The Div i s ion of Research Resources, Nat ional I n s t i t u t e s of Heal th , announces t h e establ ishment of t h e Biomedical Research Development Grant (BRDG) Program. This program i s intended t o he lp e s t a b l i s h s i g n i f i c a n t involvement i n bio­medical and/or behav io ra l r e sea rch i n new, s m a l l , and developing i n s t i t u t i o n s t h a t need a s t r o n g r e sea rch m i l i e u t o h e l p a s s u r e t h a t manpower f o r b a s i c biomedical o r behav io ra l r e sea rch , c l i n i c a l r e sea rch , o r o t h e r h e a l t h p r o f e s s i o n a l areas w i l l be w e l l t r a i n e d . The BRDG Program is a subprogram of t h e Biomedical Research Support Grant (BRSG) Program. Up t o 10 pe rcen t of t h e funds a v a i l a b l e f o r t h e BRSG Program may be a l l o c a t e d f o r t h i s new program. It is expected t h a t 12 t o 15 new awards w i l l b e made each yea r and t h a t t h e program w i l l be h igh ly competi t ive. E l i g i b i l i t y is l i m i t e d t o n o n p r o f i t and no t - fo r -p ro f i t i n s t i t u t i o n s which, during t h e la tes t 12-month per iod ending September 30, have r ece ived less than $200,000 ( d i r e c t and i n d i r e c t c o s t s ) i n Pub l i c Heal th Se rv ice (PHS) r e sea rch g r a n t s . Each i n d i v i d u a l campus of a multi-campus u n i v e r s i t y is considered as a s e p a r a t e " i n s t i t u t i o n . " S i m i l a r l y , each h e a l t h p r o f e s s i o n a l school is l i k e w i s e considered an " i n s t i t i t i o n , " even though i t may b e a component of a u n i v e r s i t y . I n s t i t u t i o n s t h a t r e c e i v e o t h e r PHS re sea rch development g r a n t support (e.g. t h e Minority Biomedical Support Program) are no t e l i g i b l e f o r t h e s e new BRDG awards.

App l i ca t ions m a y r eques t up t o t h r e e y e a r s of support ,with average funding of no more than $100,000 p e r yea r . Support may be extended by competi t ive renewal, b u t support t o one i n s t i t u t i o n may no t exceed a t o t a l of s i x y e a r s o r $600,000.

FOR PERIODONTAL DISEASE A N N O U N C E M E N T

In t roduc t ion The Nat iona l I n s t i t u t e of Dental Research (NIDR) announces a n a t i o n a l compet i t ion t o e s t a b l i s h a l i m i t e d number of c l i n i c a l r e sea rch c e n t e r s devoted t o pe r iodon ta l d i seases . The o b j e c t i v e s of t h e proposed c e n t e r s are t o a c c e l e r a t e t h e development of new knowledge f o r t h e prevent ion , d i agnos i s , and t reatment of pe r iodon ta l d i seases by b r ing ing resources , f a c i l i t i e s , and manpower i n t o focus on t h e s e problems i n a concerted way. It is expected t h a t t h e c e n t e r s w i l l c a p i t a l i z e on f ind ings accrued from recen t microbio logic , immunologic, and pharmacologic i n v e s t i g a t i o n s . This i s t h e announcement of t h e N I D R ' s i n t e n t t o e s t a b l i s h c l i n i c a l research c e n t e r s and desc r ibes gu ide l ines f o r developing t h e a p p l i c a t i o n s .

Background Pe r iodon ta l d i s e a s e s are a group of inflammatory condi t ions a f f e c t i n g t h e t i s s u e s surrounding t h e r o o t s of t h e t e e t h . It is es t imated t h a t 94 m i l l i o n Americans have some form of pe r iodon ta l d i sease , and 32 m i l l i o n of t h e s e have advanced d i sease . With age, t h e d i s e a s e becomes more seve re i n t h e i n d i v i d u a l and more p reva len t i n t h e popula t ion . Thus, pe r iodon ta l d i s e a s e , i n i t s va r ious forms, c o n s t i t u t e s a major h e a l t h problem of inc reas ing concern i n our soc ie ty .

methods are d i f f i c u l t and r e s u l t s are uncer ta in . Furthermore, pe r iodon ta l therapy c o s t s t h e American p u b l i c approximately $1.5 b i l l i o n each yea r even though only a f r a c t i o n of t h e ind iv idua l s r e q u i r i n g t rea tment a c t u a l l y r ece ive i t .

