We want to acknowledge it.

President’s Message

Vanakkam.

Respected elders and my dear friends of Vellore IMA, I am humbled by the great responsibility bestowed upon me as the President of Vellore IMA. Vellore IMA has a very long list of great State presidents. Dr.T.Kumaraguru was my father figure and my mentor. Dr.P.Kesavan is my guiding force. Dr.T.Sadagopan and Dr.S.Damodaran are my pillars of strength in IMA. Dr.L.Krishnamurthy, Dr.C.S.Palani and many more seniors are my guiding forces. The previous team headed by Dr.K.Sendan- immediate past president, Dr.Madan and Dr.Balachander did a fabulous job.

Friends, its only because of my dear friend Dr.B.Kumaran that I have become the president of Vellore IMA. He was gracious to accept the post of Secretary to work with me for the goodness of Vellore IMA. Dr.Chandramohan has cut out his task of managing the finance of Vellore IMA.

We are planning many activities in the forth coming year apart from the regular CME. We have decided to go green by planting tree saplings in Government schools in Vellore and kaniyambadi taluk choosing one school every month.

I encourage the members to actively participate in the branch activities for the betterment of our society. The cycling club and the music classes started by Dr.Kumaran, the MSV music society of VELIMA by Dr.Madan are fine examples to find talents among our doctors.

Friends i need your support all through.

Secretary's Report for February 2017 d

Month of february started with Pongal Vizha celebration and Installation on 5th

sunday afternoon. We had Painting competition for children on Jallikattu topic. The response

was excellent. Dr. ASHA had conducted games for children and adults. Tamil Pattimanram on

Influence of smart phone on youngsters with DR.Ravishankar, President of TNSB as a judge

was the highlight. Doctors who participated were. Dr.C.S.PALANI, Dr.VENKATRAMAN,

Dr.JASMINE MUSTAFA, Dr.BALACHANDER, Dr.ARULPARI, Dr.MANJULADEVI,

DR.PASUPATHY and Myself. The Miming skit done by Vidya Nikethan school children of

Ranipet was applauded well by the audience. We sincerely thank Dr.VIJAYARANI, RANIPET

IMA ,PAST PRESIDENT for arranging the skit.

The installation of new office bearers 2017 was presided by Dr. RAVISHANSKER,

President, IMA,TNSB. Dr.ARULPARI took over as President of Vellore IMA 2017. Past state

presidents of IMA,TNSB, Dr.KUMARAGURU, Dr.DAMODARAN, Dr.KESAVAN were felicitated.

Dr.Ikram was honored for his Prestigious award from the Chief minister.

We had hosted IMA NURSING HOME BOARD MEET, North zone on 19th Feb, along

with Paramedical meet. The venues were at Vellore ima hall and Kumaran Hospital. The

meeting was presided by our State president , in presence of DR.RAJA, Sec of IMA NHB. All

diginitaries were honored with MaraKandru .More than 100 participants from different

nursing home Board in north zone participated , and first time the programme was web

telecasted in IMA NHB website.

We had ENT and diabetic Medical camp at Pogia on 21st of February. More than 30

new diabetics were identified and 15 hearing loss patients diagnosed.

Talk on the importance of antenatal care and newborn care was given by Dr. Anitha

and Dr. Narmada respectively. Importance of yoga in pregnancy to antenatal mothers was

enlighted by Dr. Nandini. A lively discussion was held with AN mothers and their doubts were

cleared. About 50 AN mother's attended the program. Antenatal care and baby care

booklets were issued to the mother's. On behalf of Women's wing I would like to thank Dr.

Mangaiyarkarasi medical officer in charge of Poigai PHC for giving us the opportunity to

conduct the program. A special thanks to our branch president and Secretary for their

support.

SWINE FLU – A REVIEW - Dr. Sathish Kumar, MD.,

Good morning from Vellore District Chapter of Associations of Physicians of India.

There is increased incidence of H1N1 in the last few days in and around Vellore. As a

Physicians group we thought we will share a short guideline on how to go about H1N1. This

guidelines is based on guidelines issued by our Ministry of Health. If you have any doubt or

clarifications, you can call any of your neighbouring physician for advice. Why are we

having another review on Swine flu?

● Why are we having another review on Swine flu? Confirmed patients of H1N1 are being reported throughout India and more new

cases are being detected in India everyday. Added to that, people are more aware of the

disease so much so every day we have patients asking at least once whether the disease they

are having could be a Swine flu. Many of us, doctors have many unanswered queries and

wrong notions that need to be cleared. That is the reason for this review.

● What is Swine flu? First thing we should know is that the name is a misnomer (Misnomer means an

interpretation that is known to be false). Why? Swine influenza in reality signifies the specific zoonotic flu that spreads almost

exclusively in pigs. It spreads to humans who are closely associated with pigs like pig growers

and slayers. Inter-human spread is very rare. Less than fifty human infections of this virus are

known. It does not cause any significant disease in humans.

● Has this virus been identified before in humans? Yes. H1N1 influenza was the cause of famous Spanish flu in 1918-1920, causing

about 100 million deaths.

● Is this virus causing that kind of devastation? No. Look at the following figures:

Spanish flu (H1N1) 1918-1919 affected 500 million people and lead to the death

of nearly 100 million people.

Seasonal flu – every year – nearly 1 billion people affected – 500,000 deaths a

year.

Swine flu 2009-2010 – 620,000 confirmed cases – 14,000 confirmed deaths. So

we can see, it is not as deadly as we were made to believe. It is less potent than a

seasonal flu which many of us get every year.

● The basics: The influenza viruses are basically RNA viruses belong to the family of

orthomyxovirus. There are three genera of influenza viruses - A, B & C. The present epidemic

is by a strain of Influenza A virus called H1N1. The influenza is a common viral disease. In

India, it usually spreads during the monsoon season.

