Register for NTP Portal 1. Click the following link to access the NTP Portal main page: https://sitentp.sbasite.com.

2. Selection options are: Register, Login, and View Your Apps. View Your Apps will be available

after you Register and Login.

3. Click registration page and the following page will display: Required Information must be completed: Username, Your Name, Phone, Email, Password, Confirm Password, Security Question, Security Answer, and Captcha. Optional Profile Information is Fax, Company and Address. Once information is entered click Create User. The new User will now be created and you are ready to Login.

Login to the NTP Portal 1. From the Customer NTP Portal home page, click login here.

2. Enter your username and password.

3. Select Remember Me setting to save your Login credentials.

4. Click Login. My NTPs page will be displayed.

My NTPs (Overview) 1. On the My NTPs page you will see Start New NTP and View Site Schedule. Under My

NTPs are: Draft, Submitted, On-Hold, Approved, Rejected, Pending Closeouts, Closed and Cancelled. Closed and Cancelled by default are Unchecked. To only see Draft, uncheck all other boxes and only Drafts display. There is a Quick Jump drop down menu with NTP ID, Tenant ID, and SBA Site ID selection options. Use these to go directly to a request. Enter the ID, click GO and you are taken directly to that request.

3. Tenant ID is Carrier Site ID.

4. SBA Site ID is Site ID.

5. Click and the NTP Request Form page is displayed. The first section is labeled General. The data displayed is: Site ID, NTP Type, Carrier, Owner, RSM, Carrier Site ID and Status. Also displayed is a Comments box and CC Approval To box. The Status will be: Draft, Submitted, Approved, Pending and On-Hold.

6. Supporting Documentation - From this screen you can upload, delete, and view documentation. Column headers are File Name, Type, Size, User and Date. Note the page Refresh button. You do not need to be in Edit mode to add or delete files.

7. Safety and Compliance - On this screen are a collection of Yes or No questions and a link to our safety requirements, Gin Pole Use Request Form, and Night Work Request.

8. Assign Contractor - On this screen are: Company Information, Contact Information, and Estimated Dates on Site, as well as a Subcontractors section.

Note: If you click Edit at the top of the page, the whole NTP form can be edited.

9. When in Edit mode the available Actions are Save and Cancel.

10. If you make changes in edit mode and Save, the status becomes Draft. 11. If you click Submit, the status becomes Submitted. 12. If you select Gin Pole ‘Yes’ and click Submit the Status is On-Hold. 13. If you select Will the Scope of Night Work require installation or lifting of equipment ‘Yes’ and

click Submit the Status is On-Hold. 14. Once approved the status will be Approved. You can also Delete or Cancel NTP request. Once

approved, if you Close out by clicking Finished, you must attach Closeout documentation. RSM’s can Accept, Reject, Cancel and Close an NTP request.

15. The last section is the NTP Approval. In this section you will see the approval status of Safety and the RSM.

16. Click View Site Schedule. A drop down will display where you will select your Site ID.

NTP Request Form - Customer

General Section

1. Click Start New NTP to display the NTP Request Form. Owner and Status are pre-populated.

Owner has full rights to make any changes to an approved NTP as required. Any changes to an Approved NTP will require a re-approval from the RSM. Upload Supporting and Close-out documents. Assign Ownership to another person.

The Owner can add additional persons in the CC Approval box shown below:

Anyone listed in the CC approval will have the same rights as an assigned Contractor.

2. Select a Site ID by typing in the drop down box and select the correct site from the list of possible choices.

a. Select the NTP Type. The NTP Type will determine the Safety and Compliance

selections as well as needed documentation. Please refer to Required Documents Matrix (page 23) for more detailed information regarding NTP type and which documents are required to be uploaded

3. Select a Carrier.

4. Click Save. The General section looks like the following. The RSM is populated upon saving.

Supporting Documentation Section

b. Click Edit and move to the Supporting Documentation section. As stated previously, you do not need to be in Edit mode to Add or Delete files. Please refer to Required Documents Matrix (page 23) for more detailed information regarding NTP type and which documents are required to be uploaded

3. Select the needed documents for the NTP Type selected from the drop down menu (Building Permits, Closeout, Construction Drawings etc.). Click Pick Files. This will allow you to upload documents from your computer. Click Ok. The document is now listed in the document section.

4. From the Support Documentation section, you can Delete a file, or open it by clicking the File Name. Also displayed are the headers Type, Size, User and Date. From here you can upload more documents for the type already uploaded or choose another type.

a. Please refer to Required Documents Matrix (page 23) for more detailed information regarding NTP type and which documents are required to be uploaded

NOTE: The NTP Type will drive what needs to be checked in the Safety and Compliance section along with which documents are required. Example: Carrier Installs – Ground & Tower require all Safety and Compliance check boxes to be selected while Carrier Installs – Ground Only requires only two Safety and Compliance check boxes to be selected. Refer to the matrix of Safety and Compliance check boxes that must be selected based upon NTP Types on page 23.

