The adjustable double-action Cortical-Fix sinus elevator is an innovative instrument for oral surgery developed and patented by Microdent, and intended to augment by compaction, in a slowly and controlled manner, the second cortex of the subantral sinus area.

With the help of this device, insertion of longer implants is achieved improving, then, stability and security conditions and osseointegration in the area, often with poor bone quality.

The innovative design of the Cortical-Fix enables a very simple functionality and has a low traumatic incidence since the fixation of the device in the bone is self-tapping and follows the same surgical protocol as if it were an implant. Furthermore, once the plunger has been activated, the Cortical-Fix body is removed from the alveolus to which it is fixed replacing it by a longer, final implant with appropriate measures for excellent primary retention.

The Cortical-Fix device does not have the same dimensions as the implant, therefore it requires a different sur-gical protocol regarding the preparation of the implant alveolus. The following surgical protocol must be particu-larised according to the patient’s bone conditions.

Opening of the flap to access the bone support.

Welding with cortical drill (maximum depth of 3.0mm).

Optionally, a drilling with a diameter of 1.80mm can be performed (preferably using a drill with a stop) controlling the depth until it reaches the resistant and hard second cortex. Very low rotation speed, maximum between 300-400rpm.

Drilling with stop drill.Maximum diameter of 2.80mm for the placement of a 4.20mm implant.Maximum diameter of 3.20mm for the placement of a 5.00mm implant.IMPORTANTThe drilling sequence must be performed with extreme caution, paying close attention to the substantial change in density and hardness of the second cortex. Maximum rotation speed between 200-300rpm.

Manual drilling.Manual drills are very useful to remove the remaining bone in the alveolus that the drill tip has not been able to eliminate.

Use of the bone compactor. By using a compactor in an area with poor bone quality before using the Cortical-Fix, the bone will harden and compact.IMPORTANTThe compactor is inserted by pressing it in the alveolus in order to mould the thread until the drilling depth previously established is reached. It is necessary not to exceed the required depth with the compactor due to risk of destroying the thread in the alveolus and, consequently, annul the retentive capacity of the device body.

Fixing of the Cortical-Fix.The body of the device must be completely screwed in in order to achieve good retention and to prevent any backward movement when the plunger shaft is activated.

IMPORTANTThe device body is fixed in the maxillary with the plunger in non-activated position.

The plunger drive must be done slowly to ensure the displacement and lift of the bony wall of the sinus by the pressure exerted, avoiding at the same time a possible fracture of the bone, until the appropriate height is reached.

Once the lift has been achieved, completely unscrew the Cortical-Fix from the area.

Insertion of the widest and longest implant, according to the elevation achieved, and suturing of the soft tissue.

Prior to starting the surgical protocol for the Cortical-Fix device, practitioners need to have as much information as possible about the morphology and dimensions of the subantral area.The technique described below can be supplemented, if required, with the use of a bone graft made of either autologous materials, biomaterials of animal origin or synthetic materials.

CLINICAL CASE Nº1

Observe the pneumatisation of the maxil-lary sinus in quadrant II.

Once a Ø M-4210mm implant has been placed, proceed to drill the implant bed to place the implants corresponding to teeth 2.5 and 2.6.

Use of anaesthesia at crestal level. Note the good quality of the keratinized gums.

Full-thickness incision, slightly palatalized.

Drilling of the cortex in tooth 2.6 with a cortical drill. With a 1.8mm drill, drilling 1mm before the sinus floor. Radiographic control.

Next, insertion of a Ø 2.80mm drill to a depth of 4mm and finally a Ø 3.20mm drill to a depth of 2mm.

Insertion of a Cortical-Fix with the plunger open (deactivated).

Placing of an adaptor on the Cortical-Fix, it is then activated by slowly turning it clock-wise to its stop. It is internally acting.

CLINICAL CASE Nº2

32-year-old female patient with tooth 2.6 extracted. 16 months later, the patient decided to get an implant to replace the missing tooth.

Observe on the radiography the pneuma-tisation of the sinus in its middle part with 6.7mm. It is decided to place a Microdent 5.0 x 10mm implant.

Crestal incision followed by drilling with a cortical drill.

Continuation of protocol with a pilot drill (Ø 1.80mm). Drilling 1mm before the sinus floor.

Control radiography to check the depth of the drilling, in this case 5.5mm.

Drilling in a staggered fashion: with a Ø 2.8mm drill down to 2mm and with a Ø 3.2mm drill down 1mm more.
