Demand Draft for 2500/- towards the cost of application form drawn in favour of The Secretary, Selection Committee and payable at Chennai. Applications with out the Demand Draft will not be accepted. The name of the candidate and the course applied for (DM/ MCh) should be mentioned on the reverse of the Demand Draft. SC/ SCA and ST candidates who are natives of Tamil Nadu are exempted from payment of the cost of the Application Form.

� The Application Number will be assigned to each candidate at the time of online submission

� Random Number generated by computer will be assigned to each candidate before the

Entrance Examination.

I. GENERAL INSTRUCTIONS : - 1. (a) Candidates seeking admission to Higher Specialty courses should submit the online print out of

application form with necessary enclosures, which should reach THE SECRETARY, SELECTION COMMITTEE, 162, E.V.R. PERIYAR SALAI, KILPAUK, CHENNAI – 600 010.

on or before 01.07.2014 by 5.00 p.m.

(b) The candidates shall ensure that the online print out of application form with all particulars and enclosures reaches the Selection Committee within the date and time specified. Applications received by Speed post / courier or any other means, after the last date & time will not be accepted irrespective of the date of booking.

(c) The Selection Committee shall not be responsible for any delay in the receipt or for the loss in transit of application form etc.,

(d) Request for extension of time for submission of documents called for, after the specified date and time will not be entertained.

(e) Any supporting documents received after the due date or application without required documents / particulars will not be entertained.

(f) Non-Service Candidates should send their applications directly to the Secretary, Selection Committee at the above address.

(g) Service Candidates should get their online print out of application forwarded only through the proper channel with the remarks of the forwarding authorities in the format prescribed in the application form. If not, the applications will be summarily rejected.

(h) Advance copies from the service candidates will not be accepted.

II. ELIGIBILITY CRITERIA : 2. Candidates must be Citizen of India.

3. Candidates from other States are not eligible to apply.

4. (i)Candidates who had studied their Under Graduate and Post Graduate courses in Tamil Nadu are eligible to apply.

(ii)However candidates from other States who had studied in Tamil Nadu are eligible to apply provided they do not claim nativity in their State and give an undertaking to that effect.

(iii)Other candidates will have to produce the Nativity Certificate of Tamil Nadu Origin in the format annexed to the application form.

(iv) The Certificate of Nativity in Tamil Nadu should be issued by the competent authority only in the specified format annexed to the application form and supported by a copy of Passport/ Ration Card / Voter’s ID / Driving License. If the copy of the supporting document is not produced, then the application form will be rejected. Nativity Certificate submitted later will be summarily rejected.

(v) Permanent Residence Certificate will not be accepted. Note: (a) Nativity Certificate should be produced as per the Instructions in the Annexure - IV (b) Producing false Nativity Certificate if detected at any time later during the course of study will result in expulsion of the candidate and also initiation of criminal proceedings against the candidate.

5.(a) Candidates who hold a Post Graduate Degree of the Tamil Nadu Dr. M.G.R. Medical University or of any other University recognized as equivalent to the Tamil Nadu Dr.M.G.R Medical University and included in the schedules to the Indian Medical Council Act of 1956 and who have obtained full registration either from the Medical Council of India or any of the State Medical Councils are alone eligible to apply for Higher Speciality Courses.

( b). Non-service candidates who have completed their qualifying post-graduate degree in Tamil nadu Government medical colleges under State quota/All India quota, who are bound by the two years government service bond:

I. Non Services candidates who have completed their Post Graduate degree in Tamil Nadu Government Medical Colleges under state quota and All India Quota will be allowed to appear for Higher Speciality entrance examination 2014-15 session, for two attempts only. The first attempt will be taken as immediately after passing the post graduate course ( eg. April 2014) and for the next year, i.e April 2015, it will be taken as second attempt This will be irrespective of whether the candidate takes/writes the higher speciality entrance examination or not.

II. Being eligible and allowed for two attempts as above, if a candidate does not take or get admission

to a higher speciality course, he or she will have to complete the bond service ( 2 years of Government Service) before taking subsequent attempts of Higher Specialty entrance examination.

III. There is no limit of attempts for appearing to higher speciality entrance examination once the

mandatory two year bond service is completed after taking two attempts. IV. A candidate who is the process (middle) of completing the mandatory 2 year bond service, he/ she

will be allowed to take the higher speciality entrance examination. If selected for higher speciality course, he/she on completion of the higher speciality course, will have to complete the remaining period of 2 year bond service, in addition to the bond service executed for the higher speciality course.

