Purpose of this Guide

This Guide will help you prepare for and successfully deliver the Vale Base Metals Golden Rules Presentation training and serve as a reference while you are facilitating this course material.

Structure of this Guide

This guide is addressed to the facilitator and includes the following:

The purpose and structure of the facilitator guide.

An overview of the Golden Rules.

A lesson plan for the Golden Rules training, describing the session flow and facilitation cues.

How to use this Guide

Use this guide to prepare in advance for training facilitation and to help you stay on track with topics, content and timing.

Your Role as the Facilitator

The level of participant engagement and interaction in any given course typically relies upon the effectiveness of the facilitator. Your role in this course includes:

Preparation

Classroom Requirements: The Facilitator computer is to be prepared as follows:

• PowerPoint slides linked with shortcut from the desktop

• Connected to a Projector, with image displayed on a white screen or backdrop.

Name place cards and several markers to write on them

Participant Materials: Not at this time

Other Facilitator Preparation:

Review this Instructor Guide ahead of time to become familiar with the material and the expectations of the course delivery.

Review the presentation ahead of time to understand the slide animation in coordination with the requested dialogue

If possible, prepare relevant examples from the operation location where the information session is occurring to help participants relate the concepts to their environment. Include examples of how the Golden Rules impacts people, the workplace, community and the environment.

PURPOSE:

• The purpose of this session is to introduce the main concepts of Vale Global Golden Rules to help increase your knowledge and skill such that you can implement them into your daily work routine and at all times when performing work in the areas associated with these instructions.

OBJECTIVES

By the end of this lesson, participants will be able to:

Summarize the reasons why the Golden Rules are important to all people in all workplaces in our company

Identify the Golden Rules as they apply to you

SPECIAL RESOURCES

There are no special resources for this session.

SPECIAL NOTE

ONTARIO OPERATIONS – There are differences between the Golden Rules and ON Operations current practices. Plans are underway within the departments of the operation to meet the Golden Rule requirements. Before you make your presentation know what the plans for your area, your audiences’ area to meet the Golden Rule requirements. The primary differences occur in the areas related to:

o Working at Height (1.8 meters/6 feet vs 3 meters/10 feet) Gap analysis to be performed by each facility and a work plan developed by the end of 2013 to enable the facility to meet compliance.

o Permit System – The Golden Rules will indicate the need for additional permits than what is the current practice in the ON operations. Expectation for our people to use the permits for those tasks that require them as per our current practice.

o Management of Change – Local operations may have alternate methods (electronic or otherwise) of achieving the intent of the Management of Change process

Introduce yourself

SAY This presentation has been developed to provide you the knowledge to assist you and others at Vale Base Metals to manage the risk associate with known hazardous conditions and activity.

This is not formal training on each of the topics covered by these instructions. It is training on critical aspects of these workplace conditions and activities that we know if they are not followed have and can result in catastrophic consequences.

A previously discovered challenge is the cascading of information.

It is difficult to maintain the same message session to session. This document is an attempt to minimize the impact of cascading – try to stick to the format and the dialogue.

We recognize you need to deliver this presentation in your words – reviewing this material in advance will assist in aligning your dialogue and the key presentation points.

We all have responsibility and actions to get us home safe, and sound at the end of the day the following is the legislated requirements – In your own words (from the heart) stress that we all have a role to play and that we are all in this together. Demonstrate Caring

• Employer – develop the program (ei Confined Space Program, ZES Program, WHMIS…..)- We have world class programs for these types of hazardous work, they are the origin of the Instructions.

• Supervisor – Implement the programs as trained – ensure the Instructions are known, understood and followed at all times

• Worker – Follow the program as trained – follow the Instructions (report any concerns or problems following the instructions)

• Every level of the organization has actions to demonstrate “capable of applying”

• Second from bottom bullet – brother’s keeper, we are dependent on each other – without this interdependency we cannot achieve zero harm

No need to review exits, washrooms etc if the participants know the workplace or if this has already been reviewed in a session immediately prior to this one (check to make sure people know).

Do emphasize that this applies to all of us – if this presentation has supervisors in attendance make sure they understand that if they cannot organize work within the boundaries of these instructions they are to contact their supervisor.

The consequences of not following these instructions are so catastrophic – that no one has the authority to not follow them.

These are the reasons WHY we need to follow these rules – our co-workers, our friends, and our family.

Information that the presenter has to provide to the Audience

• Assuming that if these primary fatality prevention controls are not implemented and followed, then how are we going to get each of us to know that these instructions are crucial for zero harm to ourselves and others and that each of us have a desire to follow and promote them.

• We know that not implementing and following one of these rules will result in a high probability that you or some else may die. And if any of us are lucky enough to have made a poor decision not to implement and follow one of these rules and survives, then we as an organization must address the decision making process exposing us and others to risk not at ALARA.

• We need to recognize poor decision making, learn from it and correct it to prevent re-occurrence.

• The Corrective Action Framework will be used to determine appropriate corrective action based on the circumstances that lead to the poor decision

• We follow these rules not because we are fearful of discipline but because we know that not following them put us, and others, including our friends and family, in serious jeopardy

When it looks like most are done (from body language) move on to the asking the question

ASK Any Comments? Allow some time for people to think about this.

These should not come as a surprise – especially for people in the Production and Maintenance areas.

The Presenter may be asked about Cardinal Rules:

Cardinal rules were introduced in our organization for the same reason as the Golden rules – to provide instruction to prevent harm – the worst harm, a fatality.

The Golden Rules are global in nature, whereas the Cardinal rules were more regional. The Cardinal rules – although more in the form of primary instructions are still shared today in regional training sessions to help people identify hazards, assess risk and implement the controls to manage risk to As Low As Reasonably Achievable – ALARA.

The key aspect is any work over 1.8 meters (6 Feet) requires the Working at Height program applied.

NOTE: 1.8 meters is a Base Metals Requirement, if your local requirement is more stringent than the more stringent shall apply.

When preparing to work at a height a person is required to:

• Check that certified fall prevention or protection systems are in place.

• Use only certified equipment for fall arrest and travel restraint systems.

• Visually inspect the fall arrest and travel restraint equipment and systems prior to use and take any equipment that is damaged out of service.

• Follow the site specific work permit procedures for working at height

Scaffolds

• Check that platforms, scaffolding or temporary structures used for access are constructed by qualified persons. Structures of this nature should have a signed and attached tag that should also indicates the allowable weight limit.

• Check that there is a means to safeguard against items from falling and causing injury to another person. This will include kick-plates on work platforms and tethering of equipment that will be in use outside of the platform barricading.

Identify – we still find instances of isolation mechanisms that are mislabeled

Tag – use the proper tags as per local protocol

Lock – one lock, one key – locks used as per local protocol

CHECK- This step is critical – must know that equipment will not start.

The degree of risk of work within a confined space varies with the ability to make the atmosphere free of potentially harmful gases or other material.

Work in a confined space should only be performed if all other ways of performing the task have been ruled out.

Only people who are specifically trained and currently qualified to work in a confined space can enter.

The entry permit must be reviewed by all who are going to enter the confined space and must address the following

• Potential hazards,

• The procedures to be applied for isolation,

• Atmospheric conditions and personnel monitoring,

• Work to be completed within the space, and

• The Emergency response.
