11kv supply from RDM tripped on low set overcurrent fault while taking a trial of 55kw motor after

assembling discharge valve. RDM was informed to turn on the power supply for tailing dam panel.

From their end it was reported that DO fuses got blown away twicw while charging the panel. 11kv

line inspection will be done tomorrow.

Type of Tripping with reason and rectification

Carbon brush size of Ball Mill-1 was very low. Its need to be change as early as possible

Trip indication was coming in RM-2. Its LRS was not healthy. Problem was in its water level

sensor. After cleaning of sensor, LRS trial was taken in local & it is fine. Since stream-2 was not

running so production loss was not occur.Rod mill-2 tripped bacause of overload fault in micom 220 relay. Rod mill was stopped for rod charging

and after few minutes it was started again. At first rod mill got tripped and subsequently blower 3(

miniler ELR fault) and Ventillation fan(micom 220 overload fault).

tripped due to phase overcurrent fault. All the agitators in PMCC-1 started drawing more current

bacause of blower-3 tripping issue.

Tripped from temp. scaner. Temp Scaner was checked and no problem was found. Its due to

malfunction of temp. scaner. It was reset and started again. Now running fine and its temp is-

DE side- 47°C, NDE SIDE-22.6°C

Process dept was planning to shift the load from blower 2 to blower 3. blower 3 was started and after

12 secs motor got tripped. Micom 220 was showing excess long start fault. Blower suction was

controlled and motor started again. Now running fine taking 43A.

Tripped due to LRS fault coz of problem in water sensor. Needs to be replaced with new one as soon as

possible

Tripped on Trip circuit 110 V DC wires loose connction on 6.6kv Feeder side. It was tightened and

started again

Tripped due to thermal overload due to improper shaft position due to which it took around 24A for

some period and tripped . Shaft position was adjusted and it started again.It took around 14-16 A and

running fine now.

Tripped due to phase overcurrent fault. All the agitators in I/C-4 started drawing more current bacause

of Blower-A Emg stop by Mech. Its trip value was Ip> 0.17kA and set value is 0.7A, CT Ratio is-250/1A

V-Belts of 3C-02 were blowen so its motor connection was removed from motor TB. While opening of

motor connection it was found that its Y-Phase lug was broken. Its Re-Lug work is going on.

Motor tripped on LRS Fault. No problem found from electrical side. Possible Reason: Some person may

pressed the LRS Emg stop Button.

2P-08A (Hot water pump) feedback to DCS gone missing and in response BM-1 got tripped. Fixed and

Moving contacts of TB were checked and made clear. The same problem found in 2P-08B.

Trip circuit healthy was not coming. Its trip ckt SFU of 4A was foun blowen. It was replaced with new

one. Also it service position limit switch was not working. It is Bypassed due to unavailability of spare.

It was charged after I/C-1 line was charged by MRS. It is fine now

Blower 2 tripped on over load while making changeover from blower 3 to 2. And because of blower

trip trafo - 3 also got tripped.

Blower tripped without any indication, Due to this trafo 3 tripped on Phase over current(175A, 60 Sec).

Reset and started.

Blower B stopped Due to this trafo 3 tripped on Phase over current(175A, 60 Sec) ( Stream 1

floatation motors). Reset and started.

BM-2 VCB panel side Master relay (86) observed in trip condition. Then checked at LRS side and found

that LRS panel Emg. Stop pressed. So release it and clearnace given to Process to start the grinding

circuit.

Tripped on Ip> fault,problem was tripping of instrument air compressor,process engineers are working

to find the problem,as of now it is working fine.

Tripped due to LRS fault but no problem was found in LRS,it was reset and started .Now running fine.

Its temp scaner was showing under temp in channel no-1. It was skiped and its terminal box tightness

need to be done when RM will not running

Voltage spike arose due to RSEB power grid trip. Because of this Stream 1 & 2 HT Motors , MCC-2,4 & 5

(I>> Fault) Tripped . Reset and started. Now plant is running on Islanding Mode.

Rod Mill-1 trip coil was not getting picked. In previous shift, because of the malfunctioning of

temperature scanner, master relay was operated but motor was running continuously. All modes of

tripping checked and later spare motor feeder breaker(P-02) was replaced with Rod Mill-1 breaker.

Again there was some problem in the control ckt of that breaker so Ventilation fan motor breaker(P-

03) was used. Breakdown Time (9:30AM- 11:30AM mech b/d & 11:30AM-1:30PM elect b/d).

It was getting tripped without any indication. One cooling fan for BM was not working. Now running

fine

tripped due to Ip> fault (phase overcurrent). Blower-A was turned off by proces dept and blower-B was

started, but it was not providing sufficient amount of air. This causes overloading of agitators and as a

result Trafo-4 got tripped.

tripped due to overload twice. It was reset and turned on. Now taking 38-40A.

