Bishop Moore Catholic High School Attendance Office Attendance Policy and Test Make-Ups We understand that occasionally student absences cannot be helped. If you know your child will be absent, please follow the appropriate guidelines to ensure their schoolwork and records are not compromised: If Your Student Is Going To Be Absent Or Tardy: 1. Call the attendance line and leave a message at (407) 293-7561 x245 by 8:00 am. 2. When your student returns to school they are required to bring a completed copy of the Absence/Tardy form signed by a parent or caregiver to the Student Services office. This must be done within 48 hours of their return for the absence to be counted as excused. Please note it is the parent’s responsibility to both call the attendance line and submit a completed copy of the Absence/Tardy form. This form can be found on the school website and must be used for all absences and tardies Pre-Arranged Absences A pre-arranged absence form may be picked up from the receptionist of the Student Services Office upon the presentation of a parent/ guardian note providing a reason and dates for an anticipated absence. Pre- arranged absences should be kept at a minimum and should only be requested for reasons that require the student to be out of school that are unavoidable. Vacations are not considered unavoidable situations. All requests for pre-arranged absences will be reviewed within 24 hours. If approval is granted, the student will be asked to pick up a Pre-arranged Absence Form to notify teachers. All affected teachers must sign the form. Students must return the completed form to Student Services Office.

Attendance Policy and Test Make-Ups

We understand that occasionally student absences cannot be helped. If you know your child will be absent, please follow the appropriate guidelines to ensure their schoolwork and records are not compromised:

If Your Student Is Going To Be Absent Or Tardy:

1. Call the attendance line and leave a message at (407) 293-7561 x245 by 8:00 am. 2. When your student returns to school they are required to bring a completed copy of the

Absences During Semester Exams Or Finals:

Mid-terms are not administered to students prior to the regularly scheduled exam date. All students who

miss an exam must arrange to take the test on a scheduled make-up day.

Students who may be in danger of missing a Mid-Term Exam during regular administration must provide a

justified excuse in order to be allowed to take a make-up test. Not all requests will be approved.

All exam absences may require a physician’s note

Notes must include:

o Current Date

o Date of the Absence

o Full Name of the Student

o Grade of the Student

o Reason for the absence

o Signature of the parent and a telephone number for verification.

Pre-Arranged Absences During Semester Exams Or Finals:

Requests for Pre-arranged absences are during semester exams and finals are discouraged and should be limited to reasons that require the student to be out of school which are unavoidable. Vacations are not considered unavoidable situations. All requests for pre-arranged absences will be reviewed within 24 hours. If approval is granted, the student will be asked to pick up a Pre-arranged Absence Form to notify teachers. All affected teachers must sign the form. Students must return the completed form to the Student Services Office.

Attendance Policy and Test Make-Ups

Should a student miss a test due to illness, a physician’s note must be presented to qualify for a makeup

test.

Make-Up Test Administration:

Students must report to the Main Office for details regarding their make-up testing times and location.

Only students with pre-arranged excused absences on file and/or medical notes will qualify to take make-

up tests. Students will take tests missed consecutively (2 per day) and in order until they have finished

taking all the tests they missed during the regular exam week. After completing make-up tests, students

will resume their regular class schedule for each day.

Attendance Quick Reference Guide

If your student is sick and will not be attending school: 1. Call the attendance line before 8:00AM

at (407) 293-7561 Extension 245 and follow the instructions provided.

2. When your student returns to school they

must provide a completed copy of the Absence/Tardy form found on the school website.

Please note: Parents must call the attendance line on the day of the absence and send a completed copy of the Absence/Tardy form found on the school website. Absences not fulfilling both criteria will be recorded as unexcused.

If your student will be absent for an extended period of time:

1. Call the attendance line on the first day of your student’s absence before 8:00AM at: (407) 293-7561 Extension 245. Be sure to indicate that it will be an extended absence.

2. Contact your student’s guidance counselor so that they can get word to your student’s teachers notifying them of the absence. The phone number for the Guidance office is: (407) 293-7561 Ext 239

3. When students return to school they must submit a completed copy of the Absence/Tardy form found on the school website

Please note: Parents must call the attendance line on the first day of the absence and send a completed copy of the Absence/Tardy form found on the school website. Absences not fulfilling both criteria will be recorded as unexcused.

If you need to pre-arrange an absence for your student:

1. Provide the student services office with a note containing the following information:

Students name and grade

Date(s) of absence

Reason for absence

Signature of parent or guardian and telephone number where parents may be contacted for verification.

This information must be provided at least 72 hours prior to the anticipated absence

2. Your student will receive a pre-arranged absence form, which must be signed by his/her teachers, A Dean, and their guidance counselor, indicating their approval along with the work needing to be completed during their absence.

3. The Pre-Arranged absence form will be returned to the Student Services Office by the student upon completion.

Please Note: Pre-Arranged absences during exam periods (Mid Terms and Finals) are discouraged and require administrative approval.

If your student is going to be late to school:

1. Call the attendance line before 8:00AM at (407) 293-7561 Extension 245

2. When students arrives at school they must

provide a completed copy of the Absence/Tardy form found on the school website.

Any tardy that is not accompanied by a completed copy of the Absence/Tardy form found on the school website will be unexcused and the student will be assigned a detention. If your student participates in any extracurricular activities (athletics, drama, etc.) a doctor’s note must be provided or they will not be allowed to participate in their activity that day. Please note: Parents must call the attendance line when a late arrival is anticipated.

If need to pick your student up early from school: (Anticipated) 1. On the morning of the absence have your

student provide Student Services with a completed copy of the Absence/Tardy form found on the school website. Be sure to include a phone number for verification.

2. At the designated pick up time the parent or guardian will sign their student out from the Student Services Office.

3. If the student returns to school he/she must be signed back in by their parent or guardian at the Student Services office.

Please note: Students can only be released to individuals listed on the students emergency contact list on record with the registrar’s office.

If you need to pick your student up early from school: (Unanticipated)

1. Parents should report to the

Student Services Office to sign their student out.

2. Upon parent’s arrival the student will be called out of class and dismissed from the Student Services Office.

If your student drives themselves and has to leave early: (Anticipated)

1. On the morning of the absence have your

student provide Student Services with a completed copy of the Absence/Tardy form found on the school website be sure to include a telephone number where parents may be contacted for verification.

2. Student Services will contact the parent listed to verify the note provided.

3. The student must sign out at the Student Services office at the designated time. If the student returns they will sign back in at the Student Services office before parking their vehicle.

Please note: Students can only be released to individuals listed on the students emergency contact list on record with the registrar’s office.

3. The form can be either faxed or emailed to Student Services