V. A candidate who has not got any appointment in government service in order to fulfill the 2 year

bond service, after passing the post-graduate (MD/MS) course in 2013, will be permitted to take the higher speciality entrance examination of this year i.e 2014. If the candidate does not take or get selected for the higher speciality course, he/she will be allowed to appear for the next attempt only after completing the mandatory bond service.

6. (a) Candidates who have obtained the P.G. Degree from Medical Colleges not recognized by the

MCI are not eligible. (b)Candidates should produce the additional qualification registration certificate of their Post Graduate degree from the Indian Medical Council or the respective State Medical Council at the time of applying.

7. A candidate who has already acquired a Higher Speciality degree course in any one discipline or who has already completed the course is not eligible to apply for any other discipline.

8.Candidates who are presently undergoing Higher Speciality course in one discipline are not eligible to apply for any other discipline.

9(a)Medical Officers selected by the TNPSC and appointed in the Tamil Nadu Medical Service on regular basis, who have put in minimum of two years of continuous regular service as on 30.06.2014 are eligible.

(b)Medical Officers serving continuously for two years in Local Bodies of Tamil Nadu as on 30.06.2014 are eligible.

(c ) Contract medical consultants and 10 a (i) candidates are eligible to apply as private candidates.

III. PROCEDURE FOR FILLING AND SUBMISSION OF APPLICATION :

10. The candidates shall fill in the application form carefully.

11.Only photocopies of Certificates should be enclosed and they must be duly attested by a Gazetted Officer.

12. Applications with incomplete or insufficient particulars or without enclosures or those received after

the last date mentioned will be summarily rejected ( without any intimation to the candidates).

13. Candidates shall ensure that the online print out of application form with all particulars and enclosures reaches the Selection Committee within the date and time specified. Applications received by Speed Post/Courier /any other means after last date and time will not be accepted.

14. A candidate shall submit only ONE application form for admission to the entrance examination

15(a) The candidate seeking admission to the entrance examination is required to send his/her application print out of Online Entry only.

(b) Only recent passport size photograph should be uploaded at the time of online submission. (c) Attested recent passport size photographs identical to the uploaded photograph should be affixed

to the application form. Note: If the Photograph is not a recent one taken at the time of submitting the application, the

Application will be rejected.

16.( a ) Incomplete entry of particulars in the application form will not be taken into account and such

applications are liable to be rejected ( b) Candidates selected for Higher Speciality course should give a declaration that he / she will

forego selection / admission into the Higher Speciality course, if suppression of factual information is found at any time during or after the admission to the course and the selection / admission shall be liable for cancellation based on that declaration at any time during or after the admission to the course, and are not eligible to apply for 2 years

from the date of appearance for the Entrance Examination.

17. Candidates must enclose only Photocopies of the following documents duly attested by a Gazetted Officer in the order indicated below along with the application form failing which the application is liable to be rejected.

(a)The Postgraduate course completion certificate from the parent college where the candidate had

undergone the post-graduate degree course. (b)Postgraduate degree certificate / provisional certificate awarded by the university concerned to

which the Institution is affiliated. (c)Permanent Medical Registration Certificates of the MBBS Degree and additional qualification

registration certificate for Post Graduate degree issued by the All India Medical Council or any State Medical Council.

(d)Postgraduate Degree attempt certificate obtained from the institution where the candidate completed his/her PG Degree.

(e)Eligibility certificate from Tamil Nadu Dr. MGR Medical University for candidates who have qualified

from other Universities. This shall be produced at the time of Counselling when called for. (f) Certificate of satisfactory completion of two year bond period. (g) TNPSC Selection & Posting Order ( For Service Candidates.) (h) Nativity certificate ( if applicable ) with supportive documents

18. It shall be the responsibility of the candidate to give the correct mailing address on the main application form. If there is any change of address it should be intimated immediately to the Selection Committee in writing.

19. The Selection Committee shall not be held responsible for any loss in transit or for incorrect address given by the candidate.

20. The Government orders issued and to be issued from time to time pertaining to any of the matters contained in the prospectus should be read as part and parcel of this prospectus and such terms and conditions in the Government order are deemed to have been incorporated in this prospectus

21(i) Service candidates should send their online printout applications through the proper channel with the forwarding authority’s remarks and with the service particulars carefully filled up by the forwarding authority as prescribed in the application form. Application of the Service candidates who have completed two years of continuous regular service alone will be considered. The Heads of Departments have to issue a Certificate of completion of the required service The application should reach within the stipulated time. If the application is received after the last date, it will not be accepted irrespective of date of submission and date of forwarding by the Competent authority.

ii) a) Medical Officers selected by the TNPSC and appointed in Tamil Nadu Medical Services on regular basis, who have put in minimum of two years of continuous regular service as on 30.06.2014 are only eligible.

b) Medical Officers serving continuously for two years in Local Bodies of Tamil Nadu as on 30.06.2014 are only eligible.

