Forgiveness Portal User Guide

Thank you for choosing Choice Bank as your SBA PPP loan provider! The following document provides instruction on how to begin your PPP loan forgiveness request, through our PPP Forgiveness Portal. The Choice Bank SBA PPP Forgiveness Portal has been designed to provide a user-friendly, electronic process, that you can complete at your convenience.

1. Accessing the PortalAn email has been sent to the address provided in your original PPP Application. This email contains instructions for how to access the PPP Forgiveness Portal. Simply click on the “Login to Get Started” link to begin. If you have problems accessing the portal from the link provided, please contact us at [email protected] or at 763.398.3060 during normal business hours for assistance.

2. Enter Portal

For first-time users, you will be prompted to create a password.

Once you’ve set your password and logged in, you will be directed to your dashboard.

To begin your application, select the Marketplace tab, located on theleft-hand side of the screen. When the option for Paycheck ProtectionProgram Forgiveness appears on your screen, click the green applybutton.

NOTE: You are encouraged to complete your application within 5 business days of logging into the portal to avoid delays in the processing of your application. We encourage you to wait to setup your account until you are prepared to submit all information.

NOTE: For optimal performance, it is recommended that you use Google Chrome or Microsoft Edge to submit all information. If you are using Internet Explorer, you will need to decrease your browser view to 95% or less.

Your business name should now appear in blue text from within the portal. If you are the primary contact for multiple entities with PPP loans, you will see all of these businesses listed here. You can submit your application for each PPP loan through this same login. Please note the help text tile on the right-hand side of your screen. This section will be available across the various portal screens, providing valuable detail to you as you complete your forgiveness process.

After loading, verify your business name and information appears correctly and select ’Apply With This Business’.

There are two versions of the Forgiveness Application (Form 3508 and EZ Form 3508). The shorter, EZ, form is available for borrowers that answer ‘Yes’ to at least one of the qualification questions prompted within the portal.

Every field within the portal must be completed. Unfinished fields will result in an error message and/or affect your ability to submit your application. The stages of the application will be displayed on the left-hand side of your browser, to help guide you.

After completing your application, you will find a tab named “Estimated Loan Forgiveness Calculation.” Please review this information carefully, to ensure accuracy. Incorrect information could result in an incorrect forgiveness approval. Once you have verified, click “Next”.

NOTE: Borrowers that do not qualify for the EZ Form, will be required to upload a copy of their Schedule A (located on page 3 of the full Forgiveness application) as part of their supporting documentation. A fillable version of Schedule A is located on our website here.

TIP You do not need to complete your forgiveness application in one sitting. The portal offers the flexibility to save your current work and return at another time to finish. Simply select the blue “Save & Come Back Later” button.

Reminder: Once you create your account, to avoid delays in processing you are encouraged to complete your application and upload all supporting documentation withing 5 business days. We encourage you to wait to create your account until you are prepared to submit all information.

4. Certification and E-Consent

In this step, you will make certifications to additional SBA guidelines.

The E-Consent is optional, however we ask that you please complete this within the portal. This consent will act as your electronic signature for your application. If you do not complete the e-consent, a paper form will be required, which will result in delays. When complete, select the green “next” button.

NOTE: The e-consent MUST be completed by an authorized representative (most often the owner) of the business.



5. Review and Submit

You will now have the chance to review all information entered one more time, to ensure accuracy, before submitting. Once verified, select the “Submit” button.

6. Uploading Documentation

Upon submitting your application, the confirmation pictured below will be displayed. Click on the ‘Go to Dashboard’ button to return to your dashboard. From the dashboard, you will upload supporting documentation for your PPP forgiveness application.

NOTE: Once you submit your application, you will not be able to return to this information. Consider printing a copy of this page if you would like to retain for your personal record keeping.



6. Uploading Documentation (Continued)

From your Dashboard expand the Upload Documents Option, by clicking on it. This will expand to show a full list of all documents that MAY be needed for your Forgiveness application.

Simply drag and drop or use the browse option to upload individual files needed. Once all documentation has been uploaded, our team will begin processing your application.

NOTE: You only need to upload the documents that are applicable for the information completed in your application portion. For example if you did not complete the field for rent expense, you do not need to upload documentation for this.



7. Approval Process

If there are errors or missing information, you will receive email notifications from the forgiveness portal. Please review and address these notifications to avoid delay in the processing of your application.
