Fremantle Community Care promotes and provides support to older adults and people with a disability to maximise wellbeing, independence and access to the community.

The following services are funded by Home and Community Care (HACC) to eligible residents living within the City of Fremantle.

Dial-a-Ride Dial-a-Ride is a transport service funded by the Home and Community Care (HACC) program. Door to door transport is provided for the elderly, frail and people with disabilities.

Buses travel to the heart of Fremantle, Phoenix, Gateways and Southlands shopping centres. We organise special transport trips to Spud Shed, Spotlight, Op shops and other venues.

Social outings This service also provides weekly social outings for HACC eligible clients. Fremantle Community Care services are available to people residing within the boundaries of the City of Fremantle.

This is a fee for service program with fees and eligibility set by HACC. wellbeing as animal welfare. If age or disability prevents you from fully caring for your pet,

Volunteer Task Force services are currently available for aged people with a disability or medical condition living in the community.
