    1. GENERALITIES

    1.1 Scope

    The Manual present contains elements and standards of strict fulfilment for the employees and workers of

    our company. HOWEVER AGREE TO COMPLY WITH THE PROGRAMME, PROCEDURES AND

    PRACTICES OF SAFETY, HEALTH AND ENVIRONMENT OF OUR CUSTOMER.

    1.2 Objectives

    The objectives of the present manual are:

    Inform and norm the aspects Environmental, Health and Safety in the work for its respective execution.

    Include an internal technical/normative instrument to carry out every task according to the practices and

    disciplines of Safety and employment Health established by the laws, regulations and standards of our

    client.

    2. LEGAL REQUIREMENTS

    This Plan is based on the following documents:

    Occupational Safety & Health Administration (OSHA) 29 CFR 1926 of the United Status of

    NorthAmerica.

    3. POLICY OF THE INTEGRATED SYSTEM OF MANAGEMENT

    3.1 Mission

    "To provide Engineering, Construction and Installation services in the steel construction field with the

    highest levels of safety, quality, compliance, profitability and social responsibility, while focusing on

    achieving the complete satisfaction of our customers and the professional development of our personnel.

    3.2 Vision

    To be a highly reliable and competent company of the sector that guarantees their clients an

    excellent service.

    To reach the 100 years of services maintaining the leadership among the companies of the sector

    and particularly in the speciality of Tanks.

    3.3 Policy of the Integrated System of Management

    HAUG S.A., as a dedicated company to serve of Engineering, Construction and Assembly in the steel

    construction field, and having the commitment of the top direction and active participation of all the

    personnel is committed:

    a) To obtain the satisfaction of its clients offering them products and services of the highest quality,

    considering Safety, protection and care of Health and of Environment as top priorities elements in the

    execution of itself.

    b) To seek the constant professional development of its workers.

    c) To reach the most yield of the company.

    d) To protect the environment of any adverse effect of its operations, diminishing the environmental impact

    caused by the consumption of resources, generation of remainders and gas discharge.

    e) To guard by the safety and health of all the employees in the site through the prevention and control of

    risks in its activities.

    f) To fulfill the applicable legal requirements and the internal requirements that the company considers

    necessary to assure the quality the services that offers and safety and protection of environment on the

    activities that make.

    4. POLICIES OF WORK

    4.1 Policy of Meetings and Training

    Meetings of five minutes: before the works begin, the Supervisor will give a chat from 5 minutes to their

    personnel, orienting them on the sure work that will perform. Any worker that have some doubt should

    ask his Supervisor and this should resolve the problem. Every chat and meeting of safety that give

    should be communicated in writing in the corresponding formats, causing signing to each one of the

    workers as sign of conformity.

    Meetings weekly: Every week a meeting of Safety will be performed, Hygiene and control of the

    Environment that will have a most minimum duration of thirty minutes, in which they will intervene the

    Project Chief, Project EH&S Manager, Supervisors and workers in general. This meeting should be

    documented in a format of weekly meeting.

    Training HAUG S.A. will qualify and will coach its personnel in the different related areas from EH&S.

    Instruction to third parties every visit or supplier of HAUG S.A. that have that to enter to Project,

    previously will be instructed on the existing risks and the administrative norms and of EH&S that should

    comply.

    4.2 Policy of Use of Personal Protective Equipment (PPE)Safety glasses, hardhats, hi viz vest and steel toed work boots of safety will used by the personnel during

    the workday compulsory, and according to the degree of risk of the work be considered, will be offered

    special implements. The administrative workers that be found labouring inside the offices are excepted of

    this disposition.

    Safety of the head: The head is the most important part of the human body, that is why the use of the

    hardhat for its protection is permanent and strict in project during the workdays. It prohibited in work having

    loose hair only if it is long , it should be maintained collected in such a way that not cause danger since it

    could be trapped by machines or equipment in operations.Safety of the eyes: Safety glasses during the continuance in the work will used, except for works in offices.

    For the special works of welding, brazing, cutting and other will be provided of the adequate implements.

    Protection of the feet:For the safety of the feet is obligatory the use of steel toed work boots to avoid

    accidents. Its prohibited the use of slippers, sandals or similar. The boots of rubber with steel toed are

    permitted in humid places or contact with the water. The electricians will use dielectric boots of Safety.

    Uniform: All personnel will use a hi viz vest during the permanence in the project. According to the

    requests of the project or work the company will provide the personnel work uniforms. This should be

    comfortable and tight of such form that cannot be stalled neither to be hooked in machines and equipments

    of construction, structures, mobile parts, etc. Moreover, workers should not use shirts, pants or very wide

    overalls or deform. Clothing of synthetic fibber will not be used.

    Ear safety: All personnel will use protectors of ear in places where the noise surpass the 85 decibels dB,

    such as: compressor, pneumatic hammers, blows of constant bulge, etc.; and in those places that apply

    hand grinds , workshops where there are noises of machines and vehicle.

    Safety of arms and hands: according to as correspond should be used protection for the hands and arms.

    Previously an evaluation of risks will be made in order of determining the type of gloves to use.

    Safety of the face will be used the adequate mask in works of chemical materials, welding, cutting,

    brazzing or in those machines that throw shaving, dust or abrasive.

    Respiratory protection: it will be provided to each worker respirators adequate for each work that imply

    risk or excessive concentration of dust, smoke, gases, fog, vapors, toxic emanations or by deficiency of

    oxygen in places confined. Before using the respirator, be assured that he has been trained about it.

    Protection in works of height: Is obligatory the use of the safety harness in works of height, equal or

    more than 1.20 mts. The infringement of this norm is motive of drastic sanction.Inspection of the equipments: All the personal protective equipment will be inspected, codified and

    repaired or placed out of service.

    4.3 Policy of Order and Cleaning (Housekeeping)

    The order and cleaning is a very necessary and fundamental activity of individual responsibility of each

    person.

    The places of work, walkways, stairs and all the other areas should be maintained free of filth.

    All the wastes should be placed in cylinders placed in different places.

    The ways, halls and other places where the personnel circulates should be maintained cleared of

    obstructions and other materials that can cause any accident.

    The stores and deposits should be maintained clean and orderly.

    Every building material, cords, wires, electric cables they should be far away of the surface of circulation

    of the personnel, these should be placed in high places over the surface in order not to cause damage

    to personnel or equipment (or protected).

    It is compulsory to keep each work area in order and clean.

    The sanitary services and dining rooms after being cleaned should be disinfected daily to avoid

    infections.

    4.4 Policy of Health and Other Services

    Every worker that enter to project should be submitted to medical exam.

    In every installation or work, the personnel should include adequate dining rooms and enclosures,

    which they should be maintained always clean, free of wastes.

    The personnel should maintain the good judgment and discipline in the dining rooms, sanitary services

    and internal areas of Site project.

    The portable chemical baths should be maintained clean, disinfected and in good conditions

    4.5 Policy on Control of the Alcohol and Drugs

    HAUG S.A. is conscious that the use of alcohol and drugs in the workplace affects the normal health of the

    workers to carry out their tasks safely for themselves and others. That is why it is prohibited to consume or

    to possess alcoholic beverages and drugs during the work day or while working in the site projects.

    Measures to take:

    Carry out tests of alcohol tests to the people found involved in a serious accident.

    The Supervisor will control the behaviour of his workers in alcohol consumption signs search and

    drugs. Or in his defect he will be utilized instruments like the ALCOTEST for this purpose.

    In case it has been proved that the worker has consumed alcohol or drugs, the coordinations for

    respective analyses will be done.

    The worker that is consuming drugs by medical prescription, will report this fact to Human Resources

    Administrator. All the workers of site projects, without exception, will comply with their responsibilities that are

    indicated in our politics on the use of alcohol and drugs.

    Personal will be sanctioned, any be its hierarchy yes infringes these norms, in agreement al item 4.6 of

    the present document and legal devices in force.

