Appendix FInstructions for Completing

Demolitions-Related Reports

F-1. Target Folder. A completed target folder contains demolition orders and an obstacle folder.A samp le target folder is show n in Figure F-1 (page F-4). Refer to Cha pter 5, Section III (page5-2), for a d iscussion of the d emolition ord ers. Refer to Chapter 5, Section IV (page 5-6) for adiscussion of the obstacle folder.

F-2. Instru ctions fo r D A Form 2203-R. Use the following instructions and the sample formshown in Figure F-2 (pages F-38 through F-42) to complete DA Form 2203-R. A blank DA Form2203-R is at the end of this manu al. It may be locally reproduced on 8 1/ 2-by 11-inch paper.

a.Block1 (FILE NO.). Leave blank unless a higher headquarters provides this number. Higherheadquarters provides this number or enters it after you submit the form.

b.Block 2 (DML RECON RPT NO.). Leave blank unless a higher headquarters provides thisnum ber. Higher headqu arters provides this number or enters it after you submit the form. CompanySOP may specify the procedures for determining this number.

c.Block3 (DATE). Enter the date the reconnaissance was performed.

d.Block 4 (TIME). Enter the time the reconnaissance par ty arrived at the target site (local orZULU time).

e.Block 5 (RECON ORDERED BY ). Enter the command authority authorizing thereconnaissance action.

f.Block6 (PAR TY LEAD ER).Enter the name of NCOIC or OIC of the reconnaissance partywho was p hysically at the site when the reconnaissance was performed.

g. Block 7 (MA P NA M E, SCALE, SHEET #, and SERIES #). Obtain this information from amap of the reconnaissance area and enter the information in this block.

h. Block 8 (TARGET AN D LOCATION). Enter a brief description of the target and the distanceand direction from an identifiable landmark (railroad brid ge, crossroad, hilltop, and so forth). Forexample, Target is 275 degrees, 300 meters from the railroad br idge, 2 miles east of Han esville,on Route 2.

i.Block 9 (TIME OBSERVED). Enter the time you last saw th e target as you dep arted th e site.j. Block 10 (COORDINATES). Enter the complete 8-digit map coordinates of the target.

k. Block 11 (GENERA L DESCRIPTION (attach sketches)).When ap plicable, includ e the typeof construction, width of the roadway, number of lanes or tracks, type of pavement, number ofspans, condition of spans or entire bridge, and bridge categorization and classification. For example,

"Prestressed-concrete T-beam bridge, four simple spans supported by six concrete columns, twolanes; total bridge length is 140 feet; roadway width is 30 feet; overall bridge width is 36 feet; height

l.Block 12 (NATURE OF PROPOSED DEMOLITION (attach sketches)). State the expectedamount of destruction and the priority for placing charges, if feasible. Provide a sketch showing

the number and type of charges to use (tamped or u ntamped), where the charges should be placed,and the type of firing system required.

m.Block 13 (UNU SUAL FEAT URES OF SITE).Includ e any sp ecial features of the target orsite that might affect the meth od of dem olition (high-tension lines, rad ar installation, underwaterblasting, and so forth). Give any details that may affect the security of the target and the demolitionwork party.

n. Block 14 (EXPLOSIVES REQUIRED). Indicate the types, quantities, caps, detonators, and

so forth proposed for the demolition.

o.Block 15 (EQUIPMENT AND TRANSPORT REQUIRED). Specify the amount an d type oftransportation required (for example, two 5-ton dump trucks, one ram set with 50 cartridges, two

post-hole diggers, two demolition sets, 10 pou nd s of 16d nails, twelve 8-foot 2 by 4s). Comm entsmay be continued on the reverse side of the form.

NOTE: Troops may n ot ride in veh icles transporting explosives.

p.Block 16 (PERSONNEL AND TIME REQUIRED FOR:). Complete subsections a and b,indicating the number of personnel and amount of time necessary for placing the demolitions. Thedistance between the firing points and firing systems will be a consideration for determining theamount of time necessary to arm and fire the explosives.

q.Block 17 (TIME, LABOR, AND EQUIPMENT REQUIRED FOR BYPASS; SPECIFYLOCATION A ND METHOD). Specify the equ ipment necessary to clear the site after demolition

and the available bypasses that allow un its to bypass the site. Comments may be continued on thereverse side of the form.

r.Block 18 (REMARKS). Includ e any appropriate remarks that ar e not covered in Blocks 1through 17. Comments may be continued on the r everse side of the form.

s.Block19 (ADDITIONA L COMMENTS). Use this block as a continuation for Blocks 1throu gh 18. Identify the block being continued .

F-3. Instructions for Sketches. Use the following instru ctions and the samp le form shown inFigure F-2 (pages F-38 through F-42) to complete the necessary sketches for DA Form 2203-R.

a. General Description Sketch. This sketch should include

The avenues of approach to the target and possible bypasses in the vicinity of the target.Indicate route numbers and the direction of cities or towns.

Rivers or stream s includ ing nam e, direction of flow, and velocity in meters per second.

Terrain features, including observation points, cover and concealment, swampy areas,deep valleys, and so forth.

A compass arrow indicating north (indicate grid or magnetic).

Dimensions of the proposed target.

Number and length of bridge spans.

Height of the bridge from the ground or water.

b.Nature of Proposed Demolition Sketch. This sketch should include-Dimensions of members to be cut.

Charge calculations. Use either the formula or table method , but show you r work.
