Target Group 255 deaths in Macon County in 2006
11 died due to complications of diabetes Nurse Practitioner explained that around
80% of the patients she sees have type 2 diabetes.
Target age range: 40-80 year olds

at high risk for diabetes with pre-diabetes by increasing levels of activity, weight loss, and reducing the amount of fat and calories in the diet”
Community-oriented health diagnosis: Risk of development of type 2 diabetes
among 40-80 year olds in Notasulga, Alabama related to lack of healthy eating habits.
Objectives To identify the population in Notasulga
most at risk for developing type 2 diabetes or complications from type 2 diabetes
To increase prevention behaviors through educating the population on making healthier eating choices within a budget

center told us that the majority of the people present already had type 2 diabetes
Seemed irritated that we were interrupting their Bingo game “I’m not going to learn anything I don’t already
know.” Based on the feedback received by
participants: Processed knowledge and asked intelligent
questions Provided real-life situations
Future Recommendations Because the main meal the seniors eat is
provided by the Senior Center through the government, we would suggest taking this factor into account when planning future interventions
Because of the overall age of our audience, a program could be planned to increase exercise and an active lifestyle for seniors.
These seniors could also benefit from knowledge about different disease processes
