of two games will cause the followingprocedure:

1. Any team or individual forfeiting twice will be dropped from league play. The $25 sign-upfee will not be returned to them. In the event of a double forfeit, both teams will have defeatsrecorded against their record. The winning team also must record its lineup (names and ID numbers) for the forfeited game and must have at least a minimum of the officialnumber of players.

2. Teams or individuals who are dropped from the league will be replaced by teams on a waitinglist when appropriate. Waiting list teams willabsorb losses for all games they missed becauseof late entry.

The StudentsThe intramural sports program recognizes theimportance of two student groups that help its activitiesrun smoothly: the intramural team captains and thestudent workers.The intramural team captain is responsible for attendingall captains’ meetings so he/she is able to distribute allpertinent rules and information to teammates. This jobshould not be taken lightly, as the team captain isresponsible for the actions of his/her teammates. Eachteam and/or organization must have a team captain. Themethod by which the team captain is chosen is entirelyup to the organization. Some of the team captain’sresponsibilities include:A. Encouraging participation in all intramuralactivities.

B. Organizing teams and entering them intocompetition before the deadline date. Full namesand ID numbers must be on roster.

C. Checking the eligibility of all players.D. Notifying teams and players of dates, times andplaces of scheduled contests.

E. Seeing that participants’ names listed on thescorecard are legible and that participants listedactually are playing at the contest. A forfeit will bedeclared if an ineligible player is listed.

F. Frequently picking up information from theIntramural Sports Office and posting ordistributing all information received.

G. Attending all scheduled captains’ meetings andrule clinics. He/she will be held accountable for all information and rules discussed at these meetings.

H. Keeping his/her name, address and telephonenumber up to date with the Intramural Sports Office.

I. Paying any forfeit fee on time and seeing thathis/her organization never forfeits a game(remember that two forfeits exclude a team fromtournament play).

J. Assuming responsibility for the conduct of theteam members before, during and after allintramural contests.

K. Evaluating the program and offering suggestions.Student workers help keep the intramural sportsprogram running at its best. They also gainvaluable experience that serves them for many yearsafter leaving MSOE. Release from Liability, Assumption of Risk and Indemnity Agreement

We the above signed, are aware that participation in one or more intramural or club sports programs, or other athleticevents or activities, at the Milwaukee School of Engineering (MSOE), or in which MSOE employees, students, staffand others participate (all such programs, events and activities, collectively called “Athletic Programs or Events”), is atour own risk and we irrevocably assume all risks of harm, damage, liability, injury and loss associated with, related to,or potentially arising from or related to, such participation. We understand that participating in any Athletic Programor Event can be a dangerous activity involving risk of injury and even death. We understand we are responsible for thecost of any and all medical services we may require as a result of participating in Athletic Programs or Events.

In consideration of MSOE’s permitting us to engage in Athletic Programs and Events, including but not limited to,practicing and competing in such activities and traveling to or from such activities (hereinafter “participation” or“participating” in Athletic Programs or Events), we hereby fully and irrevocably release, waive and discharge MSOE,and its officers, directors, and employees (“Release Persons and Entities”) from all liability to the above signed, abovesigned’s personal representatives, assigns and heirs, for all loss, liability damages, or expenses whenever arising ordiscovered, and any claim or demands therefore, on account of or related to any injury to the person or property, orresulting in death of the above signed while the above signed is participating in Athletic Programs or Events, includinginjury, loss, or damages caused in whole or in part by the negligence of MSOE or the Released Persons and Entities, orany violation of the Safe Place Statute under Section 101.11 of the Wisconsin Statutes excepting only injury or deathcaused by intentional or reckless acts of the Released Persons and Entities.

We will indemnify, and at MSOE’s request, defend, and save harmless MSOE and Released Persons and Entities forany loss, damages, or expenses incurred by Released Persons and Entities by reason of any third party claim for injury loss or damages to the above signed, which we have released, waived or discharged in this General Release ofLiability.

In addition, the above signed irrevocably assumes all liability for, is responsible for, indemnifies, and at MSOE’srequest, defends and saves harmless MSOE and the Release Persons and Entities against any loss, liability, damage, orexpense arising from any actual or claimed death or injury to any person other than the above signed, or actual orclaimed damage to property, whether owned by the above signed, MSOE or third parties, including loss of use, whichactually or allegedly results from any conduct, act or omission of the above signed, resulting from participation inAthletic Programs or Events.

Furthermore, we understand that MSOE disclaims responsibility for team or club members and others who travel toAthletic Programs or Events in vehicles not owned by MSOE. We assume all risks of driving ourselves and/or others toany Athletic Programs or Events. Similarly, we assume irrevocably all risks of riding with other team or club members,spectators, coaches or others to such activities in vehicles not owned by MSOE.

The above signed hereby irrevocably assume liability for, are responsible for, indemnifies, and at MSOE’s request,defends and saves harmless MSOE and the Released Persons and Entities against any loss, damage, liability, or expensearising from or related to any actual or claimed death or injury to any person or any actual or claimed damage toproperty which actually or allegedly results from the above signed’s transportation to or from Athletic Programs orEvents in any vehicle not owned by MSOE. We hereby authorize the staff of MSOE to secure case history records,laboratory reports, diagnoses, x-rays, and other data covering this and/or previous confinements and/or disabilities. Aphotostatic copy of this authorization shall be deemed as effective and valid as the original document. We authorizeMSOE Athletic Department to provide and secure medical assistance needed on behalf of the above mentioned.
