Do you ever ask yourself in a Board Meeting… – Where and how could we be better? – Have we done everything possible that a typical well
functioning Board should be doing? – Are we being accountable and using our funds in the best
possible manner with the necessary checks and balances? – Why does our Board always seem to go in circles on every
issue we discuss? – Are we ready and well prepared for any challenge?
Now imagine a future PCN Board that…. – That knows exactly where any “gap” exists and can
pinpoint what needs to be improved. – Has “peace of mind” knowing that all risks have been
identified and a has a plan with support to address. – Can pinpoint problem governance issues ( e.g. conflict
of interest) while those issues may still be small versus a “festering board wound”

The Board Health Check-up Tool:
• We will be using AC’s Governance Functional Tool which is one of the unique instruments included as part of Accreditation Canada’s Qmentum accreditation program

• This is NOT accreditation but utilizes the same “process” and methodology Accreditation Canada uses within the “governance” portion of their accreditation process.
• The PCN “Board Governance Health Check-up Tool” will take advantage of the AC ‘s Governance Functional Tool but be tailored to fit PCNs (the tool will be relevant to the PCN setting)

The Process: How does your PCN Request a Review? • Participating PCNs will contact Accreditation Canada for
individual contract (participation for up to 3 years) • PCNs will provide contacts for all Board Members and
determine timelines for the survey • Accreditation Canada will administer the survey, gather
data and provide analytics on a yearly basis • Additional services: Simulation survey (accreditation interview) of Board
members, coaching and recommendations PMP can work with you to work on gaps and improvement
areas

to submit by the end of the Forum weekend (hand-in at PCN Registration Desk)
