Along with lung cancer (caused by cigarette smoking in 87% of cases), thesehealth conditions are the most significant causes of premature death and loss of quality of life due to healthproblems. At the same time it is well established that individuals who consume a higher plant-based diet, richin vegetables, fruits, beans, peas and whole grains, and fewer fats from the sources listed above, aregenerally at much lower risk for premature development of cardiovascular disease and many forms of cancer.As such, the results of your dietary assessment that follows and the modifications you are being asked to

    consider may seem a bit severe in some cases.     the bloodstream tends to stick to the artery walls, causing narrowing and increasing risk of heart attack,stroke and other vascular disorders. An important part of reducing heart attack and stroke involves keepingyour blood cholesterol level below 5.2 mmol/L (200 mg per dL) and more ideally below 3.9 mmol/L (150mg/dL). Even moderate amounts of animal fat consumption is linked to increased risk of colon cancer andreproductive organ cancers and is also a major contributing cause of weight gain, as saturated fat tends to bestored in fat cells, enlarging their size, and increasing body fat. Finally, high fat animal products containconsiderable amounts of a polyunsaturated fat called arachidonic acid (AA), which the body converts into ahormone known as PG-2. PG-2 makes the skin less smooth, soft and moist than it has the genetic potential

    to be. PG-2 also contributes to increased risk of cancer and cardiovascular disease by speeding up celldivision rates and increasing the stickiness of the blood. PG-2 also aggravates inflammatory states, such asarthritis. Most experts agree that it is best to follow a vegetarian lifestyle (using sound practices of nutrition) orto consume only skinless chicken breast, turkey breast, Cornish hen, fish and soy products, as protein foodalternatives to high fat meat products, and to consume non fat or 1% milk or yogurt and cheeses that are lessthan 4% milk fat or soy-based cheeses, as protein food alternatives to high fat dairy products.Congratulations, once again, for adhering to a diet that contains very little, if any, animal fat. It is a veryimportant measure in reducing risk of degenerative diseases and improving your chances for extended yearsof functional living.Your practice of using butter on bread, toast and other bread products is a cause for concern. Butter

    is very high in saturated fat and cholesterol. It is far better to use jam on bread products as an alternative.Ingesting too much saturated fat triggers the production of cholesterol in the body, which often raises bloodcholesterol levels. The fat

    content of these foods significantly contributes to weight gain and depending upon the type of fat present, canincrease blood cholesterol levels, which is a key risk factor for heart attack, stroke and other vasculardiseases.     It is impressive that you have maintained a low intake of fried snack foods and regular chocolate

    products. Fried snack foods contribute excessive fat calories to the diet, which promote weight gain,vascular disease and may increase risk of colon and other cancers. Regular chocolate bars and chocolateproducts are a rich source of saturated fat, which can elevate blood cholesterol levels, increasing risk of heartattack, stroke and other vascular diseases. Saturated fat intake also contributes to weight gain and is linkedto colon cancer and reproductive organ cancers.

    It is impressive that you have maintained a low intake of highly refined sugar-containing foods andbeverages.     Individuals who consume 5 or more servings per day of fruit and vegetables have half the cancer rates asthose who consume less than 5 servings of fruit and vegetables per day, on average.Your diet appears to lack proper attention to fruit intake. Like many vegetables, fruits contain vitamins,

    minerals and other protective nutrients that help prevent cancer and vascular disease (heart attack andstroke). Individuals who consume 5 or more servings per day of fruit and vegetables have half the cancerrates as those who consume less than 5 servings of fruit and vegetables per day, on average.Your low intake of alcohol is most impressive from a health-risk standpoint . Although moderate

    quantities of alcohol (no more than one alcoholic drink in any 24-hour period) may benefit some aspects ofcardiovascular health, alcohol consumption of just two drinks per day, on average, is associated withdoubling or tripling the risk of colon and breast cancer, and even lesser amounts are associated withincreased prostate cancer risk, according to some large studies. Alcohol is also a contributing factor to mouthand throat cancer. It causes damage to liver and brain cells, impairs driving ability, and can increase bloodpressure and blood triglyceride levels, which contribute to heart disease, stroke and other vascular diseases.The National Cancer Institute indicates that alcohol consumption is the second most important environmentalcause of cancer, after cigarette smoking, accounting for 3% of all cancer cases. It appears that the body cansafely handle one alcohol drink per day before certain health risks begin to increase. One alcoholic drink isdefined as one 12 ounce beer, one ounce of hard liquor, or one 5 ounce glass of wine.

