ENGINE DRIVEN MOBILE EQUIPMENT SAFETY

1. INTRODUCTION

An enormous range of mobile equipment are used in the refinery. Important points in their use include: selecting the right equipment for the job, making sure equipment is safe to use and keeping it safe through regular maintenance, inspection and, if appropriate, thorough examination, training employees to use equipment safely and following manufacturers' or suppliers' instructions. Accidents involving work equipment happen all the time - many serious, some fatal. However with proper training and usage these can be avoided.

2. SCOPE

This document is intended to cover safety aspects concerning control of commonly used engine driven mobile equipment entering/ operating inside KNPC premises including special features and precautions required for use in hazardous areas. This document supercedes Chapter-18 (Rev 1, Sep 02).Traffic safety aspects of vehicles are covered in the document: Traffic Safety Guidelines. Certain equipment like Welding machines, Vacuum Trucks, Hoists, Forklifts, Mobile cranes etc are not covered in this chapter as they are covered elsewhere.

3. DEFINITIONS

For convenience, following two classes have been adopted as referred

elsewhere in this document. The list gives examples only.

New equipment: means brand new one within first year of registration (Govt

4. ENTRY TO KNPC RESTRICTED AREA

4.1. GENERAL REQUIREMENTS

1. Equipment shall be in good condition to meet the requirements of

Kuwait Govt. Traffic Dept and KNPC Safety Checklist (Appendix-C).

2. The number of equipment shall be kept to the minimum essential for

the site personnel and work requirements.

3. The equipment shall be provided with the right type and size of fire

extinguisher certified by KNPC Fire Section (See Appendix-E).

Extinguisher shall be properly mounted at an easily retrievable

location.

4. Fire Extinguishers kept inside operator cabins, shall be securely

mounted so as to prevent falling down during movement.

4.2. SPARK ARRESTOR

1. Approved type spark arrestor shall be fitted properly to the engine

exhaust of equipment entering KNPC Restricted Areas. Spark arrestor

shall have quality markings such as USDA Forest Service or other

documentary proof of quality.

2. User/ User Division shall frequently check the spark arrestor and

maintain it in good condition.

3. Equipment entering to attend or contain an emergency can be

exempted with the permission of Refinery Shift Leader.

4.3 EQUIPMENT SAFETY CERTIFICATE

1. Contractor or the user shall ensure the equipment is in good condition

and submit the filled safety checklist signed by the concerned

supervisor (Appendix-C) before forwarding for certification or

renewal.

2. KNPC I&C Division or KNPC Approved Third Party Inspection shall

witness and certify the Visual Inspection and Load Test/ Bench Test

for KNPC owned or leased equipment/ Safety valves as and where

applicable. Certification of the mechanical and electrical components

of these equipment shall be done by the Work Shops & General Works

of KNPC. Certificate issued at one KNPC location is valid for any other

KNPC location.

3. Workshop & General Works Division shall certify KNPC owned or leased

equipment.

4. Safety Certificate issued by a KPC company (eg:- KOC) shall be

considered valid for equipment belonging to that company (not

contractor’s).

5. Road Tankers entering KNPC restricted area for loading / unloading

products shall meet requirements of Appendix F: Safety Checklist for

Product Road Tankers. Third Party Inspection Report shall meet

requirement of KNPC I&C Manual IC 42.

6. Inspection for first issue of Safety Certificate is usually done outside

the restricted area gate while facility for renewal is available within

premises. Users shall check compliance for requirements before

offering for inspection.

7. Validity: For all types of equipment, Safety Certificate will be issued

for a period of 3 months. For renewals, expiry date will be counted

from the current expiry date. For new equipment this shall be 6

months.

8. Third party certificate if current shall be valid beyond the expiry of

Safety certificate. Safety Certificate becomes invalid if the expiry

date of registration and fire extinguisher has elapsed. Owner/user

shall be responsible for timely renewal of above.

9. The safety certificate (or copy) shall be available with, or displayed

on the equipment.

10. KNPC Security shall check for valid safety certificate when issuing long

term (more than one day) security gate passes.

11. Contractor or the user is responsible for maintaining the equipment in

"as certified" condition during the validity of the certificate. If there is

a change, certificate become null and void and, it shall be re-

certified. Users shall be responsible for timely renewal.

12. User's supervisor shall ensure that the equipment operator checks and

signs a checklist made as per equipment operating manual each day

before initial use.

4.4 THIRD PARTY CERTIFICATE

Following equipment shall have a valid test certificate before applying for

a safety certificate. KNPC Inspection or a KNPC approved third party shall

certify KNPC owned or leased equipment. Contractor equipment shall have

KNPC approved third party test certificate. Third Party Inspection Report

shall meet requirement of KNPC I&C Manual IC 42. User Division shall renew

these timely.

1. Lifting Appliances - crane, boom truck, relamping truck etc yearly test

certificate fulfilling requirements of IC-16.

2. Safety valves on mobile equipment like air compressor - yearly test

certificate fulfilling requirements of IC-10.

3. Cargo tanks (atmospheric pressure) of petroleum product road tankers

shall be inspected as per IC 42 (Inspection Procedure for Petroleum

Products Road Tankers). Pressure relieving devices on these road

tankers shall be inspected & tested every 2 years.

