Recommendations for Surveillance of Clostridium difficile Infection (CDI) 1. A patient classified as having Healthcare Onset- Healthcare Facility Associated (HO-HCFA) CDI is defined as a patient with CDI symptom onset > 48 hours after admission to a HCF. 2. A patient classified as having Community Onset- Healthcare Facility Associated (CO-HCFA) CDI is defined as a patient with CDI symptom onset on admission to a HCF or within the 48 hour window, and symptom onset was < 4 weeks since the last discharge from a HCF. 3. A patient classified as having Community-Associated (CA) CDI is defined as a patient with CDI symptom onset on admission to a HCF or within the 48 hour window, and symptom onset was > 12 weeks since the last discharge from a HCF. 4. A patient classified as having indeterminate disease is defined as a CDI patient who does not fit any of the above criteria for an exposure setting—for example, a patient who has CDI symptom onset in the community but who was discharged from a HCF 4-12 weeks before symptom onset. 8/12/2008 Adapted from CDI Surveillance Working Group. Recommendations for Surveillance of Clostridium difficile Infection(CDI)

5. A patient classified as having unknown disease is a CDI patient for whom the exposure setting cannot bedetermined because of lack of available data—for example, a patient who has CDI symptom onset in the communityor 48 hours or less after HCF admission and for whom available medical records are not sufficient to exclude dischargefrom an HCF 12 weeks or less before symptom onset.
