1 INTRODUCTION

In 2013, the year 2 of the Round 8 Phase ll of the Health Systems Strengthening (HSS) grant, the

activities implemented covered 3 broad components, namely: (i) the Health Worker Retention

Scheme (HWRS), (ii) the Community System Strengthening (CSS) and (iii) the Health Information

System (HIS). This report presents the summary of activities implemented under the grant during

that period, the overall performance, the Capacity Development (CD) activities, and the successes

and challenges encountered in the implementation of the grant.

During the period under review, the grant recorded impressive performances and these are

reflected in the A1 ratings it achieved in the semester 1 and semester 2. Noticeable improvements

were observed in two of the HWRS and HIS components, especially the indicators on

completeness and timeliness of reports to the national level.

During the year, the CCM continued to provide its oversight leadership and guidance to the

implementation of the grant. The PR also provided timely updates on grant implementation and

progress to the CCM as well as data to populate the CCM dashboard. The PR worked

collaboratively with the SRs and SSR in devising strategies for implementation of the grant,

coordination of capacity building, and monitoring and evaluation, which ensured the basis for

good performance.

In 2013 the GFATM approved additional $2.7 million dollars to support the national rollout of the

DHIS-2. The piloting of the system was completed in the first quarter of 2013 and the national

rollout to provinces, districts and 166 hospitals was successfully completed with technical support

from Oslo University (UiO).

OVERVIEW OF THE GRANT

2.1 BACKGROUND

Zimbabwe has experienced an unprecedented decline in health service delivery due to the high

inflation, severe economic decline and rising poverty, which led to mass exodus of skilled

workforce in particular from the health public sector. This adversely affected the country’s health

delivery system and consequently health outcomes of the population. This situation seriously

compromised efforts by the government of Zimbabwe and its international partners to provide

universal access to basic health services and combat HIV, Tuberculosis (TB) and Malaria. The

deterioration of the health sector has had a negative bearing on the Government of Zimbabwe in

its quest to achieve the Millennium Development Goals (MDGs), which among others, include

having halted by 2015 and reversing the spread of HIV/AIDS and reversing the incidence of

Malaria, TB and other major diseases. In response, the Ministry of Health and Child Care (MOHCC)

and some funding partners instituted the “Public Health Retention Scheme” in 2007/2008. The

goal of the retention scheme was to reverse the outward migration of health staff from the

country; and to ensure that the numbers of new trainees entering the health system from the

clinical training schools exceed those leaving the sector.

In 2008, a Harmonized Health Worker Retention Scheme (HHWRS) was set up as a joint initiative

between the MOHCC, Health Services Board (HSB), College for Health Science, Harare City Council

and various development partners, including the Global Fund, European Commission, Expanded

Support Programme on HIV/AIDS (ESP), DFID, UNICEF, WHO and UNFPA. The HHWRS is embedded

in a national policy and is described in the emergency short term Human Resource retention policy

of December 2008 (MOHCW, 2008) and the reviewed short-term HWRH Retention Policy of April

2009 (MOHCW, 2009).

The HHWRS implemented during Phase 1 of the HSS grant has proved to be an important

contributory factor in the reduction of vacancies in the Health sector and the increased facility-

based births and in order to sustain these achievements the Phase 2 has continued to provide

funding for the payment of the retention scheme allowances for additional 2 years (2012-2013).

Goal of the Grant: The goal of the Grant is to enhance the capacity of the health system to deliver

effective scaled-up treatment for HIV, Malaria and TB.

Objectives: The specific objectives of the grant are three fold, namely:

− To reverse the current trend of high outward migration; ensuring that the numbers entering

the system from the clinical schools are larger than the numbers leaving;

− To motivate and retain trained community health workers (CHW) and strengthen their

involvement in community programs for HIV and AIDS, TB and Malaria; and

− To ensure an effective and integrated HMIS system, backed by improved communications and

IT servicing capacity

Target Group/Beneficiaries: The target group of the grant includes Health Workers, Health

Information Officers, Home-based Care givers and Village health workers, communities and people

affected and/or infected by HIV/AIDS, Tuberculosis and Malaria.

2.2 COMPONENTS OF THE GRANT

The HSS grant was designed around three main components, namely the Health Workers

Retention Scheme (HWRS), Community Systems Strengthening (CSS), and the Health Information

Systems (HIS). A brief description of each component is provided below:

2.2.1 Health Retention Scheme

The main purpose of the Health Workers Retention Scheme is to attract and retain Health Workers

in the public sector through the payment of monthly top-up salaries or allowances to critical

health workers with the grades of C5 and above. The GFATM took over the payment of the

retention scheme allowance in September 2009.

In 2011, as part of an exit strategy plan of the payment of the retention allowances, the

Government of Zimbabwe and the Global Fund agreed on an annual reduction of 25% in the

payment of the allowances by the grant. As a result, during 2013, the GF was paying 50% of the

2011 retention allowance amounts. The Institutions whose health workers benefit from the

retention scheme include the MOHCC, Councils, Colleges and Mission Hospitals. However, the staff

members from College and NUST were removed from the Retention Scheme in October 2011 per a

CCM decision on the 29th July 2011. The category of HWs with grades C5+ who are considered

critical staff and benefit from the retention allowance include medical doctors, nurses,

pharmacists, radiographers, laboratory staff, Environmental health staff, human resources, records

and information and Finance staff.

2.2.2 Community Systems Strengthening (CSS)

From January 2013, this component is

being implemented by the MOHCC and

NAC that has replaced the Zimbabwe

AIDS Network (ZAN) in the payment of the

allowances to the target group.

