The health care industry’s environment and health care reform challenges:

– Dwindling acute care volume and reimbursements

– Cost of care (too high)

– Ease of access (too little)

– Size of geographic footprint (not large enough)

o This is leading to more mergers and acquisitions

– Transitioning from the traditional inpatient-dominated model to one with more outpatient access points

A recent study by Kaufman Hall of seven counties in Illinois shows a continuing trend for declining hospital admissions.

– Study looked at the number of 2012 admissions for 71 IL hospitals:

• If you were to strip out the number of patients admitted for chronic/manageable diseases that could be treated in an outpatient setting or one-day stay, the number would be equal to 24% less hospital admissions in a year.

What is driving 2014 priorities?

2013 was a transitional year as Presence Health developed its strategy and set priorities for our journey to be a leader in transforming health care by delivering clinical excellence, outstanding value and exceptional experience to achieve better health for our communities.

2014 Priorities Established for Presence Health

What does it mean?

– Presence Health’s new Vision requires a new operating model.

– In 2013, we determined the best approach for Presence Health is to move to a more integrated model. Work is underway to determine the right leadership framework, structure and decision-making.

What is our focus for 2014?

– The focus will be to establish new processes while developing leaders to successfully implement dozens of Becoming ONE initiatives that were identified to create a more integrated and efficient health system.

– A large piece of work in this area will be the development of an IT platform that brings together data currently in a broad range of HR and Finance platforms. More information will be forthcoming on this.

Why is this priority important?

– Health care is changing faster than ever and an integrated model ensures we are a flexible organization.

2014 Focus for Operating Model

What does it mean?

– Presence Health’s portfolio is made up of the different components of our organization that we own and that provide value.

– In 2013, we assessed all of those assets to determine how all of these puzzle pieces align to the needs of our communities and the future delivery of health care. We also looked at gaps and began to explore partnerships to fill in critical capabilities needed.

What is our focus for 2014?

– We will continue to grow and balance our services to ensure we our meeting the needs of those we serve in each market.

– Heavy focus will be to ensure the right amount of outpatient care to meet growing needs of our patients.

Why is this priority important? – As health care moves to “population health management,” we need to

look at things like shelter, food and other issues that may impact the health of our patients to ensure they receive the care they need before they need more expensive hospital-based care.

What does it mean?

– Our Vision requires a new model for care delivery to patients, residents and families - a model that promotes value across the continuum and promotes health and wellness.

– The goals of this new care model are to improve the health of populations, improve the experience of individuals and lower the cost of care

What is our focus for 2014?

– Care coordination and a focus on creating networks of providers to keep patients well will be a primary element of this work.

• Why is this priority important?

– Promotes a seamless, high quality and consistent care delivery for our patients.

– Aligns us with health care reform when we begin to be paid for improved quality, access and outcomes rather than how many procedures we do.

2014 Priorities for Innovative Care Model

What does it mean?

– A culture is based on values, beliefs and norms in an organization. At Presence Health, we have an opportunity to shape our culture to engage our more than 22,000 employees and 4,000 physicians in creating an exceptional experience and outstanding value while delivering high quality care to every patient, resident and family member.

What is our focus for 2014?

– To continue to build upon the Service Promises while delivering an excellent experience to our patients, families and residents.

– To further develop strong leaders who can manage the significant amount of change underway in our organization and the industry.

– To launch new developmental programs for emerging leaders as well as high potential physician, clinician and other staff to help enhance leadership skills necessary for the future of health care operations.

– To continue utilizing Breakthrough Improvement as a way to engage all levels of our staff in problem solving.

Why is this priority important?

– This work further unites us as Presence Health.

– A strong culture also shapes the speed and efficiency with which things get done while also helping to shape the attitudes of outside stakeholders – like our patients and communities.

2014 Priorities for Culture Transformation

What does it mean? – Achieving an operating margin allows Presence Health to reinvest in

programs and services after our expenses are paid.

What is our focus for 2014? – Our goal for 2014 is to achieve a 1.5 percent operating margin

through a mix of performance improvement efforts, Becoming ONE initiatives and growth opportunities.

Why is this important? – Allows us to reinvest in services to meet the needs of our

communities. – Allows for a sustainable Mission to provide compassionate, holistic

care with a spirit of healing and hope in the communities we serve.

2014 Priorities for Financial Performance

What does it mean? – As we move toward our Vision of delivering clinical excellence,

outstanding value and exceptional experience to achieve better health for our communities, this priority is focused on ways for Presence Health to enhance our growth opportunities through greater integration, referral management and the advancement of key service lines.

What is our focus for 2014? – A primary element of this work for Presence health will be in determining

approaches for keeping referrals within the health system. – Our growth agenda is focused on shifting care from inpatient to lower

cost of care settings, including home health, virtual health and ambulatory.

Why is this important? – Our focus on growth and integration helps set us up for success to

sustain us now and well into the future to ensure we are hitting on all cylinders when it comes to delivering the most efficient and high quality care to those we serve.
