    Instructions for Filling Up of OMR Application Form A

    1 Program Code: Fill the 3-letter program code in the boxes and also darken the appropriate ovals. Program code may be seen in column 3 of thetables given in Section 10 (Page 16-24) of the prospectus. They are also written in the first column of the tables in Annexure I (Page 25-57).

    Examples:

    Guidelines for filling up the OMR Form are given in Annexure III (Page 60) of the Prospectus.

    Instructions for Filling Up the Front Page (Side 1) of the OMR Form

    Program Code for Diploma in ElementaryTeacher Education is D03

    (Pl see. Pages: 20 & 52 of the Prospectus 2014-15)

    Program code for Diploma in (Civil/Computer/Electrical/ Electronics/Mechanical) is D01

    (Pl see. Pages: 20 & 51 of the Prospectus 2014-15)

    Program Code for M.Arch/M.Ekistics is M59(Pl see. Pages: 21 & 39 of the Prospectus 2014-15)

    2 Preference Code: This field is applicable to programs of Faculty of Architecture only (M59 and B21).

    Example: Suppose you wish to apply for M.Arch/M.Ekistic group of programs M59. Look at the the entries shown in Annexure I (Page 39) of theProspectus.If M.Arch (Medical Architecture) is your First Preference, M.Ekistics is the Second Preference, M.Arch (Pedagogy) is Third, M.Arch(Building Services) is Fourth, M.Arch (Recreational Architecture) is Fifth, and M.Arch (Urban Regeneration) is sixth preference., fill the preferencecolumn in the following way:

    Branch Preference (For applicants of B.Tech. B14/Diploma Engineering (Regular) D01 and Diploma Engineering (Evening) D02programs only): Fill in the preference in the Boxes and darken appropriate ovals.at later stage.

    Preference of Centre (for B.Tech. B14 only):You should give your preferences of Entrance Test Centers in the Boxes and also darken theappropriate ovals. (This information is required to facilitate the applicants as far as possible. However, it is the sole prerogative of the University toallot the Entrance Test Centre to the applicants)

    This Field in applicable to programs in Faculty of Education only

    In the first boxon the top, candidates of B.Ed. B22, B.Ed. Nursery B24and B.Ed. Special Education B23can indicate their medium asEnglish, Hindi or Urdu by filling the appropriate oval and writing E or H or U, as the case may be, in the box.

    The second boxin Field 5 is only applicable to Diploma in ETE D03.You should indicate your medium as Urdu or Hindi by filling the appropriateoval and writing U or H, as the case may be, in the box.

    The third boxin Field 5 is only applicable to B.Ed. (Spl. Education) B23and M.Ed. (Spl. Education) M37. You should indicate the desiredspecialization by filling the appropriate oval provided against Learning Disability and Visual Impairment and by writing L or V, as the casemay be, in the box.

    The fourth boxin Field 5 (on the right side) is to be used by the candidates of B.Ed. B22and Diploma ETE D03programs for indicating theisubject codes. Subject codes are given in Code Table - 4 of Annexure IV (Page 62) of the Prospectus.

    The preference would be treated as final and cannot be changed(Giving all the five preferences is compulsory)

    For all other remaining programs,you should fill the percentage of marks obtained in the qualifying examination which makes you eligible for theprogram based on the eligibility criteria given in Annexure I of the prospectus. In case, you are eligible under more than one criteria, i f applicableto a program, you may fill the higher percentage. In case of grades or Final Cumulative Performance equivalent percentage is to be filled.

    Example: If you have secure 839 out of 1000, your percentage is 83.90%, write the percentage in the boxes as shown below:

    Educational/Professional Qualification: You should write the details of your qualification as per format given. If result of an examination is not yedeclared, the candidate should write R.A. under percentage (of aggregate).

