The communications kit sent to you in mid-May are the only communications to be sent by Baxter.
No communications will be sent directly to patients about the new HomeChoice SmartCare software – all the communications and training materials are intended
to be distributed to them by you.

• ALL patients will be swapped with HomeChoice SmartCaresoftware version
NOTE: Reactive (failure) swaps can occur at any
time and will impact proactive swap
sequences
If a patient experiences a cycler failure and requires
an immediate reactive swap, that patient will no longer have their cycler
Proactive Swaps(Excluding DaVita)
Baxter contacts patient to arrange for cycler swap
Patient schedules delivery of cycler swap with Baxter
Patient immediately informs nurse of swap for training
Reactive/Failure Swaps ALL Customers

Cycler Swaps Communications
A list of proactive patient swap sequences will be provided to all centers
(via your AE/CE), with a 1-3 week lead time
Exact dates will NOT be included, since exact dates are impacted by patient response time