Objec t ives The broad o b j e c t i v e of t h e c l i n i c a l r e sea rch c e n t e r i s t o t r a n s l a t e b a s i c concepts , e s p e c i a l l y those of microbiology, immunobiology, and pharmacology, i n t o c l i n i c a l i n v e s t i g a t i o n s of pa t i en t s wi th pe r iodon ta l d i sease . Even though t h e main t h r u s t of t h e c e n t e r r e sea rch should b e c l i n i c a l , i t is recognized t h a t t h e proper s tudy of human s u b j e c t s may encompass a l abora to ry component. Animal s t u d i e s may a l s o b e appropr i a t e . S p e c i f i c a l l y , t h e c e n t e r should develop programs t o accomplish some o r a l l of t h e fol lowing t a sks :

1. E s t a b l i s h t h e causa t ive organisms i n pe r iodon ta l d i s e a s e s ,

2 . C l a r i f y t h e hos t response t o t h e s e causa t ive organisms,

3 . Perform c l i n i c a l s t u d i e s of t h e r a p e u t i c techniques and regimens,

4 . Develop prevent ive measures.

Although t h e subs tance of t h e research program may vary according t o t h e l o c a l e x p e r t i s e , i n t e r e s t , resources , and recrui tment p o s s i b i l i t i e s , a proposed c e n t e r should develop p r o j e c t s which re la te t o t h e above.

C h a r a c t e r i s t i c s of a C l i n i c a l Research Center The sponsoring i n s t i t u t i o n is expected t o have an adeq,uate base of r e sea rch . It should provide r e sources and s e r v i c e s t o support e s t a b l i s h e d r e sea rch i n areas such as microbiology, immunology, and pharmacology and develop r e l e v a n t c l i n i c a l i n v e s t i g a t i o n s toward t h e establ ishment of such a c e n t e r . The c l i n i c a l r e sea rch c e n t e r should b e an i d e n t i f i a b l e o r g a n i z a t i o n a l u n i t w i t h i n an i n s t i t u t i o n o r among a group of i n s t i t u t i o n s and t h e sponsoring i n s t i t u t i o n should be w i l l i n g t o make a commitment of i t s r e sources and s t a f f t o i n s u r e t h e development, o p e r a t i o n , and f u n c t i o n of t h e proposed c e n t e r .

Mechanism and Length of Support The mechanism of support w i l l b e t h e g r a n t , b u t d i f f e r s from t h e r e g u l a r r e sea rch p r o j e c t g ran t i n i t s emphasis, o b j e c t i v e s , and t h e degree of coord ina t ion by t h e Na t iona l I n s t i t u t e of Dental Research. Centers may r e c e i v e support f o r a per iod of f i v e y e a r s , w i th con t inua t ion beyond t h i s pe r iod dependent upon a competing renewal a p p l i c a t i o n . Renewal a p p l i c a t i o n s w i l l be reviewed i n competi t ion wi th new a p p l i c a t i o n s , on an equa l b a s i s .

Re la t ionsh ip t o N I D R The award of c l i n i c a l r e sea rch c e n t e r g r a n t s w i l l e s t a b l i s h a s p e c i a l coope ra t ive r e l a t i o n s h i p between t h e NIDR and t h e g r a n t e e i n s t i t u t i o n . To accomplish t h i s , a program o f f i c e r w i l l b e designated by t h e N I D R t o work c l o s e l y wi th t h e s t a f f of t h e c l in i ca l r e sea rch c e n t e r . The program o f f i c e r w i l l make r e g u l a r v i s i t s t o each c l i n i c a l c e n t e r t o d i s c u s s p rogres s and t o provide a s s i s t a n c e . I n a d d i t i o n t o t h e review of annual progress r e p o r t s , p r o j e c t s i t e v i s i t s t o each center w i l l b e conducted on an annual b a s i s .