4. Inspection of portable gas containers (LPG, Nitrogen, Butane, Propane,

Chlorine etc) shall be done as per IC 17 (Inspection of Portable Gas

Containers) and pressure relieving devices on these shall be inspected

and tested once in 5 years (max) or as per manufacturer’s

recommendation whichever is lower. Note: Air receivers for mobile

compressors shall also come under this category.

4.5 ENTRY TO HAZARDOUS AREA

1. Engine driven mobile equipment shall not be allowed to

enter Division-1 Areas. Use of gasoline or gas powered

equipment/tools shall be minimized in Division-2 areas due

to higher spark risk.

2. A Hot Work Permit is required for entry of mobile equipment

into hazardous areas (see Safe Work Practices on Work

Permit System & Risk Assessment). Such areas shall be

clearly demarcated with signs indicating “No Entry of

Vehicles without Permit” and/or with removable chain

barriers.

3. A Work Permit is required for unloading of chemicals like

sulfuric acid, caustic soda etc. However a permit is not

required at product loading facilities or filling stations (oil

pier, bitumen/LPG/coke/sulfur loading terminals, petrol

stations, etc).

4. The executor shall ensure that all equipment are equipped

with exhaust spark arrestors of approved type and provided

with fire extinguishers certified by the fire station.

5. Executor shall forward valid Safety Certificate (or copy)

when applying for permit.

6. Permit Issuer shall verify the validity of the safety certificate

(Appendix-B) and, indicate its number on the permit.

7. Authorized gas tester shall ensure 15m radius around the use

or travel route is gas free.

8. Other general requirements for hot work shall be applicable

(see Safe Work Practices on Work Permit System & Risk

Assessment).

9. Driver should be given clear instructions on which route to

follow and the field operator initial shall be obtained on the

work permit before entering hazardous area.

4.6 REFUELING AT SITE

1. Refueling shall not be done while the engine of the equipment is

running. Ensure no hot work or source of ignition within 15m radius.

2. Fuel for refueling shall not be stored inside the KNPC premises

without KNPC Safety approval.

3. Safety instructions for refueling shall be displayed on the container (or

laminated).

4. Transfer of fuel from one engine's fuel tank to another shall not be

permitted.

5. Bonding and grounding shall be done before refueling.

6. Fuel shall not be transported in vehicles or containers not approved

for that purpose.

7. Equipment shall not be refueled in hazardous areas.

5. MOBILE EQUIPMENT: GENERAL SAFETY CONSIDERATIONS

1. Only trained and authorized operators shall be permitted to operate

the designated equipment. All Crane Operators shall be trained and

certified by KNPC approved third party agency.

2. Operators shall thoroughly review and understand information provided

in the operators manual with special attention given to description of

safety procedures. Follow instructions in caution stickers.

3. Entry to hazardous areas shall be as explained in 4.5.

4. The altering or disabling or interlocks or other safety devices is

prohibited.

5. Ensure that equipments intended to carry loads are not overloaded.

6. Never carry passengers on mobile equipment.

7. Operators shall carryout a pre-use inspection of the equipment. A

general checklist is given in Appendix A, however this will vary with the

type of the equipment. Necessary modifications to the checklist shall

be made in line with the Operating Manual.

8. All motorized mobile equipments shall have automatic reverse beepers.

9. Operators shall have mandatory Personal Protective Equipment. The

noise level at the operator's ear may exceed 85 dB(A); operator to wear

personal ear protectors in case of noise levels of 85 dB(A) and more.

10. Never open hot radiator cap. Attempt to open slowly only when the

radiator is cooled. Use a wet cloth and keep face away.

11. Batteries shall not be changed in hazardous areas.

12. Inspect hydraulic line connectors and hoses for leaks before applying

pressure to the system. Use paper or cardboard, not your hands, to

search for leaks. Keep in mind that hydraulic fluid escaping under

pressure can penetrate skin and cause serious bodily harm.

13. Check tires for cuts, bulges, irregularities, abnormal wear and proper

inflation.

14. A first aid kit shall be mounted in the cab.

15. Machine Maintenance:

a. Ensure the cab area is clean and free of debris and tools.

b. Clean windshield, mirrors and lights.

c. Remove all oil, grease or mud and snow from grab irons, hand

rails, steps, pedals and floor to prevent slips and falls.

d. Remove or secure any loose items such as tools, chains or lunch

boxes from the cab.

16. A preventive maintenance program shall be developed for each

equipment according to the manufacturer’s recommendation and

based on the environment and severity of use. Maintenance log book

shall be maintained to track history of the same.

17. Know work area clearances - watch for overhead or surface objects –

height barriers, lamp posts, overhead pipe racks, subsidence, holes,

drop offs, and partially hidden obstacles and wires.

18. Mount properly:

a. Do not get on or off a machine that is in motion

b. Maintain a 3-point contact with the steps and hand rails while

getting on/into the machine - do not use the controls or steering

wheel as a handhold.

c. Do not operate the machine with wet, greasy or muddy hands or

shoes.

19. Starting and Testing:

a. Exhaust fumes are dangerous – always have a running machine in

a well ventilated area.

b. Fasten your seat belt and adjust the seat prior to starting.

c. Controls should be in neutral and the parking brake set before

starting engine.

d. Start the engine only from the operator's seat.

e. Warn personnel in the area that you are starting the engine.

f. Check all gauges, light, instruments and warning devices to

assure that they are functioning properly and the readings are

within normal range.

g. Test steering, right and left.

h. Test brakes against ground speed to be certain there is no

malfunction.

i. Ensure all controls are operating properly.

