1.0 Project Background
Andhra Pradesh (AP) state has one of the longest coastal belts in India. Out of 67,362 Km of road network in the state, the
nine coastal districts cover 26,000 km of road length. Every year during the period of April-May and September-December,
cyclonic situations set in to the Bay of Bengal. With the recent changes in climatic conditions, the catastrophic events are
increasing resulting in very devastating cyclones like Laila, Jal etc where the loss is enormous. In the 97 years ranging
from 1892 to 1989, there were as many as 67 cyclones within the State.
It is essential to provide safe residential accommodation to the vulnerable population during any eventuality or disaster due
to natural calamities such as cyclones and associated tidal waves and storm surges. The project is formulated based on
experiences of Government of Andhra Pradesh in mitigating such disasters during the past and also keeping in view the
existing facilities available in the project area in Andhra Pradesh.
The Government of India has applied for a credit of US$ 255 Million equivalent from the International Development
Association (IDA) toward the cost of National Cyclone Risk Mitigation Project (NCRMP), and it intends to apply the
proceeds to payments for goods, works, related services and consulting services under this Project.
National Cyclone Risk Mitigation Project (NCRMP) Project Implementation Unit (PIU), Andhra Pradesh, headquartered atHyderabad has invited the consultants to submit proposals for providing the consultancy services for Third Party Quality
Audit for 9 coastal districts namely Srikakulam, Vijayanagaram, Visakhapatnam, East Godavari, West Godavari, Krishna,
Guntur, Prakasam and Nellore. The project Construction of Multipurpose Cyclone Shelters, Roads to Cyclone
Shelters, and Roads to habitations and Roads & Bridges under NCRMP is funded by International Development
Association and received through Government of India. This consultancy is envisaged to audit the quality of works being
implemented by various line departments working under PIU. The number of works under progress / for which
agreements are concluded in each district is shown in the table below

5.0 Progress of NCRMP works:
The district wise progress of works is shown below. The works in the West Godavari are not started and the works in
Krishna District has just started. At many places the first milestone is completed but works are not started. The line
departments are to analyze the reasons for the slow progress and if necessary penalty for the slow progress as per the
agreement conditions should be imposed.

6.0 District wise status of tests conducted by TPQA:
Apart from monitoring the tests being conducted by the implementation team (contractors & Line department) at various
stages, the Third party Quality Audit team also conducted tests to confirm its compliance with the standards. These tests are
carried out in the site itself and in some cases samples are collected and testing done in outside laboratories. The details of
tests conducted in the field till end of September 2011 are presented.

Mile stone 1 to the end of Dt: 18.08.2011- Earthwork, CD Works, CC Pavement and GSB.Mile stone 2 to the end of Dt: 18.11.2011- WBM Gr-II, Gr-III and Shoulders.Mile stone 3 to the end of Dt: 18.02.2012- BT and Road furniture.The formation width of the road is less than 7.5 m at some stretches. The reasons for thesame shall be informed.
3 GSB from Km 0.592 to Km 1.505.4 GSB: During the collection of GSB materials, sample was collected and got tested for CBR and PI values.
GSB layer is verified for width, camber, undulations and thickness. The details are given in ANNEXURE-1.FDD tests were conducted by TPQA at regular intervals. As and when, the required relative compactionwas found to be less than 98%, re-compaction was ordered. After ensuring the minimum 98% relativecompaction, further work is allowed. 10 Fine aggregate: Sieve analysis and silt content tests were conducted. Sand is conforming to zone-II.Fineness modulus is varying from 2.77 to 2.81 as against the allowable range from 2.02 to 3.5.
11 Workability of concrete: Slump cone tests were conducted for M30 grade concrete at regular intervals
and required workability was censured by TPQA during the inspections.12 CC Cubes: M30 grade concrete cubes were tested for 7 days' and 28 days' compressive strength and the
results are shown in QC summary table.13 8 cores were taken in cc road for M30 grade of concrete. The average thickness is varying from as
200mm to 206mm as against the specified 200mm thickness.The Equivalent Cube Strength is varying from 27.32 N/mm2 to 33.55 N/mm2 as against the minimumaverage required strength of 25.5 N /mm2
14 Contraction Joints were not cut as per agreement specification .They shall be provided and sealed as perMORD sub calluses 1501.8 and 1501.22
15 For WBM aggregators: Q.C.tests of AIV, FIV, and Water absorption and gradation tests was conducted.
After satisfying of these results, spreading and compaction was allowed.Grade II and III layers of WBM from Km 1.000 to km 1.820 were checked for width, camber, undulations,thickness and degree of compaction by volumetric analysis. The details are given in ANNEXURE-II andANNEXURE -III.