    4.6 Policy of Incentives and Disciplinary Measures

    Incentives

    HAUG, will establish safety incentives programs for the workers consisting on the following points:

    Monthly prize to the workers that there are outstanding in safety.

    Celebration of the ACCIDENTS ZERO goal.

    Disciplinary Rules

    All the rules, norms and procedures, contents in the present plan are of strict fulfillment by part the workers,

    without restriction of charge or category.

    In agreement with Internal Regulation of Personnel of HAUG and legal devices in force, that break the

    dispositions given will apply the following sanctions:

    Verbal Admonition.

    Admonition written

    Suspension

    Dismissal

    If the misdemeanour is serious, the worker will be fired not including the sanctions before described.

    Serious misdemeanour in Safety is considered : Cause serious wound or death, which makes the worker be under the disposal of the competent

    authorities.

    Cause damage to the installations, equipments or any properties of the company or third sides.

    Appropriated of any property or value of the company or of third sides, or that anything under its

    custody.

    Destroy the natural resources.

    The resistance to comply the regulations, norms, manual and dispositions of Safety and Environment of

    the employer. The Supervisor is responsible for the control of his personnel and he is in obligation to instruct them so

    that they do not incur in the demeanours mentioned before.

    5. RESPONSIBILITIES

    5.1 Project Chief

    Is the direct head of all the operations in Project and therefore of the Safety, Health and Environment in

    the place. Consequently, he must provide the resources and facilities needed to avoid any occurrence

    of incidents, damage or waste of the resources of the business.

    Will set a personal example and assist in the proactive promotion of EHS as a personal objective

    Implement, comply and to make fulfill the EH&S policy, plan, objectives and designated goals in the

    project.

    It reviewed all the methods and procedures of work, verifying the compatibility with in EH&S Plan.

    To participate actively in the EH&S initiatives.

    Conduct inspects all the areas of the Project.

    To make weekly EH&S meetings with all the workers of the Project.

    Participate in accidents investigations.

    Review and improve permanently the EH&S programs of project, in agreement with the updates of the

    laws, standards and technologies in the matter.

    5.2 Project EH&S Manager

    Administer the EH&S Plan of project

    To advise to the different areas form the project in EH&S aspects

    To offer the EH&S induction training to all the new workers in the project

    To train all the personnel in EH&S.

    Inspect weekly all the areas of the project, review the results to identify EH&S issues and deficiencies

    and to advise supervisors of findings.

    Lead Job Safety Analysis reviews.

    To participate in accidents investigations.

    To provide weekly reports of EH&S project a our client e internally to HAUG S.A.

    To participate in the EH&S meetings organized by our client.

    To offer first aid to rough personnel.

    Be fully conversant with our client EH&S Rules for Construction, standards and the project EH&S

    requirements.

    Evaluate the EH&S performance of all the areas e inform a Project Chief.

    5.3 Erection Supervisor

    It planned and it implemented appropriate methods of agreed work with the safety standards.

    Comply and to make fulfill the EH&S plan.

    Conduct EH&S inspections weekly in all areas of project with the goal to verify the fulfillment of the

    standards and EH&S plan and to take the corrective measures in case that some norm is being

    infringed. These inspections should be taken down in their respective formats.

    To participate in the elaboration of Job Safety Analysis

    To participate in accidents investigations.

    To participate in the audits of Safety to order of the Project Chief or of the Client.

    Arrange continuously the improvement of the working conditions to prevent accidents and employment

    illnesses.

    Verify that the Area of Maintenance comply with the preventive program of maintenance, in order to

    assuring the operability of equipments and tools.

    5.4 Quality Supervisor

    It planned and it implemented appropriate methods of agreed work with the safety standards,

    specifically in the works that it corresponds to him. To participate EH&S inspections weekly in all areas of project

    5.5 Human Resources Administrator

    Verify that every new worker enters to the business with the certification of pre medical exams

    employment.