22. Applications without the signature of the candidate will be rejected.

IV. CONDUCT OF EXAMINATION :

23. The Entrance Examination will be conducted only at Chennai as scheduled below

DM/ MCh Courses : 10.00 AM – 01.15 PM

First / Single Option : 10.00 AM - 11.30 AM 13.07.2014 Second Option : 11.45 AM – 01.15 PM

� This Entrance Examination is valid only for admission to Higher Specialty courses for the academic year 2014-2015.

� The Centre in which the entrance examination is to be written will be given in the Hall Ticket. . � The examination will be conducted only in English.

� Answers should be shaded with a Ball Point Pen in the OMR answer sheet as per the instructions given and if it is shaded with any other item the answer sheets will not be considered for valuation without any intimation to the candidate. The ball point pen will be provided in the examination hall.

24. ( a) Each candidate will be allowed to give not more than two options in the order of preference while applying for Higher Specialty courses.

(b) Options once given cannot be changed.

25 (a) Candidates opting for admission to TWO different courses will be permitted to appear for the FIRST OPTION discipline paper FIRST. The completed answer sheet and question booklet of the first option should be handed over to the invigilator after one and half hour. Thereafter the question booklet of the SECOND OPTION will be given to the candidate to be answered within the next one and half hour. The candidate will be permitted to leave the hall only after completing and handing over the second paper and not in between.

(b) Candidates who do not appear for the first option paper will not be permitted to appear for the second option paper alone.

26. Hall Tickets should be downloaded by the candidates prior to the commencement of the entrance examination. However admission to Entrance Examination does not confer on the candidate the right of admission to the course if the candidate is found to be ineligible later.

27. Candidates admitted for the entrance examination will have to come at their own expense and will have to make their own arrangements for stay.

28. Candidates with Hall Tickets only will be allowed to enter the Examination Hall..

29. a) Candidates for D.M and M.Ch courses are advised to be present at 9.30 a.m. in the Examination Center. They are required to be seated on their respective seats 15 minutes before the commencement of examination and to fill up the 1st page of Question Booklet. Question Booklets should not be opened by the candidates before 10.00 A.M. for Option I and before 11.45 AM for Option II respectively.

b)Candidates who report after 15 minutes of the commencement of the examination shall not be permitted to appear for the examination.

30. Candidates shall not be allowed to carry any text material printed or written, bits of paper, electronic devices with or without remote sensing like Pagers, Cellular Phones or Electronic Diary etc., and pencil /pens inside the Examination Hall.

31. Candidates appearing for single / two options will not be allowed to leave the examination hall before the end of the examination.( Single option 11.30 AM and double option 01.15 PM ).

32. Candidates shall maintain silence. Any misconduct will result in forfeiture of the right to continue the examination. His / Her answer will not be considered for valuation. Further the candidate will not be allowed to apply for Higher Speciality Courses for 3 years (Three Years).

V. SCHEME OF ENTRANCE EXAMINATION : 33. All the Candidates seeking admission should appear for an Entrance Examination conducted by the

Selection Committee.

34. Each paper will be of Postgraduate Degree standard and will contain 120 objective type questions with 4 responses. Out of the 120 questions, 60 questions will be from Basic Sciences and the other 60 questions will be related to the Speciality concerned.

35. Each question will carry 1 mark. There will be No Negative marks for incorrect responses.

36. Indicating more than one response, will be deemed as incorrect response and will be given zero mark. The Maximum marks will be computed to 90.

