• NorWest Co-op has served the Inkster community since 1972, taking a multi-service, holistic, team approach to health care.

• NorWest is the only health cooperative in Manitoba. Members of NorWest play an active role in deciding the future of the co-op.

• HANS KAI is currently available in French and English. HANS KAI was developed for a Canadian context in partnership with Robert Cliche Health Co-op in Beauceville Quebec. Robert Cliché and NorWest have worked to standardize the HANS KAI program

What is Hans Kai?

• Small groups that meet at minimum once monthly to support one another in making healthy lifestyle choices.

• Groups participate in 4 key activities – preparing healthy food/snack, engaging in physical activity, checking specific health indicators, and socializing and supporting each other.

• Groups attend a health school to learn about Hans Kai, be provided with the education, tools and resources to run their own group.

• Groups are leaderless and are self directed (with support from Hans Kai team as needed).

Hans Kai Background – How it all Started

• NorWest participated in a co-op tour of HANS KAI in Japan in 2007.• HANS KAI started in Winnipeg in 2009 with inviting key stakeholders such as

Winnipeg Regional Health Authority, City of Winnipeg, Get Better Together, Universities etc… to an information day about HANS KAI. These stakeholders formed a steering committee that continues to advise the program.

• Health School and HANS KAI standardized curriculum and tools/resources were developed with best practice models and in collaboration with Robert Cliché Health Co-op.

• Research is provided by an external company and has received ethics approval.• National Network currently being formed to oversee HANS at a national level.

Hans Kai HANS KAI Implementation Plan• Identification and outreach of interested participants.• Recommendations for potentially interested groups from the steering committee

members and other partners.• Letters explaining HANS KAI (HANS) and its benefits are sent to local businesses.• Newsletter and poster advertising throughout the community.• Health fairs, community events and neighbourhood gatherings include interactive

activities and information designed to introduce HANS KAI (i.e. Bike blenders to make smoothies, blood pressure and glucose checks, physical activity games or chances to try new activities, general information, healthy snacks).

• Personalized outreach for golf tournaments etc…

Key Objectives of Hans Kai 1. To create social networks that support personal wellness. 2. To increase awareness of the connection between personal wellness and

healthy lifestyle choices, and those factors within and outside of their control.

3. To increase one’s ability to take action to improve health. 4. To develop peer support and leadership skills to encourage others to

make healthier lifestyle choices across all sites. 5. To reduce social isolation by developing connectedness with others. 6. To maintain or improve measurable health indicators. 7. To increase access to primary care services and linkages to community

programs and services . 8. To collaborate and share resources and lessons learned with other

organizations interested in creating HANS groups in their communities.

Hans Kai Health SchoolsHealth Schools• Health Schools are for participants interested in joining Hans – it prepares them for their

group meetings after completing the health school. • HANS team members schedule facilitators for the Health School. One HANS staff

(preferably 2) is assigned to be the main lead and consistent contact at each health school session to develop the relationship, assist with continuity and ensure the participants have their needs met. The staff(s) also liaise with the other presenters and participants to ensure all needs and expectations are communicated.

• Health Schools are 8 sessions. (2 hours each, can be adapted for shorter time frames).

Session 2 - Health Indicators

Learning ObjectivesParticipants will…1. Understand metabolic syndrome and risk factors2. Identify factors that affect blood pressure, understand normal and high blood

pressure values and when to report to their health care provider 3. Understand the differences between the different types of diabetes and risk factors4. Be aware of healthy blood sugar ranges5. Be able to check blood pressure, blood sugar and waist circumference and know

when to report their numbers to their health care provider6. Learn about risks associated with large waist circumference Tools: Toolkit with glucometer and supplies, blood pressure monitor, tape measure,

scale, PowerPoint, blood glucose waiver, handouts with instructions for health indicators and values of each

Session 3 - Eating for Health

Learning Objectives: Participants will….1. Understand how to use Canada’s food guide (in life and for planning HANS snacks)2. Understand key messages in Canada’s food guide (4 food groups, serving sizes)3. Know guidelines for food safety4. Learn about fibre, fats, sodium and other nutrients5. Able to read a food label and determine if there is a ‘little’ or a ‘lot’ of a nutrient in

the food product i.e. fat6. Recognize the importance of nutrition for healthTools: PowerPoint or jeopardy game, Canada’s food guide, snack list, food models and

labels, portion plate, arteries with plaque

Session 4 - Lets Get ActiveLearning Objectives:Participants will...1. Understand the benefits of being physically active2. Understand the barriers to activity and how to overcome them3. Demonstrate correct exercise techniques4. Understand current health status before beginning an exercise program and how to reduce risk of injury5. Understand the 5 determinants of activity: cardio, body, composition balance, flexibility, and strength

Session 6 – Primary Care Through the Years

Learning ObjectivesParticipants will…1. Know about local over the phone health service supports (Health Links)2. Understand when to get periodic medical exams through the lifecycle3. Understand how to prepare for common lab tests4. Know the meaning of different lab tests, when and why you may go for those tests5. Understand when and why medications may be needed for certain health

conditions6. Understand health risks of smoking or exposure to second hand smoke

Session 7 Working Together Learning ObjectivesParticipants will…1. Brainstorm activities they may like to do in their Hans group2. Understand cooperative values and how they relate to Hans3. Identify optional sessions (guest speakers) available to support their group4. Be able to use the consensus model to make decisions as a group5. Be aware of their own boundaries, group boundaries and how to handle any

conflict if it were to arise in the group6. Gain skills to have effective time management at their group

Hans Kai How it Works

• Groups meet at least once every month for 2 hours.

• Groups are self directed with participants having an active role in how their group runs.

• After the 8 session health school, HANS groups receive their tool kit of resources.

Hans Kai HANS KAI GROUPS• Groups are now meeting on their own at a mutually agreed upon time and

location.• The groups are functioning independently and are community led

(Leaderless as the groups make decisions by consensus)• Groups meet a minimum of once a month for 2 hours. At each meeting

members participate in physical activity, eat a healthy snack, have time to socialize, and check their health indicators (weight, blood pressure, blood sugar)

• Group members responsibilities include: picking a snack, shopping for the snack, reminder phone calls, bringing the tool kit and contact HANS team for supports when needed.

• HANS group members often mentor other groups in the start-up stage.

Hans Kai Ongoing Support• HANS staff are available to support the groups as needed with requests for

speakers on particular topics, activity requests, health school session refreshers, and more. Each HANS group is assigned a main contact with a back up for support.

• HANS staff also support the groups with ensuring the tool kit is stocked, support with health snacks if needed and ensuring the groups are aware of any changes to health information.

• HANS staff also assist the group if any group difficulties arises.• HANS staff provide mandatory motivation and personality sessions at the 3

month mark.• HANS staff follow up with the groups every 3 to 6 months to see how they

are doing if we have not heard from them.

Hans Kai Results and demographics- As of September 2013 there are 14 HANS groups in Manitoba- 11 Community and 3 Workplace groups- 2 Health Schools are currently in session - Number of participants in HANS: 89 (33 in Workplaces and 56 in

Community)- Average age is: 51.6 (42.6 in workplaces and 54.4 for community)- 84 women and 5 men- 46% have a chronic condition (workplaces 10% and community has

63% have a chronic condition)