The CSS component of the grant has a

total of 11,260 community health workers

being paid a US$42 per quarter

allowances as incentives for the work they

are doing. The MOHCC is responsible for

the payment of 120 Village Health

Workers (VHWs) per district while the

National AIDS Council (NAC) is the new

Sub Recipient for the management of the payment of 80 Secondary Care givers (SCGs) per district.

The duties of a CHW /VHW include treating minor ailments, providing medical and psychosocial

support to chronically ill patients at home, following up on patients, identifying problems in the

community and advising the rural health centres accordingly. The CHW/VHWs are expected to

work for 2-3 days per week. However, due to the increasing burden from HIV and AIDS and TB, the

job of the VHWs has now become almost full time.

2.2.3 Health information Systems (HIS)

The role of the Health Information System (HIS) Component of the grant is to ensure an effective

and integrated Health Management Information System (HMIS), supported by improved

communications and IT servicing capacity. The component of the grant supports a phased

integration of existing HIV and AIDS, TB and Malaria M&E systems into the National HMIS, and the

communication. This is being achieved through operationalization of the integrated national HMIS

through the following activities: (i) Training of Health Information Officers at national, provincial

and district levels (ii) computerisation of relevant training of the NHMIS from district level up; (iii)

provision of appropriate communication systems: radios in sentinel rural clinics without access to

land line telephone, and broadband internet at district, provincial and central level; and (iv)

procurement of computers to be used at district level where the main data sets are collated (v) the

provision of cell phones to 1,200 rural clinics and as well as negotiate for the provision of network

coverage for areas that are not covered. The HIS department also ensures timely submission and

transmission of national data from the rural and other health facilities to the head office where the

data is collated and shared with all stakeholders.

The SRs and SSRs and their roles in the implementation of the grant is provided below:

Ministry of Health and Child Care (SR)

� Health Information & Disease Surveillance Unit

The Health Information & Disease Surveillance (HIDS) Unit of the MOHCC is responsible for the

collection, storage, processing, analysis, reporting, utilization and dissemination of health

information at all levels from the Primary sources of data to the Head Office. It services all the

three disease components of the grant and provides routine health statistics to report on key

indicators for all the grants. The HIS unit manages and reports on the HIS component of the

grant as an SR, including reporting on four indicators. The HIS unit is leading the effort to

improve the health communication infrastructure of the MOHCC to ensure timely submission

of quality reports at all levels.

� Nursing Department

The Nursing Department is one of the Departments under the MOHCC and is responsible for

the development of national standards and policies for the nursing services in Zimbabwe. The

department oversees the implementation of general nursing care, including community

nursing and patient nursing. The community nursing covers the VHWs that offer limited

health services across all the three disease components (Malaria, TB and HIV/AIDS) in the

communities in which they live. The department is also involved in infection control and the

implementation of mental health, midwifery and maternal and Child health (MCH). The

Nursing Department co-manages the CHWs component of the grant with NAC.

National AIDS Council - (SR)

In 2013 Zimbabwe AIDS Network (ZAN), SR for the HSS grant since Phase 1 was replaced by NAC.

Following the withdrawal of the donor financing to ZAN (through the Joint Funding

Arrangement1) and the implications on the organization’s operations, the SR agreement with ZAN

was terminated during the year. The uncertainty of the future of the organization, the reduced

capacity and the absence of core funding from the major donors (the GF funding was on the

principle of additionally), are the factors the PR took into account in the decision to terminate the

SR agreement. As an interim measure NAC was requested to support implementation of this

component of the grant on the basis of the SR agreement signed in June 2013.

For the remaining of Phase 2 NAC will be the SR for the payment of the allowances to the

Secondary Care Givers (SGCs). The roles of the SCGs within the communities are to prepare,

monitor and support clients in comprehensive care of HIV and AIDS, TB and Malaria. The focus is to

maintain regular contact with clients and their families and:

− Train family members in how best to care for clients

− provide physical, emotional and spiritual support to both the clients and family members

− Refer clients to clinics and other health services

− Provide information and education on all the 3 diseases and monitor treatment adherence.

Health Service Board (SR)

The Health Services Board (HSB) is the SR responsible for the management of the retention scheme

component of the grant. It is a parastatal organization established by the Health Service Act,

Chapter 15:16. The functions of the Board include:

− To appoint persons to offices , posts and grades in the Health Services

− To create grades in the Health Services and fix conditions of services for its members

− To supervise and monitor health policy, planning and public health

− To enquire into and deal with complaints made by members of the Health Services

− To supervise, advise and monitor the technical performance of hospital management

boards and state aided hospitals

− To handle appeals in relation to disciplinary powers exercised by hospital management

boards over members of the health services

− To exercise any other functions that may be imposed or conferred upon the Board in terms

of the Act [Chapter 15:16]

HSB mandate under HSS Round 8 grant is to manage the payment of the retention allowances as

part of efforts to reverse the current trend of high outward migration, and ensuring that the

numbers entering the public health system from clinical schools are larger than the numbers

leaving the sector.

Crown Agents (SSR)

Crown Agents (CA) is an SSR under the Retention Scheme component of the Grant and it ensures

timely payment of the top up retention allowances to the 19,678 (2013), an increase from 18,860

(2012) critical health workers. It is an international development organization that provides

capacity building and international development services in public sector transformation. It

operates in more than 110 countries, working for international development agencies. It

established its presence in Zimbabwe in 1980, mainly involved in the management of the

technical support which the country received soon after independence. Under the

implementation of the grant, CA has been the paying agent since September 2009. Their main role

is to verify the authenticity of the beneficiaries, submit disbursement request to UNDP through

HSB, and ensure that the retention allowances are paid to beneficiaries.