37. The number of Questions in each subject is as shown below. a) BASIC SCIENCES: SUBJECT NO.OF QUESTIONS Anatomy 10 Physiology 10 Bio-chemistry 10 Pharmacology 10 Pathology 10 Microbiology 10 Total 60 The Basic Science questions will be of Postgraduate standard and would be related to the discipline

concerned. b) CONCERNED DISCIPLINE: 60 Questions will be related to the discipline concerned e.g. Cardiology, Neurology, Cardio Thoracic

Surgery etc., and the standard would be of Post Graduate Degree

38. Candidates are not allowed to take back the question booklets and any request for the issue of Xerox copies of question booklets/ answer sheets/ keys will not be entertained

VI. DECLARATION OF RESULTS : 39. a. The individual Entrance Examination Marks for 90 will be published in the websites: (http:

www.tnhealth.org / http://www.tn.gov.in). b. The Secretary, Selection Committee will publish the merit list in the websites

http://www.tnhealth.org. / http://www.tn.gov.in

40. There will be no Rechecking/ Revaluation/ Retotalling of answers.

VII. MERIT LIST : 41. Two merit lists - 1) Open Merit List and 2 ) Service Merit List will be published Open merit list ( for both service and non- service )for each discipline of the Higher

Speciality course will be prepared by computing the entrance examination marks and experience marks, which will be calculated to a maximum of 100 (Entrance Examination Marks to a maximum of 90 + Experience Marks to a maximum of 10).

Service merit list will be published separately.

42. a) The Individual Mark (Service / Non-service) is calculated by computing the marks secured by the Candidate in the Entrance Examination and Experience Marks of one mark for each year after completion of the P.G. Degree which is calculated from the last date of the month of passing Examination to 30.06.2014. (E.g. If a candidate scores 50 Marks out of 90 marks in Entrance Examination and has completed Two Years after P.G. Degree as on 30.06.2014, the candidate’s total Marks will be 50+2=52).

b) Fractional values of a year will not be counted for awarding marks for experience

43. Determination of inter se merit (rank) of candidates obtaining equal marks : In case of two or more candidates obtaining equal marks in the total aggregate of 100 marks, the

Inter-se merit (Rank) of such candidates shall be determined on the basis of the following order of preference:- a. Marks obtained in the entrance examination

b. Least number of appearances in the Postgraduate Degree examination. c. Seniority in age. d. Random number assigned (Higher value will be given preference)

VIII. METHOD OF SELECTION AND ADMISSION : 44.(a) The final preparation of the selection list / waiting list for admission of candidates to the various

disciplines of the Higher Speciality courses will be made on the basis of the merit list for each Higher Speciality.

(b) The merit list alone, one for each Higher Speciality Course will be considered. The allotment of seats will be purely on the basis of merit in respect of the options given by the candidates.

45.a)The Selection will be made by Counselling based on Merit only and no communal reservation will be followed.

b) 50% of seats in each speciality and in each college are reserved for service candidates, in addition to those service candidates selected in the open category. In case an odd number seat remains in any speciality in the total seats sanctioned for that speciality, that seat shall be marked for open category and filled by merit (as illustrated in Example 1).

The odd seat stated above is referable to each speciality and not to each college ( as illustrated in Example 2)

Example 1 : In MCh (Urology ) course, there are 8 seats in Madras Medical College, 5 seats in Stanley Medical College and 6 seats in Kilpauk Medical College. Thus there are totally 19 seats with 10 seats for the open category and 9 seats for the Service Category.

Example 2 : In DM ( Nephrology ) course there are 6 seats in Madras Medical College and 1 seat each in Stanley Medical College and Kilpauk Medical College. Of the 6 seats in Madras Medical College 3 each will be allotted to the open and service categories. The single seat in Stanley Medical College and Kilpauk Medical College will be allotted one each for the open and service categories as per Merit .

c) If there are no Non-service candidates opting for any Higher Speciality Course, the same will be allotted to service candidates.

d) If there are no Service candidates opting for any Higher Speciality Course, the same will be allotted to Non-service candidates.

e) For the newly sanctioned seats after the I Phase of counselling, reallotment of courses / colleges will be permitted as per merit from rank one. Joined and waitlisted candidates only can attend the counselling for the allotment of above seats

f) Candidates who are absent at the time of counselling without a valid reason for absence will not be considered for admission. However the Selection Committee reserves the right to permit the candidate to attend the counselling if a valid reason for absence is communicated to the Selection Committee. The candidate so permitted can only claim if the seat is available at the time when the candidate actually attends.

46. a) Candidates securing less than 50% of Entrance Examination Marks in other communities and 40% of the Entrance Examination Marks in reserved communities (BC, BCM, MBC, SC, SCA & ST) will not be considered for Counselling. (Entrance Examination Marks =Marks for 90).

b) Strict discipline should be maintained by the candidates for smooth conduct of counselling. If the candidates are found to indulge in any untoward activities, they will be debarred from the counselling and selection for the present and subsequent sessions.

c) The candidates appearing for allotment by personal appearance should bring the documents enclosed along with application in original . In any case candidates without original certificates / documents, shall not be allowed to participate in the counselling. Candidates who have deposited their original documents to Institutions / Universities and come for counselling with a certificate stating that “ their original certificates are submitted with institution/ University” will NOT be allowed to participate in the counseling .

47. Any change or modification and relevant information pertaining to this admission process will be made available immediately on the websites. (www.tnhealth.org / www.tn.gov.in.) The candidates are instructed to frequently visit the websites from the date of application till the end of counseling without fail. All information including the date of counseling will be put up in the website. Call letters can only be downloaded from the websites. Candidates will be permitted for counseling accordingly.

48. Unauthorized absence of candidates for one month or more will be treated as discontinuation and the vacancy will be filled up by the Selection Committee depending on the availability of the time before cut-off date

49. Option exercised at the time of counseling is final and no request for change of discipline is permitted.

50. The candidates who are selected at the time of counselling shall pay the tuition fees in total for a sum of Rs.30,000/- (Rupees Thirty thousand only) by way of Demand Draft drawn in favour of the Secretary, Selection Committee, Kilpauk, Chennai – 10 payable at Chennai, at the time of receipt of the allotment order. After the receipt of the joining report from the Deans of the respective colleges, this amount will be transferred to the colleges concerned. This fee is not refundable.

All candidates attending the counseling for D.M. / MCh courses have to remit a non-refundable amount of Rs.500/- by means of Demand Draft drawn in favour of the Secretary, Selection Committee as processing fee .

51. a) The candidates who discontinue the course shall pay discontinuation fees to the Deans of the respective colleges the sum as specified below by way of Demand Draft drawn in favour of The Secretary, Selection Committee, Kilpauk , Chennai -10 payable at Chennai.

52. The admissions to DM / MCh Courses for 2014-2015 will close on the cut off date of admission (i.e. on 30-09-2014) as per the Guidelines issued by the Medical Council of India and the Tamil Nadu Dr. M.G.R. Medical University, Chennai – 32.

56. ( a ) All the selected service candidates at the time of joining should execute a bond as shown in the

Annexure VI for a sum of 25,00,000/- (Rupees twenty filve lakhs Only) to the Government that they would serve the Government till the date of superannuation with three sureties. One surety should be from the Tamil Nadu Medical Service in the same rank as that of the candidate or Medical Officers of Higher rank with leftover service of 5 years. Two sureties should be the spouse/parent of the candidate. PAN numbers of the sureties should be furnished.

The prescribed form will be available in the colleges at the time of admission. The bond will become infructuous if the service candidates serves the Government of Tamil Nadu after the completion of the Course until superannuation. ( b ) All service candidates of Tamil Nadu who have less than 5 years of service after passing

Higher Speciality Degree course have to serve the Government for a period of 5 years from the date of Passing the examination irrespective of the date of superannuation, if the Government required their services. They will be paid salary till superannuation . After superannuation salary will be given as per the Government Order in force. They have to furnish an undertaking to this effect at the time of joining the course.

57. All recipients of stipend should execute the surety bond available in the Office of the Dean at the time of admission.

58 (a) All the selected Non Service candidates at the time of joining should execute a bond as shown

in the Annexure VI for a sum of 25,00,000/- (Rupees twenty five lakhs only) for Higher Speciality Courses 2014-2015 session undertaking that they shall serve the Government of Tamil Nadu for a period of not less than 5 years, if their services are required by the Government of Tamil Nadu. During the above period they will be paid a salary on par with the fresh recruits of the Government of Tamil Nadu Medical Services and the Government of Tamil Nadu will requisition their services, if required, within a period of 2 years from the date of completion of their Higher Speciality Course. The bond should be executed by the candidate with three sureties who are Income Tax Assessees. Two of the sureties should be the spouse/parent of the candidate with proof of address, photocopy and details of immovable properties. The PAN Number of the sureties also should be furnished. The bond will become infructuous if the candidate serves the State Government of Tamil Nadu if required for a minimum period of 5 years.

(b) All the selected Non service candidates who have not completed the mandatory 2 year bond

period after that PG course (MD/MS)done in Tamil Nadu Government Medical Colleges under the state quota/all India quota will have to give an undertaking that they will complete the remaining period of the 2 years bond period or 2 years bond period as the case may be which has already been executed by them at the time of joining their PG course.

Non service candidates will be paid stipend till the age of 58 years only.

