Speaker profiles and abstracts (where supplied) Biography Dr Nachi Palaniappan, Consultant Oncologist, Velindre Cancer Centre

Trained in Wales and now working as Consultant Clinical Oncologist specialising in head and neck and urological malignancies since January 2011. My interests include practicing advanced radiotherapy techniques and involved in implementing and expanding this service at my institution.

Abstract

Advances in radiotherapy Radiotherapy plays an important role in the non surgical management of patients diagnosed with cancer and 40% of patients diagnosed with cancer are cured of their disease with radiotherapy treatment either alone or in combination with other modality. Delivery of radiation has evolved over the last 100 years and major improvements were seen in the last 2 decades with advances in the field of engineering and computers. The major advances include ability to shape the field, deliver high dose to the region of interest and reduce the dose to surrounding normal tissue and the ability to verify the position of the patient prior to delivering the treatment with the use of soft tissue imaging. Curative doses of radiation are normally delivered over a number of fractions. In room soft tissue imaging helps us to identify the change in shape and position of the target and enable us to account for those day to day variations prior to delivery of radiation to the region of interest on every day for the entire treatment duration. Movement of target during treatment due to respiration can also be accounted for now with the help of 4D CT scan and various breath holding techniques. The various technologies that are now routinely used in clinical practice are intensity modulated radiotherapy (IMRT), image guided radiotherapy (IGRT), volumetric modulated radiotherapy (VMAT), and stereotactic ablative radiotherapy (SABR). Recent advances in PACS technology

DICOM as an imaging standard has been around in its current form since 1993. The

advances in PACS over the last 4 to 5 years have been driven mostly by the introduction of

new IHE profiles such as XDS, XDS-I and CDA. This, coupled with a deeper understanding

of the different applications of DICOM, has ushered in an era of new possibilities for:

Better data management in PACS

More efficient ways of sharing PACS data across health boards using

XDS XDS-I

Better dose management solutions

My presentation will highlight how these new technologies improve the

quality of care delivered to patients, how it enhances collaboration among

medical practitioners and how it gives life to the realisation of a “central PACS” across NHS

Wales.

Radiology training - where are we now and where do we want to be? What is the purpose of a training scheme and is it responsible for shaping the future workforce in Wales? The delivery of radiology services has changed significantly over the past 10 years, in order for the specialty to survive, training must also adapt to the service requirements. In its current format the training programme will not support a sustainable workforce in Wales unless considerable changes are made There are significant challenges facing the current and future radiology workforce in Wales. The presentation will outline the issues and challenges, proposed and realised strategies to address these matters. It will also give an opportunity to discuss and explore how we propose to tackle the problems. Biography Dr Craig Parry, Consultant Radiologist, Cardiff and Vale University

Dr Craig Parry is a Consultant Radiologist subspecialising in gastrointestinal imaging at the University Hospital of Wales Cardiff. He developed an interest in diagnostic imaging during undergraduate medical training at Guy’s, King’s and St Thomas’, London where he undertook an intercalated BSc in radiological sciences and medical imaging. On completion of his medical degree in 2001 he progressed with a surgical training rotation as a foundation to a career in radiology. In 2006 he joined the All Wales Radiology Training scheme and on completion in 2011 undertook further higher specialist training with a one year fellowship in abdominal imaging at Sunnybrook Health Sciences Centre of the University of Toronto, Canada. He joined the Department of Medical Imaging at the University Hospital of Wales as a consultant 1 year ago and is developing a specialist interest in hepatobiliary imaging. Abstract

Advances in abdominal MRI imaging

Diffusion imaging is a valuable tool in diagnostic magnetic resonance imaging that despite

being a well recognized and established technique is often under utilised in the investigation

and diagnosis of abdominal and pelvic disease.

The strength of diffusion imaging lies in its sensitivity and ability to detect

inconspicuous lesions, but acquisition of diffusion sequences can be susceptible to artefact

and interpretation can prove challenging.

However when evaluated in conjunction with standard abdominal MRI sequences it is

complimentary when characterizing lesions and often invaluable in detection of lesions. On

occasions it can prove the sole component of the study that determines clinical outcome.

The technical aspects of diffusion in abdominal imaging will be explored and clinical cases

will be used to emphasize its value in lesion detection and evaluation. Particular reference

will be made to cancer staging and the common pitfalls highlighted.