ACTIVITIES IMPLEMENTED

3.1 RETENTION SCHEME

3.1.1 Payment of Retention Scheme Allowances

During the year under review, a total of 19,678 critical health workers (nurses, doctors,

pharmacists, Environmental Health Technicians, Radiographers, and Laboratory Scientist) were

paid top-up allowances under the retention scheme component of the grant. Analysis of the data

shows that there were more appointments (2,689) for all the health professional categories put

together during the period, than terminations (571). Table 1 illustrates the in post, appointments

and terminations by category of health staff tracked in the performance framework and at

program level in 2013. The net loss or gain for each professional category during the year is also

shown in Table 1.

Performance of In Post: The total in post at December 2013 was 19,678 compared to 17,542,

reported in December 2012, representing a 12% increase in the net gain of health workers across

all the health professionals that were paid and tracked in the Performance Framework and at

programme level. The decision of government to relaxed recruitment in the public health sector at

the beginning of the year to enable them recruit critical health staff could explain the increase in

health workers paid retention allowance. The decision by the GFATM to remove the ceiling placed

on the number of health workers who could be paid the allowance and rather placed the limit on

the approved budget also contributed to the positive shift. Similar, the ability of the public health

system to retain and attract more critical health staff following this retention scheme and other

motivational packages by other donors and government may explain the observed increased in

the number of in post in 2013 over 2012.

Performance of Annual Retention Rate: Annual retention rate measures the ability of the public

health system to recruit and retain health workers in the system. During the year under review, the

annual retention rate for doctors increased to 94.6% from 93.9% reported in 2012 while that for

nurses increased from 97.5% reported in 2012 to 99.6%. The good performance could be

attributed in part to the increase in the number of new appointments of nurses following the

removal of government embargo. However the non-reporting of the in-post statistics of HWs of

NUST and College had a negative bearing on the performance. The Staff of these two institutions

were dropped from the retention payroll by a decision of the HSB and the CCM because their

salary levels were deemed to have improved. Subsequently, the management of the two

institutions refused to report their in post data to the HSB as they had previously done. HSB has

requested the Permanent Secretary (PS) of the Ministry of Health and Child Care to engage the

Permanent Secretary of the Ministry of Higher and Tertiary Education to intervene.

It is instructive to note that the salaries of 8 Provincial Medical Directors and 3 Principal Directors

were also paid by UNICEF, under the Health Transitional Fund (HTF), and were subsequently

removed from the retention allowance payroll, and thereby creating opportunities for more health

workers to be recruited.

3.1.2 Human Resource Database

HSB and the HR Department of MOHCC are developing a Health Human Resources Database to

replace the current excel-based database. The Web based database seeks to capture all the health

workers in Zimbabwe in 2013 with the assistance of HITRAC. The database automatically updates

the information once changes are made and thus allowing real time reflection of what is on the

ground. The facilities are able to analysis, print and share the information in the most effective and

timeous manner.

The UNDP joined a team comprising CDC, MOHCC and Hitrac to Masvingo in September 2013 to

assess progress made in the rollout of the database to provincial level. The field visit was also used

to document lessons learnt and challenges encountered from the implementation of the database

in an effort to improve its utilization.

3.1.3 HSB Oversight over implementation of HWRS

Implementation of Attendance Register: The HSB with the support from UNDP designed a Daily

Attendance Register (DAR) in 2013 with the objective to standardize the logging registers use by

the institutions which differed from one institution to another. UNDP led the procurement process

after receipt of specifications from the HSB. Technical evaluation of the bids was conducted by the

two institutions and a printing company was identified for the production of the registers, which

are expected to be delivered by the end of the first quarter of 2014. In total, 1,070 registers with

100 page leaflets are being printed for smaller institutions and 1,000 copies with 200 page leaflets

for the Provincial, District and Central hospitals

Activities of Auditors: The five Auditors of the HSB carried out 59 field visits during the year as

part of the SR oversight to mitigate the risks of implementing the retention allowances. The sites

and dates when the field visits were carried out are appended to this report (See Annex 1).

The major findings and recommendations from the exercises carried out by the Auditors have

been summarized in Table 2.

ISSUES RECOMMENDATIONS/ACTIONS TAKEN

1. Payment of Non-Deserving Members

In some of the health institutions, HWs whose

appointments have been terminated for various

reasons (resignations, discharges due to disciplinary

action, deaths and retirements) continued to draw

allowances, the terminations had occurred for durations

in excess of 12 months.

The findings also applied to members of staff who were

on full time MDL and have been away for more than a

year but remained on the retention scheme payroll

Recommendations: (i) Communication on termination of HWs should be

strengthened and immediate to the Provincial/National office. Districts

should not wait for the cutoff date as this causes delays in removing the

member from the data base. (ii) All members on full time MDL should be

removed from the retention scheme payroll as stipulated in the information

pack. Furthermore, only those on attachments of more than three months

duration should benefit from the scheme.

Update on Actions to implement recommendations:

(i) All none deserving members (retired, resigned, discharged, deceased and

those on fulltime MDL) have since the findings been removed from the

database. HSB sent communication to all the affected individual members

for refund of funds undeservedly paid to them. (ii) Retention allowance Pay

slips have been introduced since December 2012 to help reduce issues of

undeserving members remaining on the database after they would have left

service.

2. Maintenance of the Retention Scheme Database

The districts are slow in updating the monthly

information coupled with wrong information

being relayed to the next level of reporting. This

problem is major and prevalent in all Health

Institutions. Employee names, ID and EC numbers are

incorrectly captured. Transfers in and out are not

adjusted whilst there were situations where names of

some members of staff are omitted from all data bases

at, district and provincial levels yet were receiving the

retention allowances.