20. Machine Operation

a. Operator shall familiarize with the controls before operating

the machine.

b. Only the operator is permitted to ride on the machine.

c. While backing up use extra care and sound the horn to clear

the area.

d. A signal man shall guide the operator while reversing

equipment like Nitrogen Trailer, Garbage Truck, Dump Truck,

Concrete Truck etc.

e. If a malfunction is observed, stop and do not operate until the

proper repairs have been made.

f. Drive or operate at speeds compatible with working conditions.

g. Do not ride on neutral gear downhill. Select a gear that will

prevent excessive speed when going downhill. Do not park on a

steep incline.

h. Drive at a speed at which it can be stopped safely.

i. Do not permit anyone to stand or pass under the bucket or lift

arms. Barricade where possible.

j. Follow the manufacturer's load capacity limits. Identification

plates are attached to all machines.

k. If the machine is stuck, back it out or stop engine and get help.

l. Do not make mechanical adjustments while the unit is in

motion.

m. Always follow the manufacturer's recommendations for pulling

or towing.

n. Lower all the hydraulic equipment before shutting down or

getting off the machine.

21. Operating a Power Take Off unit (PTO).

a. Shut off the engine and wait until the PTO stops completely

before getting off, disconnecting or servicing the PTO unit.

b. Wear snug fitting clothing when operating the power take-off

or when near rotating equipment.

c. When operating stationary PTO equipment, always apply the

parking brake and block the rear wheels front and back to

prevent any unnecessary movement.

d. PTO shields are mandatory on all PTO-driven equipment.

22. Shut Down/Parking

a. Park on level ground.

b. When parking on a grade, position at right angles to the slope,

block the wheels and set the parking brakes.

c. When parking, lower all loader, buckets, hydraulics to the

ground.

6. EQUIPMENT

6. 1. NITROGEN TRAILER

Nitrogen trailers are required to enter units during start-up, shutdown and

emergencies. Trailer driver has to carefully reverse the trailer in between

units to reach the required location.

1. Entry of Nitrogen trailers shall only be on a hot work permit.

2. Only those trailer drivers who have been trained and certified shall

sign executing authority column. Authorization shall be limited only

to entry/exit and positioning of the trailer. (Note: Simplified and

customized work permit training may be arranged.)

3. The trailer shall have a spark arrestor, fire extinguisher and valid

safety certificate unless waived by the shift leader during

emergencies.

4. Trailer driver must be accompanied by a helper who shall guide the

trailer while reversing.

5. Permit for truck entry shall be closed before issuing a permit to give

connections. Likewise disconnecting permit has to be closed before

issuing a permit for taking empty trailers out.

6. 2. AIR COMPRESSOR

1. Some compressors are built with an internal cooling fan. When the

power switch is turned on, the cooling fan will start up and operate

continuously even when the pump has automatically shut off.

2. Turn the power switch to the "off" position when you are done using

your air compressor.

3. Do not direct air stream at body.

4. Keep in mind that some compressors may be set to start automatically

at preset pressures.

5. Do not touch moving parts. Keep all guards in place.

6. When the compressor is used to supply breathable air as in the case of

blasting jobs, precautions as mentioned in SHE Document: SWP on

Surface Preparation & Painting shall be taken.

7. Do not adjust a regulator to result in output pressure greater than

marked maximum pressure of the tool or attachment you're using.

8. Do not tamper the PSV mounted on the receiver. Do not operate

without proper safety valve in place.

9. Drain liquid from the air tank daily or after each use. Failure to

properly drain liquid from the air tank may cause rust from moisture

buildup, which weakens the tank and could lead to a violent tank

explosion.

6.3. TRUCKS

6.3. 1. GARBAGE TRUCK

General Safety - Safe Operation:

1. Only trained and authorized operators shall be permitted to

operate the designated equipment.

2. Stand clear when tailgate or packing mechanism is opened

or in motion.

3. Do not place hands or feet near any moving part or packing

mechanism while the engine is running.

4. Whenever the tailgate is in any elevated or raised position it

must be securely propped or blocked so it cannot fall.

5. Do not work on or near hydraulic pump, PTO, driveline.

universal or any rotating part while the engine is running.

6. Stop engine and remove ignition key before entering body.

7. Keep access door closed when ejector panel is in motion.

8. Do not drive or move the unit with the tailgate in any

position except fully down and locked.

9. Never attempt to disconnect any hydraulic line unless

pressure is released in the circuit and the machine elements

to which the hydraulic actuator is attached are supported or

blocked from movement.

10. Instruction and warning decals are important. Clean or

replace all decals if they cannot be read. Use them for

quick reference information.

11. Do not stand in the hopper or on the hopper sill while

adjustments are being made on the packing mechanism with

the machine running.

During repairs to the tail gate, packing mechanism, or hydraulic drive

system, a lockout procedure should be employed where the power shall be

shut off, the ignition key removed, and a tag out sign placed on the

steering wheel warning against any attempt to start the unit.

6.3.2. BOOM TRUCK

The boom truck is a truck with a telescopic lifting boom mounted on it.

General

1. Do not modify or attempt to modify the structure or operation of the

truck or boom.

2. A copy of the Operator’s Manual shall remain with the equipment at

all times.

3. Maintenance inspections.

a. Garage technician inspections.

b. Pre-operation inspections.