    To offer each worker the services of social insurance ESSALUD and complementary insurance. In

    every case these services should be maintained brought up to date. To offer the facilities like feeding and others with he purpose of maintaining an atmosphere of healthy

    and healthful work.

    To transact the necessary documentation for the entrance of the workers to the project.

    5.6 Planning

    To make the planning of all the activities of the project contemplating the aspects of security, health and

    environment during all the work.

    5.7 Responsible for warehouse

    Maintain the stock of personal protective equipment such as hardhats, protectors for ears, steel toed

    work boots, gloves in their diverse uses, high-viz outer garment, facial protectors and safety glasses.

    This stock should be compatible with the quantity of the scheduled personnel, in such a way that all

    they include these requests.

    Order that the warehouse includes enough fire extinguishers and to inspect them.

    Maintain order and cleaning in the warehouse.

    Order that every material or flammable product, chemical or hazardous materials be stored in other

    different areas al warehouse of materials. To maintain a file with MSDS (Material Safety Data Sheet) of

    existing dangerous products in the project

    Every acquisition of equipment, implement or material of safety should include the review and approval

    of the Project EH&S Manager. Similarly, it will be done with the dangerous materials and products.

    Order that the warehouse carry a KARDEX everyday on the safety equipments delivery to the workers.

    To inspect equipments, tools and other objects of warehouse, placing to the elements in good state the

    tape of the color code corresponding to the trimester in course (yellow).

    To retire of use and to signalize the elements in badly been

    5.8 Responsib le for Maintenance

    Comply the programs of preventive maintenance of machines, equipments, installations and electric

    tools.

    Comply with the requests of corrective maintenance that indicate the Supervisors of Safety or the users

    of equipments and tools.

    To inspect machines, equipments, installations and electric tools. to put the tape of color code to the

    elements that are in good state

    To give to the warehouse, to retire of use and to signalize the elements that are in badly been

    5.9 Supervisors of Field

    Know the dispositions and the EH&S Plan of project.

    Order the works of the personnel to its charge in sure form, according to the dispositions of Safety.

    Not to allow the new workers carry out works unless they have received EH&S induction training as

    much of HAUG S.A. of as our client.

    Conducting weekly EH&S tool box meetings.

    Conducting daily EH&S meetings given to all the workers.

    To participate in the elaboration of Job Safety Analysis.

    Ensure that the work under their control has been planned and undertaken in such a way that

    recognized hazards and residual risks are managed and controlled making use of all available tools

    Ensure that EHS concerns are identified, communicated and understood by the workers under their

    supervision.

    Monitor and evaluate site personnel to identify workforce EHS performance issues for improvement,

    To verify that each worker has a uniform and basic equipments of Safety, like hardhats, protective

    lenses, protectors of ear, gloves, hi viz vest and steel toed boots.

    Control strictly the new workers under his charge.

    To make sure that all the equipment, tools and other elements to be used in the work are in good state

    Request special permission to the Project Chief on works of high risk, as: Raise critic with crane, works

    in confined places, works in height, works in heat and others.

    Request sanctions for the workers who infringe the norms of Safety in agreement to the procedure that

    is established in this Plan.

    5.10 Workers in General

    Work in a safe manner at all times, which is indispensable requirement to maintain the employment,

    obeying the dispositions of EH&S Plan.

    Communicate any act or substandard condition to their immediate supervisor.

    Attend the courses, talks and EH&S meetings..

    Obey the dispositions, norms, rules and instructions that give the company and Supervisors.

    Use the equipment of Safety that the company handed out by the company according to the task.

    Do not carry out unsafe works. If it is obliged it to communicate al Chief Project.

    Employees are responsible for their personal safety and the safety of their co/workers, through both

    their acts or their omissions

    Maintain the area in order and clean.

    Stopping their immediate or impinging work where they consider the work being performed is at risk

    or unsafe.

    5.11 Sub contrators and third sides

    The sub contractors are in the obligation to obey the norms of Safety, Health and Environment of

    HAUG and of our client.

    HAUG is in the obligation to instruct them and to reach them the pertinent documentation on these

    aspects.

