Getting StartedYour first step to wellness is to register on BlueAccess at bcbst.com/Nissan-Wellness if you haven’t already. Registering is easy and gives you access to our online wellness tools and educational resources.

Register for BlueAccessVisit bcbst.com/Nissan-Wellness and follow these steps:

l Choose the Register Now option under Log In/Register for BlueAccess if you haven’t already registered.

l Select the first option: Register as a Member.

l You’ll need your BlueCross Wellness Only Member ID card to sign up.

Explore Your Online Wellness Resources

Member Wellness PortalYour Member Wellness Portal is there to help you keep up with your LiveWell Program. Visit the portal to find resources that can help you get and stay healthy:

l Take your Personal Health Assessment (PHA). l Use a health tracker to keep up with your stress level, nutrition, weight, exercise and more.

l Sync your nutrition and fitness devices or apps. l Communicate with your Health Coach. l Take an online health course.

Visit Your Member Wellnes Portal l Visit bcbst.com/Nissan-Wellness. l Log in to BlueAccess. l Click My Health & Wellness. l Choose Member Wellness Portal.

AlwaysOn Wellness AppThe AlwaysOn app lets you access many of the online tools in the Member Wellness Portal:

l Complete your PHA.

l Sync your nutrition and fitness devices, and view your activity.

l Set health goals and access tracking.

l Send a secure message to a Health Coach.

*You may be asked to enter your Subscriber ID and Group Number.

Download the AlwaysOn App from the App Store or Google Play.

l Open the app and tap New User Registration at the bottom of the screen.

l Enter your first name, last name, date of birth, and zip code. You’ll be asked to review and verify your personal information.*

l Create a username and password, then enter your phone number and email address, and accept the Terms & Conditions.

Complete Your Personal Health Assessment Taking your Personal Health Assessment (PHA) is an important part of committing to living a healthier life.

Your PHA helps us identify and evaluate characteristics and behaviors that impact your health. We may contact you via phone or email to offer suggestions and advice to help you live a healthier lifestyle.

How to Complete Your PHA on Your Computer

l Visit bcbst.com/Nissan-Wellness and log in to BlueAccess (see figure A).

l Click the My Health & Wellness tab, select the Member Wellness Portal, and then click Get Started (see figure B).

l You’ll be taken to your personalized wellness page where you’ll be prompted to take your PHA. Click Get Started! and complete your PHA.

If you don’t have access to a desktop or laptop computer, or if you have questions about your PHA, please call us at 1-866-843-3509 (option 4) and a Health Coach will help you complete your PHA.

Healthy MaternitySM Management ProgramIf you’re expecting, our Healthy Maternity Management Program can provide you with support throughout your pregnancy and help you have a healthy delivery. Moms-to-be who enroll in our program can expect:

l Personalized one-on-one support from a maternity nurse

l Postpartum support for up to eight weeks after your baby is born

l Details about your baby’s immunizations

l Helpful prenatal information and online pregnancy resources

Our maternity nurses are available at 1-866-843-3509 (option 6), Monday – Friday, 7 a.m. to 6 p.m. CT.

Enroll in the maternity program as soon as you learn you’re pregnant. You can enroll online by logging in to BlueAccess, choosing the My Health & Wellness tab and clicking on Healthy Maternity, or by calling 1-866-843-3509 (option 4).

Health Coaching With our Health Coaching program, you have access to a personal Health Coach who can help you set goals related to:

l Quitting Tobacco

l Physical Activity

l Nutrition

l Weight Management

l Blood Pressure

l Stress Management

l Cholesterol Levels

Contacting Your Health Coach

Your Health Coach is available to you for health- related guidance and advice via telephone or secure messaging.

l Call your coach at 1-866-843-3509 (option 4).

l On your desktop or laptop computer, message your coach via the Member Wellness Portal on BlueAccess (see page 7).

l On your smartphone, contact your coach through the AlwaysOn mobile app, which can be downloaded from the App Store or Google Play (see page 8).

Fitness Your WayTM

We know that getting fit and staying fit can be a challenge. So we offer Fitness Your Way, a flexible, low-cost fitness program for you and your covered dependents (age 18 and older). Fitness Your Way provides:

l Affordability. One-time enrollment fee of $29 and a $29 monthly membership fee per member.

l Unlimited access. The extensive network includes more than 9,500 participating fitness locations including national and local chains, and community and women-only facilities.

l Flexible membership. No long-term contract is required – only a three-month initial commitment and unlimited use of any facility within the network.

l Online convenience. Find fitness centers, enroll and manage your activity online.

To get started, visit bcbst.com/Nissan-Wellness and register or log in to BlueAccess.

l Click My Health & Wellness.

l Select Member Discounts & Fitness Your Way to visit our Blue365® member discounts page.

l Click the Fitness Your Way tile.

l Select View Details then Redeem Now.

l From there you can find fitness center locations, enroll and create a Fitness Your Way account.
