How to make the diagnosis of childhood TB?

• Need three elements to make the diagnosis:

1. Infection– Contact

– Positive skin test

2. Symptoms of chronic disease– Cough

– Fever

– Weight loss

3. Radiological changes suspicious of TB

4. Bacteriology

Role of the CXR in the diagnosis of TB

• The role of the CXR– Exclude other chronic lung disease

– Evaluate children not responding ( wrong diagnosis)

– Define the underlying cause of the pathology

– Add diagnostic certainty if TB is suspected

– Remember:• Not a good diagnostic tool with poor sensitivity and

specificity

• Large intra- and inter –reader variability

• Able to identify an acceptable CXR

• Identify the commonest radiological abnormalities on a CXR caused by TB

Pneumonic consolidation with lymph gland enlargement.

• Fundamentally flawed

• About 20% of the radiological pictures are atypical• Glands not visible• Interstitial lung picture• Pneumonic opacification

• Follow-up CXR after 2-4 weeks

What about the CXR that is atypical of TB: can it be TB?

• In approximately 20% of children with confirmed TB the CXR does is not typical of childhood TB.

• The images include:

– Perihilar infiltrates

– Interstitial pneumonia

– Bronchopneumonia

– Segmental confluent opacification