Before beginning boom truck operations, the operator shall

conduct a basic inspection of the boom truck. An inspection

checklist shall be kept in the cab of the truck. The checklist

shall document condition of the following items among other

things the boom for visual inspection for cracks, hydraulic leaks,

and other defects; function of lift controls

4. Report any unusual observation or occurrence during truck and/or

boom operation that may indicate required maintenance, repair or a

safety defect.

5. Before using the aerial device, the truck transmission shall be placed

in neutral (please check N or P), parking brake shall be set and the

outriggers set. The outriggers shall be positioned on pads or a solid

surface. Outriggers shall be fully extended irrespective of whether

load is being lifted or not. If outrigger footing is positioned onto

weak soil or mud, use boards, gravel, or whatever means are

necessary to obtain firm conditions. Use the following stability

guideline:

Firm conditions: concrete, cool asphalt

Weak conditions: bare soil, gravel, lawn, hot asphalt, similar

materials

6. Position the truck on as level a surface as possible before beginning

aerial device operations. If the truck must be parked on an incline of

12 percent or greater, wheels shall be chocked. Provided they can be

safely installed, wheel chocks shall be installed before using the aerial

device on an incline. If chocks cannot be safely installed, the boom

truck should not be used.

7. Operational Safety

a. Keep objects and personnel clear of aerial device operations at

all times.

b. After positioning and leveling the truck (truck must be

approximately level) and before beginning aerial device

operations, set up a perimeter system of cones or barricades

and caution tape for the purpose of keeping pedestrians away

from the work area

7. Before moving the boom truck for travel, the boom shall be inspected

to ensure that it is properly cradled and outriggers are in the stowed

position.

6.3.3. DUMP TRUCK

Truck whose contents can be emptied without handling; the front end of

the platform (box) can be pneumatically raised so that the load is

discharged by gravity. The main hazard is related to the stability of the

end-dump unit when the box is in the raised position.

1. When the center of gravity of box and load is not roughly between

the frame rails of the unit, there is a risk of tip-over.

2. Stability is adversely affected by one or more of the following

factors:

a. the unit is not on a level surface when dumping

b. a large amount of material is in the upper portion of the

raised box

c. material does not flow out of the top portion of the box, or

does not flow out of one side of the top portion

d. the rear wheels settle unevenly as the load moves to the rear

during dumping

e. wind may exert lateral loads, especially if the box is long, as

is the case with end-dump semi-trailers.

3. Stability may also be affected by the unit's mechanical condition:

a. poor rear suspension systems on one side of the vehicle

b. uneven tire pressures in rear wheels

c. worn or inadequate components of the lifting system such

as pins

d. worn or inadequate lifting cylinders.

4. Maintenance can play an important role in preventing tip-overs.

a. Check tire pressures daily. Tire pressures should be equal

on each side of the vehicle.

b. Examine and lubricate pins and bushings regularly.

c. Inspect suspension systems under load to ensure that they

work properly and provide even suspension. Weak

suspension systems should be replaced immediately.

d. Inspect hoist cylinders regularly. Worn cylinders should not

be replaced with smaller cylinders or with cylinders rated

at lower operating pressure.

e. Make sure that repairs to boxes leave bottom and sides

clear and unrestricted. Rough patchwork repairs near the

top of the box can catch and hold sticky materials.

5. Loading

a. Loading of the box front-to-back must meet allowable gross

weight and axle weight limitations. From side to side it is best

to load as evenly as possible.

b. If material is likely to flow poorly, lighten up the load in the

top end of the box. A slightly smaller load will be better than

a full load that causes a tip-over.

c. Box liners will help most materials flow better during

dumping. Liners also help to keep the box in good condition.

6. Dumping

a. Operators should be trained to recognize areas hazardous to

dumping, such as soft or uneven surfaces and inadequately

compacted fill.

b. Before dumping, operators should ensure that the tailgate is

unlocked and that the vehicle is on a reasonably level

surface. Dumping on surfaces that are not level is one of the

main causes of tip-overs.

c. Before spreading material by dumping it from a moving truck,

make sure that the entire length of travel is reasonably level.

d. Trucks should not dump when they are parked side by side

with another vehicle. When a dump unit tips over, it is often

the operator in the adjoining vehicle who is injured. Dumping

operations should be spread out.

e. Other personnel such as dozer operators, surveyors, and

spotters should be warned not to work near a dumping truck

in case it tips over.

f. Workers on foot should not congregate in areas where

dumping is under way.

g. When dumping wastes follow approved waste disposal

procedures.

6.4 BACK HOE

Back hoe is an excavator whose shovel bucket is attached to a hinged boom and

is drawn backward to move earth. Back hoe loaders (combination of back hoe

and front loader) can dig, scrape, load material, lift and with specialized

attachments do almost any required task.

1. Before starting and while operating a backhoe, look for people

or obstructions in the vicinity of the backhoe.

2. Employees working within an area where a backhoe is operating

must wear flagging garments (i.e., orange vests) and other

mandatory PPE.

3. Prior to starting, learn the locations of underground and

overhead utility lines, ditches, stumps, boulders, and other

hazards or obstructions in the work area.

4. Always extend the backhoe stabilizers prior to starting an

excavation task.

5. Always swing the backhoe arm uphill when excavating on a

hillside.

6. When transporting material in a loader bucket, keep the loader

bucket as low to the ground as possible to maintain backhoe

stability.

7. Only raise the loader bucket for the purpose of dumping

material.

8. Reduce speed when turning, crossing slopes, or driving on

rough, slick, or muddy surfaces.