Recommendations: All districts should urgently take the necessary steps to

ensure accuracy in the presentation of the data to the next level of reporting

It is further recommended that a data clean-up exercise be conducted at

Central Hospitals, Provinces and Districts to clean the database. This can be

facilitated at HSB level through the sharing of databases with provinces and

districts.

Update on actions to implement recommendations

HSB now provides institutions with the database of the last month of every

quarter to enable them compare with their database and provide feedback

where necessary.

HSB HR Auditors have been instructed to ensure that at every institution

they visit they clean up the database.

3. Clocking in and out Registers

No consistency in the completion of the clock in and

out sheets in most of the health institutions. In some

cases the sheets had not been completed for months

especially in Rural Health Institutions where supervision

from the District is limited. However at all the Central

Hospitals the registers were being adhered to.

Recommendations: DHE members who conduct supervisory visits to some

of these satellite stations have been advised to conduct spot checks to

ensure that members of staff are completing the registers.

4. Communication System

Poor communication on the number of days HWs have

worked per month from the rural health centers to the

districts. The districts were submitting information

indicating that each member of staff had clocked 22

days yet there was no information to substantiate that

this had happened. This is prevalent in most of the

Districts.

Recommendations: DHE members should ensure that Health Centers

submit their clocking in and out sheets to the district monthly to curb

payment to those who probably would not have been at work or would

have worked for less number of days..

Update on actions to implement recommendations

Institutions have already been advised to ensure that they use the daily

attendance register when compiling the master attendance sheet.

5. Delays in Submitting Changes to the Database

It was noted that in some cases, changes to the

database were being submitted late. For example in

some cases new members of staff were disadvantaged

as their names were not submitted timeously for their

retention allowances to be effected.

Recommendations: Human Resources staff should always ensure that

information is submitted on time to the next level and to follow up on the

status of the information submitted on a regular basis.

Update on actions to implement recommendation

The date for submission of monthly changes has shifted from the 18th to the

25 of every month in order to give Provinces more time to compile the data

for onward submission to the HSB.

In general, the monitoring exercises conducted by the Auditors have enhanced the quality,

accuracy and timeliness of databases at Provincial, District and national level and this is reflected in

the improved quarterly reports submitted to the PR. The accuracy of statistics on the number of

staff in post against those on establishment, terminations (through dead or resignations) and the

appointments of new staff members improved considerably.

Development of Operational Plan: The HSB developed an Operational Plan (OP) to guide the

activities of the auditors for 2013. The OP detailed how the 5 auditors would carry out their

activities. The OP further provides information on sites and dates each Auditor will visit as well as

due dates for reports at the national level. On average, the Auditors spent at least 10 days every

month carrying out spot checks to health facilities to ascertain the accuracy of the reported data

and physically verify the existence of the workers and some of the activities mentioned above.

Quarterly review meetings 2013: Three quarterly meetings were held in 2013 and brought

together personnel from 11 reporting institutions who maintain the retention database. The

achievements, challenges and the best strategy to implement the retention scheme were

discussed at the meetings. Staff from Crown Agents, the paying agent also participated in the

meetings.

During the meetings, the findings during the OSDVs from the Auditors and UNDP were discussed.

Among the issues discussed include the following:

− Information Pack - meeting recommended that there was need to update the retention

scheme information pack, starting from January 2014 with the new grant i.e. HIV/AIDS NFM. It

was agreed that a team would be set up to develop the new information pack;

− Late submission of monthly returns – Institutions should conduct disciplinary procedures

against members who delay in submitting the monthly changes;

− Manpower Development Leave (MDL) – Members on post basic training courses in community

nursing and nursing science should not benefit from the scheme and their names should be

removed from the database.;

− Members on acting position – The meetings recommended that all staff on acting positions

should have a written communication from the institution indicating that the member is on

acting position to qualify them for allowances of the position they are acting.

The meetings further tasked Human Resources Auditors to check and ensure that Treasury

concurrence is sought in order to fill those posts with acting members.

− Payment of undeserving members - Institutions should make efforts towards the recovery of

all funds paid to undeserving members;

− Communication - all communications between provincial offices and districts should be in

writing to ensure that all communication reaches the district level and also ensure that they

are documented for future reference.

The quarterly meetings were generally successful. Major issues were highlighted and in most cases

explanations offered to the effective management of the health retention scheme. Members were

encouraged to give prompt feedback to other colleagues in HR at their institutions, provinces and

districts so that everyone is aware of the discussions and recommendations from the meeting.

3.1.4 UNDP Oversight over the Retention Scheme

Disbursement to Crown Agent: The SSR agreement for the HSS retention allowances, Crown

Agent (CA) was extended during the period. UNDP continued to disbursed funds to CA on a

quarterly basis upon receipt of DR through the HSB and after previous disbursements have been

fully acquitted. In total, four quarterly DRs were made to CA during the year.

On-Site Data Verifications (OSDVs): As part of UNDP oversight over the health retention scheme,

three field visits were carried out to Matabeleland South, Manicaland and Masvingo provinces to (i)

verify the existence of HWs who were paid allowances and (ii) proper record keeping, including

completion of the daily attendant registers. In all, 17 health facilities were visited and records on

attendance verified from the daily registers. The provinces, districts, sites or communities visited

and the findings per site have been summarized in the Table 3.

3.1.5 Achievements

UNDP and HSB managed to strengthen their oversight role of the grant through OSDVs conducted

during the period. Observations made and recommendations from the OSDV informed the

development of action plans to address issues noted in the field. It is worthy to note that the HSB

managed to address most of the issues from the field visits. Most of the individuals who had been

paid allowances that they did not deserve had either paid back or were in the process of doing so.

A total of 19,678 Health Workers in grade C5 and above were successfully paid their health

retention allowances for 2013.