    Likewise the suppliers will comply with the norms of Safety, Health and Environment of HAUG and our

    client.

    6. HAZARDS IDENTIFICATION AND RISK ANALYSIS (HIRA)

    Previously a begin a works will elaborate hazards identification and risk analysis of all work, step by stepconsidering the hazards, risks and safety measures to apply. One will consider any change in the

    7. INSPECTIONS

    Introduction

    The inspection constitutes one of the main techniques used for the identification of substandard

    conditions in the place of work and verification of the fulfilment of the appropriate standards of work.

    Objectives

    Identification of occupational risks in order to eliminate them or to treat them in the smaller possible

    time and of the suitable way more.

    Identification of defects or faults in the facilities, vehicles, equipment, machineries and tools.

    Verification of the implementation of the remedial actions derived from them inspection.General Procedure of the Inspection

    Preparation

    To make a list of the equipment, tools and materials to inspect.

    To review the previous reports of inspection in search of critical aspects for the pursuit.

    To prepare a route to have sufficient cover in all the area of inspection.

    To compile the control lists to use during the inspection.

    Inspection

    To follow the route predetermined and to use a list of control previously prepared. To describe in clear form, makes specific and concise the detected problem, contemplating its specific

    9. TRAINING

    Introduction

    Based on the hazards, risks, occupations, equipment, machines, tools, conditions of work and other

    elements, that is to say, previous to an analysis of qualification necessities, a Plan of Qualification will be

    Objectives

    To generate a high level of safety conscience in all our workers and employees.

    To offer to our personnel an optimal level of knowledge in the aspect of Enviroment, Safety and

    Induction to the New Personnel

    All worker of HAUG S.A. will obligatorily receive a Course of Induction to the New Personnel, before

    entering to toil in the Project. This course will be distributed by Project EH&S Manager on an appropriate

    atmosphere.

    Also all worker will obligatorily receive the Course of Induction offered by our client, before the beginning

    of the workings in the project.

    Daily Meetings of 5 minutesThey are offered to all the personnel of the project, being distributed by the Supervisor of area. In these

    char them will touch to agreed subjects to the hazards and existing risks in the work area.

    Weekly Meetings

    It will be realized weekly during 30 minutes by the Project Chief altogether with the Project EH&S

    Manager.

    EH&S Courses

    Workers will be received specify training in different subjects from safety, health and environment

    12. SAFETY STANDARDS

    HAUG S.A. comply strictly standards of our client during a activities in the present project.

    12.1 Housekeeping

    The order and the cleaning constitute fundamental activities that they must be executed by all the workers

    who toil in the project.

    Good housekeeping is an important element of accident prevention.

    Good housekeeping will be planned at the beginning of the job and be of a primary concern of all

    supervisors and workers throughout the entire project. This will be a part of the daily routine and clean up

    will be a continuous process.

    Some basic rules to consider are:

    The areas of work, passages, stairs and all the other areas will stay free of obstacles, equipment, and

    materials.

    In diverse sites appropriate containers for sweepings will be placed, which will be used for the

    positioning of remainders.

    The liquids (such as paintings, reliable, dissolvent, oils, and fats) and any other material or containers

    that have contained chemical, will be rejected in agreement with the procedures and regulations

    established for toxic wastes. The storage areas will stay cleanings and the materials properly stored.

    The electrical cords, wires, cables, hoses, and other temporary systems, will stay outside the surfaces

    to walk and in a high position.

    All the pieces of wood, rejected material and sweepings will have to be removed immediately of the

    work area.

    All the remainders of reliable, oily rags, and incendiary fuels, will stay in containers closed resistant to

    the fire until they are retired of the work site.

    12.2 Personal Protective Equipment

    Double eye protection is required for certain operations such as welding, cutting and brazing.

    All the personnel will use auditory protective equipment in the designated areas that exceed 85 dB.

    Fall protection is required for employees working above/below ground when not protected by an approved

    scaffold or staging.

