The Diabetes TeamThe team will consist of consultants, nurses, dietitians,podiatrists and psychologists employed by Kingston Hospital. Weare here to help you to understand and to manage yourdiabetes in the best possible way.
What do I need to bring to my appointment?• If you are home blood glucose monitoring please bring
your glucose diary with you. • List of medications.• Please ensure your rotuine blood tests have been done
prior to your appointments.
How to get to Leatherhead HospitalThe Hospital is a 20 minute walk from Leatherhead station. Freecar parking is available on-site for patients and visitors.

Diabetes service closer to homeIf you live in the Leatherhead area, you may be able to access yourdiabetes care closer to home. This innovative new service will beavailable to those people with diabetes requiring additional supportand help with their diabetes management.
The team delivering this service will have the right skills to supportpeople with diabetes requiring more complex care within their owncommunities.

Why is this service changing?
These developments are taking place to ensure that local people arereceiving care in the community, closer to home.
I am currently being cared for by a consultant in hospital.Will the new service be inferior to the care I am gettingnow?
No. You are being referred to a team of specialists, overseen by ahospital consultant who will now provide your care.
Does this mean I will not need to go to the hospital for mydiabetes at all?
Your clinical needs will continue to be assessed and if necessary youmay be referred back to the Leatherhead Community Hospital.
