meet for Wonca

The 2006 Wonca-Europe conference brings us toFlorence, one of the worldÕs pillars of culture and histo-ry. Wonca-European conferences are the annual meet-ing place of general practitioners and other scientistsinvolved in building ever-stronger primary care. It is ameeting that has earned its place on the agenda. Eachconference is based on WoncaÕs infrastructure forresearch, teaching and quality development, but eachconference is different - and each one is special. Whatmakes Florence special is the link to our common his-tory: its streets and palaces, its museums and churchesall breath the Renaissance that shaped the Europeanindividual human being and its norms and values. We, medical generalists and family doctors, focus ourcare on the individual - our patient - in pursuit of thebest care and treatment. Human values are an inte-gral part of our professional endeavours, and wehave to address patientsÕ perspectives in increasing-ly more powerful biomedical interventions we haveat our disposition. That is precisely the excitingtheme of this conference and ÒFlorenceÓ can help uskeeping track of the human face of medicine.Florence is also an ideal stage to experience theproverbial Italian hospitality. And in this respect, againFlorence makes a difference. Our hosts are the CentroStudi e Ricerche in Medicina Generale (CSeRMEG) afounding member of Wonca-Europe. In preparing forthis conference, they have partnered with theAssociazione Italiana Medici in Famiglia (AIMeF) andthe Scientific Interdisciplinary Association of Familyand Community Medicine (ASSIMeFaC), alreadyWonca member organization, and SIMeF/FIMMG(Societ� Italiana Medicina di Famiglia/FederazioneItaliana Medici di Medicina Generale), SIMG (Societ�Italiana di Medicina Generale), SNAMID (Societ�Nazionale di Aggiornamento Interdisciplinare),SIQuAS (Societ� Italiana per la Qualit� dellÕAssistenzaSanitaria), ACP (Associazione Culturale Pediatri),EGPRN (European General Practice ResearchNetwork). This demonstrates the richness of generalpractice/family medicine in Italy. Colleagues andfriends of all these groups and organizations havedone their utmost to prepare the conference and willcontinue to make it happen. I am convinced that theirdedication to the academic programme and their per-sonal involvement will touch you all for a truly firstclass scientific and social conference. Dear Colleagues,

as the European regional president of WONCA I amdelighted that the meeting in 2006 is going to takeplace in Italy. There are many reasons for that. The annual conferences of WONCA Europe havebecome the most important meeting for the disciplineof general practice and family medicine in Europe.They have become a meeting place, where practition-ers from different places bring together their expertiseand exchange their experience among peers.Europe is a continent, marked by its variety of culturesand characterised by tolerance. One could hardly think of a better meeting place thanFlorence for hosting such an event. The exciting andattractive capital of Tuscany is certainly one of themost interesting venues an international organisationof any kind may hope for.I am absolutely sure that family physicians all overEurope and other parts of the world will find their stayin Italy a very pleasant one.The other reason is as important as the previous one. Over the years, we have witnessed a rapid develop-ment of family medicine in Italy.The contribution of various organisations of Italianfamily medicine is becoming more and more promi-nent and it is therefore sensible to have our annualmeeting in a country that has so much expertise tooffer to European colleagues.There is no doubt that the meeting of general practi-tioners in Florence will be another successful eventthat we will like to remember. I am certain that you will take the opportunity and visitFlorence which is going to be the gathering place forfamily physicians throughout Europe for 2006.

perceived by that self as an illness, to look at it as apart of a biological whole.

2. RISK AND SAFETY:The 2002 Wonca Europe Definition of GeneralPractice states Family Doctors should protect theirpatients from unnecessary and dangerous interven-tions. 30 years after his provocative book ÒMedicalnemesisÓ Ivan Illic we must take our responsibility torethink to the influence of our daily activity, and thatof medicine as a whole, in the health and life of thepeople, finding the safest balance of our interven-tions. In western world, patientsÕ expectations areoften unrealistic, because they are exposed to mis-leading information, and lead sometime to misun-derstandings, overdiagnosis and overtreatments. We are fully aware of the limits of evidence-basedmedicine, meanwhile evidence-based medicine is apowerful instrument that can be of great help forGPs willing to be really accountable and patientscommitted.

3. FAMILY PRACTICE RESEARCH:Special attention will be paid on building research capa-bilities among General Practitioners, and some impor-tant examples of independent research will be shown. Research should be built into practice, and viewedas a ÒnormalÓ - not separate - setting of care.Lack of evidence should stop to turn into arbitrarysolutions for the many Ògrey areasÓ of our dailywork: sharing uncertainties, between doctors andpatients, should prompt rigorous and originalresearch methods to find rigorous answers.