9. Only dismount (or mount) a backhoe when the engine is shut

off.

10. Never adjust or work on a backhoe unless the engine is shut off

and hydraulic pressure has been de energized.

11. Be aware that braking conditions may vary due to mud, loose

gravel or other conditions. There are always obstacles on the job

such as rocks, stones and debris. Soft soil conditions exist

around edges and deep holes.

12. When operating the loader be sure the back hoe is secured in the

upright position and carry the loader bucket low, close to the

ground. This allows maximum visibility and stability. Stay in gear

when traveling downhill. Use the engine's RPM to help maintain

control.

13. When working on backhoe, take precautions. Have it safely

blocked before working under it or through the loader arms.

Work with the bucket down. Follow the manufacturer’s

recommendations for routine maintenance.

14. Before beginning the back hoe digging process, lower the

stabilizers so the wheels are off the ground leveling the unit.

15. Check the boom swings to avoid hitting anyone or anything that

may be in the boom path.

16. Remember, the Operator is permitted to operate the backhoe

loader only from the driver's position. Never when he is on the

ground.

17. Operators should never use a backhoe loader without an operator

cab that does not have Rollover Protective Structure (ROPS) or

Falling Object Protective Structure (FOPS)

18. When operating on a slope, be sure the unit is level with the

stabilizers. Swing loads uphill to avoid a balance problem while

on the slope and place soil well away from the edge of the

trench.

19. When loading dump trucks, the dump truck driver must be out of

the cab and away from the process.

20. Never swing a load over the cab.

21. Back hoes get a lot of wear and tear and maintenance is required

from time to time. Be sure the boom is adequately blocked to

avoid a serious injury in case it falls. Never rely upon hydraulics

to hold the boom. It must be blocked to avoid a fall.

22. Chalk the tires when working on the vehicle and keep the bucket

teeth in good shape.

23. When refueling or handling fuel turn the engine off as well as

electrical or spark-producing motors. Ground the fuel nozzle or

funnel against the filler neck to avoid static electricity.

6.5. FRONT END LOADER

Front-end loaders are versatile and can be a valuable tool for construction

work. However, they change the center of gravity and the stability in the

tractor-loader combination, so they can be dangerous if not operated

properly. Changing the center of gravity presents a potential tipping hazard

for the operator.

6.5.1. Working with a Loader

1. Use a front–end loader only for its specific purpose. Never use it for

such things as removing fence posts, towing or to knock something

down. Never allow people to ride in the bucket. Never walk or work

under a raised loader. Raise and lower the loader arms slowly and

steadily. Never move or swing a load with people in the work area.

2. Load the bucket evenly, and avoid overloading to prevent upsets. Do

not exceed the load limits listed in the operator’s manual. Keep the

bucket low while carrying loads and operating on hills. A hole or

sudden bump can easily upset the tractor if the bucket is carried and

the rougher the terrain, the greater the danger of an upset.

3. Unsecured loads such as large, round bales carried without a bale fork

or grapple can roll back and crush the operator. With loaded buckets,

drive forward when going uphill. Back downhill with loads. Allow extra

room to turn and manoeuvre because of the extension of the bucket.

4. Travel slowly with front–end loaders filled. Carry the load low, to

maintain stability and visibility. However, make sure the load is high

enough to avoid contact with the pavement. Be careful when turning

with loads, especially those that may shift or slide. Add extra weight

on the rear of the tractor to counter-balance the weight of the load of

the front end loader. Check the operator’s manual for specific

recommendations.

5. When operating a front–end loader, watch the road or field. Avoid

holes, rocks, loose fill or any other obstacle that could upset the

tractor. Watch for overhead wire and obstacles when the loader is

raised. If working inside buildings, watch for low ceiling beams and

doorways to prevent being pinned or crushed between them and the

tractor. Also, be sure there is enough ventilation so carbon monoxide

from the tractor engine will not build up.

6. Be very careful when back filling. The weight of the fill material plus

the weight of the tractor and loader could cause the new construction

of collapse. Avoid undercutting high banks. Watch for falling rocks and

cave-ins if a high bank must be undercut. Stay away from the outer

edge when working along high banks and slopes.

7. When leaving the front–end loader, lower the bucket to the ground.

8. When working on front–end loaders, take precautions. Have the loader

safely blocked before working under it or through the loader arms.

Work with the bucket down. Follow the manufacturer’s

recommendations for routine maintenance.

9. While operating, two people should work together; one operating the

machine and the other directing the operator and watching for

dangers.

10. Know the pinch points and wrap points on the loader/backhoe.

11. Keep work area level; avoid developing ruts by occasionally back

dragging the bucket to smooth the surface.

12. When assessing the risks involved with the use of a FEL consider:

1. the potential for carried objects or loads to roll back or fall on

the operator

2. the capability of the front axle, wheels and tyres of the tractor

to accommodate the weight imposed by the FEL when it is fully

loaded

3. lift capacity of the tractor’s hydraulic system

4. is there adequate clearance between tractor front tires and FEL

frame to eliminate contact during turns

5. stability of the tractor operating a fully loaded FEL

6. appropriateness of the selected FEL to lift the load

7. operating conditions related to:

(a) density of material to be handled (e.g. nature of material and

whether wet or dry)

(b) dimensions of the load to be lifted

(c) speed of travel which affects stability on bumps or turns,

(d) load height during travel

(e) terrain surface (direction of slope, evenness and hardness of

surface)

8. whether the Rated Operating Load (ROL) of the FEL will be

exceeded.