    All respiratory protective equipment will count on the approval of the National Institute Occupational

    Safety and Health of EE.UU.

    The respiratory equipment will be used, stored and maintained in accordance with the requirements of themanufacturer.

    The respiratory equipment will be selected with base in the risks to which the worker will be exposed.

    The use of steel toed work boots of safety is required during all the works in the project.

    The equipment that has undergone any type of alteration and/or that is in inoperativo state will be retired

    of use in immediate form.

    12.3 Tools in general

    Every tool should be used only for the purpose that was designed.

    The tools should be maintained in optimum conditions of operation and use and they can just be used by

    qualified personnel.

    12.5 Scaffolds

    Will be established norms that guarantee that the scaffolds and work platforms systems be installed, they

    inspect, use and be disarmed according to safe practices and the pertinent rules. Nobody, will modify,

    alter or damage any scaffold. The modifications which are required will be performed only under

    authorization. Scaffolds will only be erected, modified or dismantled by qualified scaffolders.

    All scaffolds comply with standards of our client.

    Every scaffold must be reinspected and certified for use every 7 days and will use a scafftag system.

    The use of harness is obligatory when it works in scaffolds, which will have to be hooked to a safe point

    outside the scaffold. Also the use of elastic chin straps of plastic is obligatory.

    12.6 Ladders

    All ladders comply with standards of our client.

    The ladders will be reviewed before each use and periodically a authorized person will carry out

    inspection, registering the results in the respective format.

    The ladders that present deficiencies you would not used, being retired of use.

    Tools will never be put, materials or objects upon the steps.

    In same ladders it will not be able to work more than a person.

    12.7 Material Handling and Storage

    The manual manipulation refers to the task that the force of the worker is carried out by its means. It

    Use adequate gloves to avoid cuts or sharp pains in hands.

    If the load is heavy request help to raise it; but it is preferably recommended the use of mechanical

    carts.

    12.8 Hazardous Materials

    EH&S area will support responsible of warehouse about the degree of hazard of the material or

    substance, consequently the adequate material for the different processes of the production will be

    acquired.

    The responsible of warehouse will be performed weekly inventory of the hazardous materials of project.

    The management and utilization of the hazardous materials will be according to the indications of the

    manufacturer or supplier. It will serve to instruct the workers by means of the sheets of instruction of the

    manufacturer or Material Safety Data Sheets (MSDS).

    For the storage of these materials previously should be coordinated with the Project Chief to adopt the

    measures of the case.

    Every container that has contained hazardous materials should be signalized.

    A Hazardous Communication Program will be implement and spread a personnel of project.

    Material Safety Data Sheets (MSDS) in both English and Spanish will be maintained at the job site and

    will be available for examination from the EHS Supervisor.

    12.9 Fire Prevention and Protection

    A procedure written of storage, use and disposition of inflammable materials will be elaborated.

    The work area will stay clean, ordered and free of combustible materials and incendiary fuels.

    12.10 Welding and Cutting

    applicable lifting equipment as required.

    All rigging equipment is to be properly inspected color coded on a quarterly basis (yellow in our case). Arigging register will be maintained by each contractor for all rigging equipment used on the project.

    The Project Chief in coordination with Erection Supervisor and Project EH&S Manager will evaluate the

    situation and will decide he type of lifting.

    Equipment operators will be instructed in our policies and rules for operation and in equipment

    maintenance. They will be trained, certified, and qualified for each class of lifting equipment.

    The signal person (rigger) will be specifically designated and instructed in the use of proper hand signals.

    A chart containing the proper hand signals will be maintained in a prominent location

    on the job site.Equipment will be repaired in designated, safe locations.

    The condition of all cables, slings, hooks, and clamps will be inspected in accordance with the safety

    requirements.

    Personnel are not permitted to walk or work underneath suspended loads.

    12.15 Motor VehiclesAll driver will instructed in defensive handling and will have a certification respective.

    The conductors of vehicles should possess an internal license to conduct inside the project offered by the

    client if thus is required.