    Body Mass Index

    Your Body Mass Index, which is calculated by dividing your weight in kilograms by your height in meters

    squared (wt/ht2) is known to be associated with risk of heart disease, adult onset diabetes as well as breastcancer in postmenopausal women. In men, a high body mass index may also increase risk of prostatecancer. The Body Mass Index also provides a good indicator of a persons overall percentage of body fat,unless the person has increased muscle development from weight training, in which case the Body MassIndex is not a true reflection of health risk or body fat percentage. However, waist circumference remains agood indicator of health risks even in individuals who have muscle development from weight training.     This is regarded as Grade 1 obesity and is associated with a mild to moderate increase risk for the

    degenerative diseases mentioned above. We suggest that you strive to get your body mass index below26, unless you are highly muscular with a body fat percentage below 13% for men and 16% for women.Fortunately, you are not that far away from being within the optimal range of the Body Mass Index, which is20-25. This is very attainable in your case and is a very worthwhile goal, as you will significantly reducecertain health risks by losing this modest amount of weight.     Your present waist circumference is 29. Your waist circumference is at or below 32 inches, which does notcarry an increased risk of the development of cardiovascular disease or adult onset diabetes. It is importantthat you strive to maintain a waist circumference below at or below 32 inches throughout your entire life, asincreased weight gain in the abdominal area is highly correlated with a shortened life span, often due tocardiovascular disease and diabetes.

    Exercise and Physical Activity

    It is important that you speak with your physician prior to making any changes to your current physical activitypatterns. Also keep in mind that a high level of cardiovascular fitness does not confer complete protectionagainst heart disease. As such, you still require a periodic physical exam from you attending physician, whichshould also include blood work to check for other cardiovascular disease risk factors (e.g. high cholesterol,high blood pressure). This is an important consideration as many physically fit individuals die each year fromheart attacks because they had other risk factors for heart disease that went undetected or were ignored.

    Maximum attainable heart rate = 220 minus your age The low end of your aerobic heart-rate zone is calculated by multiplying your maximum attainable heart

    rate by 0.6 The high end of your aerobic heart-rate zone is calculated by multiplying your maximum attainable heart

    rate by 0.85

    Based upon your present age, your aerobic training zone is between 102 and145 heart beats per minute.Exercising at 60 percent of your maximum attainable rate is so comfortable that you could maintain aconversation without getting winded. Unknowingly, most people perform aerobic exercise at a pace that putstheir heart rate between 70 and 75 percent of their maximum attainable rate. If you are just starting withaerobic exercise, you should aim for about 65 percent of your maximum attainable rate.

    1. It is most ideal to perform at least 2-3 different exercises for each body part (i.e. For chest--BenchPress, Incline Press, Flies)

    body sculpting, general anti-aging purposes, using 8-10 repetitions per set to muscular failure6. Remember that to gain muscle size and definition you must ingest at least 0.5 grams of protein for

    every pound you weight (or about 1.2 grams of protein for every kilogram you weigh). Some eliteathletes require up to 2.2 grams of protein per kg, if they may spend three or more hours per daytraining. As an example, if you weigh 80 kilograms, you should consider ingesting 100 to 150 grams ofprotein each day (1.2 grams of protein per kg of body weight) to build your muscles, depending on the

    intensity, duration and frequency of your workouts. Note that your weight in pounds can be convertedinto kilograms by dividing your weight in pounds by 2.2. In your particular case, your proteinrequirement would be approximately 65 grams per day, if you performed 30 - 45 minutes of aerobicendurance exercise 5-7 days per week, and resistance training, at least 3-4 days per week (30-60minute routine), calculated as 1.25 times your weight in kilograms.     Speak to your health practitioner or fitness professional about a customized aerobic, resistance training, andstretching program that best suits your individual circumstances.     Smoking cigarettes increases free radical exposure and depletes the body of antioxidants, B-vitamins andminerals. Note that the free radicals contained in cigarette smoke cause lung cancer and increase risk forcancer of the mouth, throat, stomach, pancreas and possibly colon. Cigarette smoking is considered to be thenumber one environmental cause of cancer and is linked to more than 30% of all cancer cases. Eighty-seven

    percent of lung cancer cases occur in smokers. Cigarette smoking also contributes to emphysema and otherrespiratory diseases, and is a cardinal risk factor for cardiovascular disease. Cigarette smoke constricts bloodvessels, promotes the deposition of cholesterol into the artery wall -narrowing arteries, and increases thestickiness of blood cells promoting the development of abnormal clots that plug up the arteries. Togetherthese effects increase risk of heart disease, stroke and other vascular diseases.

    Feeling chronically fatigued in the absence of disease is often associated with multiple vitamin and mineraldeficiencies.