Motor was tripped due to thermal O/L due jam in crusher. Motor was checked and found ok. Also

motor temp is normal.

Motor was tripped due to tI Start fault. Motor was checked and found ok. But crusher was found jam.

Motor was started again after removal of jam. Now running fine

Tripped on Ip> Fault, Problem is due to over load of stream 2 cell motors, Motors are overloaded

Becoz Blower A was stopped at that time while stopping Stream - 2 . Now reset and started and OK.

Tripped two times on Ip> Fault, Problem is due to over load of stream 1 cell motors, Motors are

overloaded Becoz Blower B was stopped. Now reset and started and OK.

tripped due to false reading of temperature scanner. Channel 2 was showing 126 deg celcius.

Terminations were checked and tightened, still it was showing 102 degree celcius. It is bypassed now.

It was tripped from Aux-A type relay(Buchholz relay). Trafo temp was checked and found ok. It was

reset after removing of gas from Buchholz relay. Now running fine

Trafo tripped due to Ip> fault. Process dept turned off the Blower (Blower-A) before reducing rotors

load. Later it was reset and turned on.

Trafo 4 tripping causes 4P-01C/D + MCC-8 supply Off as well. This makes primary sump overflow and

entire reagent area motors inactive.

Motor was not getting start from DCS/Local. It was checked and found that its CC was not working.

VCB was change with spare VCB from Panel-2. Now working fine

It was tripped due to Ip> Fault. Process dept turned off the Blower (Blower-C) before reducing rotors

load. Later it was reset and turned on. Now running fine

It was tripped due to tripping of MCC-1. MCC-1 was tripped from ELR. It was reset and started again.

Now running fine

6.6kV incomer-2 feeder got tripped because of "U/V fail" under voltage fault. Its 'R' phase fuse in Line

PT was found blown away. Now 6.6kV Incomer-1 is in line.

Running Motor was stopped without any indication from feeder side and DCS Side. No problem found

while checking. Started later. Running fine now.

humming sound was noticed. problem not checked yet.

Termination tightness checked, Insulators were cleaned. Now the sound was reduced. Blowering done

for the panel.

Some body has pressed emergency just after start of motor it was reset and started after 30minutes of

as per start inhibit time

OLTC-1 was not working its ckt need to be check

Ball mill 1 LRS insulator support damaged. Flash over. Total 9 nos were defective and replaced by new insulators.

Ball mill 2 LRS insulator support damaged. Flash over. Total 9 nos were defective and replaced by new insulators.

Rod mill 1 LRS checked. Insulation value was less. All the six insulators were replaced with good one.

Ball mill 1, 2 water level in LRS was lowered by 30mm to increase the gap between the insulators and water

level. For this one no 10mm hole was drilled in LRS tank.

Ball mill 1 NDE side bearing NU 230 M was replaced as the old bearing was giving scratching sound. Grease

SCH motor NDE bearing replaced. 6320 C3. Motor fan replaced with the fabricated fan brought through Adarsh

5AG-01, 5AG-02 lead agitator motor non drive end bearings were replaced with 6209-2RS bearing.

Ohline shifting work completed near SKM gate through M/s Navdurga electrical jaipur

11KV/440V 1500KVA trafo 440V side cables were removed and connected to 11KV/6.6KV/440V transformer .

Raise borer M/c power supply given .

Main V.fan greasing done. With Temp Gun. Motor DE Cover= 68 deg C , Motor body= 77degC, NDE cover=

Masibus Scanner: Channel 1=36degC, 7= 110degC, 8=78degC.

Raise borer m/c connections removed from the back of the techno office room.

Power given to raise borer m/c at south ramp.

The 33KV no 1 feeder tripped at early morning 1AM. One no 33KV PT fuse blown out at the SKM incomer feeder.

The tripping was cause due to one cable bursting and eathing at the DSC side cable tray near yard. 33KV No 2 ckt

Raise borer m/c pump connections removed in the evening as the pilot hole drilling is over.

33KV no 2 feeder cable jointing done

33KV no 2 feeder line charged after cable jointing work.

Raise borer M/c is taking 490A @ the boring of 3.5 mtr dia .

Hydro fill plant 6.6KV trafo power supply energized on 6.0PM.

SCH was giving problem. 3 core cable was laid out from oil cooling panel to float switch so that relay R1 can be

energized directly. R2 elay is kept charged through permanent short ckt.

The mill was switched off in 4PM due to the union after the accident in lead plant.

33kv no1, no 2 circuits were taken in to ckt and 6.6KV no1, 2 feeders switched on and the bus coupler is kept off.

