Utilisation de benzodiazepines en Belgique (données INAMI, mars 2009)
- Augmentation de la consommation de somnifères entre 1997 et 2008 : 16%
- Augmentation du coût lié à ces médicaments (même période) : 22%
- 9.4 % des belges prennent un somnifère plus de deux semaines
(6.7% des hommes, 12% des femmes)
- Nette augmentation avec l’âge (25% des personnes de plus de 75 ans)
- Très forte utilisation dans les homes et MRS (>30%, jusqu’à 68%)
- Plus le degré de scolarisation est élevé, plus le % d’utilisateurs est faible
(9.4% en moyenne, 18.5% des personnes faiblement scolarisées, 6% des personnes ayant suivi l’enseignement supérieur)

Withdrawing benzodiazepines
• Most people become dependent after >6 weeks continuous use
• Drugs with useful adjunctive properties for assisting in the discontinuation of benzodiazepines (-, carbamazepine, propranolol)
• Duration: schedules from 4 weeks to 2 years !• Recommandation CNS drugs 2009: < 6 months• Explanation: newly GABA receptor subtypes are synthetized
and some subtypes are down-regulated; restoration- regeneration is very slow
• Use long acting drug, diazepam (and its metabolites) for withdrawing ?

Insomnia in depressed patients
• Sleep disturbances in 90% of patients with major depression (mainly insomnia)
• SSRIs increase serotonin level and are antidepressants and anxiolytics through 5-HT1A receptor activation.
• SSRIs and serotonin also induce activation of 5HT2 receptors, leading to insomnia and changes in sleep architecture. Fluoxetine is associated with a 30% increase in the number of nocturnal awakenings
• Antidepressants with 5HT2 blocking properties (mirtazapine, nefazodone, trazodone) alleviate insomnia, improve sleep architecture. Nefazodone is associated with a 15% decrease in the number of nocturnal awakenings

Drug therapy versus behavioral therapy
• A meta-analysis (21 studies, Am.J. Psych. 2002) showed that both drug therapy and behavioral therapy had similar magnitudes of effects.
