The start-up rate of the boiler is limited by the following items:
1. Allowable heat up rate of refractory, to achieve acceptable refractory life.
2. Elimination of temperature stress that could cause leaking tube joints.
3. If sulfur or HCl is present in normal fuel effluent, start the heating process on clean fuel until the dew point is passed.
During this period of operation, carefully observe that all components function properly.
Be prepared to shut the unit down should a lack of proper operation become evident.

Gradually reduce load and run the fuel system to low fire position. When the unit is at the low fire position, blow down the boiler along with water column
gage glass and the feed water control valve. Following manufacturer's
recommendations, remove from service all combustion equipment. The until

cool on natural draft after removing the fan from the service.
The steam pressure should be allowed to drop naturally without opening vents or other intentional means of taking steam from the unit to hasten the
lowering of steam pressure. A cooling rate exceeding 100F per hour to
150F per hour by excessive opening of vents should be avoided to limit the
drum distortion and resulting strains on tube joints. Vents can be fully
opened when the steam pressure drops to 15 to 20 psig.

The first automation-related error by the flight crew, the selection of Romeo instead of Rozo, was a simple one, based on the method
used to generate a selection of navaids from the FMS data base,
using the single letter identifier. All navaids having that identifier are
displayed, in descending order of proximity to the airplane. The one
closest to the airplane is presented first, the second is further from
the position and so on. Selecting R resulted in a display of 12 NDBs,
each of which used the "R" as an identifier. Choosing the first
beacon presented in this list resulted from a logical assumption by
the pilot.

Speed brakes
After the GPWS alerted, the first officer initiated a go around and correctly followed AA's procedures regarding GPWS escape maneuvers. However,
neither pilot recognized that the speed brakes (spoilers), deployed earlier to
increase the descent rate, remained deployed, and no effort was made to
retract them. The evidence indicates that few cues were available to the
pilots to recognize the speed brake extension and the airline had no
procedure at the time to require speed brake retraction as part of a GPWS
escape maneuver. Nevertheless, because of the critical effect of speed
brakes on maximum performance maneuvers, the flight crew should have
recognized that the spoilers were still extended during the attempt to avoid
the terrain, and should have retracted them early in the escape maneuver.
