Dr. Thomas Fogarty, inventor of the balloon catheter and numerous other important medical devices, is eager to train the next generation of inventors with his newest idea: an educational center for innovators called the Fogarty Institute. The institute is opening at El Camino Hospital later this month. Fogarty, 73 and a former Stan- ford professor, is most famous for his balloon catheter, a tube that helps remove blot clots in arteries. More recently, he invented the AneuRx stent graft procedure, a device and method that helps prevent bulging blood vessels from rupturing. He says wants to look at common diseases, not unusual or rare ones, from his new El Camino facility. Stanford can handle the rare ones, he said. Atrial fibrilation, or irregular heart beats, is a common dis- ease that Fogarty believes can be treated more efficiently, with virtually no pain and with a shorter procedure. He invented a new device, the “CyberHeart,” to deal with the disease, which is currently under medical review. Fogarty expects the CyberHeart will “cause a paradigm shift” in medicine if it works, he said. But Fogarty’s interests range beyond the heart. Diabetic patients, for instance, need a bet- ter way to receive insulin, he said. Right now patients take insulin shots through a syringe. “We could probably come up with a novel delivery system. The end goal is to create an artificial pancreas,” he said. Another process, stapling the stomach to reduce obesity, could also be rethought, he said. Cur- rently, doctors cut the body from the outside to get to the stomach for stapling, But Fogarty has a new process in mind in which a device is inserted through the mouth. the patent. “It’s a very good name. It’s very descriptive. It decreases the size of the stomach so you start to feel full with minimal amounts of food intake, and that’s less invasive.” Most doctors, he noted, do not like to challenge the usual way of doing things. “They don’t want to deal with a non-standard approach because they have all kinds of liabili-ties the minute they do,” he said. But in his role as inven-tor, Fogarty has no fear of such a fate. Despite a prolific career spanning 40 years, he’s only been sued twice for malpractice, and both suits were dismissed by the judge, he said. “If you want to be an inventor, be prepared to be thought of as crazy, inappropriate, dishonest, out of bounds — and you’re going to have to accept that,” he said softly. “I guarantee you that’s what’s going to happen.”

‘A whole bunch of innovators’

When Fogarty was in medical school in his 20s, he and his pro-fessor first introduced the balloon catheter to the medical world. Four major medical journals, including the New England Journal of Medi-cine, rejected him. More than 40 years later, Foga-rty’s invention has helped millions of patients. Before the catheter was invented, 50 percent of patients who had surgery for coronary artery disease died. Of the 50 per-cent who survived, half of them had an amputation, he said. The balloon catheter changed all that, Fogarty said. Today, the same surgeries have mortality

rates as low as 1 percent. Fogarty hopes to replicate such successes with his Fogarty Insti-tute, a 4,000-square-foot laborato-ry located at El Camino Hospital. There, physicians with a penchant

for innovation, along with engi-neers, technicians and patients who want something better, can meet to exchange ideas and improve medical treatments, Foga-rty said. It is the only place of its kind, according to both Fogarty and hospi-tal administrators. “I hope we’re going to create a whole bunch of innovators,” he

said. “When technology is not available, you think of how many patients continue to be without a technology that can really help. That hurts, if you’ve ever cared for patients.” A younger generation of inven-tors needs encouragement in the face of a tighter regulatory atmo-sphere, Fogarty said, and the seasoned inventor plans to be a mentor to them. The El Camino location also should make the pro-cess easier, as doctors can examine first-hand the existing medical technology and processes in use. Fogarty and his trainees will also have direct access to doctors and patients so they can discuss ways to make surgery less painful, less invasive and more successful. But the best help in improving treatment does not come from the doctors, engineers and scientists, Fogarty said: “The heroes of inno-vation in medicine are the patients, from my perspective.” V

Q : On medical TV shows, doctors always order a

“CBC.” What is it and why is it such a standard test?

A: TV shows emphasize the drama of a CBC in

the emergency room, but this test is typi-cally done as part of a rou-tine physical examination. “CBC” stands for Complete Blood Count and it is a key blood test for diagnosing a patient’s condition because it provides important informa-tion about the types and num-bers of cells in the blood. Evaluating blood test results is like reading Cliff Notes for “War and Peace.” You get a comprehensive overview of a large and complicated entity in a relatively non-invasive way. A CBC provides basic informa-tion about the general state of your health so your doctor can diagnose what is going on inside. An increase or decrease in the numbers of the different types of white blood cells can help identify infection, an allergic or toxic reaction to certain medications or chemi-cals, and numerous other medical conditions. Some universal symptoms — like fever, fatigue or weight loss — can indicate a more serious condition. The CBC can often diagnose whether that is the case. This test finds infections and diseases of the blood, detects anemia, monitors a patient’s response to medica-tions or special treatment (such as chemotherapy and radiation) and much more.

Q: In the wake of the recent toy recall, should my child

and I be tested for lead?

A: There are no safe levels of childhood exposure to

lead, so if your child played with a recalled toy for a month or more, take them to their doctor for a blood test, the quickest way to find out if your child has accumulated hazard-ous amounts of lead. Read and heed the recall lists and get any toys containing lead out of your house.

Remember that toys are less of a threat than old houses and buildings (1978 or older), because they are more likely to have lead-based paint. If lead paint exposure is possible, then blood tests are recom-mended. Lead poisoning is seldom triggered by a single event of ingestion or exposure. The com-mon danger is exposure on a regular or daily basis that leads to the accumulation of lead in the body. That’s why the lead-tainted toys are so threatening for children, because youngsters tend to handle and mouth their toys a lot. During your next exam, talk with your doctor about known and potential exposures. This will help evaluate whether your or your family’s blood lead levels should be mea-sured.

Q: I hate needles! Any sug-gestions for helping me

get over my phobia?

A: I’ve practiced medicine for many years and I have

yet to meet someone who likes needles! The prick can be uncomfortable, but the fear of the needle is usually much more painful than the test itself. It is the anticipation of the shot that prompts the anxiety. Your nurse or doctor will try to reduce your anxiety by explaining what is going to happen and why. Appreciation of the benefit of the vaccine or test can help reduce angst. Topical anesthetics can be used to numb the skin before needle procedures, which diminishes the discomfort and gets you on the path to defeating your needle-phobia. Don’t let your fear of hypo-dermic needles keep you away from the doctor. Skipping booster vaccinations or impor-tant blood tests can lead to life-threatening problems. V

NEW DEVICE COULD HELP PREVENT SEIZURES A device created in Mountain View and designed to prevent epileptic seizures before they start is currently being tested in trials across the country. Called the Responsive Neuro-stimulator System, or RNS, the device is nested in a cavity carved into the bone of the skull. Accord-ing to its developer, Mountain View-based NeuroPace, battery-powered electrodes are placed in the parts of the brain that are damaged, and a computer chip within the RNS monitors the patient’s brain waves. When it senses buildup of electrical energy preceding a seizure, it stimulates the affected area and disrupts the impending seizure pattern, the company says.