● The natural course: The influenza viruses spread mainly through respiratory droplets. Occasional

transmission through fomites has also been proven. When a patient acquires a disease, after

an incubation of 7-10 days, they get symptoms of the disease. The symptoms include body

pain, fatigue and respiratory symptoms like nose block, runny nose, sore throat and non-

productive cough. Then they develop a sudden episode of a high grade fever with chills.

Usually the fever subsides within 2-3 days after which the cough, fatigue and body pain lasts

for a variable period of time. Usually the disease is not fatal unless the patient is

predisposed. These predisposed patients are the high risk patients who need active antiviral

treatment. For this Ministry Of Health, India has issued a guideline:

Category- A Patients with mild fever plus cough / sore throat / with or without body ache,

headache, diarrhoea and vomiting will be categorized as Category-A. Usual non-influenza

respiratory infections do not have GI symptoms while flu patients can have GI symptoms.They

do not require Oseltamivir and should be treated for the symptoms mentioned above. The

patients should be monitored for their progress and re-assessed at 24 to 48 hours by the

doctor.

No testing of the patient for H1N1 is required. Patients should confine themselves at home and avoid mixing up with public and high risk members in the family. Category-B

i. In addition to all the signs and symptoms mentioned under Category-A, if the patient

has high grade fever and severe sore throat, may require home isolation and

Oseltamivir;

ii. In addition to all the signs and symptoms mentioned under Category-A, individuals

having one or more of the following high risk conditions shall be treated with

Oseltamivir:

● Children with mild illness but with predisposing risk factors. ● Pregnant ● Persons aged 65 years or older ● Patients with lung diseases, heart disease, liver disease, kidney disease, bloodd

disorders, diabetes, neurological disorders, cancer and HIV/AIDS ● Patients on long term cortisone therapy. No tests for H1N1 is required for Category-B (i) and (ii). All patients of Category-B (i) and (ii) should confine d

themselves at home and avoid mixing with public and high risk members in thed

family. d

Category-C In addition to the above signs and symptoms of Category-A and B, if the patient has

one or more of the following:

● Breathlessness, chest pain, drowsiness, fall in blood pressure, sputum mixed with

blood, bluish discoloration of nails

● Children with influenza like illness who had a severe disease as manifested by the

red flag signs (Somnolence, high and persistent fever, inability to feed well, convulsions,

shortness of breath, difficulty in breathing, etc)

● Worsening of underlying chronic conditions. All these patients mentioned above in

Category-C require testing, immediate hospitalization and treatment.

● How to recognize? To tell it shortly – you cannot differentiate influenza clinically from any other

pathogen causing similar symptoms. Investigations are the only way you are can tell with any

assurance that this is influenza. In seasonal influenza seen in non-tropical countries, 60% of

patients with a triad of fever, respiratory symptoms (nasal symptoms, sore throat and cough)

and systemic symptoms (myalgia, malaise and fatigue) have influenza. But if you see in a

place like India, where the spread of this disease is not restricted to any particular season, it is

near impossible to tell who is having a viral flu and who is having a bacterial respiratory

infection.

● How to confirm? Currently there are two types of tests available – a screening and a confirmatory test

like in HIV detection.

● Labs available to confirm: ○ CMC & GH ○ Hi-tech lab

● How much does it cost? Rs 3,750

● How to treat? Patients, who need treatment, have to be given Oseltamivir. This is available in only

designated treatment centers. In Vellore, it is CMCH and Government hospital,

Adukamparai. This has been purposefully restricted by government to these centers and

medicine not available in open market - to prevent artificial demands, private stockpiling of

drugs and to prevent treatment of unnecessary people leading to drug resistance. The

dosage in adults is 75mg twice a day for five days. The drugs can be obtained from the local

Government Hospital with a prescription request.

● Are their vaccines to prevent H1N1 influenza? Two types of vaccines are available:

○ Nasal : Given as inhalation. Gives similar lifelong immunity. Given for ages above 3 years.

Dosage 0.25ml in each nostril for children and 0.5ml in one nostril for adults. Should not be

given for patients with runny nose or asthma.

○ Injectable : Protects against seasonal flu as well as H1N1.For ages 6m to 3 years dosage is

0.25ml im and for ages >3y it is 0.5ml IM. Costs around Rs 750.

● Who needs the vaccine? Vaccines are needed for only people in high risk groups. It can be avoided in other

patients since for all others it causes a very mild disease especially with the pandemic

officially already ended.

● What do I do when I see a patient with fever, myalgia and cough? If the patients do not belong to the high-risk group mentioned above (Category B)

and is clinically stable as is most of the patients we see, just advise them to take rest and give

symptomatic treatment with oral hydration and Paracetamol. Ask the patient to come for

follow-up. If the condition worsens, refer the patient to one of the designated treatment

centers, since referring to any other hospital is going to be useless. On the other hand, if

patient is sick at the first visit or belongs to one of the high risk category, refer at the first visit

itself.

If the patient wants investigations in private, explain to him / her that even if he /she can

afford that, treatment is possible only in the designated centers. Not only that, the patient

have to go to Chennai to do the test, since the nasopharyngeal swab needs to be

transported in a viral transport media in a refrigerated container if taken outside

Chennai. It is going to be cumbersome and increases the chance of false negativity of the

test.

Summarizing: ○ The Swine flu we are seeing should be correctly called H1N1 influenza. ○ Clinically impossible to differentiate. ○ There is no need to panic as it causes mortality in only high risk patients. ○ Tests and treatment are required only for high risk and sick patients. ○ Vaccines available can be given to high risk patients.