    Irregular eating patterns tend to contribute to an inadequate intake of essential vitamins and minerals andoften leads to at least marginal deficiencies in many vitamins and minerals.

Psoriasis is a chronic skin condition that causes skin cellsto grow too quickly, resulting in thick, white, or red patches on the knees, elbows, scalp, hands, feet, or lowerback.     After menopause, women become less able to clear cholesterol from their blood stream, which greatlycontributes to the fact that heart disease is the number one cause of death in postmenopausal women.During the pre-menopausal stage of life, high circulating estrogen levels increase the production ofLDL-cholesterol receptors, which enable cells to extract LDL-cholesterol (low density lipoprotein-cholesterol,which is known to increase risk of heart attack and stroke) from the blood stream and use it for variouspurposes. The dramatic drop off in circulating estrogen levels in menopause reduces the ability of cells toproduce LDL-cholesterol receptors. As a result there is a strong tendency for cholesterol to accumulate in theblood stream, stick to the walls of the arteries and cause narrowing of coronary blood vessels; leading toheart attack. As a high saturated fat diet is the main culprit in raising LDL-cholesterol levels, postmenopausalwomen should adjust their diet to lower their saturated fat intake in order to keep blood cholesterol levelsbelow 5.2 mmol/L (200 mg per dL). This implies that the one should choose lower fat protein foods consistingof chicken, turkey, Cornish hen and fish, and that all milk and yogurt products consumed are non-fat or 1%varieties.     OSTEOPOROSIS

    The decline in estrogen levels that accompanies the menopausal years also permits calcium to leak out ofbone into the blood stream, where it will eventually become filtered by the kidneys and exit the body in theurine. This of course, can lead to osteoporosis, which increases risk of fractures. Osteoporosis is reaching

    epidemic proportions in our society largely due to insufficient calcium intake and accumulation in bone,especially between ages 11 and 24, and loss of calcium from bone during the menopausal years. It should benoted that Canadian statistics indicate that complications from osteoporotic hip fractures (e.g., thedevelopment of pneumonia) result in more deaths each year than the combined mortality rate from breastand ovarian cancers. The lifestyle recipe to help prevent osteoporosis during the menopausal years is asfollows:

    1. Ingest 1,500 mg per day of calcium. This can be through a combination of calcium from food andsupplements. Note that a high potency multivitamin and mineral should provide 400-500 mg of calciumas a starting point.

    2. Supplement with 600 to 1,000 IU of Vitamin D. For general health reasons women should considertaking a high potency multiple vitamin and mineral throughout adult life that contains 400 IU of VitaminD. Studies show that postmenopausal women ingesting an additional 200 to 400 IU of Vitamin D perday may reduce their risk of hip fractures by approximately 50%. As we age, our kidneys reduce theirability to convert 25-hydroxyvitamin D to 1,25-dihydroxyvitamin D, which is twice as powerful a form ofVitamin D, than is 25-hydroxyvitamin D. However, studies indicate that by increasing blood levels of25-hydroxyvitamin D, through the intake of Vitamin D supplements (600 - 1,000 IU per day), apostmenopausal women can compensate for the drop off in 1,25-hydroxyvitamin D synthesis, andthereby, significantly reduce her risk of osteoporotic fractures.

    3. Perform weight-bearing and/or resisted exercises 4 to 7 times per week. Weight bearing exercise such

    as walking or jogging, and weight training exercises, place increased stress on the spine and hips,which respond by holding their calcium in bone to help withstand the physical stresses acting on thebone structures. The ground breaking study performed at Tuft University by ME Nelson et al(JAMA,1994) showed that postmenopausal women can increase their bone density, without usinghormone replacement therapy, by simply ingesting more calcium and performing a specific series of 5weight training exercises, twice per week on a year round basis. The 5 gym-based exercise stationsincluded: Hip Extension Machine, Back Extension Machine, Abdominal Flexion Machine, KneeExtension Machine and Lateral Pull-Down cable station.

    BREAST CANCER

    It is well documented that women who are overweight during the postmenopausal years have approximatelya three times greater risk of developing breast cancer. This is likely due to the fact that as fat mass increasesthere is a greater conversion of the hormone androstenedione to estrone within fat tissue. Higher circulatingestrone hormone (one of three types of estrogens made by the female body) levels are associated withincreased risk of breast cancer, as estrone is known to increase the cell division rate of breast cells. In turn,this leads to a greater chance of genetic mutations occurring, which may be cancerous. This is exactly thesame mechanism through which hormone replacement therapy has been shown to increase breast cancerrisk. Thus, postmenopausal women are well advised to attain and maintain an ideal body weight.