6.6. BULLDOZER

1. Travel slowly over rough terrain.

2. When making turns close to buildings or passing through narrow

passages, make sure there is enough clearance.

3. Be careful to avoid stumps and other immovable obstacles.

4. Do not use a dozer as a battering ram.

5. Do not stand with your feet under the blade when changing the

angle or removing the blade.

6.7 AUGUR DRILLING MACHINE

Operators and helpers may be exposed hazards when an auger strikes materials

beneath or above the surface. Major hazard also include injuries with hidden

materials on ground, contact with underground lines etc.

1. Follow the instructions in the manufacturer's operating and preventive

maintenance manual.

2. Look for obstacles that may need to be removed. Hand digging may verify

the presence or absence of underground material, including utilities.

3. Except for the operator, employees should not be near the auger when it is

operating.

4. Employees using hand tools should not move or remove spoil-pile while the

auger is operating.

5. The operator should sit or stand at the operator's station while operating the

auger.

6. Do not modify the operator's station or disable safety controls beyond

manufacturer's recommendations

7. Remain a safe distance (a minimum of 10 feet) from the auger when helping

the operator.

8. Do be sure the feed ram (mast) is vertical from side to side with respect to

the machine before lowering. Lower slowly to be sure feed ram (mast) will

clear other parts of the machine and fit correctly in the feed ram (mast)

rest.

9. Do Not move the machine with the Feed Ram (mast) raised. Always lower

the Feed Ram and raise the jacks before moving the machine.

10. Do Not attempt to dig unless the jacks are firmly placed and set on a hard

surface to eliminate thepossibility of turning the truck and digger over.

11. Do Not attempt to move the machine with a load suspended from the winch

line. Always keep the Feed Ram vertical and the auger on the ground when

lifting a load. Do not swing with a suspended load. Failure to heed this

warning may cause serious damage and/or personnel injuries.

12. Operator or helper shall not use face covering clothes like “gathra” while on

the job.

6.8 BOREHOLE DRILL RIGS

1. The equipment should never be used unless guards and safety devices

are in place and in good working order.

2. For stability ensure that front and rear legs are fitted and that the

mast is vertical before unit is used in stand-alone mode.

3. Before commencing sampling, wear ear protectors

4. Operator or helper shall not use face covering clothes like “gathra”

while on the job.

5. The area around the drop weight shall be clear of all personnel whilst

sampling is in progress. There is a safety zone around the drop weight

extending to 2 meters in all directions. This area must be clear at all

times.

6.9.CONCRETE MIXER

1. Never operate the machine without all guards in place and firmly

attached to the machine.

2. Make certain that the mixing drum is clear of obstructions and

stand clear of the machine while making power cord connections

3. When cleaning, lubricating, or adjusting the mixer, disconnect the

power plug.

4. Keep all people (except operator) a minimum of five (5) feet away

from the machine during operation.

5. Do not plug or unplug the motor while standing in or around damp

or wet ground

6. Do not start the motor if the drum is fully loaded.

7. Do not overload the motor. Always start the mixer before loading.

8. Never attempt to move the mixer while the motor is on.

9. Make certain that the work area around the machine is clear of

debris and obstructions to prevent tripping and/or falling onto the

machine.

10. Never let the mixer run unattended even for a brief moment. Turn

off the motor when the operator leaves the machine.

11. DO NOT overreach while working with the mixer. Keep proper

footing and balance at all times when loading or unloading the

mixer.

6.10 READY MIX CONCRETE TRUCKS

The following points shall be considered prior to setting up a pump truck:

1. Safest location for setting up the pump, considering soil conditions,

excavations, outriggers . Setting outriggers of a concrete pump with placing

boom is one of the most critical jobs of the concrete pump operator and

should always be done in accordance with the manufacturer’s recommended

procedure.

2. Priming and washout area

3. Who will be providing the hand signals

4. Hose handling safety procedures

5. Pump truck and outriggers need to be 1’ back for every 1’ of depth from an

excavation

6. Safe clearance for outrigger extension and placement

7. Safe access for ready mix trucks

8. Proper number and securing of required safety slings, chains, or cables, for

any part of the pump that can fall and cause injury

9. All clamps are securely pinned

Any employee that is assigned the task of guiding the discharge hose must be aware

of the following safety guidelines:

1. Good communication with the pump operator will minimize the amount of

hose handling required for the job. Let the pump truck do the work.

2. Keep Hose at Arms Length! Do Not hug the hose! Pressure within the line can

cause the hose to whip forcefully when it discharges, and cause severe

injury.

3. Keep the hose end 2’ above the concrete pouring elevation to minimize

splatter.

4. Do Not kink the hose, as it can whip forcefully when the pressure is

discharged.

5. Never reach into the receiving hopper, valve area, or water box! The

machine may cycle unexpectedly from operation or line pressure!

6. Never pass under an extended outrigger, or stand near an outrigger when it’s

moving.

7. Never stand between the pump truck hopper and a ready mix truck as it is

backing up to the pump truck.

8. Be careful around the chute of a ready mix truck as it can swing suddenly.

9. Know where the pumps horn and emergency stop buttons are located.

10. Air ingression to concrete pumping delivery pipelines has revealed itself as a

considerable hazard under certain circumstances. Injuries to placing crew

personnel have been sustained when trapped air is momentarily compressed,

then released, causing the end hose to whip violently.