Safety and Compliance Section

1. In Edit mode Safety and Compliance will look like the following with Check boxes for gin pole usage and disclose any night work that will be needed. Note the Check boxes are blank. Check the boxes that pertain to the Scope of Work you are requesting and click Save.

2. All check boxes with asterisks are required to be answered in order to Submit. If any box is not

checked you will receive a warning when you click Submit. You must then go back into Edit mode and check the box you missed previously. If Gin Pole is required, then documents must be uploaded. If Gin Pole is not required, documents are not required.

3. If there is any Night Work that is required: a. • If the carrier is performing just simple antenna, jumper, testing, or site integration they will

be required to review the Night Work Checklist and complete the attached Night Work JHA. They will not require written approval from Safety/Risk Management. The contractor will be required to complete a Site Specific JHA, of their own format, meeting SBA’s at a minimum (or utilize ours attached) while following the attached checklist items, addressing the multiple hazards associated with working at night. They will not need SBA Safety approval for this SOW

4. Will the scope of night work require installation or lifting of equipment:

a. If the Carrier/Vendor is actually performing any hoisting activity they will be required to have written approval from our Safety/Risk Management team. The attached Night Work Checklist has all the required information on how they can request approval. For this SOW, the contractor will be required to submit a Rigging Plan and an Illumination Plan requiring SBA Safety approval. As well as, having a Site Specific JHA (of their own format meeting SBA’s at a minimum or utilize ours attached) addressing the multiple hazards with working and hoisting materials at night.

5. Avetta ID Number:

a. Avetta is 3rd party company employing an internet-based evaluation program that reviews and maintains contractor’s health, safety, environmental, insurance, and company-specific information for SBA.

b. Any Contractor that will be performing work on an SBA tower site will need to be registered and in good standings with Avetta.

Assign Contractor Section

1. In Edit mode the Assign Contractor section will look like the following. All data fields are blank. All fields are required. Enter Company Name, Company Number, Contact Name, Number and Email.

a. The Contractor that is assigned by the Owner of the NTP has rights to change the construction dates and upload supporting and close-out documents.

b. Any change to the construction dates will require a new approval from the RSM.

2. In the Estimated Dates section click the Show Site NTP Schedule to see what dates are

available. The LEGEND will show what is open and what is closed. Your dates will display in Green once the form is submitted.

1. When you click Save, the completed form will look like this. Once you see which dates are open, close the calendar and click the Calendar Icons to select Start and Finish dates. Also enter any Subcontractors that will be on site.

4. In Edit mode your Actions are Edit, Submit, Cancel NTP and Outlook Reminder.

5. When you click the Outlook Reminder, you will be prompted to Open, Save, or Cancel. Select Open.

6. When you open, the following appointment setter will display. Here you can click Save and Close and the dates are now on your Outlook calendar. If you choose to Save, you can then click the X and close out the pop up window and the dates will be on your Outlook calendar.

NTP Approval Section

1. In this section you are able to follow whether there has been an Approval either by Safety or an RSM. In the example below, there is no Safety approval needed and the RSM is Pending. These fields are locked and cannot be edited. When approved or rejected you will see the RSM or Safety Approver name, date and time it was approved/rejected.

3. Click Submit and the status is now Submitted. On the main NTP page you will now see your request in the grid with a Status of Submitted. As requests are Approved or Rejected you will be able to track the status of all entries on the My NTPs page.

NTP Request Form – RSM 1. When a Customer submits a request, you will receive an Email letting you know a Request Form

is waiting for your approval along with a link that will take you directly into the NTP Request Form. Here you can review the whole request.

2. At the top of the page are your Actions: Edit, Approve, Reject, Cancel NTP, Close NTP and Outlook Reminder.

1) Edit - Allows you to edit any field, add or delete documentation, change request dates etc.

Anything the requestor can do you can do as well.

2) Cancel NTP – When you click Cancel you will receive a warning message asking if you really want to Cancel. If you click Cancel the request will remain. If you click OK the request will no longer display.

3) Approve – Upon Approving, the remaining Actions are Edit, Cancel NTP and Close NTP.

Note the window to enter any closeout instructions.

4) Reject - Upon Rejecting, the remaining Actions are Edit, Cancel NTP and Close NTP.

When you reject a request, you must enter a Reject reason which will display on the main page. The same information will also display on the Requestors NTP Request Form.

5) Close NTP – When you click Close NTP you will receive a warning message asking if you want to close the NTP. If you click Cancel the request will remain. If you click OK the request will no longer display.

6) Outlook Reminder will allow you to enter the dates onto your Outlook calendar.

When you choose any of the above Actions, the requestor will receive an Email letting them know if their request was Approved, Rejected, Cancelled or Closed.