59. The bonds are governed by clause (c) under exemption under Article 57 of Schedule 1 of the Indian Stamp Act, 1879 (Central Act II of 1879). Hence the bonds executed need not be stamped.

XII.. HEPATITIS-B VACCINATION: 60. Candidates selected for admission to the various Higher Speciality courses should take Hepatitis-B

Vaccination and submit a Medical Certificate as proof thereof to the Head of the Institutions at the time of admission. The Heads of Institutions should not admit any candidate without the Medical Certificate and also ensure that the candidate is periodically vaccinated during the course of study.

XIII. COMMUNICATION : 61. a) All Communications will be sent only to the mailing address given in the application. Candidates

are requested to give any change of address immediately. The Intimation / Call letters will be available only in the websites for downloading by the eligible candidates.

b) Candidates should inform the change of address, change of postings etc., immediately to the

Selection Committee. If the change of address is not intimated to the Selection Committee in time, the Selection Committee will not be responsible for delay in communication. The results will be published in the Websites www.tnhealth.org and www.tn.gov.in. The Counselling Schedule will be available in the above websites.

62. The candidates who join Higher Speciality Courses should not indulge in any kind of agitation/strike/ragging activity inside/outside the college campus during the course of study. Candidates found to take part in any such activities mentioned above will be expelled from the course/college, at any part of the course of study and criminal action will be taken against them.

63. Any candidate appearing for the entrance examination is deemed to have read the content in this Prospectus and agrees with all the conditions and clauses and will not have the right challenge any of the Regulations after appearing for the Entrance Examination

INSTRUCTIONS FOR PHOTOGRAPHS

1. Recent passport size photograph with light background is required.

2. Polaroid photograph are NOT acceptable.

3. Photograph must be snapped on or after 1st June , 2014.

4. Photograph must be taken with a placard indicating name of candidate ( as in the application) and

date of taking photograph.

5. In case the name and date are written on the photograph after taking it, the application will be

rejected.

6. The name and date on the photograph must be clear and legible.

7. Photograph should NOT have cap and goggles. Spectacles are allowed.

8. One photograph should be pasted ( NOT STAPLED ) on Application Form.

ANNEXURE IV

INSTRUCTIONS FOR NATIVITY CERTIFICATE

1. This Certificate should be issued by an officer of the Revenue Department not below the rank of Tahsildar in the Taluk concerned.

2. This Certificate should not be issued by the Special Tahsildars, Deputy Tahsildars such as Loans, Land Acquisition, Election, Excise and HQs, Deputy Tahsildars, Special Deputy Collectors, Assistant Commissioner of Agricultural Income, Excise, Elections

etc.

3. This Certificate should be signed also by the Village Administrative Officer.

4. The Certificate should bear the stamp of the office of the Officer signing the Certificate.

5. Any foreign national irrespective of the period of study in the State will not become eligible to apply for this course. No certificate of Nativity in the State of Tamil Nadu, should therefore be issued to Nationals of other countries for this purpose. 6. The certifying officers should insist upon clear proof of the Nativity of the Parent or Guardian

of the candidate and satisfy themselves on the genuineness of the residential qualification. They should be particularly careful in the case of candidates whose mother tongue is not

Tamil and whose place of birth is other than Tamil Nadu and who have studied MBBS /

MD / MS /DNB outside Tamil Nadu.

7. Residence Certificate will not be considered as Nativity Certificate. 8. The guidelines prescribed for the issue of Nativity Certificate and they are as follows as per

Letter No.RA.V(B)16932/2000, dated 3-4-2000 of the Special Commissioner and Commissioner of Revenue Administration, Chepauk, Chennai-5.

(i) The parents/guardians of the applicants/students or the applicants themselves

should have permanently resided continuously for a period of five years in Tamil Nadu. (ii) Permanently residing for a period of five years should be supported by documentary

evidence. (iii) The family ration card, Electoral Roll, Census List if taken recently, documents like sale

deed,tax receipt etc. relating to the property owned by either of the parents or by the applicant may be verified.

(iv) The Transfer Certificate issued by the School authorities where the applicant had studied last may be verified to know whether he was in the state for five years.

(v) Enquiry in the village / place of residence of the neighbour / Village Administrative Officers regarding continuous residing.

(vi) To ensure that wrong or incorrect address had not been furnished to obtain the certificate.

(vii) The birth place of the present, place of residence of the parent / father, permanent assets, mother tongue, place of education, place of marriage of the applicant / parents, the period of stay in and outside Tamil Nadu can also be considered before issuing certificate.