The annual retention rate of doctors and nurses in 2013 improved compared to 2012, an indication

that the public health sector had been able to attract and retain critical health staff.

3.1.6 Challenges in 2013

There were delays in the submission of Disbursements for Q13 (1 Jan-31 Mar 2013) and Q14 (1 Apr-

30 Jun 2013) by the Crown Agents (Table 4) which affected the timely payment of monthly

allowances to beneficiaries. Delays in payment of allowances could be disincentive to HWs and a

potential source of disaffection among staff.

3.2 COMMUNITY SYSTEMS STRENGTHENING

3.2.1 Distribution of Cell Phones

In 2013, a total of 1,150 cell phones donated by Econet were handed over to the Nursing

Department of the MOHCC and eventually distributed to the Provinces and Districts. The donation

is in support of the piloting of the eco-cash paying mechanism to Village Health Workers (VHWs) in

three provinces. The payment of allowances through eco-cash is expected to reduce the hassles

and risks of carrying large sums of cash at the end of each quarter and the challenges experienced

in the acquittal of funds disbursed.

3.2.2 Community Health Workers

The 11,512 CHWs trained in year 1 of implementation of the grant were maintained and continued

to receive allowances in 2013. The Directorate of Nursing under the MOHCC continued to pay

7,540 VHWs from 62 districts in the country at a monthly rate of US$15, including administrative

charges. NAC, the interim SR for civil society also paid 3,720 SCGs.

NAC assumed the role to pay the SCGs that were previously managed by ZAN in June 2013, and in

August 2013 the SR paid the allowances to the SCGs for January-August 2013. The allowances for

Sept-December 2013 were paid in February 2014; and currently there is no backlog on the

payment of the allowances to the 3,540 secondary caregivers.

The major constraint encountered in the payment of the allowances during the period included

timely payment of allowances by MOHCC and ZAN (when they were in charge of CSO) and delays

in the acquittals of funds disbursed to ZAN and MOHCW.

It is important to note that, during the reporting period, the payment of allowances to 180 SCGs in

3 urban districts (Bulawayo, Harare & Chitungwisa) which had hitherto been done by the MOHCC

was transferred to NAC.

3.2.3 Change in Payment Mechanism of VHWs

The MOHCC Department of Nursing Services faced a number of challenges in the payment of the

allowances to the VHWs since the beginning of the grant and this resulted in the majority of the

VHWs not being paid allowances for the last 3 quarters of 2013 (1 April-31 December 2013). In

addition, the MOHCC also experienced some difficulties in acquitting the funds disbursed to them.

In view of these challenges, the CCM recommended that UNDP pilot the payments of VHWs

allowances in three provinces using mobile money facility provided by Eco cash. The 3 provinces

being piloted for the new payment mechanism are Midlands, Mashonaland East and Matabeleland

South and have a total of 2,760 VHWs. Following the recommendation, UNDP engaged Econet, the

parent company of Ecocash services in the last quarter of the 2013 for further details of their

services, Terms Of References (TORs) for the engagement of Ecocash was developed and a contract

signed between UNDP and Econet in the first quarter of 2014 for the implementation of the pilot

for the first 6 months of 2014. The lessons learnt from the pilot phase will inform the national

rollout of the new payment system starting from July 2014. Upon further discussions with the

MOHCC, it has been agreed that Ecocash payment mechanism will be used to pay the three

quarters arrears of 2013 and the first quarter allowances to VHWs in the pilot provinces by the end

of April 2014.

3.2.4 Evaluation of the CSS Component of the Grant

Following the evaluation of the CSS component by the Local Fund Agent,

PricewaterhouseCoopers (PwC) in April 2012, the Global Fund sent the communication to the PR

of the six key findings of the assessment in a Management Letter dated 12 October 2012.

The evaluation raised concerns over (i) inadequate supervision and linkage of community health

workers/volunteers with the health system, resulting in fragmented work by these different

groups and therefore difficult to see how they contribute to achieving national goals, (ii)

intermittent supply of health commodities to community health workers/volunteers to facilitate

their work, (iii) non capturing of CHWs Data into the national health information system, (iv)

inconsistency in payment of allowances, including lack on how the few who receive allowances

are selected, (v) non standardization of the training curriculum for the different community groups

doing similar work, and (vi) inadequate use by the 3 disease programs of the 11,260 community

workers supported by the HSS grant.

UNDP, in consultation with the MOHCC and other partners duly responded to all the issues raised

by the CSS evaluation, and after satisfying itself of the responses, GFATM deemed the CPs as

having been fulfilled.

3.3 HEALTH INFORMATION SYSTEMS

3.3.1 Roll out of District Health Information System 2 (DHIS2)

Through UNDP, the Global Fund provided additional funding of US $2.7m to support the national

rollout of DHIs in the country. The activities covered by the additional funding are detailed in Table

5. This was timely in addressing the funding gap for the national rollout in 2013 and ensuring that

implementation of the system was not delayed.

Activities Implemented: The additional funding facilitated the procurement of 309 laptops,

training of 600 health workers on the new system and the training of two MOHCC staff in Routine

Health Information System (RHIS) in India as part of enhancing national capacity building in HIS.

Having RHIS skills is very important at all levels of the health services because improved health

information is directly linked to good management and service delivery.

In addition, funding from the Global Fund enabled the University of Oslo, Norway, to provide

technical assistance to the MoHCC in the implementation of DHIS-2. Subsequently, all key health

workers at sites where DHIS- 2 has been installed have since been trained accordingly. And with

the inclusion of in-patient dataset in the DHIS-2, health workers in admitting hospitals have also

been trained in International Classification of Diseases (ICD-10) coding concepts to equip them in

the relevant skills.

