- 50 000 000 infected people- 50 000 deaths per year
Human = 3 tapeworms: Tænia solium, Tænia saginata, T. asiatica.
T. solium most frequent( South America, South East Asia, Africa)
T. asiatica = Taiwan, Thaïland, en Koree, China, Ethiopia, Indonesia, Madagascar

EPIDEMIOLOGY
Contact human-pork => taeniasis / pork cysticercosis
Fecal peril=> contact with eggs from T. solium=> human cysticercosis
Self contamination => human cysticercosis
Tourism / migrations = change in area of transmission (imported cases)

HOW TO CONTROL CYSTICERCOSIS
GALVMED / OMS Strategy
GALVMED: one dose oxfendazole by mouth and one vaccine into
neck muscle = at the same time then a second vaccination 3-4 months later + oxfendazole.
OTHERS: vaccine given four weeks apart and oxfendazole at the time
of second vaccination,
vaccine four weeks apart then repeated at six months and oxfendazole every two months.

Deux molécules utilisées : le praziquantel et l’albendazolebonne tolérance et un faible coût (en zone tropicale)Albendazole = plus grande régression des cysticerques + meilleure diffusion cérébral
Le traitement étiologique n’est jamais une urgence thérapeutique = instauré en milieuhospitalier.
Contre-indiqué dans la cysticercose oculaire, dans les encéphalites et en cas de chargeparasitaire élevée (> 100 kystes) = risque d’exacerbation de l’inflammation et d’œdèmecérébral.
Les calcifications ne requièrent pas de traitement antiparasitaire (le parasite est déjà mort)
L’efficacité évaluée par l’examen radiologique (TDM ou IRM), réalisé à trois et six moisaprès traitement pour suivre la régression des lésions.
La réponse au traitement est très variable d’une forme à l’autre et d’un individu à l’autre.Aucun protocole thérapeutique optimal n’a encore été établi.

complications majeures du traitement antiparasitaire = réaction inflammatoire locale sévère.
exacerbation des signes = entre le second et le cinquième jour de traitement.
Certaines localisations sont plus à risques : sous-arachnoïdienne, intraventriculaire, spinale oulors de lésions multiples.
L’inflammation peut conduire à un infarctus cérébral, une hydrocéphalie aiguë, unehypertension intracrânienne ou un œdème massif.
corticothérapie instaurée deux jours avant le début du traitement et poursuivie quelques joursaprès = dexaméthasone (0,2 à 0,5 mg/kg par jour), ou la prednisone (1 mg/kg par jour).
Attention = les stéroïdes et les carbamazepines diminuent les concentrations plasmatiques dupraziquantel mais pas de l’albendazole
Si encéphalite ou œdème cérébral sévère = associé les corticoïdes à un diurétique osmotiquecomme le mannitol à la dose de 2 g/kg par jour.

monothérapie par carbamazépine, valproate de sodium ou phénytoïne.
même après traitement curatif, les larves calcifiées restent épileptogènes=> traitement anticonvulsivant poursuivi jusqu’à deux ans après la dernière crise
Si pas de crise = pas de traitement en prophylaxie.
récidive possible des crises en cas de multiples lésions malgré le traitementanti-helminthe
cas de récidive = faire des cures séquentielles prolongées d’albendazole ET depraziquantel (sensibilité différente de chaque kyste à chacun des médicaments,pour un même patient)
