III. Programme and contents:

The final programme will be very much in line with the one that has been placed online but an updated version will be given to you on the day of the course.

There is always the possibility of a lecture or demonstration using prosections or osteology over-running the time and thus may necessitate some flexibility in the programme. However, due to the wide range of topics that will be covered during the course, we will adhere to strict time-schedule. Thus we will endeavour to stick to the session times (i.e., morning and afternoon) and hence all breaks, and start and finish times during both days will be followed as per programme.

Applied Surgical Sciences and Critical Care: The course will be taught using a series of interactive lectures with OSCE focus and OSCE scenarios. It will use clinical scenarios to discuss specific subject areas such as Cardiology and Endocrinology. All major areas as per the Part B syllabus will be covered including:• Gastric outlet obstruction• Metabolic alkalosis• Central lines and invasive monitoring, CVP waveforms• Starlings Law of the heart• O2-Hb dissociation curve• Crush injury/compartment syndrome• Small bowel obstruction• Pancreatitis• ARDS• Burns: Assessment, immediate management, fluid resuscitation - difference b/w colloid & crystalloid• Shock/SIRS/MODS• Epidural injections and complications• Renal failure and relevant drugs • Hypothermia• Coagulopathy, DIC• TURP syndrome – hyponatraemia, hypotension• Grades of liver injury

DOCTORS ACADEMY CARDIFF MRCS OSCE COURSES – APRIL 2014 11

www.doctorsacademy.org

Clinical Skills and Procedural Skills Course:This course has assumed significance due to the huge emphasis on clinical and procedural skills in the recent exams. About 25% of the total marks in the OSCE will be based on clinical and procedural skills. The course is delivered by hands-on sessions with scenarios mimicking the real exam. It will take you through the common procedures that are tested in the exam, allow practice in simulated exam conditions and will provide structured feedback for further development. The stations covered will include: • Safe use of local anaesthesia • Suturing techniques • Surgical excision of a cutaneous lesion • Urinary catheterisation • Cannulation and venepuncture of high-risk patients • Epidural technique • Lumbar puncture • Organising a theatre list • Appropriate radiology requesting • Discharge letter and transfer documentation

In addition, in line with the recent MRCS exams, stations on chest drain insertion and FNAC have been added.

Surgical Anatomy and OSCE:The course will be predominantly delivered by small group demonstration of anatomical prosec-tions and osteology. The format for Saturday morn-ing will also essentially be the same as Friday but on Saturday afternoon there will be mock vivas and OSCEs. Details regarding the format will be explained to you during the course. The lectures and group demonstrations will be a mix of didactic and interactive sessions, depending on the topic. Being a two-day revision course, the course is aimed at consolidating and brushing up your already acquired knowledge and focus on key aspects to pass the exam, rather than overwhelm you with extensive information!

Clinical Examination and Communication Skills Course:This course will be conducted in the format of small group teaching/mock OSCE, focusing on the clinical areas and communication aspects tested in the exam. You will have the opportunity to practise both clinical examinations and communication skills. The course is intended to aid your ability to obtain a focused history, interpret relevant data, communicate effectively, and perform clinical examinations slickly and present the findings to the examiner in a systematic and concise manner. In addition, you can use this course to consolidate your previously acquired knowledge, streamline your thought process and obtain essential exam-preparation tips from the tutors.

IV. What to prepare:

The courses will be very intense and exhausting. Hence please read as much as possible beforehand so that you will obtain maximum benefit from the courses. Prior knowledge will also help you to focus on important pieces of information during the lectures and demonstrations. All relevant handouts and course material will be given to you during the courses.

There are a few presentations we would like you to review before attending the course. Please follow this link, enter your course registration number that you received by email when you registered and click on validate to view the presentations online:http://www.courses.doctorsacademy.org.uk/Number_Validation.aspx

Some guidance on:(i) Travel: http://doctorsacademy.org/Course/SurgicalAnatomyand-mockOSCE/downloads/Travelling_to_Cardiff.pdf

(ii) Accommodation: If you have not already booked your accommodation, you may consider the options below. The hospi-tal and the university where the courses are held are very close to the city centre. The University is about 5 min walk from the city centre and the hospital is about 15 minutes from the centre. Cardiff Bay is quite far from the venue and we would hence suggest not booking your accommodation in the bay area. We recommend a few accommodations nearby: 1. The cheapest option might be: Travel Lodge in Whitchurch, Cardiff: http://www.travelodge.co.uk/hotels/450/Cardiff-Whitchurch-hotel

Travel lodge in City Centre, Cardiff: http://www.travelodge.co.uk/hotels/81/Cardiff-Central-hotel

DOCTORS ACADEMY CARDIFF MRCS OSCE COURSES – APRIL 2014 17

www.doctorsacademy.org

Both the travel lodges are quite close by. However, the one in city centre can be busy during the evenings/night and you may get disturbed. Thus the one in Whitchurch might be better. It will cost about £5 by cab to the hospital and about £7-£8 to the University.

2. There is a Holiday Inn as well that is not too far from the venues: http://www.holidayinn.com/hotels/us/en/cardiff/cdfcy/hoteldetail

You can try hospital accommodation but we cannot be sure if they will be able to offer you a room. The best option will be to speak to the accommodation manager of the hospital and the number of the main switch board of University Hospital Wales is 02920 747747.

(iii) Places to dine in Cardiff: http://doctorsacademy.org/Course/SurgicalAnatomyand-mockOSCE/downloads/Places_to_Dine.pdf

Last but not the least let us take this opportunity to reiterate that all the four courses will be intense, focused and packed with information. The days will be very long and you will only get the chance to have some dinner, sleep, have breakfast and return the following day! This is of particular relevance if you are attending the full MRCS course (Wednesday to Sunday) that will cover all the 18 exam stations configuring the four broad content areas. Hence please ensure that you are sufficiently rested before the days and do not have any plans for extensive social evenings during the days of the course.

We look forward to seeing you soon.

Please do not hesitate to contact us at [email protected] if you have a query in the interim.

Best wishes,