11. Hose whipping accidents can be avoided if people take the proper precautions

when air has been taken into the delivery system. Each person involved has to

know what to do, and knowing is a matter of education. Everyone needs

specific knowledge, and each person has to heed the warnings to protect

themselves. Communication between the personnel is crucial.To avoid injury

by a hose moving from release of trapped air, personnel must be out of the

end-hose movement area.

6.11. COMPACT EXCAVATORS

1. Know the worksite: Inspect work area for holes, drop-offs or

unstable ground.

2. Know the weight limitations of operating surfaces and clearances

3. Start excavator with steering levers and hydraulic controls in

neutral.

4. Move steering levers and pedals smoothly. Hydrostatic drive

systems are responsive. Excavator will move immediately when

controls are activated.

5. Check blade location before moving travel control levers or

pedals. Excavator will move in the direction of the blade when

steering levers are moved away from the operator.

6. Keep your attachment low when traveling.

7. Do not exceed the rated lift capacity. Read and understand the

load rating chart on the excavator.

8. Keep bystanders away from the maximum swing area. Never lift a

load over another bystander.

9. Always look in the direction of travel before moving the excavator

or rotating the house swing (slew).

10. Do not swing loads on a slope. Swing loads low and slowly. Do not

travel across slopes which are more than 15 degrees to the side or

to the back of the machine, or 25 degrees to the front. Keep boom

centered.

11. Never operate excavator’s controls from the ground. Always

activate the controls from the operator’s seat.

12. When getting On and Off use the grab-handles, track and skid-

resistant surfaces when getting on or off the excavator.

13. Do not jump off the excavator; Lower the attachment to the

ground when operator goes out.

6.12 CONCRETE VIBRATOR

1. Do not operate a machine that is powered by a gas or diesel

engine in a poorly ventilated space. These type of engines produce

toxic gases which cause serious health problems.

2. Pneumatic Machine: Check that the compressed air hose and the

hose coupling are not damaged.

3. Never attempt to loosen any compressed air hose that is

pressurized. First

4. Switch off the air at the compressor and then allow the machine

to run to a stop to discharge any existing compressed air.

5. The admissible sound of 85 dB (A), can be exceeded because of

the machine and/or the application. Long time exposure to loud

noise without ear protection can cause permanent damage to

hearing.

6. Long time exposure to vibration can cause damage to the hands

fingers and wrists. Do not use these vibrators if operator

experiencing discomfort, cramping or pain.

7. Operators and other personnel in the immediate vicinity of the

working zone shall have the mandatory PPE and also Dust mask

(dusty environments) and Eye Protection equipment.

8. Do not carry out maintenance work while the machine is moving or

while the engine is running. Before connecting vibrator, clear the

compressed air supply line of possible impurities, contaminants

and water.

9. Connect the vibrator to the quick release coupling securely. Insure

that locking pin is in place.

6.13 ROAD GRADER

1. Do not dismount from the grader with the engine running - lower all

attachments and stop engine first.

2. Before backing up, use extra care to ensure persons and equipment

are clear of the grader.

3. Know and use hand signals required for particular jobs and know

who has the responsibility for signaling.

4. Start and operate the grader only from the operator's seat.

5. Park the grader on level ground whenever possible and apply the

hand brake. On grades, park the grader with the wheels securely

blocked.

6. Select a gear that will prevent excessive speed when going

downhill. Do not coast downhill.

7. Note and avoid all hazards and obstructions such as overhangs,

ledges, slide areas, electrical lines, underground cables water

mains or gas lines.

8. Watch for bystanders and never allow anyone to be under or to

reach into the grader and its attachments while operating.

9. Use extreme care when working with hydraulic systems. Relieve the

hydraulic system pressure before performing any service.

6.14 COMPACTOR

The soil compactor has been exclusively designed for the compaction of

sand, gravel, crushed aggregates, semi-cohesive mixed material, concrete

block pavement etc.

1. Before starting the engine, check to ensure that the soil compactor

cannot slip out of control.

2. When running the machine, the operator is not allowed to leave the

operating controls of the soil compactor.

3. In addition, he must always have a sufficient visibility on the soil

compactor's zone of operation. The operator must be assisted by a

second person if, because of the operating conditions, the visibility on

the zone of operation is restricted.

4. Soil compactors must be used and operated in a way ensuring their

stability. The machine's stability is especially endangered on slopes

and brinks. Thus, keep clear of slopes and brinks.

5. When working on slopes, the operator must always walk on the uphill

side.

6. Compaction work on slopes exceeding the maximum gradeability of

the soil compactor is prohibited.

7. When working/traveling on slopes, always use extreme precaution and

work directly in uphill or downhill direction.

8. Moist and loose bases considerably reduce the grip of the soil

compactor on slopes. Increased danger of accident!

9. Passing unevenness or kerbs is only allowed at reduced speed.

10. In addition, the soil compactor must be operated in a way excluding

any risk of injury caused by the handle swinging towards the operator.

6.15 MOWERS

Mowers are the most common type of lawn care equipment. To avoid injury

with power mower equipment, one must pay close attention to his

surroundings. Whether you use a riding mower or a walk-behind mower,

follow these guidelines for lawn mower safety:

1. Conduct a premowing inspection of the lawn and remove any debris,

rocks, limbs, or other items that could become a projectile. Look for

concealed hazards such as holes.

2. Keep hands and feet away from moving blades.

3. Fill the tank with gas (if that is the fuel used) before beginning work.

(By filling the tank initially, you can avoid having to fill the tank later

when it is hot.)

