    Dear Friends:During Fiscal Year

    2009-10 the Health Trust board and staff took time to examine the results of our funding and advocacy efforts for the past five years (2005-2010) and to update our strategic grant making plan for the next five years (2010-2015).

    One of the most distinguishing features of the Health Trust’s funding and advocacy to date has been the board’s decision to “stay the course” with sustained support in our key strategic health priority areas: diabetes, oral health, support of health professions and access to care. It is our intent to continue supporting these areas of need until a significant impact can be seen.

    From our perspective, this philosophy yields two major benefits: 1) It recognizes the advantage of investing more deeply into our four health priority areas as we strive to make long-term impacts in the community’s most difficult health care challenges; and 2) It gives us the opportunity to use our “lessons learned” to implement refine-ments in our priority areas rather than significantly diverting our attention and resources to new areas every few years.

    In most instances, the lessons learned from these funding investments have helped to validate our methods and approaches while others have pointed the way to consideration of alternatives.

    For example, our new 2010-2015 strategic plan consolidates two distinct grant cycles into one with the result of creating a more streamlined and competitive grant making process that is open to a broader range of non-profit agencies.

    Prudent planning in all Health Trust endeavors has been the founda-tion on which we have built strong community partnerships that sup-

    port our mission to improve health care in the Pajaro Valley. We take this opportunity to salute the dedicated, nonprofit organizations (see facing page) who received Health Trust funding during FY 2009-10. Each offers innovative health care programs benefiting children and adults in our community across the Health Trust’s four strategic program areas.

    We also proudly recognize numerous com-munity donors to the Health Trust whose generos-ity during FY 2009-10 helped to fuel the success of our partner organizations in their efforts to positively impact the quality of life and health for those in greatest need in our community.

  • Grants Awarded for Fiscal Year 2009-2010

    Following in alphabetical order is a list of grants and program support during this period:

    Aptos High School, Cabrillo College, Pajaro Valley High School, Watsonville Adult Education, Watsonville High School — $5,000 in scholarships for Pajaro Valley residents pursuing careers in the health professions.

    City of Watsonville Parks & Community Services Department — $15,000 to support the Get Out, Get Fit Youth Camp that will combat obesity and develop healthy lifestyles by engaging youth in a variety of fitness and wellness classes, physical activities, and outdoor field trips.

    Dientes Community Dental Care — $10,000 to support the delivery of dental services to low-income, uninsured adults from the Pajaro Valley.

    Ecology Action — $7,500 to establish the “Boltage” Program at Starlight Elementary School in Watsonville, a fun and safe way to encourage children to be more active by walking or riding their bikes to school.

    Healthy Kids of Santa Cruz County — $16,000 to provide access to health insurance to children in the Pajaro Valley who have incomes at or below 300% of the federal poverty level.

    Life Lab Science Program — $6,500 to create a workshop series based on a new pre-school curriculum guide titled, Sowing the Seeds of Wonder (SSOW). The book and the workshop series emphasize exploration in the garden and highlight early childhood healthy eating and nutrition practices.

    Mesa Verde Gardens — $8,500 to support the Daycare Gardens project, which will build organic gardens in 10 to 12 daycare homes and facilities serving low-income children in Pajaro Valley.

    Planned Parenthood Mar Monte — $5,000 to support the Teen Success Nutrition Series aimed at reducing obesity and diabetes risk among pregnant and parenting teens by increasing knowledge of and access to local, affordable nutrition and exercise options.

    Salud Para La Gente — $10,000 to support the delivery of dental services to low-income, uninsured adults from the Pajaro Valley.

    Santa Cruz County Office of Education (Watsonville Community School) — $7,500 to

    support the Physical Education for Body, Mind, and Spirit Program at Watsonville Community School. The program blends the philosophies, movements, and practical aspects of the martial arts with the fundamental principles of fitness,

    nutrition, and physical health.

    Watsonville High School Health Careers Academy — $7,000 for enhancement of its program that provides a specialized course of study for high school students interested in entering the health professions.

    United Way of Santa Cruz County — $2,000 in support of the Community Assessment Project, a biannual study conducted county-wide to gather data regarding a number of community indicators, such as health, safety, and the environment.

    Program Support:Diabetes Health Center — $470,186 to increase awareness and help prevent and reduce the incidence of acute and chronic complications of diabetes by providing culturally competent Diabetes Prevention and Self Management Education and Medical Nutrition Therapy Services to all who need them.

    Regional Diabetes Collaborative — $ 85,016 to bring together organizations and individuals from throughout the tri-county area to work on issues such as diabetes education and prevention, public policy, clinical management and best practices.

    The Foundation invites

    grant proposals through

    a Request for Proposals

    (RFP) process. Deadlines

    and awards amounts vary.

    Complete details are outlined

    in each RFP. RFPs are released

    and publicized through

    our website and through

    targeted communications.

    We encourage all interested

    organizations to visit our

    website periodically for RFP

    notices and/or to request that

    their organization be added to

    the Trust’s RFP mailing list. All

    grants made are health related

    and must address one or more

    of the Trust priority areas:
