Purpose of this Presentation

• The purpose of this presentation is to present to you, the user, information on how to download and submit an E-form through the Del Sol Children’s Ministry website.

• Throughout this presentation, you will be presented images that show you how to find the website, fill out a form, and submit a document.

• In addition to this, there will also be a direct link to the website, so feel free to stop the presentation at any time to go download an E-form to make sure you know how it’s done.

Del Sol Children’s Ministry Website

• The Del Sol Children’s Ministry has created its own website where church members can view information on upcoming events such as AWANAS, Vacation Bible School, and Preteen Camp. In addition to this, there are also electronic forms, known as E-forms, that parents can fill out and submit directly to the Ministry Director, Cheryl Reed.

Types of E-forms

There are 2 types of E-forms that can be found on the Del Sol Children’s Ministry Website:

1. Information Sheets2. Registration Forms

Requirements to access the forms

There are several requirements that need to be followed in order to access the E-forms through the Del Sol Children’s Ministry website:

1. First, you need the latest version of Internet Explorer or Firefox.2. Second, you need to download the latest version of Adobe Reader or Adobe Acrobat on yourcomputer.

Things to keep in mind

1. First, you DO NOT need to have an account through the Del Sol Children’s Ministry website or be a member of Del Sol Church to access any Information sheets or Registration forms. These forms are open to the entire El Paso community .

2. Second, in order to fill out a Registration form, you MUST be the parent or legal guardian of the child who you are registering. If you are neither the parent or guardian of the child, you will need to submit additional documentation that proves your relationship with the child in order for the church to allow the child to be picked up by you.

Information Sheets• Information sheets are a method of communication for parents to voice their

own comments or concerns on what activities are being done in a certain program.

• For example, some forms will ask if the child is allergic to certain foods or if there are certain activities that the child may prefer to do in class such as arts and crafts, puppet shows, or watching movies.

• This helps ministry leaders know how they can improve with how they conduct the classroom and accommodate the needs of both the parent(s) and the child.

• These forms are separated according to the grade of the child.• For preschoolers, these information sheets are separated according to the

name of their classroom. For example, if your child attends class in the Caterpillar room, you will select “Caterpillar Preschool Information Sheet.”

• For grades 1 through 4, you can select the link marked “What I like for Children in 1st through 4th Grade” and for grades 5 through 6, you can select the link marked “What I like for Children in 5th through 6th Grade.”

How to complete an Information sheet

1. Go to the Del Sol Main Website.

2. Select the “We Offer” link and click on the sub-category marked “Del Sol Kids.”

3. At the Del Sol Children’s Ministry website, click on the link marked “E-forms.”

4. At the “E-forms” section, select the Information sheet you want to fill out. (Note: Remember that the information sheets are separated by the grade of the child. Refer back to slide 9 form more details.)

5. Once you select the correct form, take a few minutes to fill out the form. (Note: Remember, these forms are an evaluation of how a certain class or program is conducted and it is important that you answer as honest as possible.)

Registration Forms• Registration forms are a simple way of placing your child in a

certain program that is offered through the Del Sol Children’s Ministry.

• These forms ask for background information of both the parent(s) and the child.

• The parent(s) will also need to provide the name and phone number of someone other than themselves whom the church can contact in case there is an emergency. (Note: If possible, it is highly recommended to give at least two emergency contacts.)

• The forms may require consent from the child’s parent(s) to take their photograph for certain class activities such as arts and crafts.

• The forms may also ask for the child’s T-shirt size since the church gives them out during and toward the end of every program.

How to complete a Registration form

1. Like searching for an information sheet, if you are going to fill out a Registration form, first go to the Del Sol Main Website.

2. Then, select the “We Offer” link and click on the sub-category marked “Del Sol Kids.”

3. At the Del Sol Children’s Ministry website, click on the link marked “E-forms.”

4. At the “E-forms” section, select the Registration form you want to fill out. (Note: These forms can be found below the list of information sheets on the “E-forms” link.)

5. Once you select the correct form, take a few minutes to fill out the form. (Note: Remember to answer all background questions about you and the child who you are registering. Also, it is important that you give us an emergency contact should you not be able to pick up the child.)

After completing either an Information Sheet or a Registration form

Once you have completed either an Information sheet or a Registration form, you have the option of either submitting the form electronically or printing out the form and submitting it directly to the Ministry director.

6. To submit electronically, simply click the icon marked “submit form”

7. To print out the form, simply click the icon marked “print.”

After filling out an E-form • After submitting an Information sheet or a Registration form, you will

receive a confirmation on your e-mail that the form was submitted.• The Ministry Director may contact you within 48 to 72 hours if there is

any additional information you may need to submit.• If you have any other questions, comments, or concerns, you can