4. Replace loud or faulty mufflers.

5. Shut off the engine before unclogging, servicing, or adjusting the

mower and before removing the grassbag. For added protection,

remove the ignition wire before working on the machine.

6.15.1 Walk-Behind Mowers

In addition to the general guidelines for mower safety, follow these

guidelines for walk-behind mower safety:

1. Do not bypass the safety device that stops the blade when the

operator releases his/her grip on the handle.

2. Mow across slopes rather than up and down slopes.

3. Work slowly and patiently when mowing tall grass or tough

weeds. Forcing the mower may cause repeated clogs and engine

stalls.

4. Never leave a running mower unattended. If mower has to be

stopped momentarily, cut the throttle to idle and make sure

the mower will not roll away.

6.15.2 Riding Mower

In addition to the general guidelines for mower safety, follow these

guidelines for riding lawn mower safety:

1. Before starting the engine, make sure the transmission is out

of gear and the mower blade clutch is disengaged.

2. Slow down when turning and when working on slopes. Mow up

and down slopes rather than across them.

3. Always look behind before backing.

4. If mower hits a large rock or stump, stop the mower and

inspect the blades and shaft. Replace damaged blades.

5. Never leave a running lawn mower unattended. Before leaving

the seat, park the mower on a flat area, disengage the mower

blades, and remove the ignition key.

6.16 AERIAL PLATFORMS AND MANLIFTS

1. Only authorized, properly trained, qualified persons shall use or operate this

equipment.

2. The operating and maintenance instruction manuals issued by the

manufacturer must be followed.

3. Load limits of the boom and basket will not be exceeded. Shock loading

(sudden stops or starts) of the equipment shall be averted.

4. Aerial lifts shall not be “field modified” unless the modifications are

certified by the manufacturer. The insulated portion must not be altered in

any manner that might reduce its insulating value.

5. Prior to use, the equipment shall be given a warm up period. The hydraulic

system and the lift controls shall be checked and tested daily before use to

positively determine that such features are in safe operable working

condition. Malfunctions or unsafe operational conditions shall be reported.

Equipment, which is not in proper operational condition, shall not be used.

6. Lower level controls shall not be operated unless permission has been

obtained from the employee in the lift, except in the case of an emergency

only. Ground controls shall be positive override.

7. The truck shall not be moved unless the boom is lowered, the basket cradled

and secured, and the outriggers retracted.

8. Employees shall not ride in the bucket while the truck is traveling.

9. When employees are in the bucket of an aerial lift, the emergency brake of

the equipment must be set. Wheel chocks or outriggers shall be used to

provide added protection. When the equipment is on an incline, wheel

chocks shall be used regardless of whether or not outriggers are used. The

truck should sit approximately level when viewed from the rear.

10. When outriggers are used, they must be set on a solid surface, or on pads.

11. Employees shall not belt to an adjacent pole or structure. When working

from an aerial lift, a harness or a body belt shall be worn and a lanyard

attached to the boom. A body belt can only be used for fall restraint, not

fall arrest.

12. When a boom must be maneuvered on a street, all of the necessary

precautions shall be taken to eliminate accidents with traffic and

pedestrians.

13. The operator must always face in the direction in which the basket is moving

and he must be able to see that the path of the boom or basket is clear

when it is being moved.

14. Employees shall not stand or sit on top of the edge of the basket or on

ladders placed in the basket. Employees’ feet shall be on the floor the entire

time that he/she is in the basket.

15. Employees shall not wear climbers while in the basket.

16. When two workers are in the basket(s), one of them shall be designated to

operate the controls. One employee shall give all signals, which shall be

thoroughly understood by all persons concerned.

17. When two line workers are working from the basket, care shall be taken to

eliminate one man contacting poles, crossarms, or other grounded or live

equipment while the second line worker is working on equipment at a

different potential.

18. In no case shall more than one energized conductor or phase be worked at

one time.

19. Clearances: The aerial lift, together with the workers in the basket, and all

tools and equipment must maintain proper clearances from unprotected

energized conductors.

20. When using hydraulic or pneumatic tools in a bucket, the operator shall be

sure that hoses or lines do not become entangled in the operational controls.

21. All aerial devices shall be equipped with suitable flashing warning lights.

22. All controls must have protective guards to prevent accidental operation of

the controls.

23. Periodic electrical tests shall be made of the boom insulation and aerial

basket liners.

24. Equipment or material shall not be passed between a pole or structure and

an aerial lift while an employee working from the basket is within reaching

distance of unprotected, energized conductors or equipment.

25. When used near energized conductors or equipment, it shall be properly

grounded or barricaded. Ground crews must not touch the equipment until it

is safely clear of energized conductors.

7. MANAGEMENT SYSTEMS

7.1 Support Resources KNPC SH&E Dept shall assist in the implementation of this document. 7.2 Management Records

All records must be maintained in compliance with this document by the user.

7.3 Audit requirements

Application of this document must be audited as part of safety audits to assure compliance.

7.4 Standard renewal process

This document must be reviewed and revised as necessary and, at a minimum, not later than 3 years from the date of the latest revision.

7.5 Deviation process

Deviations from this document must be authorised by DMD after consultation with SH&E Department. Deviations must be documented, and documentation must include the relevant facts supporting the deviation decision. Deviation authorisation must be renewed periodically and no less frequently than every 3 years.
