Drs. Richard and Bruce Winter

Dear Dr. Winter, I need implants in my upper and lowerarches. I have had several consultations and opinions all overthe board! Some of the consults said I only needed 4 implantsfor the upper and 4 for the lower and others have said I needjust two for the lower. I did tell them I wanted to be able tohave teeth that didn’t come out and I don’t have a lot of bonein either arch. What are your thoughts? Thanks, Jack J.

Dear Jack, there are several good questions here. I will tryand break them down and I encourage you and the readers toread my articles on Upgradeable Dentistry and AdvancedTreatment Planning under “publications” at:www.hamptondentalassociates.com where I have writtenarticles about these very questions. I also outline in variouspublications the differences between removable overdenturesand fixed implant bridges and show examples of each type ofimplant treatment.

First of all, without a consultation and x-rays I cannotcomment on your specific situation. The number of implantsyou require is dependent upon the quality and quantity ofyour remaining bone, your budget and the type of prosthesisyou require. Whether the implant restoration should be“fixed” or non-removable by the patient or whether it isacceptable to remove the implant supported denture and takeit out at night is a personal decision and both have advantagesand disadvantages. Most people that lose their teeth want“fixed” teeth. In addition, the patient’s health, medication(s)and anatomy all play a role in deciding upon the ideal implantrestoration. It is important to note that sometimes we can startwith a removable implant denture due to budgetaryconstraints and add implants over a year or two and Upgradeto a fixed restoration or bridge when finances allow!

Next, we can discuss the number of required implants. Inthe upper arch most people have already been wearing adenture or partial and they have already undergone bone loss.As bone is lost in the upper jaw the sinuses expand and inorder to place implants in this area there is a need to “growthe bone” by doing a sinus grafting procedure and possibly ablock grafting procedure. This sinus augmentation is aprocedure that will allow implants of normal length to beplaced in the posterior upper jaw. A way to bypass this is azygomatic or long implant that is used in an “all-on-four”procedure. This procedure will place 4 implants at angles thatare used to support a full arch of denture teeth or a “hybrid”prosthesis. This may be a predictable procedure but replacing14 teeth (24 roots) with 4 implants doesn’t provide as large amargin of safety as using 7-9 implants per arch. This is amatter of philosophy and the treating implant dentist andsurgeon will be able to explain their philosophy based upon

their education, skill and judgment. As far as the two-implant option given to you for your

lower arch, this is strictly to support a lower overdenture, which is removable. In order to have a fixed restoration(non-removable) you would require a minimum of 6maxillary and 5 mandibular implants in my practice. Thatbeing said if you are able to place more implants and over-engineer the situation you are always better off, as theseextra implants will redistribute chewing forces over 6-9implants, instead of just four.

We offer complimentary implant consultations orcomprehensive examination and x-ray visits to our readers. Please call us at 414-464-9021 to schedule your visit! Welook forward to meeting you. You may see examples of ourdentistry at www.winterdental.com

We would like to also congratulate Dr.Rick on his ABOIaward that he received last weekend at the AAID AmericanAcademy of Implant Dentistry Meeting in Orlando.

The Menomonee Falls School Boardapproved a legislative platform Jan. 12 thatdetails its concerns about funding and pro-gramming for public education.

The platform highlights areas such as stan-dards and “flexibility” in other regulations.

“The state needs to sustain adequate fund-ing for required state programming. The statehas significantly increased the expectations forperformance, but the funding needed has notbeen adequately addressed. The state hasincreased the technology needed to meet statetesting requirements, increased requirements forstudent interventions, and mandated identifica-tion and intervention for special educationneeds; none of these recent requirements areadequately funded. These have placed addi-tional pressures on the already limited districtbudget,” district officials said in the statement.“The tools of Act 10 have been implemented. Our district has reached a practical limit to usethese tools, and remain competitive in thesoutheast Wisconsin region.”

District officials noted that in response toAct 10, the district changed to a highdeductible self funded plan while districtsalaries and benefits rank in the “middle” ofneighboring districts.

“ We have reached a practical limit inorder to retain experienced staff and attractstaff in hard to hire areas,” district officials stat-ed.

To address funding issues affectingMenomonee Falls, district officials suggested:

• “Increase the revenue limit each year ofthe 2015-17 biennial budget to avoid furtherimpact on class size, programming, mainte-nance, and staffing.”

• “Focus specific support for decliningenrollment districts through greater categoricalaid.”

•“Increase open enrollment reimburse-ment to align with the base educational cost of$9200 per student.”

• “Increase the reimbursement rate for spe-cial education to 30% of the prior year’s coststhat are eligible for aid. This rate has droppedsteadily from 70% in 1980 to 26% in 2014.”

The statement also asked the state to allowfor flexibility with the Sept. 1 school start date.By state law, public schools cannot begin untilSept. 1 or later each fall.

“Providing flexibility for local schoolboards to adjust the calendar will help betterprepare students for the national expectationsof the Advanced Placement Testing whichbegins on May 1st. Our parents value ourapproach to college and career readiness, butthis still stands in our way,” the platform said.

All board members agreed with that por-tion of the platform. The platform also calls for“a level playing field needs to be available toall systems receiving public tax dollars. We askthat the measurement systems, state aid, andstate mandates be equally applied to all sys-tems educating children with public funding.Innovation and waivers should be equallyaccessible to all publically funded schools.”

That portion of the platform calls for thestate to “Increase local school board controland authority in setting any waivers or modifi-cations to the state required accountability sys-tem and educational standards. Any assess-ments or standards waived for one publicallyfunded educational option should be availableto all local school boards.”

The section also states: “Alternatively, cre-ate one state-wide accountability system for allpublically funded schools to ensure equalresponsibility to the taxpayers.”

That section referenced voucher schools, which receive state vouchers but do not have tocomply with all the state testing requirements orregulations. Board member Paul Tadda indicat-ed he did not agree with that portion of the plat-form.