4. CARING FOR THE WORLD:The growing involvement of African and SouthAmerican members in WONCA and the extraordinaryimportance of inequalities in the health outcomeswill be discussed in this session. As it is in our ancient tradition of Mediterraneancountry we would like to build bridges betweenEurope and other Countries: western world shouldallow the developing world speak Òhis wayÓ by itself.

5. QUALITY OF FAMILY MEDICINE:From problem-oriented, to goal- and patient-orientedquality in medical care. This is the ongoing discussion where FamilyMedicine has to find out its own position.

6. EDUCATION:An open discussion on the different experiences ofthe schools of general practice in Europe and somepedagogic proposals coming from the Italianschools will be shown.

Each main topic will have a dedicated chairman dur-ing the three days of the conference that will sum-marize the contents in the last hour. There is a lot ofwork to do together: so É

Dear Colleagues,I have the honour and the pleasure to invite you atthe 12th WONCA Europe Regional Conference thatwill be held in Florence (ITALY) on August 27-30,2006. It is a great opportunity for the Italian FamilyMedicine that I am sure will convey its original con-tribution to the debate and the thriving of theEuropean General Practice/Family Medicine to whichit fully belongs. Our hope is that Florence may helpGeneral Practitioners/Family Physicians from all overEurope and the Mediterranean Area to work in a sucha creative and rigorous way that a new scientific par-adigm for our profession may come forward indicatingthe way toward a new Renaissance for Family Medicine.For all this the title chosen for the Conference has been

AND HUMANITIESWe imagined the conference works developing alongsix main topics:

1. BIOLOGY AND HUMANITIES:As stated in the 2002 Wonca Europe Definition ofGeneral Practice, General Practitioners/Family Doctorsare the first point of medical contact: an open andunlimited access regardless of the age, sex andhealth problems. They manage diseases at an earlystage and often in the absence of high-tech instru-mentation. Moreover they deal with acute and chron-ic illnesses in their psychological, social, cultural andexistential dimensions. That describes what GPs fromall over the world know by experience: culture hassomething to do with medicine and is responsible forthe complexity perceived by GPs.

Alas medicine lacks of a sound biological modelcapable to solve the antinomy nature/nurture. Thebio-psycho-social paradigm is of great help and addsvalue to the family doctorsÕ job, yet it canÕt answer tothose questioning the relevance, for example, of lin-guistics or anthropology or sociology for a clinician.At the moment, since neurobiology appeared, pri-mary-care can do a little better: human subjectivitystops to be a metaphysical trait of the animal andstarts to display its nature of extended phenotypicalcharacter deeply rooted in genetics and in the bio-logical development of the nervous system. Thishuman self, soundly embodied in his proper uniquebody leading his proper and unique life, allows thephysician, challenged in the observation of an event

The organization of a European Conference is aunique event in the history of Italian General Practice.For this reason the aim of the organizing committee isto prepare in Florence 2006 an event to be remem-bered as one of the most important offered byWONCA Europe to its members and all those caring forGeneral Practice/Family Medicine.We surely want you to have a marvelous stay and nodoubt Florence will be able to marvel you; but we donot want for you only a beautiful show.We would like you to go back home pleased by thecertainty to have lived a very exciting and scientifical-ly relevant experience in being where important top-ics, for general practitioners from all over Europe andthe Mediterranean area, were stated and discussed.For this reason the conference will try to bridge thegap between biology and humanities facing six maintopics.Each one of them will have a dedicated chairman thatwill follow the works during the three days of the con-ference and will summarize in a 10 minutes presenta-tion the State of the Art in the last plenary session. Soyou may follow the lectures and sessions of your maininterest but before leaving the conference you will beinformed about what of relevant has been told in theother ones.Everyone must be able to bring back home a clearidea about the settled achievements emerged duringthe ongoing process of the conference.As we believe that every contribution should find theproper place we are planning new criteria for the ref-erees and a room for each ÒongoingÓ activity ofWONCA like for example the organizations in collabo-rative relation and the special interest groups.We strongly want Florence 2006 to become anotherimportant step for all GPs attending, on the waytoward a thriving General Practice both for Italy andEurope.We are lucky enough to have at our disposalLeonardoÕs land: the best place to build a newRenaissance for Family Medicine.

research) as to the object observed. Research meltsinto daily practice, and becomes part of care.

Quality may be further improved: goal- and patient-orientation stop being attitudes and become essentialprofessional strategies grounded in the neurobiologi-cal model explaining illnesses, the diagnostic and thetherapeutic processes. Recognition of uncertaintiesmay prompt rigorous and original researches to findrigorous answers. Local conditions may legitimatelydeserve a peculiar local ranking of priorities leading tospecific solutions.

Education may be much better tuned and in someparts even totally rearranged. ThereÕs surely a greatneed for a more widespread teaching of EBM but theworldwide spread idea that the ÒartisticÓ and empath-ic side of medicine may be Òstolen to the MasterÓ tellsthat good teachers are important but good theorieson how humans learn are even more.