    The conductor before operating the vehicle or heavy equipment will inspect the system of brakes,

    mechanical and electric system, state of the tires and other elements.

    Every vehicle of the company will leave in commission of services with authorization of the Project Chief.

    In the project, the maximum velocity of circulation it will be the one that set the client

    The conductor of the vehicle is responsible for any damage or wound that cause as a result of an

    accident or incident in the public way or project.

    Every conductor or passenger should use the seat belt that carries in the cabin.

    He is prohibited to travel in the platform or hopper of the trucks, small trucks. Thus he is prohibited to

    travel in the stirrups and bumper of the vehicles and heavy equipment.

    The vehicles and heavy equipment should have their sonorous alarm of backward movement that be

    audible besides the environmental noise. It is prohibited to use the alarm musical or another

    unconventional sound.

    In the Project the vehicles should stationed in the places and form that indicate the client

    In the huts is obligatory the revision of the vehicle on the part of the Agents of Caution. Every material,equipment or active that enter or leave should be revised in the huts with the corresponding guide of

    remission. The conductor is the only head of any infringement to this norm.

    12.16 Confined SpaceBefore entering a confined space should will tramit to PERMIT REQUIRED CONFINED SPACE.

    Confined Space training will be provided to all employees who will be entering any permit or non-permit

    required confined space areas.

    The Confined Space Entry procedure will be used to control the work being conducted within the space.

    Adequate ventilation will be provided to those work areas, as needed, to provide and maintain a safe

    atmosphere. Ventilation units will be both portable and fixed, so as to provide the necessary flexibility as

    work areas are changed.

    On job sites where conditions warrant, oxygen deficiency meters, combustible gas meters, and other

    monitoring devices are provided. No person will be allowed to enter or work in an explosive or toxic

    atmosphere.

    In the entrance of the place the permission of work in confined spaces will be published the relation of

    personnel who has entered to the confined space, the hour of entrance and the time that will last their

    workings Periodically gas measurements will be made in the carried out place of work by personnel

    properly trained and authorized

    12.17 Construction Non DestructiveThe Quality Supervisor elaborated procedures of radiographic tests before the accomplishment of these

    types of work

    Only trained and qualified personnel are to perform radiography work.

    Each radiographer is required to wear a film badge, which records radiation exposure.

    A dosimeter is to be worn by each individual in radiation work. The dosimeter reading is to be recorded

    daily in the Quarterly Dosimeter Report.

    During this works will comply with Peru Regulation D.S. 009-97 EM Radiologic Safety.

    12.18 Abrasive Blasting and Painting

    All personnel will use Personal Protective Equipment respective during the accomplishment of abrasive

    blasting and painting

    Painting equipment and procedures are to be carefully examined frequently to assure potential hazards

    are eliminated. Items examined are rigging, scaffolding, staging, ladders, ropes, and cables. Of particular

    importance are vapour concentrations, fire and explosion hazards, toxic materials, and corrosive liquids.

    In enclosed spaces, the atmosphere will be checked for oxygen deficiency and combustible gas before

    starting work.

    Signs will be posted specifying no smoking, fires, or open lights within 20 meters of solvent handling

    areas.

    Extinguishers against fires in the zone of work will place.

    The Paint Superintendent will inspect the job site no less that weekly.

    12.19 Signs, signals and barricades

    The posters, signboards and posters of Safety must be made in agreement with the regulations of Safety

    The posters must be allusive to the Safety and must be placed in places where the personnel has easy

    access to its reading

    The signboards according to are their message must be put when hazard is noticed, warning, not to

    12.20 Excavation, trenching and shoring

    Prior to opening an excavation, provisions are to be made to accommodate underground installations.

    The walls and faces of all excavations, in which employees are exposed to danger from moving ground,

    will be guarded by a shoring system, sloping of the ground, or some other equivalent means.

    Excavations are to be inspected by a competent person after every rainstorm or other hazard increasing

    occurrence, and the protection against slides and cave-ins will be increased if necessary.