    Intervening Period: Write gap period, if any, since passing the qualifying examination. (Refer to section 5.6(ii) Page 10 of the Prospectus for details)

    Permanent Address: Write your permanent address with Pin Code. Also write your Contact Telephone Number. (Refer to section 5.6(ii) Page 10 ofthe Prospectus for details)

    Name of the Candidate: Write your name, in capital letters, as given in your High School (Class-X) Certificate of the Board/University. Each letteshould be filled in one Box. Leave a Blank Box between first, middle and last name. You should also darken appropriate ovals under each Box(Example: Candidate's Name : ASHISH WINSTON KHAN)

    For applicants of M.Tech. (Mechanical Engineering) M48 only: Fill in the preference in the Boxes and darken appropriate ovals. The preferencewould be treated as final and cannot be changed at later stage.(Giving all the three preferences is compulsory)

    Relaxation Under Special Provisions: Candidates claiming relaxation under NCC or NSS certification may fill the appropriate oval. Also write C oS accordingly in the box. The candidate should specifically read sections 4.4 and 4.5 (Page 8) of the Prospectus for details

    Applicant's Category:Fill in the Category Code in the Box and darken appropriate oval. Candidates should specifically read section 3 (Pages 6-7) othe Prospectus for details.

    Foreign National: Fill 'Y', if the applicant is a Foreign National, else fill 'N' in the Box and darken appropriate oval.

    Kashmiri Migrant: Fill 'Y', if the applicant is a Kashmiri Migrant, else fill 'N' in the Box and darken appropriate oval. (Applicant has to provide propecertificate, at the time of verification of documents, to claim this category)

    Gender: Fill in your Gender Code and darken the appropriate oval. (M - for Male and F - for Female)

    Date of Birth: Enter date of birth as given in your High School (Class X) Certificate in the space provided and darken the appropriate ovals(Example: If your date of birth is September 01,1996, fill it as shown below):

    State of Domicile: Codes for Indian States and Union Territories are listed in Code Table-1 of Annexure IV (Page 61). Write the code for state oyour domicile in the Boxes and darken appropriate ovals.

    Mobile Number: Write your or parent's mobile number (For SMS Alert) in theBoxes and also darken the appropriate ovals.

    Pin Code: Write Pin Code of your mail ing address in the Boxes and darken the appropriate ovals.

    Mailing Address: Write your complete mailing address with Pin Code where you want your Admit Card to be delivered. Write in CAPITAL LETTERS onlyusing Black Ballpoint Pen. Also write your Contact Telephone Number. Any request for change in address will not be entertained at a later stage.

    Photograph:Paste a recent coloured photograph of the size and specifications as described in Annexure III of the Prospectus (Page 60).

    Visible Mark of Identification: Write visible mark of identification.Candidate's Signature: You must sign within the Box using black ballpoint pen.

    Candidate's Thumb Impression: You must also put your thumb impression within the Box at the location mentioned in the Application Form.(lefthumb impression for male/right thumb impression for female).

    Instructions for Filling Up the Back Page (Side 2) of the OMR Form

    Mother's Name: Write your Mother's name in CAPITAL LETTERS. Each letter should be written in one Box. Leave a Blank Box between first, middle& last name and also darken appropriate ovals under each Box.

    Father's Name: Write your Father's name in CAPITAL LETTERS. Each letter should be written in one Box. Leave a Blank Box between first, middle &last name and also darken appropriate ovals under each Box.

    Qualifying Exam Details:

    Qualifying Examination Code: For each program, minimum eligibility qualification requirement is given in Annexure I (Pages 25-57) of theProspectus. Identify the qualifying examination of your program of interest by consulting Annexure I and find out its code by referring to CodeTable - 3 of Annexure IV (Page 62) of the Prospectus. In the Boxes, write code of the identified qualifying examination and darken the appropriateovals.

    Example: Senior Sec. School Certificate is the qualifying examination for admission to Bachelor of Dentistry (BDS) (Program Code: B20). As per CodeTable 3 (Page 62), the code for Sr. Sec. School Certificate (equivalent to XII) is 2. Write 02 in the Boxes as shown below:

    Board/University: The codes for Boards/Universities are given in Code Table - 2 (i) & (ii) of Annexure IV (Pages 61, 62). Write the code of theBoard/University from where you have passed the qualifying examination in the Boxes. Also darken the appropriate ovals.Example: If you are applying for B.Tech. and have passed the qualifying examination from CBSE Board, the code will be 104. Write this code in theBoxes as shown below):

    Year of Passing/Appearing: Write the year of passing the qualifying examination or if you are appearing in it, in the Boxes provided and darken thecorresponding ovals.Example: If you have passed the qualifying examination or you are appearing in 2014, write as shown below:

    Percentage:The candidates of programs listed in the following table should fill the percentage of marks as specified in column-3:
