• Poor recruitment of GPs
• “Zombie Practices”
• Demographic bulge of GPs over 50 + NHS pension changes
• Increased workload
• Reduced resources
• Nationally driven objectives GPFV/STPs/Vanguards
• Extended hours from April 2018
• CCG need for LCS ‘Single Contract Holder’
• “Work life balance / imbalance’’
• The prospect of a wasted career
• Patients being ruined by the system
• ………..what is there to lose?!
What has happened in Norwich?
What will OneNorwich do?
• Give Norwich practices One Voice for system working through the 4 elected posts
• OneNorwich will be a Not-for-Profit organisation (CIC)
• Ambition for OneNorwich to hold the MCP contract in partnership with others
• Embeds Norwich practices into Norfolk's STP footprint (CRG, PCSC, I/T)
• Gives the Leadership Board a formal entity
• Enable General Practice at Scale through identified workstreams
• Enable GP practices to work closer together and have the same objectives
• Give practices much needed support from a primary care focused organisation
• More inclusive for all practices and staff
• Develop sustainable clinically excellent general practice
What is the Role of the Leadership Board?
• Chaired by elected members
• Provide strategic direction for General Practice in Norwich
• Provide options for practices about function and form for the future to ensure sustainability
• Approve and develop business cases for General Practice Working at Scale
• Be the forum for collaborative working with the Norwich system providers
• Programme Board for PMS Money before submitting to NHSE & CCG committee
• EAHSN MCP Readiness Assessment, of OneNorwich in August 2017
• To drive the evolution of OneNorwich from a simple alliance into a formal functioning provider by 1st April 2018
The 10 Key Aims
• Make OneNorwich an official company structure to ensure readiness for MCP contract
• Develop GP neighbourhoods and tune them according to their demographics
• Release ‘time capacity’ in GP practices to enable GPs to ‘stay in their practices’
• Shift appropriate secondary care into primary care i.e. paediatric diabetes
• Integrate out of hospital care across health & social care
• Prepare for the expected growth of population over the coming years
• Utilise GP & others clinical skills and abilities to maximum effect
• Develop sustainable services over the long term
• Work smarter NOT harder
• Secure maximum value for money for commissioners
Norwich New Model of Care Business Case
The Business case:
• Implementation of Primary Care at Scale as part of the Norwich New Model of Care - £3 per head of primary care transformation money + an additional 50p per head of population from April 2017 until April 2019.
The business case high level detail:
• Support to fund the 4 elected executive team and the Head of Transformation role and management team
• Support to fund the Norwich New Model Leadership advisory group
• Helping to deliver the 10 high impact actions
• Develop the neighbourhoods across Norwich i.e. City 1,2,3 and 4
• Move appropriate treatment into primary care with the correct funding envelope
• Drive forward the MCP across the city + Potential for OneNorwichto be MCP Contract holder or lead provider