conducted and it is ensured by TPQA that the gradation satisfies the requirement of MORD table-1500.17 Proportion of the concrete ingredients and w/c ratio as per MIX Design is ensured during TPQA visits.
Slump tests were also conducted and the required workability is ensured.
8 C.C Cubes: M30 grade C.C Cubes are casted and tested for 7 days' and 28 days' compressive strength.The 7 days' compressive strength is varying from 24.00 N/mm to 24.89 N/mm. The 28 days'compressive strength is varying from 31.56 N/mm to 32.89 N/mm.
9 The M30 grade CC road was completed from Km 0/672 to km 1/186. The outer measurements of widthand thicknesses are verified. The thickness is varying from 197 mm to 220 mm as against the specifiedthickness of 200mm. The width of CC road is varying from 3.67 to 3.78m as against 3.75 m as required.Approximate compressive strength is also assessed using Rebound Hammer strength of varies from28.60 N/mm to 29.60 N/mm as against the avg requirement of 3025% N/mm2. The Results areenclosed in Annexure-III.
10 6 Nos. of Cores are taken to verify the thickness and compressive strength of CC Road. The average
thickness is varying from 199 mm to 238 mm and the equivalent cube strength of cores varies from 26.00N/mm to 29.89 N/mm as against the avg requirement of 25.50 N/mm2. The Results are enclosed inAnnexure-IV.
11 Contraction of joints is provided at spacing of 2.5m to 2.8m. The joints war not sealed so far. MORD subclause 1501.8.3. specifies, "After minimum 28 days after casting the concrete pavement, joints groove atcontraction joint shall be widened to a 10 mm width and to a depth 18 to 20mm and shall be sealed".
12 GSB: Proper gradation was censured by conducting sieve analysis at regular intervals during TPQA visits.CBR test was also got conducted and CBR value obtained is 25%. During TPQA visits FDD tests wereconducted and whenever the Relative compaction was found to be less than 98%, recompaction of GSBlayer was ordered. In that manner proper compaction of GSB was ensured. GSB layer width and
thickness are verified and the details are given in Annexure13 WBM (Gr.-II): from Km 0/0 to km 0/672: AIV test, FIV tests and Gradation tests were conducted beforeallowing spreading. After completion of WBM grade II layer width and thickness are verified and thedetails are given in Annexure
14 Volumetric analysis tests were also conducted on WBM Grade-II layer as per clause 405.13 of PMGSYQuality Assurance Manual Vol-2
15 The WBM Grade-II layer was badly disturbed from Km 0/400 to Km 0/600, which is to be rectifiedimmediately. NCR was also issued.
16 The formation width varies from 6.0 meters to 7.0 meters from Km 0/672 to Km 1/186. The formationwidth shall be widened to 7.5m.

8 In CC pavement, key wall is not provided as per agreement drawing.9 The thickness of CC road is varying from 198 mm to 204 mm as against 200mm thickness specified.
10 The width of CC road is varying from 3.73 m to 3.78 m. The specified width is 3.75 m.
11 M30 grade concrete Cubes are casted and tested for 7 days compressive strength and the results arevarying from 25.18 N/mm to 27.67 N/mm.
12 M30 grade C.C Cubes are casted and tested for 28 days compressive strength also and the results arevarying from 33.11 N/mm to 33.78 N/mm.
13 Approximate Compressive strength of C.C.Road is assessed with rebound hammer at chi: 300m and chi:468. The values are varying from 31.7N/mm to 31.8 N/mm and given in Annexure-I. The strength ofconcrete by rebound hammer method varies by 25 percent.
14 Cores are to be taken in CC pavement to verify the internal thickness and compressive strength ofconcrete.MORD Sub Clues 1501.8.3 specified the following for contraction joints
15 i)The contraction joint shall consist of a mechanical sawn joint groove 3mm to 5 mm wide and up to 1/4the to 1/3 rd depth of slab.ii) The swan joint shall be cut as soon as the concrete has undergone initial hardening and is strongenough to bear the weight of the crew and cutting machine.iii) These joint shall be subsequently widened to 10 mm width and 20 mm depth by appropriate saw tohouse the sealant.iv) These joint shall be spaced 2.5 meters to 3.75 meters.
16 i) Can this road, contraction joints are by M S plate inserting. The width of the joint varies from 5 mm to 8mm and depth of joint varies from 7mm to 14 mm.ii) The spacing of contraction joint varies from 3.8 m to 4.2 m.

14 Approximate Compressive strength of C.C.Road is assessed with rebound hammer at chi: 080m and chi:180. The values are varying from 30.8 N/mm to 31.1 N/mm2 and given in Annexure-II. The strength ofconcrete by rebound hammer is approximate and its variation is 25 percent.
15 Cores are to be trimmed in CC pavement to verify the internal thickness and compressive strength ofconcrete.
16 Spacing of Contraction of joints from 3.8 meters to 3.85 meters. But the Contraction of joints shall be at
spacing of 2.5 m to 3.75 m17 Joints groove at contraction joint was widened to a 7mm to 10 mm width and to a depth 16 to 20mm which
was sealed with bitumen.18 Cracks are observed near Contraction joint at ch:75m and chi: 25m,the behavior of crack shall be
observed and suitable action is to be taken as per MORD Cl.1501.26.19 Shoulders are constructed to support the CC road but the width varying from 0.2 M to 1.0 M only. The
thickness is varying from 130 MM to 150 mm. They are to be widened and completed.20 FDD tests were conducted on Side Shoulders by TPQA at regular intervals. As and when, the required
relative compaction was found to be less than 98%, suggest they shall be scarified ,remixed, watered ato.k. re-and re-compacted to achieve a minimum 0f 98% was ordered. After ensuring the minimum 98%

17 Hector Kilometer stones and kilometer stones are provided as per agreement.18 Cautionary, direction and place identification sign boards are fixed at site. But they are not semi reflective
type as specified in the agreement. They shall be rectified immediately. TPQA shall be informed afterrectification for verification.
19 The field officers are requested to provide the guard stones at outer edge of all the sharp curves.20 O.K CARDS are being issued at every stage of work.21 Reflective stickers may be pasted on electrical poles to prevent accidents.22 The Project informatory board was erected with all details at site as per the guide lines given by SPIU,

8. Alkali Aggregate Reactivity Tests: Samples are collected and sent to Civil Aid for testing of Alkali Aggregatereactivity test for Fine and Coarse aggregates and the obtained results are within the limit as per IS2386(Part VII) -1963.
9. C.C Cubes: M30 grade C.C Cubes are casted and tested and the test results are summarized in QC testSummary.
10 Cement Coating of Steel: The protection of reinforcement against corrosion prior to the placement ofconcrete is done by applying a Cement slurry coating on the steel.
11 Water: Sample is collected and sent to Natural Resources Development Co-Operative Society Ltd., Ongole.Results are within the specified range as per IS: 456-2000 & IS: 3025-1986.
12 Brick Test: Water absorption and Crushing strength tests are conducted as per specification IS 3495, andthe test results are summarized in QC test Summary.