    The determination of the angle of repose and design of the supporting systems are based on careful

    evaluation of weather conditions, soil consistency, and other operations.Piling, cribbing, shoring, etc., are to be designed by a qualified person and will meet all accepted

    engineering requirements.

    In excavations which employees may be required to enter, excavated or other material will be effectively

    stored and retained at least 1/2 meter or more from the edge of the excavation unless other effective

    retaining devices are used.

    Support systems are to be planned and designed by a qualified person when excavation is in excess of 5

    meters in depth, adjacent to structures or improvements, or subject to vibration or ground water.

    Materials used for sheeting, sheet piling, cribbing, bracing, shoring, and underpinning will be in good

    serviceable condition.

    Except in hard rock, excavations below the level of the base of footing of any foundation or retaining wall

    is not permitted unless the wall is underpinned and all other precautions are taken to assure the stability

    of the adjacent walls for the protection of employees involved in excavation work.

    Precautions are to be taken to assure sidewalls are supported if heavy equipment is to be located near

    the edge.

    Where oxygen deficiency or gaseous conditions are possible, the atmosphere in an excavation is to be

    tested. Where adverse atmospheric conditions may be present, emergency rescue equipment will be

    provided.

    Shoring and/or bracing will be placed in every trench over 1 meter in depth regardless of soil type, except

    solid rock, unless banks are sloped to the angle of repose in accordance to soil types as based on

    recognized tables, e.g., OSHA regulations.

    Shoring and bracing are carried down the trench.

    All excavated material will be placed a minimum of 1/2 meter back from the trench edge.Adequate means of exit will be provided for every 5 meters of lateral travel in trenches more than 1 meter

    in depth.

    Where bracing of shoring trenches is not practical or economical due to unstable ground, moveable steel

    trench shields may be used.

    12.21 Sanitation

    Proper sanitation on the jobsite and adequate facilities are required to prevent unsafe drinking water and

    inadequate or disease prone sanitary facilities.

    12.22 First Aid

    IDENTIFICATION AND EVALUATION OF ENVIRONMENTAL ASPECTS

    Before the beginning of the labors there will be realized an evaluation of environmental risks detailed inthe place of work, identifying the possible impacts and determining the measures of control to adopt. For

    it there will elaborate a counterfoil of identification of environmental risks Any risk evaluation will be sent

    to the client for his review and approval, being later registered and filed.

    MONITORING AND CONTROL OF THE ENVIRONMENTAL MANAGEMENT

    The check of the fulfillment of the Plan of Environmental Managing will be realized by means of the

    monitoring and respective control, using for it the inspections and environmental audits, executed by the

    Project Chief.

    ENVIRONMENTAL AUDITS

    The Project EH&S Manager will realize environmental audits monthly, in which the fulfillment will happen

    of exposed in the Plan of Environmental Managing. Later a report will be sent to the Project Chief of

    HAUG S.A. by copy to the client, indicating not conformities (if they had) and the necessary corrective

    policies (including persons in charge and period) for the respective raising.

    MEASURES OF MANAGING, CONTROL AND MINIMIZATION OF ENVIRONMENTAL IMPACTS

    Storage of Materials in Stores Our materials will be located in strategic, due surrounded and put up

    signs places. Additional fire-extinguishers will be located against fires and first aid kit of the first aids

    in the place.

    Managing of Dangerous Materials The employment of dangerous materials in the labors of the

    Project does that one presents a risk for the environment, the safety and labour health. Every

    chemical product will possess(rely on) a safety leaf called leaf(sheet) MSDS (Material Safety Data

    Sheet), which details the characteristics, risks and measures of contingency in case of emergencies.

    One will take an inventory of the dangerous materials, as well as of the consumption.

    impermeable, in sizes and dimensions adapted to prevent them the content of the engines, tanks

    and cylinders being from spilt on the soil and for ende to prevent the possible spillages or

    escapes(fugues) from managing to strike to the environment and the health.

    One will possess(rely on) a kit of emergency consisting of spades, beaks(peaks), absorbent rags

    and black markets to face to any contingency.
