To whom it may concern,

Greetings of Peace!

For my explanation letter, I too want to give my side of the story. It was April 8, 2009

when Mrs. Emmlyn Rose Siajuat-Lim arrived at the E.R. of AGH together with her was

Mr. John Edgar Lim her husband, Justin Neil Lim and Gene Mitchelle Perez to file a

medico legal case regarding their injury. And I as a nurse assertively assess their needs. I

have to do a skin test and then I took a new T-syringe from the supplies, and then, took

the ready to use syringe from the medicine tray and hooked it with a new needle from the

T-syringe I first took from the supplies. I testify that we put on new needles every time

we do skin test.

I as a nurse too, I am also aware of any Nursing Principles especially sterility and other

Nursing Procedure. And I would like to emphasize it is part of our orientation and

practice inside the emergency rooms that we replace new needles every time we use it.

The thing that I would like to apologize, is that I have used wrong sets of words coming

out of the blues that cause them trouble to this time

I deeply apologize to the incident happened and hoping for your kind consideration. And

will assure you it will not happen again.