24 Stilt floor columns: reinforcement details are verified randomly H2, F4 column 4-20 mm dia & 4-16 mm dia
stirrups 8 mm dia at a spacing varies 148mm to 156mm as against of 150 mm c/c. And D4 & F3 column 8-16mm dia stirrups 8 mm dia at a spacing varies 147mm to 155mm as against of 150 mm c/c. The spacing ofreinforcements is not in uniform. The same was got rectified before concreting.Stilt floor column measurements are verified and the observed measurements were fairly tallying the drawingmeasurements.Stilt floor columns D5, C1, & B4 approximate compressive strength is also assessed using ReboundHammer. Strength varies from 23.17 N/mm to 33.91 N/mm as against the avg requirement of 3025%N/mm2.
25 Stilt floor roof slab completed with M30 grade concrete, before laying the concrete the reinforcement detailswere thoroughly verified with reference to the drawing. Any minor adjustments in spacing and cover etc.,
were got attended and the concreting was allowed.Stilt floor roof slab thickness are verified as 147 mm to 152 mm as against of 150 mm. and the thickness ofslab in portion B3-4 to F3-4 is 172 mm to 177mm as against of 175 mm thick.
26 STAIRCASE- The stair case and Ramp portion are constructed with M30 grade concrete. Honey Combing isobserved on the sides of ramp slab. NCR is issued to rectify the honey combing.During TPQA inspections, when concreting is going on TPQA monitored the proportioning of the ingredients,water cement ratio, slump and placement of concrete etc. Casting of cubes was also carried out.At the Time of one TPQA inspection, curing arrangements were not properly done. TPQA has got itimproved.The stair case Rise and Treads are verified, and the observed measurements. Rise is varying from 133 mmto 182 mm against of 150mm. Treads are varying from 260 mm to 300 mm as against of 280mm. All thetreads and raises of each step shall be equal to prevent accidents.
27 First floor columns reinforcement details are verified spacing, cover etc were verified before concreting.28 First floor roof slab completed with M30 grade concrete, before laying the concrete the reinforcement details
were thoroughly verified with reference to the drawing. Any minor adjustments in spacing and cover etc.,were got attended and the concreting was allowed.
29 Honey Combing is observed on the sides of First Floor Slab & Beams i.e. on B1, B2, B4, & B7 NCR isissued to rectify the honey combing.
30 First floor Brick work is in progress, the mortar joints in brick masonry work should not exceed 12 mm.Irregular brick work done, and It is not to plum bob at some places. Workmanship is not satisfactory.

2 21% value of the work is completed. Steel placement work for plinth beam is inprogress.The progress ofwork is slow as per mile stone.
3 Works to be done as per Combined Milestone:Mile stone 1 to the end of Dt: 07.03.12- Ground Floor and Plinth Level.Mile stone 2 to the end of Dt: 07.08.12- First Floor Roof Slab Cast .Mile stone 3 to the end of Dt: 07.01.13- Completion of Works.
4 Laying of PCC under plinth beam work completed.5 The center line measurements for columns are verified and found fairly tallying with the drawing. Hence
the previously given NCR is closed.6 The material test registers are not shown for verification during inspection. The same shall be produced to
TPQA during inspection. One technical person appointed by the contractor is available at site duringinspection.Materials.
7 Sand: a) Sieve Analysis tests are conducted. Sand is confirming to Zone II of IS 383 table 4. Finenessmodulus is varying from 2.48 -2.59 as against the allowable limits of. 2.4 to 3.2.
8 Graded Aggregates : Sieve analysis for 20mm graded metal are conducted and observed values are
within the specified range as per IS 383 table-29 Alkali Aggregate Reactivity Tests: Samples are collected and sent to Civil Aid for testing of Alkali
Aggregate reactivity test for Fine and Coarse aggregates and the obtained results are within the limit asper IS 2386(Part VII) -1963.
10 Cement: TPQA collected the cement sample and tested for all the required tests on cement such asfineness, initial setting time, final setting time, consistency etc and the values observed are within thespecified range as per IS 4031.
11 Steel: Steel samples are collected and sent to Civil Aid for testing of Physical and Chemical properties andthe obtained results are within the specified limits as per IS 1786.
12 Water: Sample is collected and sent to Panchayatraj internal water quality monitoring Laboratory Z.P

4 The marking for Footing positions are given with the help of Total station instrument. The linearmeasurements of the center line of columns are verified and fairly tallying with the approved drawing ofType-B Cyclone shelter with a design SBC Value of 7.5 t/m2.
5 The soil sample was collected by the contractor at a depth of 2.5m from the ground level and sent to Govt.Polytechnic college to test the SBC value of soil and the result obtained is 9.0 t/m2 as against of 7.5 t/m2 asper approved drawing. The confirmatory test for SBC is conducted by TPQA. The SBC obtained is 8.5 t/m2
6 The field officers have furnished the test results for Soil SBC value, Mix Designs, Steel reports and Waterobtained from Department of Civil Engineering in Govt. Polytechnic and Andhra university at VisakhapatnamDistricts.
