9TH GRADE TOOL KIT

Transition begins officially for students with special needs at age 14, but the process begins with the onset of education.

The goal is to provide students with tools they will use beyond school for; independent living, work, or furthering their education.

WHO QUALIFIES

• Students must be enrolled in a school as a freshman

• Legally blind (20/200 in best eye corrected or peripheral vision less than 20 degrees)

• Registered in Federal Quota program

• Have an active IEP

KIT PARAMETERS

The tool kit must be non- consumable

Must be an APH Federal Quota product

RESPONSIBILITIES

The parties to this contract work together; CSMT, schools, and the individual student

Each has the responsibility to help the student earn the tool kit

CSMT will…..

• Provide the tools for the students

• Track the effectiveness of the program;

1. create a data base of tools

2. survey the tool use

3. work to explore other funding options

SCHOOLS WILL …..

• Advise students and their parents of the program

• Hold IEP meetings to discuss the tools for each individual

• Set the criteria for either graduation or certificate for each student

• Notify CSMT of the tool requests• Train the student to use and maintain

the specific tool(s)

STUDENTS will …..

requirements set by the school

TRAINING

Training includes learning to maximize the use of the tool;

• All applications of the tool

• Understand the manuals (if applicable)

• Learn to use the tool effectively

MAINTAIN

• To keep the tool in good working order all recommended usage and storage will be advised

• Cleaning and proper use will be discussed

• Information on upgrades or parts will be made available

Upon meeting the criteria in the IEP for the Tool Kit the student will be given the tools.

DOLLAR AMOUNT

Generally, the amount will be set at $3,000 per student.

The amount will depend on the student needs, and the cost and availability of the tool requested.

Only products already approved for Federal Quota purchase are currently available.

DOCUMENTATION• The local IEP team keeps all the

contract paperwork

• We provide a sample contract

• CSMT trusts that you are requesting kits only for authorized students (we need the name of the student to verify they are enrolled in the Federal Quota registration)