It is worthy to note that, CDC through Research Triangle International (RTI), and UNICEF through

the Health Transitional Fund (HTF) also provided both funding and technical support for the

national rollout out. Prior to the national rollout of the system, MOHCC with support from CDC/RTI

piloted the system in Manicaland Province in the first quarter of 2014, the lessons learnt from this

pilot informed the national rollout.

Meanwhile, to ensure that all health facilities countrywide are fully covered, the MOHCC is

developing a 3-year (2014-2016) action to further guide the roll-out of DHIS-2 to health facility

level and also use it to mobilize resources for the implementation.

The DHIS-2 software is used in more than 30 countries in Africa, Asia, Latin America and the South

Pacific. In Africa, countries that have successfully adopted the new system are Kenya, Tanzania,

Uganda, Rwanda, Ghana and Liberia.

Technical Support from University of Olso: Following an MOU between the Global Fund and

University of Olso, the former provided technical support for the smooth rollout of DHIS2 to all the

district and central hospital from July to December 2013. Three system experts from HISP Oslo

undertook two missions to the country in July and December 2013 and provided expertise in HIS,

and development, ICT architecture and infrastructure. The 2 in country visits were aligned with the

plan for the national roll out for 2013.

The mission assembled and trained a core national team to support the implementation of the

DHIS-2 in the country. Furthermore, an assessment of the server room was carried out and

recommendations made to bring it up to an international level. The two servers to support the

DHIS-2 were configured and an assessment of the pilot of the DHIS-2 in Manicaland Province

undertaken. In addition, the Olso team developed a plan to inform the rollout of DHIS-2 to the

facility level by 2016.

The MOHCC is currently developing a TOR to guide a Memorandum of Understanding (MOU) with

Oslo University to provide a long term TA to the further strengthening of DHIS-2 in the country.

External Training in Routine Health Information System, (RHIS) Strengthening and

Management: As part of the approved DHIS2 budget, two female staff of MOHCC attended two

weeks international training workshop in New Delhi, India, from 23rd September to the 4th October

2013 on strengthening the Routine Health Information Systems Management. The international

workshop was uniquely designed, a large scale, sustainable response to address limited public

health capacity. The workshop was jointly organized by The Public Health Foundation of India,

Measure Evaluation and University of North Carolina. The participation of the staff is part of the

long term efforts to build national capacity in health information system in the country.

Frontline SMS software system: The primary system used to transmit weekly surveillance data

from health facilities to national level, through district and provincial levels was the fixed

telephone network. At some health facilities, two-way radio communication was used as a back-up

to the fixed telephone system. In 2010, only 8.8% of the rural health facilities had a functional fixed

line telephone, and only 11.3% had a functional VHF radio. However, these two methods of

communication collapsed after 2006 resulting in very little surveillance data being transmitted to

the national level. In 2008 the system suffered the worst when the situation was compounded with

the massive cholera outbreak in all districts. As a result, from Week 30 to Week 52 of 2008, the

reporting of weekly surveillance data to national levels was zero percent. The system picked up

slowly in 2009. The weekly surveillance system witnessed a vast improvement in 2012 in timeliness

and completeness of the weekly disease notifications from below 50% in September 2010 to

above90% in 2012 as a result of the use of cell phone Frontline SMS software system. The number

of health facilities reporting weekly surveillance data also increased significantly from 530 in 2010

to 1,165 in 2012. The improvement in the coverage and reporting of surveillance data follows the

provision of additional 1200 cellphones to the MOHCC and availability of timely air time for the

cellphones from the Global Fund grants and timely provision of technical support by CDC/RTI.

3.3.2 Improving Internet Connectivity

Availability of timely quality health data is critical for

planning, implementation and monitoring of health

programs. Timelines and completeness of health

reports have been very low following the economic

challenges the country experienced since 2000 and

thus affected the performances of health programs.

Many of the communication needs at district level to

facilitate timely submission of reports were unfulfilled

because of poor fixed telephone line network and

unavailability of internet connections. The MOHCC and

her partners are committed to restoring the more

reliable and efficient HIS that used to exist in Zimbabwe

to ensure timely transfer of health data from the district

and provincial levels to the national level.

To address the internet connectivity problem of the

MOHCC, a total of 82 health facilities are being

provided with fixed internet. UNDP hired the services of

two internet service providers i.e. Liquid and Telone, to install 54 fibre and 28 VSAT technologies at

the 82 sites. Installations at almost all the sites were completed and assessment of their

functionality carried out in the first quarter of 2014. In addition, the MOHCC was provided with

mobile internet (dongles) to selected hospitals to facilitate the resuscitating of the IMMIS. When

fully functional, the availability of internet is expected to facility timely transmission of data from

the district to national level to inform timely decision making.

3.3.3 Strengthening Collaboration with CDC/RTI

UNDP continued to collaborate effectively with CDC/RTI to help improve the health information

system of the MOHCW. UNDP and CDC/RTI worked closely to support the Weekly Disease

Surveillance System reporting (WDSS); CDC/RTI provided technical and financial support to the

MOHCC to maintain the system and ensured the continue functioning of the system without

interruption. Similarly, UNDP collaborated with CDC/RTI and other partners to develop the 2013

costed HIS Workplan, including the roll out of the DHIS-2 to all districts hospitals in 2013. The

development of the Plan enabled the country to mobilize the additional $2.7 million from the GF

to address the funding gap for the national rollout of DHIS2 in 2013. DHIS2 was rolled out to all

provincial and district levels as well as about 166 hospitals at the end of the year, and managed to

integrate 11 reporting systems (-5, HS3-5, IMMIS, RDNS, HIV, TB, and EID, etc.) into the DHIS-2

thereby minimizing multiple reporting systems within the MoHCC. When completed, the DHIS-2

will provide a platform to host the ePMS and other reporting systems.

