Our Resident Services Coordinators provided 575 resident interventions.
Susan Cramer, BSW and Janet Salsbury, MSW serve as case managers and tireless advocates for our residents. They help residents navigate medical and insurance benefi ts and connect with appropriate community resources; offer crisis intervention and supportive counseling; help residents with civil rights issues; coordinate family support; intervene to address social isolation; and help families to coordinate health care services and identify home management needs for continued independence.
In addition, Susan facilitates a monthly book group, recruits educational speakers, leads a bi-monthly Women’s Political Forum and coordinates the annual Northaven Wellness Fair with 35+ vendors. Janet facilitates monthly low-vision and diabetes support groups. She also supports the garden group, organizing excursions, scheduling speakers and coordinating 69 garden barrels for residents.

I feel compelled to write to you about how much I love and admire the care I see at Northaven Assisted Living. I am constantly impressed by the expertise and excellent patient care by your nursing staff…
Your residents have diffi cult medical and psychosocial issues. However, they thrive with the individual nursing and staff attention at Northaven. The daily structure, personal interactions and social activities at Northaven allow residents to thrive... Your patient-directed approach keeps residents out of the hospital. Your skilled care allows your residents to enjoy the end of their life at the highest level.