Ball mill 1 DE side grease cover opened. The cup was not in exact position. Grease is coming out. Ball mill 1

motor needs to be opened to rectify this fault. Fixed back as it is greasing done.

11KV/6.6KV/440V 1500KVA transformer was energized from 6.6kv mill side power. Power given from 6.6KV

Greasing done on Main V.fan 630KW

Ball mill no 1 drive end side grease which was lying inside motor approximately 7KG was removed.

Greasing done on V.fan

Capacitor reforming done in the spare inverter module

KTSelvan & Ravi went to Udaipur on official tour.

CVR ventilation fan started. It is taking 150A load.

Sh Sukhmayan was deputed for 33KV yard work at DSC

Ball mill 1 : NDE side bearing NU230E ( Fag) changed, DE side bearing NU236E( NSK japan make ) & 6320

changed.( Grease ring(oil ring in NDE side is AJ180 & this is damaged. ).

All the three new bearings are SKF make.

Ravikant Baipai sent to Udaipur for grease seal.

U/G 11KV cable joint near hoist room opened, cable re-routed through cable trench and straight through jointing

made. 8AM to 4PM. Cable charged back.

Ball mill-1: Motor DE side oil seal 200*180*15 mm fixed with juggad fittings and trial taken.

Total Mill power was taken through 33KV ckt2 @ 10AM. 33kv CKT 1is in no load.

SCH crusher complete set replaced with new crusher, new oil cooling system, new overpressure fan.

Sukmayan was deputed to DSC 33KV yard work

DGMS electrical inspector visited SKM for inspection

KTselvan Went to (DGMS ) Udaipur for submitting HT file. File was rejected as it was not in stipulated New

format.

VVIP visit took place at SKM.

Compressor house power shutdown taken from 3.0PM to 6.30 PM for jointing the VCB panels.

High mast light work completed

Went to DGMS Udaipur office on personal taxi to submit the compr house substation application.

By train on 6-7-13 went to delhi. Came back on 8-3-13 by flight. V.fan motor inspection work.

Power to south ramp given from mill side through cable. 11KV/440V transformer switched off and 440V cable

connection removed from PCC side.

BC-3 magnet coil insulation failure at bottom. Removed , repaird and brought back on 6-4-2013 for fixing. It was

tested at Udaipur and found ok.

M/s Zitron visited the SKM for additional fan requirement study.

Went to Udaipur for magnet inspection @ Sakthi elect

Mine shaft supply given from mill via feeder ie no 14 & 38

Ball mill no 2 shaft sheared off near motor coupling.

The BC-3 Magnet was put in to operation. Working OK.

SCH motor (removed from use) DE side NU320 (SKF) bearing was changed with new bearing ( SKF). The bearing

was damaged due to low grease.

ABB engr Raj kumar was attending the problem of UMC 22 for the past 4 days. MOM signed today. There is a

small problem in the UMC 22 which needs to be resolved.

Setting for trafo 3, 4 HT feeders changed from 0.7, 1.0 to 0.7, 0.4( I>0.7, t>0.4). Setting for trafo 440V incomer in

all 1 to 8 no trafo changed to ( t> 0.65 from t>4.0)

V.fan setting for 1 no channel changed from 70 to 75 deg for tripping. No1 channel temp reading jumped from

48 to 64 deg C.

V.fan AC indoor card damaged & sent to Udaipur. Now the AC is not working.

Hydro fill plant 30VDC battery charger batteries failed in B shift end. Battery ircuit opened and the system

started with 30VDC rectifier ckt without battery.

Trafo-5 was tripped due to high set over current while the CT changing work is going on. MCC 5 Both front and

Both mills started at 12:30 AM.

Shutdown taken from MRS for Incomer 1 (33KV) inorder to change the Line PT fuses ( R phase and Y phase).

Incomer - 1 and Trafo – 1 back panel cleaning done. Incomer 1 GO switch in 33KV Switch yard maintenance

New reservoir pump started with VFD panel after cable laying work from hydro fill plant.( job went for a week).

850 mtr cable 95 sqmm laid out. 45kw motor taking 33A at full speed.

15kw submersible pump and starter fixed in the main reservoir sump and started.

PB& ZN floculant drawing traced and changed as in the panel.

MLDB 4 at PCH fixed with digital ELR.

On trafo 4, oil leakage was found. And in there LT side bushings of Y and B phases were found loose. So it was

630kw vfan motor received with duct assembly at SKM site.

