We are therefore looking to enhance our Community Nursing Teams with the appointment of new Advanced Nurse Practitioner roles. These roles will support General Practice and the Community Nursing Teams deliver safe, effective and person centred care to patients within the primary care setting. Initially the role will be to develop and deliver a service to support the timely review, treatment planning and care coordination for patients within the nursing and residential care home settings. As the service evolves there is an opportunity for the post holders to inform future service development with the inclusion of a responsive service to support the management of other acutely unwell, frail and complex patients within the community.

You will be required to be educated to degree or Specialist Practitioner Qualification and have or be working towards your Masters in Advanced Practice. You must already have completed your clinical assessment and non-medical prescribing qualifications to apply for this role and be committed to completing the MSc in Advanced Practice. You will require excellent communication skills and be able to work as a member of the wider Health and Social Care Team with the ability to work autonomously utilising your own initiative, clinical judgement and decision making.

An exciting opportunity has arisen for an individual to work as a Frailty Nurse Practitioner within our Integrated Assessment Team. Early identification, effective assessment and management of frailty have been identified as significant priorities for NHS Fife to optimise patient outcomes. The Integrated Assessment team is a rapidly developing multidisciplinary team which provides comprehensive assessment for patients who are identified as having frailty syndromes to ensure effective

identification of patient pathways and destination of care is reached in a timely manner. You should be a qualified Nurse with significant acute care experience with a genuine interest in working with older people and a keen desire to develop within the service. Nurse Prescribing and clinical decision making qualifications are an ultimate requirement for this post and you will be expected to work towards obtaining these skills.

You should have evidence of significant post registration professional practice experience to undertake and fulfil the key areas for this post including completion of relevant postgraduate courses to Masters degree level, or working towards Master degree level or ability to demonstrate working at Masters degree level appropriate to clinical area.

Applications are invited from suitably qualified and experienced Advanced Nurse Practitioners who already possess the non-medical prescribing and clinical assessment skills qualifications. We will however consider staff with suitable experience of working within this area of practice who have a desire to develop and become a Nurse Practitioner through completion of the above qualifications. During this time you will be paid in accordance with Annex 20 arrangements for pay and banding of trainees.

This post will be based within Integrated Assessment Team who provide cover in Admissions Unit 1 which comprises 37 beds and 7 Assessment trolleys, but the team provide support to all front door areas, i.e. Emergency Department and Acute Surgical Unit.

Ward 15 is a 26 bedded Acute Medicine of the Elderly ward which specialises in the acute care of patients with frailty syndromes. You will be a Registered Nurse educated to degree level or equivalent with significant post registration experience.

With the increasing number of frail older people in the population, there is a real focus on how Health and Social Care partners work together to support older people. The multidisciplinary team in Medicine of the Elderly works closely with the front door Frailty Team and the Discharge Hub to ensure person-centred care for frail patients through proactive management of the inpatient journey and discharge with appropriate support. You will be required to work flexible shifts including days, nights and weekends. As part of the Emergency Care Directorate you will be required to fulfill the role of Duty Charge Nurse for the directorate on a rotational basis. This post would suit pharmacists with experience of hospital, primary care or community pharmacy with considerable clinical experience and who have supervised or trained other staff.

You will have a Masters degree in Pharmacy or equivalent and be on the practising register of the General Pharmaceutical Council.

It is essential that you have either: a relevant post-graduate qualification in clinical pharmacy/ prescribing sciences or be an independent prescriber. You will be expected to have completed both the diploma and independent prescribing qualification within an agreed timescale, as well undertaking clinical assessment skills and consultations skills training.

We have a strong commitment to radiology trainees from the South East Scotland training programme. You will work in the new state of the art Emergency Department at Victoria Hospital, Kirkcaldy, which opened in 2012. The department has an annual attendance of 65,000.

As an EM consultant group we believe strongly in providing a sustainable work/life balance. The job plan includes a PA for each on call. Clinical administration time has been factored in as per RCEM guidance. There is a 24 hour middle grade rota with no gaps.

We perform well in key areas such as sepsis, stroke thrombolysis and compliance with the 4 hour target. We have an excellent relationship and collaborate closely with all in-patient teams. EXCITING SECONDMENT OPPORTUNITY

If you have well developed skills and experience in working with people who are experiencing homelessness, this could be an exciting career development opportunity for you to develop these skills within the community.

The NHS Fife Intervention Project is a highly innovative 18 month programme to test a new approach to providing services in NHS Scotland hospitals for those patients who are homeless. The project will be managed by Shelter Scotland and is supported by a multi-agency Steering Group led by Fife Council Housing Services and the Fife Health and Social Care Partnership. The key aims of this project are to:

improve working links between health and housing services provide homeless individuals with the right support to improve their health

outcomes reduce the re-admission rates among this cohort evaluate the preventative value of this approach in reducing pressures on

NHS Emergency Departments in Scotland.

The project will employ 2 staff and be run from Victoria NHS Hospital in Kirkcaldy, Fife. There will be close partnership working with hospital staff to engage patients who are identified as having insecure housing. Workers will follow a five-stage process to engage with the patient, undertake an immediate crisis response, work with them to identify key goals, put this into action focused on helping the person to develop skills and resilience, and finally support them to move on.

The project will focus on emergency admissions at VHK and Out of Hours. The target group will be individuals of any age and families who are homeless or at immediate risk of homelessness.  This project will be evaluated by Healthcare Improvement Scotland, who will work alongside the staff team in identifying outcomes and learning.

A key aim of this project is to share learning and build capacity with key stakeholders across Fife. Applications are thus being sought from staff in Fife Council Housing Services and NHS Fife who are interested in being seconded to staff the project. Key skills required include: needs assessment; experience of casework or care/support planning & delivery; experience of collaborative working. A significant programme of staff training and development will be a feature of this project.