MONITORING AND EVALUATION

4.1 ON-SITE DATA VERIFICATION

Follow Up on Health Workers Trained: As part of the risk mitigating measures instituted by

UNDP in the management of the Global Fund (GF) grants in Zimbabwe, field visits were conducted

in June 2013 to follow up on Health Workers (HWs) and Community Health Workers (CHWs)

trained under the four GF grants. The exercise was used to confirm the participation of the HWs

and CWHs trained, interacted with a sample of them in order to assess their utilization of skills and

knowledge acquired at the trainings, validate financial records on the trainings and payment of

allowances made to CHWs, and followed up on implementation of previous recommendations.

During these field visits, the UNDP teams (integrating staff from finance, program and M&E)

provided technical support in the respective areas to Implementing Partners (IPs). All SRs and SSRs

also participated in the field visits.

The findings and recommendations of the OSDVs were shared with the respective partners for

implementation.

4.2 GRANT PERFORMANCE

A total of 14 indicators were tracked by the grant at impact (3), outcome (3) and output (8) levels.

The 8 output indicators are distributed per the component areas as Health Retention Scheme (4),

and the HIS (4).

During the period under review, 7 output indicators were reported on quarterly in the first two

quarters, whilst an additional indicator on Annual Retention Rates (ARR) for Nurses and Doctors

was reported on in the last period of the year. All the seven impacts and outcome indicators were

reported on in the last semester of the year

Overall, the grant performed well in 2013 and was rated A1 for all the first semester and is likely to

record another A1 in the second semester of the year. The year 2013 maintained the good

performance of the previous year with significant improvement with the HIS indicators following

the national rollout DHIS2. Table 6 provides a summary of the performances of the grant by

indicator during the reporting period.

4.3 GRANT RATINGS

The GFATM rates the grants after the submission of report by UNDP and verified by the LFA. The

GFATM changed the frequency of reporting in Zimbabwe from quarterly to Semester with effect

from July 2012. In 2013, 2 semester reports were submitted and rated A1.

Table below provides the Grant Ratings for the HSS grant since the beginning of Rd 8 Quarter 1 till

Semester 2. In general, the grant has performed creditably achieving A ratings throughout the

grant period, except for Quarter 1 and quarter 3.

5 FINANCIAL SUMMARY

5.1. DISBURSEMENTS OF FUNDS IN 2013

A total of US$14,770,465 was disbursed by the Global Fund to UNDP in 2013. An interest of

US$47,238 was generated, bringing the total income for 2013 to US$ 14,817,703.

Cumulatively, an amount of US$74,045,239 has been disbursed of which US$69,372,030,

representing 94% of the total income generated, was spent in the implementation of the activities

by the PR and SRs/SSRs. Table 8 provides the details of the cumulative amount received, the

interests generated and the expenditures by implementing partner.

5.2. DISBURSEMENT IN 2013 BY SUB-RECIPIENT

Fig 1 provides the distribution of grant funds in 2013 by proportion of disbursements to PR, SRs

and SSRs. A greater proportion of the amount (78%) was disbursed to Crown Agents. UNDP

accounted for 11% of the disbursement, MOHCC (4%), ZAN/NAC (5%), and HSB (2%).

5.3 DISBURSEMENT BY COMPONENT I.E. HWRS, CSS & HIS

This section presents an analysis of the disbursements of the grant in 2013 according to the three

components. Of the total of US$13,992,545 disbursed during the year (excluding UNDP), the

Health Workers Retention Scheme (HWRs) accounted for 89.9% or US$12,580,809, Health

Information System (HIS) 0.3% and the Community Systems Strengthening (CSS) 9.8% of the total

amount.

CAPACITY DEVELOPMENT PLAN

The overall goal of the Capacity Development (CD) Plan is to strengthen the capacities of Sub-

Recipients (SRs) to improve their performances in implementing their programmes leading to

better performance of the Global Fund grants and provision of effective health delivery systems.

The Phase 2 plan aims to address Organisational Level or Institutional Systems Strengthening. We

are achieving this through focusing on organisational policies, procedures and frameworks, which

allow SRs and SSRs to operate and deliver on their mandates and enable individual capacities,

which was a focus of CD in Phase 1, to connect and contribute to the achievement of

developmental goals.

Round 8 grants, namely:

− The Zimbabwe AIDS Network (ZAN)

− The Health Services Board (HSB)

− NatPharm

All capacity development activities are cross cutting and are financed through all the four Global

Fund grants (HIV, TB, Malaria and HSS).

Cross cutting key achievements

� Procurement and selection of various technical experts, or firms, to support

implementations of activities, such as HSB Staff Satisfaction survey, NAC Risk

Management Framework, MOHCW Oversight and Accountability Management

Procedures and Management and Leadership Technical teams.

� Recruitment of a consulting firm and the undertaking of field work for the

assessment of storage conditions of 52 urban and rural peripheral health facilities,

the findings of which will inform the design and submission of an action plan on

the required improvements of storage of health products in these facilities.

� Procuring services providers to implement the action plan recommended by the

assessment of the 52 storage sites.

� Midterm review of the 2010-2014 MOHCW HR Strategy to ensure coordination of

all training activities.

� Procuring a Refrigerated Truck for the MOHCW Laboratory Services

� Completion of the NatPharm Roadmap

� Procuring a Service Provider for installation, Networking and trainings of Pastel to

Key contributions of the HSS Grant

The total contribution of the HSS grant to Capacity Development is $62,780.60 for two main

objectives; (i) Strengthened Risk and Results Based Management, Accountability and Oversight at

National and Provincial Level and (ii) Comprehensive, Accurate, & Timely Data Capture, Analysis &

Reporting from District & Provincial levels. The main implementers were the Ministry of Health and

Child Care (MOHCC) and the Health Service Board (HSB). Table 9 summaries the programmatic

and financial status of the six activities under the HSS grant.