Plant - Both the streams are stopped due to the fatal accident in Mines at 4:00AM

4P-01C, motor windings burnt and DE side bearing was damaged. e/f was shown in vfd. New 180kw(287A) motor

was installed and ID run test was successful. Old motor details: Serial No: NAD2X 10507 TOP, Frame size:

ND3152X… New motor Details: Serial No: NAD2X 11876 TOP, Frame size: ND3152X. Old motor bearing -SKF

NU319C3, This bearing was completely dry, inner race cracked, oil seal burned, motor rotor rubbed at drive end

33kv black-out at 3pm from CPP. Entire plant was taken in load at 6:30 pm

BM-1 energy meter was replaced with spare motor feeder (panel no 2)

5AG-03G motor was replaced with newer one. Abnormal sound was noticed in motor.

Regrind Mill motor was tripped due to False alarm in Temperature scanner. Problem due to loose connection in

Hydro fill plant Transformer (6.6KV/440V) - NGR to Earthis not proper. It was rectified and started. Transformer

Feeder No 31 Charged by Mines Electrical for 212 mrl substation(mines). Micom 220 was found faulty and

replaced from spare feeder.Shutdown given.

11KV Overhead Line was tripped due to earth fault. It was checked from Mines side by Mr. Bhanu. No problem

was found. After that it was started again.

7P-02 (1.1KW/2.35A) motor was replaced. Old motor was having bearing problem.

33kV Incomer-1 shutdown was taken from MRS end. Incomer-1 & Transformer-1 breaker were turned OFF from

Since new ventilation fan motor connection was to be done in Raise Borer M/C panel, P-22 in PMCC-2 so

Shutdown was taken for trafo-5. This ventilation fan motor was installed in Mines area. Also ELR+CBCT was

change in P-22, PMCC-2. After termination of Motor connection from feeder side its shutdown was cleared.

3BC-03 ventilation fan trial taken. Taking 8.6A. ( Rating:5.5KW/9.9A, MPCB Setting:9A). Motor is runnig fine.

Tripped from temp. scaner. While check it was found that its channel no-4 showing false reading. It was skiped

and started again. Channel no 4 need to be check in ROD mill shutdown.

5AG-03G (22KW/40A) has been changed with the spare one due to some abnormal sound in the motor. Load

Trafo-3 was tripped without any indication in SPAJ-140C Relay. It was started again and no further tripping occur

mcc 4 Tripped on Io>> fault. Fault has come from from ball mill 2 inching drive winding termination weak

insulation. Also loose bolts were also found inside that terminal box. Trial was taken for that motor after

3c-02 tripped on temp scanner tripped on temp scanner channel 3. it was skipped and started. Also all HT

motor's winding temp signals were skipped in temp scanner.

3C-02 TRIPPED DUE TO THERMAL OVERLOAD.IT WAS JAMMED , SO INFORMED TO PROCESS DEPT. TO RECTIFY

Rod mill-1 Motor starting get delayed because of LRS problem. On LRS the limit switch LSR was not released

properly. Motor started after rectifying this.

SCH ASB main I/C got tripped due to ELR. Leakage current was 2.6A. Reset & started it. Faulty welding m/c

Hydrofill MCC control supply got tripped while checking feeder 7P-34, Reset & started it.

Ball Mill-1 tripped due to fault in cooling fan. Fans supply was checked from feeder end and visually from field

side and both found ok. Ball mill started later on.

3C-02 motor V belts got broken. Motor end connections removed.

CAPACITOR BANK-2 TRIPPED DUE TO ALL FAULT, PROBLEM WAS IN ITS EARTH GO WHICH WAS FOUND

Pb area crane was not working. While checking control ckt neutral wire is somewhre shorted with ground. Work

is postponded to tomo General shift Due to unavailability of time and manpower.

Rod Mill-1 trip coil was not getting picked. In previous shift, because of the malfunctioning of temperature

scanner, master relay was operated but motor was running continuously. All modes of tripping checked and later

spare motor feeder breaker(P-02) was replaced with Rod Mill-1 breaker. Again there was some problem in the

control ckt of that breaker so Ventilation fan motor breaker(P-03) was used. Breakdown Time (9:30AM-

Paste fill Plant feeder was charged on 28-june-14.

6.6kV incomer-2 feeder got tripped because of "U/V fail" under voltage fault. Its 'R' phase fuse in Line PT was

found blown away. Now 6.6kV Incomer-1 is in line.

SCADA system was checked and started by vollkraft engineer

6.6KV Bus coupler-2 (panel-39) charged for charging the bus bar for panel no 43 and 44

11kv supply from RDM tripped on low set overcurrent fault while taking a trial of 55kw motor after assembling

discharge valve. RDM was informed to turn on the power supply for tailing dam panel. From their end it was

3BC-07 control supply is tripping frequently due to problem in PCS. It was rectified and working fine now.