Admin i s t r a t ive Items and Costs The c l i n i c a l r e sea rch c e n t e r d i r e c t o r should b e a r e spec ted s c i e n t i s t a b l e t o provide both s c i e n t i f i c and a d m i n i s t r a t i v e l eade r sh ip . The d i r e c t o r w i l l b e r e s p o n s i b l e f o r t h e o rgan iza t ion and ope ra t ion of t h e c e n t e r and f o r communication wi th t h e NIDR on s c i e n t i f i c and o p e r a t i o n a l matters. The r e sea rch program f o r each c l i n i c a l c e n t e r w i l l b e guided by i t s own i n t e r n a l review board c o n s i s t i n g j o i n t l y of s t a f f members and o t h e r e x p e r t c o n s u l t a n t s n o t p a r t of t h e c l i n i c a l c e n t e r program. This board w i l l adv i se t h e c l i n i c a l center d i r e c t o r of i t s assessment of p rogres s on c u r r e n t p r o j e c t s and conduct an i n i t i a l review of new i n i t i a t i v e s .

The c e n t e r g r a n t may inc lude p i l o t p r o j e c t s as w e l l as a c l u s t e r of i n t e r r e l a t e d r e g u l a r p r o j e c t s . Funds may be u t i l i z e d f o r c e n t r a l support s e r v i c e s , equipment, s u p p l i e s , r enova t ions , c o n s u l t a t i o n s e r v i c e s , t r a v e l , and p u b l i c a t i o n c o s t s . Funds may a l s o b e u t i l i z e d f o r p r o f e s s i o n a l , t e c h n i c a l , and a d m i n i s t r a t i v e personnel . However, each p a r t i c i p a t i n g s c i e n t i s t w i l l b e expected, w i t h i n t h r e e y e a r s of j o i n i n g t h e c e n t e r , t o o b t a i n independent r e sea rch support from sources o t h e r than t h e c e n t e r g r a n t , I n t h i s way, t h e c e n t e r may cont inue t o provide support f o r o t h e r s c i e n t i s t s who e n t e r t h e c e n t e r program. Support f o r research-r e l a t e d c o s t s of p a t i e n t involvement and medical care may b e authorized. The program cannot provide funds f o r new cons t ruc t ion . For t h i s f i r s t compet i t ion f o r p e r i o d o n t a l d i s e a s e c l i n i c a l r e sea rch c e n t e r s , t h e I n s t i t u t e w i l l r e s e r v e funds t o i n i t i a t e one o r two c e n t e r s during FY 77. There w i l l b e a l i m i t e d expansion of t h e centers program during t h e next few yea r s . The expected range of support should b e $150,000 - $200,000 f o r t h e f i r s t yea r w i th modest increases i n subsequent yea r s .

TITLE: DIABETES I N PREGiVANCY: EFFECTS ON MOTHERS AND OFFSPRING

INTRODUCTION The Center f o r Research f o r Mothers and Children (CRMC) of t h e Nat ional I n s t i t u t e of Child Heal th and Human Development (NICHD) i n v i t e s a p p l i c a t i o n s f o r g r a n t s t o s tudy pregnancies a s s o c i a t e d wi th pre-diabetes m e l l i t u s , g e s t a t i o n a l d i a b e t e s , and o v e r t d i a b e t e s of e i t h e r t h e a d u l t o r youth-onset type and t o s tudy t h e o f f s p r i n g of d i a b e t i c pregnancies. There is p a r t i c u l a r i n t e r e s t i n feeding p a t t e r n s and developmental f a c t o r s r e l a t e d t o t h e la ter appearance of o b e s i t y and of d i a b e t e s i n t h e s e g e n e t i c a l l y predisposed i n d i v i d u a l s .