Activities 2.4.1, 2.4.2 and 2.4.3 Staff Survey

The Ministry of Health and Child Care (MOHCC) continues experiencing loss of trained,

experienced, key professional and experts required to deliver, high quality, effective and efficient

health care system in Zimbabwe. Staff wastage has been a major concern to the MOHCC for a long

time and for many reasons. A manpower study conducted in 1983 revealed that there was a high

wastage of qualified health manpower (MoH, Report, 1983, p 5) ranging from 12% to as high as

38%. In 1990, staff shortage was ranging between 42% to as high as 58% in most departments

(Equity in Health). In an effort to reduce the high levels of staff attrition in the health sector, the

HSB conducted a comprehensive staff satisfaction and motivation survey that will inform the

development of a non-financial incentive plan. The plan will be a resource mobilisation tool that is

aimed at increasing staff retention levels.

The findings in the report highlight the high level of dissatisfaction with working conditions and

environment, which stands at 99%. There was widespread consensus on the reasons for this high

level of dissatisfaction as indicated below poor corporate governance, adverse working conditions,

and extremely low remunerations, lack of career advancement and promotion opportunities. The

report recommends some activities to be considered in the non-financial incentive plan. The HSB

endorsed the report in November 2013 and a stakeholder meeting is planned for 2014 to finalise

the non-financial incentive plan.

Activity 3.6 - GIS Mapping: Over $29 000 was disbursed to the MOHCC, IT GIS Programme to

update the national Health facility database. The funding allocated to this activity complimented

the UNOCHA contribution to the project.

Activity 4.2 - Monitoring & Evaluation Training: An M&E for the 5 HSB Provincial Auditors

Training was held from 4 to 6 March 2013. Other HSB senior management also attended the

training which was held at Parirenyatwa Hospital in Harare. Items covered during the training

included:

� Introducing M&E

� Evaluating HSB Programmes

� Setting up and M&E System for HRH Strengthening Programme

� Defining and Developing Indicators

� Introducing Results Based Management

� M&E Requirements for GF Grants

� Planning for M&E – Introducing the Logical Framework

� Improving Data Management for HRH Strengthening Programme

A training Manual was also developed for the HSB, particularly to improve the management of the

HRH Strengthening Programme through capacity building of staff to collect, collate, verify, report

and utilize data for decision making.

Activity 4.3 - Training on the Health Retention Scheme Database

The Database training held from 14 January to 1 February at HSB offices developed the capacity of

the HSB staff supporting the HRS database through one to one on the job training. The training

targeted HSB staff in the IT department, Human Resources Staff Establishment, Training and

Recruitment Sections. Database systems that the HSB staff required training was on MySQL and

Linux for the ICT unit and MS Access and Excel. The training included;

SUCCESS STORIES

7.1 SUCCESS STORIES

National Roll out of DHIS2: DHIS2 was successfully piloted in the first quarter of the year and

nationally rolled out to all provinces and districts and selected hospital by the end of the year. The

speedy implementation of DHIS-2 ensured that monthly T-5 reporting of most of the health

facilities in the country reported using DHIS2

A total amount of $2.7 million was successfully mobilized from the Global Fund to support the

implementation of the DHIS-2. The additional resources followed the development of the 2013 HIS

Plan

Improvement in T-5 Monthly Reporting: The completeness of the T-5 monthly reporting of the

MOHCC which had underperformed miserably in 2010 of just about 50% improved to 100% by the

end of 2-2013.

Internet Connections: Fixed and mobile internets access (fibre, VSAT and dongles) were

successfully provided to all provincial and district offices as well as admitting hospitals linked to

IMMIS. This facilitated the timely transmission of reports to the national level to inform decision

making

Collaboration with CC/RTI: Collaboration among donor and technical partners helps eliminate

duplication of efforts and join forces together for the betterment of development of a country.

During the year, UNDP continued its collaboration with the CDC funded Research Triangle

International (RTI) to support the strengthening of the health information system of the MOHCC

UNDP and HSB strengthened its oversight role over the implementation of the health retention

scheme. Field verification visits were carried out by UNDP, HSB and Crown Agents and the

observations addressed timely.

7.2 CHALLENGES

The main challenge faced in the implementation of the HSS grant in 2013 was the significant delay

in the submission of the acquittals by the Nursing Department of MOHCC resulting in the non-

payment of allowances to VHWs for 9 months.

The ZAN organizational changes and the transition of the SR-ship to the new SR have also affected

the implementation of the CSS component during the first half of 2013.

CONCLUSIONS

The HSS grant continued to exhibit excellent performances during the year which reflected in the

A1 rating by the Global Fund.

Some significant achievements were recorded during the year and some innovative approaches

either adopted or were initiated during the year with the collective efforts to enhance the

implementation of the grant.

A new more efficient mode of paying VHWs and internet connectivity for sites were explored and

initiated to be completed in 2014.

Similarly, observations and recommendations from field visits on the implementation of the health

retention scheme were implemented whilst UNDP collaborated effectively with CDC/RTI and other

stakeholders.

The net effect of all these interventions and collaborations is to contribute to the improvement of

the public health system in Zimbabwe. In spite these impressive achievements, some challenges

with experiences mainly with acquittals by the Nursing Department and thus affecting timely

payment of allowances to VHWs.

Late submission of Disbursement Requests and by HSB/CA in some of the quarters also delayed

timely payment of the top up allowances to deserving critical health staff.
