2B - Better Care Management and Lower Costs: Solving for Social determinantsThe shift to value-based care puts more pressure on providers to improve outcomes and reduce the cost of care. However, research shows that more than 80% of health outcomes are unrelated to medical care. Instead, they are attributable to outside social and economic forces, such as housing, education, unemployment, low income, transportation, access to green space, loneliness, inequality and other non-medical factors. Considering 68% of patients have at least one social determinant challenge, clinical data can only go so far to improve health outcomes and reduce the costs associated with more complicated health conditions, no-shows and readmissions. Attendees of this session will learn how to include patient-centered engagement strategies based on social determinants of health to fill the 80% gap in caring for the entire patient.

3D - Use Lean to go from Red to BlackWhat is lean and how can it help turn us from red to black? Through storytelling and images the speaker will convey the value of understanding the process, who the process impacts, as well as identifying constraints, and non-valued added steps. The discussion begins by understanding what a process is followed by the discussion of eliminating waste. Eight types of waste are typically used to provide a category when identifying non-value-added steps.
