errors cannot be excluded, since (a) prediction of the postoperative IOL position is based on the population mean and may significantly deviate in individual cases, (b) postoperative IOL dislocation (e.g. due to capsular phimosis etc.) cannot be predicted, (c) errors in the transmission of the biometric data cannot be excluded, and (d) accuracy of the biometric raw data cannot be verified by CZM. CZM shall not be liable for any potential postoperative refractive errors in connection with the IOL power proposals submitted.
Carl Zeiss Meditec AG (CZM) provides a service for medical practices to calculate the IOL refractive power. Calculation of these power proposals will be made on the basis of the biometric data of the patient using a proprietary calculation algorithm. The results of IOL calculation performed by CZM should be regarded as non-binding proposals specifically matched to the CZM IOL product line. Please note that these proposals cannot replace the professional expertise of the health care specialist. Despite careful processing of the data transmitted, postoperative refractive

Pathology/previous surgery Comment
Corneal pathologies like keratoconus and pellucid marginal degeneration as well as corneal scars or keratoplasties create asymmetrical shapes of the cornea. This means the that the two main meridians are not always consistent and that the astigmatism cannot be corrected with toric IOLs or needs
to be accounted for in the IOL Calculation. In these cases, please fill out this questionnaire completely and provide us with an up-to-date corneal topography in color.