Risk and safety of medical interventions may be bet-ter assessed by the accountable empathic advocacy ofthe Family Doctor outlined by the 2002 Wonca EuropeDefinition of General Practice. Family Doctors wellinformed on EBM, protecting their patients fromunnecessary and dangerous interventions, are able tooffer the safest management of illness for the individ-ually tailored best of patients.

EXPLANATION OF THE PROGRAMME

The underlying thread of the 12th WONCA EuropeConference is the neurobiology of both patient ill-nesses and doctorsÕ actions oriented at the Care forthe World.

The six main topics are described by the following key-words:

Caring for the world: ability to cope, addictions,bridges between national experiences, cardiovascularrisks and diseases, diabetes, elderly care, gastroen-terology, health education, illness, inequalities, men-tal disorders, negotiation, new cultures in Europe,nutrition, palliative care, partnership, primary caredevelopment, rehabilitation, respiratory problems;

Biology and humanities: anthropology, clinicalmethodology, clinical reasoning, cognition, communi-cation, complementary medicine, compliance, con-versation analysis, counselling, decision making, dis-course analysis, empowerment, epistemology, historytaking, humanities, infant observation, language, nar-rative medicine, neurobiology, placebo, problem solv-ing, story telling, subjectivity, symptoms analysis;

Family practice research: building research capabilities,epidemiology, methodologies, needs in western world,needs in developing countries, practice based researchnetworks, qualitative research, research case histories,research in Family Medicine a lost cause?, solo research;

Quality: accountability, aims, case management, clas-sifications, doctor development plans, indicators,methodologies, patient evaluation, practice develop-ment plans, practice management, practice organiza-tion, settings, technology;

PROGRAMME PRESENTATION

Biology and humanities: a gap to be bridgedGeneral Practitioners/Family Physicians are the firstmedical contact providing to those who consult themopen and unlimited access regardless of the age, sexand health problems. They manage diseases at anearly stage in their development and often in theabsence of most of well known pathognomonicsymptoms and signs and high-tech instrumentation.Moreover they deal with illnesses also in their psy-chological, social, cultural and existential dimensions. These very pinpoint characteristics, far from being aslogan, describe what GPs from all over the worldknow by experience: culture has something to dowith medicine and is responsible for the complexityperceived by GPs. Alas medicine lacks of a sound bio-logical model capable to solve the antinomynature/nurture since neurology and physiology ofmind in the early 20th century underwent a schism aseach of these proceeded along a separate way. Soprimary-care, Cinderella among all the other highlyscientific and hyper technological medical special-ties, tried hard to solve the unsolvable, exploringempirically the antinomy nature/nurture and itseffects on clinical action. The bio-psycho-social para-digm seemed to be of some help but was unable toanswer to those people questioning the relevance,for example, of linguistics or anthropology or sociol-ogy for a clinician. At the moment, since neurobiolo-gy appeared, primary-care is doing a little bit better:human subjectivity (and so its cultural manifesta-tions) stops to be a metaphysical trait of the humanbeing, and starts to display its nature of extensivephenotypical character, deeply rooted in geneticsand in the topobiological development of the nerv-ous system, from the gastrulating embryo onward.This human self soundly embodied in his properunique body, leading his proper and unique life,allows the physician, challenged in the observation ofan event perceived by that self as an illness (an inabil-ity to cope), to look at it as a part of a biologicalwhole; thus Doctors are able to embrace again biolo-gy in its evolutionistic flowing. The consequences forthe clinician assuming this point of view are many:

Caring for the world may finally come true. Themany multifaceted and locally specific cultural deter-minants of individual inability to cope, cause ofinequalities in health outcomes, may legitimately betaken into account, by every GP in any part of theworld, while deciding the best way to manage the ill-nesses of his/hers very specific patient. The strength-ening of patientsÕ competences (alimentary, neu-rolinguistic, neuromotor, social and so on) maybecome one of the biological priorities in decidingstrategies of care for them.

Family practice research may become lively and rich.The many Ògrey areasÓ, even those culturally or social-ly determined, of our daily work may be enlightened,observed and studied without hindrance; we are surethat the only ÒmustÓ has to be the appropriateness ofthe research tools (e.g. clinical trials vs. qualitative

Risk and safety: advocacy, Evidence-Based Medicine,health information, medicalization occupationalhealth, patientsÕ expectations, pleasure as a risk factor,safe care, screening and preventive medicine.

These themes will allow participants, while easily surf-ing trough their preferred conference sessions pre-senting, discussing and facing their own everyday realproblems, to be exposed to new ideas, state of the artknowledge and reliable guidelines.Every moment of the conference will be an occasion to

think globallyin a rigorous scientific manner to