SCIENTIFIC PROGRAM REQUIREMENTS The CRMC seeks s t u d i e s aimed a t b u t n o t l i m i t e d t o understanding t h e pathophysiologic mechanisms which c o n t r i b u t e t o abnorma l i t i e s encountered i n t h e pregnant d i a b e t i c and h e r o f f s p r i n g . Research is encouraged on d e t e c t i o n of pre-diabetes i n t h e female populat ion of ch i ldbea r ing age. Funct ional d e f i n i t i o n of optimal d i a b e t i c c o n t r o l during pregnancy i s needed as w e l l as determinat ion of t h e opt imal t i m e and method of d e l i v e r y as r e l a t e d t o maternal and f e t a l s t a t u s , and t o t h e i n f a n t ' s a d a p t a t i o n t o e x t r a u t e r i n e l i f e and maintenance of homeostasis. I n v e s t i g a t i o n s are requested t o e x p l a i n t h e a b e r r a t i o n s i n embryogenesis a s s o c i a t e d wi th t h e i n t r a u t e r i n e environment i n d i a b e t i c pregnancies as w e l l as s t u d i e s focused on developing b e t t e r techniques t o d e t e c t f e t a l d i s t r e s s . S tud ie s should b e d i r e c t e d toward c l a r i f y i n g mechanisms r e spons ib l e f o r congen i t a l malformations, s t i l l b i r t h s , neona ta l d e a t h s , and p e r i n a t a l complications. Areas of p a r t i c u l a r i n t e r e s t i nc lude developmental s t u d i e s of intermediary metabolism and hormonal i n t e r a c t i o n s i n o f f s p r i n g of d i a b e t i c , g e s t a t i o n a l d i a b e t i c , and nond iabe t i c women. Also of i n t e r e s t are s t u d i e s on t h e impact of prepregnancy d i a b e t e s and g e s t a t i o n a l d i a b e t e s on t h e morphology, func t ion , and metabolism o� t h e p l a c e n t a and on u t e r i n e blood flow.

Prevent ion of d i a b e t e s m e l l i t u s i n g e n e t i c a l l y predisposed c h i l d r e n is a l s o a major concern. S tud ie s of childhood feeding p a t t e r n s and t h e development of d i e t a r y h a b i t s , tastes, and a p p e t i t e are r e l e v a n t , as are sea rches f o r o t h e r p r e s e n t l y unrecognized developmental antecedents of d i a b e t e s and obes i ty . The CRMC a l s o seeks s t u d i e s of i n f a n t s and c h i l d r e n born of d i a b e t i c women which emphasize t h e i r phys io log ic , anatomic, o r developmental aber­r a t i o n s . I n t e r e s t a l s o e x i s t s i n exp lo r ing t h e even tua l e f f e c t of t h e d i a b e t i c i n t r a u t e r i n e environment on growth and development of t h e o f f s p r i n g .

SIGNIFICANCE TO NICHD AND CRMC PROGRAM GOALS An important goa l of t h e CRMC is t o understand mechanisms underlying normal growth and development and t h e i r d i s t o r t i o n by t r a n s i e n t o r chronic even t s such as d i s e a s e o r f u n c t i o n a l d i s o r d e r i n t h e mother o r i n t h e f e t u s . Maternal (diabetes h a s had major e f f e c t s on p e r i n a t a l m o r t a l i t y , on t h e outcome of pregnancy, and on i n f a n t morbidity. This metabol ic d i s o r d e r may a l s o i n f l u e n c e d i e t a r y h a b i t s and somatic cond i t ions which could r e s u l t i n obes i ty . Developing new knowledge

about t h e complex processes by which maternal d i s e a s e o r dysfunct ion may a f f e c t t h e long-term h e a l t h of t h e mother, h e r r ep roduc t ive a b i l i t y , and t h e well-being of h e r o f f s p r i n g is a major concern of t h e Center f o r Research f o r Mothers and Children.
